Hepatitis B core-related antigen as a multifaceted biomarker in chronic hepatitis B: implications for immune activity and hepatocellular carcinoma prediction.

BACKGROUND/AIMS: Hepatitis B core-related antigen (HBcrAg) reflects both serum hepatitis B virus (HBV) DNA and intrahepatic covalently closed circular DNA activity, offering potential advantages over conventional biomarkers in monitoring chronic hepatitis B (CHB). This study evaluated the clinical utility of HBcrAg across immune phases and its prognostic value for hepatocellular carcinoma (HCC). METHODS: In this retrospective study, 281 CHB patients from Daejeon St. Mary's Hospital (September 2022-July 2024) were classified into HBV phases. HBcrAg was quantified using the ultrasensitive iTACT

Drug resistance mutations of hepatitis B virus (HBV) influence the yield of the virions of the human hepatitis delta virus (HDV) and their infectivity.

UNLABELLED: This work analyzed 30 drug resistance mutations (DRMs) in the surface antigen of hepatitis B virus (HBV) (i.e., HBsAg) selected by several FDA-approved anti-HBV nucleos(t)ide analogs. All DRM-bearing HBsAg variants supported the formation and secretion of hepatitis delta virus (HDV), and only 13/30 DRMs considerably decreased the yield of secreted HDV virions. However, the majority of DRMs (23/30) greatly reduced HDV infectivity, and four of them even caused the complete loss of infectivity, which was determined by analyzing the levels of HDV RNA genomes in the infected cells, and

Clinical insights into the association between chronic liver disease and osteoporosis.

Osteoporosis is a common complication among children with chronic liver disease (CLD) and cholestasis. This condition is multifactorial, as many different liver dysfunction-related mechanisms contribute to bone loss. The overall prevalence of osteoporosis in patients with advanced CLD ranges from 12% to more than 55%. Patients with CLD have a 2- to 4-fold higher risk of hip, vertebral, and nonvertebral fractures than do matched healthy individuals. Vertebral fractures are particularly common, often occurring in 20% to 30% of cirrhotic patients, even in those whose bone mineral density scores d

HBV pgRNA induces chronic inflammation in an IL-1β-dependent manner.

INTRODUCTION: Chronic hepatitis B (CHB) is a widespread form of hepatitis B infection with advanced complications if unsupervised and untreated. It was previously reported that pregenomic RNA (pgRNA) can be detected in blood circulation as an indirect marker of HBV transcriptional activity. The aim of this study was to investigate the transcriptomic profile of pgRNA-positive patients with CHB in comparison to pgRNA-negative patients and to evaluate its role in innate immunity. MATERIALS AND METHODS: A total of 88 patients with CHB who were receiving nucleoside analogs (NAs) were enrolled in t

Recombinant polyclonal antibody GIGA-2339 potently and pan-genotypically neutralizes hepatitis B virus.

Existing hepatitis B virus (HBV) treatments control viral replication in chronic hepatitis B (CHB) patients but fail to prevent hepatitis B surface antigen (HBsAg) production; excessive HBsAg production leads to HBV-specific immune tolerance. GIGA-2339 is a first-in-class, recombinant polyclonal antibody (pAb) therapeutic consisting of over 1000 human IgG1/IgK antibodies targeting HBsAg epitopes developed for the treatment of chronic HBV infection. As Hepatitis D virus (HDV) utilizes HBsAg produced by HBV for viral entry, GIGA-2339 may also provide benefit to HDV/HBV co-infected patients. Memo

Droplet Digital PCR Measurement of HBV DNA in On-Treatment Chronic HBV Patients: Association With HCC Development and Outcomes.

Low-level residual viremia can be present in chronic hepatitis B (CHB) patients on nucleos(t)ide analogues (NUCs). Due to the detection limits of current HBV DNA assays, the association between residual viremia and HBV-related HCC has not been well studied. This study included NUC-treated HBV-related HCC patients with unquantifiable serum HBV DNA by the conventional Cobas-Taqman assay (< 20 IU/mL) at HCC diagnosis. The HCC patients were matched in a 1:1 ratio with NUC-treated non-HCC controls. Serum samples at the time of HCC diagnosis, 1 year before, and 2 years before diagnosis were retrospe

The clinical significance and optimization strategies of HBeAg seroclearance in chronic hepatitis B treatment.

HBeAg seroclearance represents a critical milestone in the treatment of HBeAg-positive chronic hepatitis B (CHB), which is closely associated with disease remission, functional cure, and reduced hepatocellular carcinoma risk. As HBeAg is a secreted viral protein associated with active HBV replication, infectivity, and host immune tolerance, its disappearance usually indicates a transition toward improved immune control rather than simple loss of a laboratory marker. This review comprehensively explores the immunological mechanisms underlying HBeAg seroclearance, thoroughly examines its impact

AURKA regulates HBV-induced hepatic stellate cell inflammation and liver fibrosis by modulating K48-linked ubiquitination and proteasomal degradation of TLR3.

Chronic hepatitis B (CHB), caused by persistent hepatitis B virus (HBV) infection, continues to pose a major global health threat. Aurora kinase A (AURKA) has been implicated in fibrosis, yet its role in HBV-induced liver fibrosis remains unclear. This study investigated the function and mechanism of AURKA in hepatic stellate cell (HSC) activation and fibrogenesis during CHB. Bioinformatics screening of GSE83148 dataset and validation in human CHB liver specimens, LX-2 HSCs treated with conditioned medium (CM) from HBV-replicating cells, and a persistent HBV recombinant cccDNA (rcccDNA) mouse

Programmable Nanobody-Targeting Chimeras Enable Intracellular Viral Protein Degradation.

Chronic hepatitis B virus (HBV) infection remains a major global health challenge, largely because current antiviral therapies seldom achieve sustained clearance of the viral surface antigen (HBsAg), a central driver of immune tolerance and viral persistence. Here, we present a programmable, nanobody-based targeted protein degradation platform, termed Nanobody-Targeting Chimera (Nab-TAC), designed for the intracellular clearance of HBV antigens. To systematically identify functional degradation modules, we developed a split-luciferase complementation-based quantitative screening platform that

Hepatocellular carcinoma development after hepatitis B surface antigen seroclearance in chronic hepatitis B patients: a 10-year single-center cohort study.

BACKGROUND: Hepatitis B surface antigen (HBsAg) seroclearance is considered a functional cure in chronic hepatitis B (CHB), yet the risk of hepatocellular carcinoma (HCC) may persist. This study aimed to evaluate HCC incidence and identify risk factors after HBsAg seroclearance. METHODS: This retrospective single-center cohort included 259 CHB patients who achieved HBsAg seroclearance between 2013 and 2023. Patients with pre-existing HCC were excluded. During follow-up, 22 patients developed HCC. Clinical, demographic, and laboratory variables were analyzed. Independent risk factors were dete

Characteristics and Outcomes of Patients With Imaging-Defined MASLD and Histological Steatosis Grade S0.

INTRODUCTION: Metabolic dysfunction-associated steatotic liver disease (MASLD) is a prevalent chronic liver disease, and liver biopsy remains the gold standard for diagnosis. However, a subset of patients diagnosed with MASLD by imaging paradoxically exhibit steatosis grade S0 on liver biopsy, posing a diagnostic challenge. This study investigated the outcomes and causes of steatosis 0. METHODS: Steatosis 0 was defined as imaging-detected MASLD with a biopsy-defined steatosis grade S0. We used global cohorts to evaluate outcomes, histologic progression by paired biopsies, and possible causes.

Association of IL-10 (rs1800896) and TLR3 (rs1879026) polymorphism with progression of hcv-related liver disease and hepatocellular carcinoma in Egyptian patients.

BACKGROUND: Hepatitis C virus (HCV) is a significant global health concern, particularly in Egypt, where its prevalence is estimated at 4.5%-6.7%. The Egyptian genotype, HCV-4, accounts for most infections and is a leading cause of cirrhosis, chronic liver disease, and hepatocellular carcinoma (HCC). A previous study was conducted in Egypt to examine certain single-nucleotide polymorphisms (SNPs) in both toll-like receptor 3 (TLR3) and interleukin-10 (IL-10) and their association to interferon therapy. However, there is still a remaining gap to examine the correlation of both TLR3 and IL-10 ge

Chronic HBV Infection Disrupts CCL5-Secreting cNK Cells and Attenuates Liver Accumulation and Activation of DCs and HBV-Specific T Cells.

Understanding the immunological mechanisms underlying chronic hepatitis B virus (HBV) infection-induced immune tolerance is critical for developing effective therapeutic strategies, yet the mechanisms by which natural killer (NK)-cell subsets regulate dendritic cell (DC)-mediated antiviral immunity during chronic HBV infection remain unclear. Here, we demonstrated that chronic HBV infection disrupted intrahepatic DCs accumulation and activation by suppressing CC chemokine ligand 5 (CCL5) production from hepatic NK cells. Importantly, chronic HBV infection reduced the number of hepatic conventi

Short-chain Fatty Acids and Bile Acids Signaling in Chronic Hepatitis B.

Chronic hepatitis B (CHB) remains a major global public health challenge. Current therapies based on nucleos(t)ide analogues and interferon mainly achieve long-term viral suppression, whereas only a small proportion of patients attain a functional cure, defined as sustained hepatitis B surface antigen loss with hepatitis B virus (HBV) DNA below the limit of quantification for at least 24 weeks after treatment discontinuation, with or without anti-HBs seroconversion. Emerging evidence from the gut-liver axis indicates that gut microbiota-derived metabolites, particularly short-chain fatty acids

LIMITED DIAGNOSTIC ACCURACY OF FIB-4 FOR DETECTING SIGNIFICANT LIVER FIBROSIS IN CHRONIC HEPATITIS B: A CROSS-SECTIONAL STUDY.

BACKGROUND: Liver disease linked to the hepatitis B virus (HBV) is a serious public threat worldwide and in Egypt. The incidence of HBV infection in Egypt is about 1.4%. Accurate liver fibrosis assessments are crucial in the evaluation and follow-up of HBV patients, but traditional biopsies have limitations, prompting the use of non-invasive methods like the vibration controlled transient elastography (VCTE), which may not be available in limited-resource treatment centers. OBJECTIVE: This study aimed to assess the diagnostic accuracy of Fibrosis-4 (FIB-4) and Aminotransferase to Platelet Rat

Factors influencing hepatitis C treatment initiation in a high-prevalence Brazilian state.

Hepatitis C virus infection remains a leading cause of chronic liver disease worldwide, affecting more than 70 million people, many of whom remain undiagnosed or untreated. In Brazil, Rio Grande do Sul State has the highest hepatitis C detection rate, underscoring the need for coordinated strategies to eliminate it by 2030. Underdiagnosis and barriers to treatment initiation persist despite the availability of highly effective antiviral therapies. This study aimed to estimate the proportion of diagnosed individuals who initiated treatment and to find factors associated with non-initiation of t
