Pelioid-type well-differentiated hepatocellular carcinoma in a patient with a history of taking oral contraceptives: report of a case.

We report a case of pelioid-type well-differentiated hepatocellular carcinoma (HCC) in a 54-year-old woman with a history of taking oral contraceptives. She was not infected with hepatitis viruses and her liver function test results were normal. Contrast-enhanced computed tomography showed an irregular-shaped and remarkably enhanced tumor with central necrosis. The tumor was vaguely nodular without capsules, and consisted of multiple pelioid cysts and sinusoidal dilatations with transitional forms between them. The pelioid cysts were directly surrounded by neoplastic cells with fatty degenerat

Takayasu arteritis associated with hepatic sinusoidal dilatation.

Hepatic sinusoidal dilatation (HSD) is pathological entity that is characterized by peliosis hepatis (PH) like lesions, with vascular lesions that consist of multiple cyst-like, blood-filled cavities within the liver. To the best of our knowledge, neither PH nor HSD have been associated with systemic vasculitis. We describe herein two cases of idiopathic HSD associated with Takayasu arteritis (TA), diagnosed at an early stage of vasculitis. The same endothelial target in HSD and TA, and the favorable outcome of HSD with treatment of TA, suggest a pathogenic link between the two diseases.

Gadolinium chloride pretreatment ameliorates acute cadmium-induced hepatotoxicity.

Cadmium is a known industrial and environmental pollutant. It causes hepatotoxicity upon acute administration. Features of cadmium-induced acute hepatoxicity encompass necrosis, apoptosis, peliosis and inflammatory infiltration. Gadolinium chloride (GdCl3) may prevent cadmium-induced hepatotoxicity by suppressing Kupffer cells. The effect of GdCl3 pretreatment on a model of acute cadmium-induced liver injury was investigated. Male Wistar rats 4-5 months old were injected intraperitoneally with normal saline followed by cadmium chloride (CdCl2; 6.5 mg/kg) or GdCl3 (10 mg/kg) followed by CdCl2 (

Liver hemorrhage due to idiopathic peliosis hepatis successfully treated with hepatic artery embolization.

Peliosis hepatis is an extremely rare condition that may cause fatal hepatic hemorrhage and liver failure. We report a case of liver hemorrhage due to idiopathic peliosis hepatis. A 60-year-old woman was admitted to our hospital with slight right hypochondriac pain. She went into hemorrhagic shock, and computed tomography (CT) showed multiple low-density areas in the right liver with massive subcapsular blood collection. Selective transfemoral arteriography of the celiac artery revealed no signs of vascular malformation or tumor stain, but showed signs of pooling in the right posterior segment

Adhesion and host cell modulation: critical pathogenicity determinants of Bartonella henselae.

Bartonella henselae, the agent of cat scratch disease and the vasculoproliferative disorders bacillary angiomatosis and peliosis hepatis, contains to date two groups of described pathogenicity factors: adhesins and type IV secretion systems. Bartonella adhesin A (BadA), the Trw system and possibly filamentous hemagglutinin act as promiscous or specific adhesins, whereas the virulence locus (Vir)B/VirD4 type IV secretion system modulates a variety of host cell functions. BadA mediates bacterial adherence to endothelial cells and extracellular matrix proteins and triggers the induction of angiog

Adhesins of Bartonella spp.

Adhesion to host cells represents the first step in the infection process and one of the decisive features in the pathogenicity of Bartonella spp. B. henselae and B. quintana are considered to be the most important human pathogenic species, responsible for cat scratch disease, bacillary angiomatosis, trench fever and other diseases. The ability to cause vasculoproliferative disorders and intraerythrocytic bacteraemia are unique features of the genus Bartonella. Consequently, the interaction with endothelial cells and erythrocytes is a focus in Bartonella research. The genus harbours a variety

Congenital portal vein aneurysm associated with peliosis hepatis and intestinal lymphangiectasia.

Portal vein aneurisym (PVA), peliosis hepatis (PH) and intestinal lymphangiectasia (IL) all are very uncommon entities. Herein, we presented a unique patient with these three rare entities who was admitted to our hospital because of portal hypertensive ascites rich in protein and lymphocyte. PVA was extrahepatic and associated with coronary vein aneurysm. Peliosis hepatis was of microscopic form. Lymphangiectasia was present in peritoneum and small intestine. Diagnoses of these rare entities were made by imaging techniques and histopathological findings. Patient also had hydronephrosis caused

Clinicopathological study of hepatocellular carcinoma with peliotic change.

Peliosis hepatis-like blood-filled cavities are frequently observed in the tumors of hepatocellular carcinoma (HCC). This finding is generally referred to as 'peliotic change' in HCC. However, the clinicopathological features of HCC with peliotic change (PHCC) are not fully understood. These issues are addressed in the present study. Among 294 consecutively surgically resected HCCs, the clinicopathological features of PHCC were compared with those of a common type of HCC (control). PHCC was observed in 116 (39.5%) of 294 HCCs. The mean tumor diameter of 3.4±0.9 cm of the PHCC group was signifi

Multifocal peliosis hepatis: MR and diffusion-weighted MR-imaging findings of an atypical case.

Peliosis is a rare benign disorder that is characterized by the presence of diffuse blood-filled cystic spaces and can occur in the liver, spleen, bone-marrow, and lungs. We present a 10-year-old boy with Fanconi anemia who presented with peliosis hepatis due to androgen treatment. Magnetic resonance (MR) imaging revealed multiple non-enhancing masses. Some of the lesions revealed fluid-fluid levels and extrahepatic extension on MR images. Diffusion-weighted (DW) imaging showed restricted diffusion. Fluid-fluid levels and extrahepatic extensions are unusual findings for hepatic peliotic lesion

Multiple distant metastases of hepatocellular carcinoma to the oral cavity.

We describe the palliative therapy for multiple metachronous oral metastases of hepatocellular carcinoma in a 72-year-old patient with emphasis on maintaining oral functions by local debulking procedures in due consideration of the dissemination of the neoplasm to multiple organs. The first oral metastasis was the first evidence of multiple tumor spread after partial hepatectomy. Palliative chemotherapy with sorafenib was cancelled after three weeks due to tumor progression and hemiplegia following brain metastasis and stroke. Periodical screening of the patient for oral findings allowed the n

Hepatitis B genotype C correlated with poor surgical outcomes for hepatocellular carcinoma.

BACKGROUND: Genotype B and C are the predominant hepatitis B virus (HBV) strains in Taiwan. We aimed to investigate the role of genotype in HBV-related hepatocellular carcinoma (HCC) after resection. STUDY DESIGN: From October 2005 to November 2008, 64 patients who underwent liver resection for HBV-related HCC were enrolled. HBV genotypes were determined by molecular method. Patient characteristics, biochemical, tumor, and viral factors were evaluated for their prognostic significance. RESULTS: During a mean follow-up of 26.6 ± 13.2 months, patients infected with genotype C had higher HBV vi

Vascular disorders of the liver: recommendations from the Italian Association for the Study of the Liver (AISF) ad hoc committee.

This review summarizes the document elaborated by the Italian Association for the Study of the Liver (AISF) ad hoc committee "Vascular disorders of the liver" on the primary circulatory liver diseases, which include Budd-Chiari syndrome, obstruction of the hepatic portion of the inferior vena cava, portal vein thrombosis, sinusoidal obstruction syndrome (veno-occlusive disease) and hereditary hemorrhagic telangiectasia. A characteristic of the primary circulatory liver diseases is that portal hypertension usually precedes liver dysfunction. Significant overlap exists amongst the diseases and r

[Peliosis hepatis: report of one case].

Peliosis hepatis is a rare vascular condition of the liver characterized by the presence of cystic blood-fi lled cavities distributed randomly throughout the liver parenchyma. We report a 46 year old female, with dyspepsia. An abdominal Doppler ultrasound showed a hyper vascularized solid liver nodule of 5 cm diameter. Magnetic resonance imaging and CT scan showed the same hyper vascularized nodule. With the possible diagnoses of primary hepatocellular carcinoma or focal nodular hyperplasia, the patient was subjected to an excision of hepatic segment VI, where the nodule was located. The patho

Assessment of non-cirrhotic portal hypertension associated with thiopurine therapy in inflammatory bowel disease.

Thiopurines represent an effective and widely used immunosuppressant in the therapeutic armamentarium of inflammatory bowel disease. However up to 25% of patients may be unable to continue the drug due to side effects. The incidence of hepatotoxicity associated with thiopurine use is reported between 0% and 32%. Veno-occlusive disease, peliosis hepatis, perisinusoidal fibrosis and nodular regenerative hyperplasia have all been described with thiopurines. Recent trials of 6-tioguanine, although successful in patients with allergies to azathioprine or mercaptopurine, have been compromised by inc

Peliosis hepatis in cats is not associated with Bartonella henselae infections.

Peliosis hepatis is a vasculoproliferative disorder of the liver with infectious and noninfectious causes. In humans and dogs, Bartonella henselae has been linked to peliosis hepatis. Although domestic cats are the natural reservoir of B. henselae and although peliosis hepatis is common in this species, an association between this condition and infection with B. henselae has never been investigated in cats. In this study, 26 cases of peliosis hepatis in cats were tested for B. henselae infection by nested polymerase chain reaction and immunohistochemistry. The authors failed to detect B. hense
