Clinical course and prognostic risk factors of SARS-CoV-2 vaccine-related hepatitis: differentiating severe acute respiratory syndrome coronavirus 2 vaccine-associated liver injury from autoimmune hepatitis.

BACKGROUND AND AIM: Reported cases of acute liver injury with autoimmune features post-COVID-19 vaccination raise questions about whether this represents vaccine-triggered autoimmune hepatitis (AIH) or self-limiting drug-induced autoimmune-like hepatitis (DI-ALH). We report follow-up data to determine if the disease course is self-limiting or immunosuppression-dependent. METHODS: Members of the International AIH Group and the European Reference Network on Hepatological Diseases who contributed cases to our original cohort provide follow-up data at 6 months, 12 months, and at last follow-up.

Simvastatin Alleviates ConA-Induced Autoimmune Hepatitis by Inhibiting CD4+ T Cell Activation via Calcium-NFATC3 Pathway.

Although simvastatin plays a crucial role in lipid management, tumor therapy and acute liver injury, its potential effects in autoimmune hepatitis (AIH) has received limited investigative attention. Our study demonstrated that in the ConA-induced AIH model, HMG-CoA reductase (HMGCR), the pharmacological target of simvastatin (SIM), was significantly upregulated in T cells, particularly in CD4+ T cells. Furthermore, our results showed that simvastatin treatment in ConA-induced AIH model reduced the level of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) and alleviated liver

Screening and identification of a novel PGAM5-specific inhibitor for attenuating multi-organ injury.

INTRODUCTION: The necroptosis regulator PGAM5 drives a pathological cycle of mitochondrial dysfunction and necroptotic signaling, contributing to multi-organ injury and representing a potential therapeutic target. Despite its clinical relevance, few PGAM5-specific small-molecule inhibitors have been developed. OBJECTIVES: We aimed to identify a safe and effective natural small-molecule inhibitor targeting PGAM5 as a novel therapeutic strategy. METHODS: Global PGAM5 knockout mice and pancreas-specific PGAM5 knockdown mice were used to clarify the regulatory role of PGAM5 in pancreatic injury

Autoimmune hepatitis overlap syndrome: Clinical presentation, treatment response, and disease evolution: Tertiary centre experience.

BACKGROUND:: Autoimmune hepatitis (AIH) overlap syndromes are conditions wherein features of AIH coexist with those of primary sclerosing cholangitis (PSC) or primary biliary cholangitis (PBC). We aimed to evaluate their clinical presentation, treatment response, and disease outcome in an Arab cohort. METHODS:: AIH overlap patients, based on the Paris criteria for PBC/AIH overlap (n = 19), and revised International Autoimmune Hepatitis Group (IAIHG) score for PSC/AIH overlap (n = 17) were reviewed at a single center in Saudi Arabia, from 2001 to 2024. Biopsy-proven standalone AIH patients (n

Not always autoimmune hepatitis: Hepatic steatosis is associated with autoantibodies positivity in patients with hepatocellular dysfunction.

BACKGROUND: Autoantibodies, including anti-nuclear antibodies (ANA), are typically present in autoimmune hepatitis (AIH), but may be detected in other liver conditions, including metabolic dysfunction-associated steatotic liver disease (MASLD), even in the absence of an autoimmune disorder. AIM: To identify predictive factors for the non-confirmation of AIH diagnosis in patients with liver dysfunction of unknown etiology and positive autoantibodies. METHODS: A retrospective cohort study was conducted in a university-affiliated hospital, including consecutive adult patients with liver dysfunc

Association between osteoporosis and waitlist mortality in liver transplant candidates.

Osteoporosis is a common comorbidity in end-stage liver disease (ESLD), yet its impact on waitlist survival remains underexplored, particularly in non-Western populations. This prospective cross-sectional study evaluated bone mineral density (BMD) and its association with mortality in 702 adult candidates, including 424 (60.4%) males and 278 (39.6%) females with an overall mean age of 45.13 ± 13.67 years, awaiting their first deceased-donor liver transplantation at a major referral center in Iran. BMD was assessed via dual-energy X-ray absorptiometry (DXA) at the lumbar spine, femoral neck, an

The Geoepidemiology of Autoimmune Hepatitis.

Autoimmune hepatitis (AIH) is typically a chronic, inflammatory, and progressive liver disease of autoimmune origin. Currently, two recognized types are identified in clinical practice: type 1 and type 2. The disease can occur in all geographic regions and racial/ethnic groups worldwide, but shows marked variations. This pattern reflects a complex interplay between genetic factors and environmental triggers. In this review, we aim to examine the geoepidemiological features of AIH and explore how these features relate to its etiopathogenesis, providing an integrated analysis of this complex con

Antinuclear antibody targets in autoimmune hepatitis and drug-induced liver injury: Diagnostic relevance of nucleosome antibodies.

BACKGROUND & AIMS: Anti-nuclear antibodies (ANA) are detected in 70-100 % of patients with autoimmune hepatitis (AIH) and in 22-33 % of patients with drug-induced liver injury (DILI). However, little is known about the specific antigens which they bind to. This study aimed to investigate the antigen specificity of ANA in patients with AIH and DILI and to assess the diagnostic value of these antibodies in differentiating between the two diseases. METHODS: We performed a retrospective, cross-sectional analysis of ANA-positive patients with AIH or DILI treated at a tertiary referral center. ANA

Azathioprine-induced acute generalized exanthematous pustulosis in a woman with autoimmune hepatitis.

Acute generalized exanthematous pustulosis (AGEP) is a severe cutaneous drug reaction characterized by sterile pustules on erythematous skin. Azathioprine (AZA)-induced AGEP is rare, with few cases documented. We report a 50-year-old female with autoimmune hepatitis who developed a widespread pustular rash five days following the beginning of AZA therapy. Histopathology confirmed subcorneal pustules accompanied by eosinophilic infiltrates, indicative of AGEP. The discontinuation of AZA and systemic corticosteroids resulted in swift remission. This instance highlights the significance of prompt

Human leukocyte antigen variants and clinical features of primary biliary cholangitis: Cumulative contributions.

This letter to the editor highlights the importance of considering potential cumulative contributions among human leukocyte antigen (HLA) alleles in shaping the clinical manifestations of primary biliary cholangitis. Complementing the overview by Curto et al, which focused on non-HLA candidate genes, this paper emphasizes that specific haplotypes of HLA-DRB1, HLA-DQA1, and HLA-DQB1, as well as HLA-G*01:01:01:08/UTR-1, may modulate disease heterogeneity, predisposition to primary biliary cholangitis and autoimmune hepatitis overlap syndrome, disease progression, and poorer therapeutic response.

Clinical Characteristics of Chronic Pancreatitis in a Saudi Arabian Population: A Retrospective Cohort Study at Riyadh Second Health Cluster.

BACKGROUND: Chronic pancreatitis (CP) is a progressive inflammatory condition with regional differences in prevalence and etiology. Though alcohol and smoking are primarily the risk factors in Western populations, CP resulted from alcohol is rare in Saudi Arabia due to cultural differences. Our study aim to characterize the Etiology, clinical presentation, and complications of CP in Saudi Arabian adults who are minimally alcohol exposure. METHODS: Our retrospective research highlights on patients who are 14 years of age and older that diagnosed with CP, who are treated at King Fahad Medical C

Dosing and Route of Steroid Induction Therapy in Non-Severe Autoimmune Hepatitis Do Not Affect Time to Biochemical Response.

BACKGROUND AND AIMS: Optimal steroid induction therapy in autoimmune hepatitis (AIH) is unclear. We aimed to investigate the relationship between steroid dosing, route of administration, and time to biochemical response to help standardize AIH practice patterns and reduce excess steroid exposure. METHODS: We conducted a retrospective cohort study of adult patients with non-severe AIH receiving induction steroid therapy. Patients were stratified into three treatment groups: (1) low-dose oral steroids (n = 39; prednisone < 40 mg/day or budesonide 9 mg/day), (2) high-dose oral steroids (n = 32;
