Hypoxia-induced GP73 promotes the Warburg effect in hepatocellular carcinoma via HIF-1α.

BACKGROUND: Transarterial chemoembolization (TACE)-induced hypoxia is a key driver of hepatocellular carcinoma (HCC) progression. This study investigated whether Golgi protein 73 (GP73) drives the Warburg effect and epithelial-mesenchymal transition (EMT) via hypoxia-inducible factor-1α (HIF-1α) under hypoxia, thereby promoting HCC malignancy. METHODS: HepG2 cells were exposed to hypoxia (1% O₂). Lentivirus-mediated GP73 overexpression (LV-GP73) and siRNA-mediated HIF-1α knockdown (si-HIF-1α) were established. Four groups were compared: H + LV-NC (negative control), H + LV-GP73, H + LV-GP73 +

Sesquiterpenoid Dimers from the Roots of Two Species of the Genus Inula with Cytotoxic Activity on Hepatocellular Carcinoma Cells.

Guided by the feature-based molecular networking approach, twenty-five previously undescribed sesquiterpenoid dimers (SDs) designated Inulaenoids A-Y (1-4, 8-25, and 27-29) along with four known analogues were isolated from the roots of Inula helenium L. and Inula racemosa Hook. f. Their structures and absolute configuration were unambiguously elucidated by spectroscopic data, X-ray diffraction, and theoretical calculations. Structurally, compounds 1-19 are rare SDs featuring two eudesmane units directly constructed through C-C bonds, including C-13-C-13', C-13-C-11', C-11-C-13', and through a

A DNAzyme-Driven DNA Nanotracker for Orthogonal Dual-Channel Imaging of an Oncogenic Signaling Axis and Its Pharmacological Modulation.

Pathway-targeted therapy has emerged as a promising strategy in cancer treatment, yet its efficacy relies on the accurate analysis of key signaling axes in living cells. The miR-140/FEN1 signaling axis, which is balanced in normal hepatocytes but dysregulated in hepatocellular carcinoma, represents a potential target for cancer therapy. Here, we present a DNAzyme-driven DNA nanotracker (DDN) for the orthogonal imaging of this pathway in dual channels with high sensitivity. The nanotracker is constructed by assembling DNAzyme-powered DNA walkers, which are responsive to miR-140 and FEN1, onto g

A hospital-based micro-elimination strategy for hepatitis C in western China: a retrospective before-after study.

OBJECTIVE: Hepatitis C virus (HCV) infection remains a major public health burden in China, with low rates of diagnosis and treatment. Hospital-based "micro-elimination" strategies have been proposed to identify and link high-risk patients to care, but evidence from real-world implementations, particularly in western China, is limited. This study aimed to evaluate changes in HCV screening, diagnosis, and follow-up rates following the implementation of a hospital-based micro-elimination strategy at a tertiary hospital in western China. METHODS: This retrospective, uncontrolled before-after stu

Changes in the apoptotic index, along with gene and protein expression levels, in splenomegaly due to portal vein stenosis in rats.

BACKGROUND: The spleen is an important peripheral immune organ, and splenomegaly (SM) is a common complication of portal hypertension. However, the regulatory mechanisms underlying splenic apoptosis remain poorly understood. MATERIALS AND METHODS: A portal vein stenosis-induced SM model was established in Sprague-Dawley rats, using a sham-operated (SO) group as the control. Apoptotic changes in splenic tissue cells were evaluated using the terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL) assay, immunohistochemistry staining, quantitative real-time polymerase chain reaction

Clinicopathologic characteristics and diagnostic value of PD-L1 in immune checkpoint inhibitor-related liver injury: a single-center retrospective study.

BACKGROUND AND AIMS: Immune-mediated liver injury caused by checkpoint inhibitors (ILICI) differs from chemotherapy- or targeted drug-related hepatotoxicity in pathogenesis and management. Biomarkers to distinguish ILICI from other anticancer agent-induced liver injuries are lacking. We compared the clinicopathological features, treatments, and outcomes of ILICI versus other anticancer agent-related liver injuries to identify distinguishing biomarkers. METHODS: We retrospectively included patients with liver injury during anticancer therapy at Fujian Cancer Hospital between June 2022 and Nove

State-Dependent Decoupling of a CREM-Centric cAMP-Responsive Regulon and Survival in Hepatocellular Carcinoma.

BACKGROUND: The cAMP-responsive element modulator (CREM) has emerged as a cAMP-responsive transcriptional node associated with dysfunctional lymphocyte states in chronic hepatic infection and some immunosuppressive tumor microenvironments, but its regulatory output and isoform usage by specific cell-types remain insufficiently defined in hepatocellular carcinoma (HCC). Because CREM produces activating isoforms and repressive inducible cAMP early repressor (ICER) isoforms through alternative promoter usage, bulk or single-cell CREM mRNA abundance may not faithfully describe the net direction of

AASLD AGA clinical Practice Guideline on management of primary biliary cholangitis.

BACKGROUND AIMS: Primary biliary cholangitis (PBC) is a chronic cholestatic liver disease associated with progressive fibrosis, cirrhosis, liver transplantation, and premature mortality. Despite effective therapies, important challenges remain in the diagnosis and management of PBC, including identification of antimitochondrial antibody-negative disease, assessment of response to ursodeoxycholic acid (UDCA), risk stratification, treatment of patients with inadequate response, management of cholestatic pruritus, and prevention of recurrent disease after liver transplantation. The aim of this gu

Scalable manufacturing of human iPSC-derived functional liver organoids for acute liver failure treatment.

RATIONALE: Acute liver failure (ALF) is a life-threatening disease associated with high mortality rates. Human induced pluripotent stem cell (hiPSC)-derived liver organoids (LOs) hold considerable promise as cell-based bridging therapies for ALF. However, the scalable production of highly consistent and mature LOs remains a major barrier to their clinical translation. In this study, we aimed to establish a scalable manufacturing platform for the reproducible generation of highly uniform and functionally mature liver organoids. METHODS: We generated LOs from hiPSCs and established a differenti

Work-life chaos: Enhancing mental health during times of strife.

The practice of medicine can be both be professionally fulfilling and inherently demanding. Physicians are often perceived as invincible, yet recurrent exposure to patient suffering, high-stakes decision-making, and institutional pressure can profoundly impact mental health. These challenges may be particularly pronounced in organ transplantation, where transplant teams navigate life-and-death decisions, complex longitudinal care, and the responsibility of stewarding scarce organs. Using narrative reflection, this article highlights the dialectic that is inherent in the culture of medicine but

Prognostic value of microRNA expression in pancreatic ductal adenocarcinoma: a systematic review and meta-analysis.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) ranks among the deadliest malignancies, with limited survival benefits despite recent therapeutic advances. Few prognostic biomarkers are available for PDAC monitoring in clinical practice. We conducted a meta-analysis to evaluate and synthesise evidence on the prognostic significance of miRNA expression in PDAC. METHODS: The protocol was registered with PROSPERO (CRD42023463542). We conducted a systematic search in PubMed, CENTRAL, and Embase on January 27, 2026, to identify studies assessing miRNA expression levels in patients with PDAC wi

Discovery of Novel Schisandrol B Simplification Derivatives as PXR Agonists to Promote Liver Regeneration.

Partial hepatectomy (PHx) and liver transplantation are the most effective interventions for end-stage liver diseases, yet druggable targets that promote donor or remnant liver regeneration are critically lacking. Pregnane X receptor (PXR), a key regulator of liver regeneration, is a promising therapeutic target. Previously, we identified the natural product Schisandrol B (SolB) as a PXR agonist to promote liver regeneration. To further improve its PXR activity and selectivity, the present study performed structural simplification of SolB and identified BZZ-034, a ring-opened analogue with an

In-Capillary Integrated CE-LIF Platform with Bioinformatics-Guided Exosome Capture and Isothermal Amplification for Ultrasensitive Multiplexed Detection of HCC-Derived Exosomal MicroRNAs.

Exosomal microRNAs (Exo-miRNAs) are promising liquid-biopsy biomarkers for noninvasive cancer detection; yet due to their short length, low abundance, and high sequence homology, the determination of Exo-miRNAs suffers from insufficient sensitivity, limited multiplexing, and operational complexity, impeding their high-throughput clinical applications. Here, we introduce an in-capillary integrated inline analytical platform specifically tailored for hepatocellular carcinoma (HCC)-derived Exo-miRNAs. The platform combines four functions within a single capillary: (i) tumor-specific exosome captu
