A prospective cohort study of dietary intake and clinical-nutritional biomarkers in patients with periampullary malignancies undergoing pancreatoduodenectomy in Southeast Brazil.

PURPOSE: Patients undergoing pancreatoduodenectomy for periampullary malignancies are at high nutritional risk. The aim was to characterize longitudinal changes in reported dietary intake, clinical-nutritional biomarkers, and inflammatory-nutritional indices after surgery and contextualize them through cross-sectional comparisons with age- and sex-similar controls. METHODS: This prospective longitudinal cohort included 24 patients and 20 contextual controls. Patients were assessed preoperatively and at 3 and 6 months after discharge; controls were assessed once. Dietary intake was estimated u

Assessment of serum micronutrient concentration in chronic pancreatitis: insights from the nationwide COMBO randomised trial.

INTRODUCTION: In patients with chronic pancreatitis (CP), pancreatic exocrine insufficiency (PEI) and poor dietary intake may cause malnutrition. International guidelines recommend monitoring key micronutrients (MN), however clinical implications remain uncertain. This study aims to evaluate the prevalence, risk factors, complications, and longitudinal course of low serum MNs in patients with CP. METHODS: This is a predefined analysis of the COMBO trial, a nationwide stepped-wedge cluster randomised trial on evidence-based CP management. Nutritional biochemical assessment included vitamins A,

OLFM4 promotes intrahepatic cholangiocarcinoma progression through a PKM2-glycolysis-histone lactylation feedback loop.

Metabolic reprogramming is increasingly recognized as a key characteristic of intrahepatic cholangiocarcinoma (ICC) initiation and progression. Among these metabolic alterations, glycolysis is central for supporting tumor growth. Elucidating the precise molecular mechanisms of glycolysis in ICC is critical for developing precision therapies. Here, comprehensive integrative analyses revealed that olfactomedin 4 (OLFM4) is upregulated in ICC specimens and is associated with adverse patient outcomes. Mechanistically, OLFM4 promotes ICC progression by recruiting ubiquitin specific peptidase 10 (US

Metabolic Barriers to Cancer Immunotherapy: Multiscale Mechanisms from Organelle Dysfunction to Organ-Specific Constraints.

Immune checkpoint blockade (ICB) has reshaped cancer therapy, yet primary resistance and acquired relapse remain common despite therapeutic targeting of inhibitory receptor pathways. This limitation suggests that checkpoint release may remain insufficient when antitumor lymphocytes lack the metabolic, organelle-level, spatial, or systemic conditions required for tissue entry, functional recovery, and durable effector activity. In this review, we organize metabolic barriers to ICB into a multiscale framework comprising four interconnected axes. The Local Resource Axis captures how nutrient comp

Surgery as Metabolic Therapy: Expanding the Role of Gastrointestinal Surgery Beyond Anatomical Reconstruction.

Gastrointestinal surgery comprises procedures used to treat diseases and injuries of the digestive tract. Evidence now shows that some gastrointestinal procedures are also systemic metabolic interventions. Bariatric and metabolic surgery intentionally treats obesity and associated metabolic disease and can improve type 2 diabetes, metabolic dysfunction-associated steatotic liver disease, and cardiometabolic risk, sometimes before substantial weight loss occurs. Other procedures, including hepatectomy, pancreatic resection, and bowel resection, are generally performed for non-metabolic indicati

Ginsenoside Re attenuates ferroptosis induced by hepatic ischemia-reperfusion injury via the BAP1/SLC7A11 axis.

Hepatic ischemia-reperfusion injury (HIRI) is a complex clinical complication involving inflammation and multiple forms of regulated cell death, among which ferroptosis plays a critical role. Ginsenoside Re (Re) exhibits prominent antioxidant capabilities, but its exact molecular targets in HIRI are unidentified. Here, using in vivo HIRI and in vitro hypoxia/reoxygenation (H/R) models, we found that Re administration exhibited an optimal therapeutic window, significantly alleviating hepatocellular damage, inflammatory responses, and oxidative stress. Sequentially, Re suppressed ferroptosis, as

Protective Effects of PD-L1-Overexpressing MSCs and Their Derived Extracellular Vesicles in Type 1 Diabetes.

This study was conducted to address the urgent need for novel therapies for type 1 diabetes. We investigated whether programmed death-ligand 1 (PD-L1)-overexpressing mesenchymal stromal/stem cells (PD-L1-MSCs) and their extracellular vesicles (PEVs) can modulate immune responses and protect pancreatic islets, and whether these effects depend on PD-L1 signaling. Our results demonstrate that both PD-L1-MSCs and PEVs suppress T-cell activity, enhance regulatory T-cell populations, delay the onset of diabetes, and reduce immune cell infiltration in pancreatic islets. These findings suggest that PD

Multivariate GWAS Analysis Reveals a Common Genetic Factor Linking Dry Eye Disease to Gastrointestinal Comorbidities.

PURPOSE: Dry eye disease (DED) frequently overlaps clinically with irritable bowel syndrome, gastroesophageal reflux, and peptic ulcer, yet their shared genetic architecture and pathogenic pathways remain elusive. METHODS: Univariate genome-wide association studies (GWAS) were conducted for DED and four gastrointestinal diseases. Genomic structural equation modeling defined a shared genetic factor mvDED-GI. FUMA annotated peak variants for subsequent gene-set, tissue, and cell enrichment. Transcriptome-wide association study (TWAS) plus fine-mapping prioritized causal genes, and multi-layer e

Benefits and risks of induction immunosuppression in pediatric autoimmune liver disease from SPLIT Registry.

The optimal induction regimen for pediatric liver transplantation (LT) in patients with Autoimmune Liver Disease (AILD) is not well-defined. This study compared outcomes between corticosteroids only, anti-thymocyte globulin (ATG), and IL-2 receptor antagonists (IL2RA; Basiliximab/Daclizumab). A retrospective review of 445 pediatric LT recipients with AILD (Autoimmune hepatitis, primary sclerosing cholangitis, or autoimmune sclerosing cholangitis) was conducted using the SPLIT database. Re-transplants were excluded. Primary outcomes included biopsy-confirmed rejection, culture-proven infection,

Evaluation of Pirfenidone for Recurrent Acute Pancreatitis: Protocol for a Randomized Trial.

INTRODUCTION: Recurrent acute pancreatitis (RAP) is defined as a syndrome of two or more episodes of acute pancreatitis (AP), with a period of resolution in between. Management of RAP is largely supportive, with no available drug therapy to prevent recurrence or progression to chronic pancreatitis (CP). Our preclinical data suggest that pirfenidone can improve parameters of RAP and decrease progression to CP. Therefore, we decided to conduct a pilot trial to evaluate pirfenidone in patients with RAP. METHODS: We have designed a double-blinded, multicenter, randomized, pilot clinical trial inv

Antimitochondrial antibodies in rheumatic diseases: beyond primary biliary cholangitis.

Antimitochondrial antibodies (AMAs) targeting the E2 subunit of the pyruvate dehydrogenase complex are the serological hallmark of primary biliary cholangitis (PBC), but may also occur across systemic autoimmune rheumatic diseases. This narrative review integrates clinical and experimental evidence on AMAs in rheumatic diseases, delineating disease-specific patterns and clinical implications beyond the hepatobiliary context. AMAs and other mitochondrial autoantibodies have been reported in systemic sclerosis, systemic lupus erythematosus, rheumatoid arthritis, Sjögren disease, and inflammatory

Development and Internal Validation of a Preoperative Risk Stratification Model for Difficult Laparoscopic Cholecystectomy in Acute Cholecystitis.

Background: Difficult laparoscopic cholecystectomy (DLC) in patients with acute cholecystitis (AC) is associated with greater operative complexity and increased perioperative morbidity, yet reliable preoperative risk stratification remains limited. This study aimed to develop and internally validate a preoperative prediction model for DLC and to compare its predictive performance with that of an alternative C-reactive protein-albumin-lymphocyte (CALLY)-based model. Materials and Methods: We retrospectively analyzed consecutive adult patients who underwent laparoscopic cholecystectomy (LC) for

Falciform Ligament Inflammation Complicated by Umbilical Vein Thrombophlebitis, Left Portal Vein Thrombosis, and Multiple Hepatic Abscesses: A Case Report.

Falciform ligament pathology is a rare cause of acute upper abdominal pain and may resemble more common pancreatic and hepatobiliary disorders. A woman in her 50s presented with sudden severe epigastric pain radiating to her back and marked hyperamylasaemia of 1,747 U/L. This combination fulfilled clinical and biochemical diagnostic criteria for acute pancreatitis, although computed tomography (CT) demonstrated no pancreatic inflammation and instead showed severe inflammatory change centred on the falciform ligament, with associated focal fat infarction, left portal vein thrombosis, and gas lo

Acute Gangrenous Cholecystitis During Pregnancy: A Case Report.

Acute cholecystitis is one of the most common nonobstetric surgical emergencies during pregnancy. Progression to gangrenous cholecystitis is rare but is associated with significant maternal and fetal morbidity. We report the case of a 26-year-old primigravida at 25 weeks' gestation who presented with severe right upper quadrant pain, fever, and vomiting. Laboratory evaluation revealed elevated serum inflammatory markers. Transabdominal ultrasonography demonstrated gallbladder wall thickening, pericholecystic fluid, and biliary sludge. Despite 24 h of conservative management, her clinical condi

Gallbladder Volvulus Mimicking Immune Checkpoint Inhibitor-Related Acalculous Cholecystitis: A Case Report.

Gallbladder volvulus (GV) is a rare surgical emergency whose clinical and radiological presentation closely mimics acute cholecystitis. This diagnostic difficulty is amplified in patients receiving immune checkpoint inhibitors (ICIs), as gallbladder inflammation may initially be misattributed to an immune-related adverse event (irAE). We report the case of an 82-year-old woman with microsatellite instability-high (MSI-H) duodenal adenocarcinoma treated with pembrolizumab. Following a temporary discontinuation of immunotherapy and initiation of corticosteroids for a suspected gastrointestinal i

Exploiting Acquired Drug Resistance in Pancreatic Cancer through Selective Covalent Targeting of MAP2K4/7.

Pancreatic ductal adenocarcinoma (PDAC) is highly lethal and remains largely refractory to targeted therapy despite its near-universal dependence on oncogenic KRAS. Single-agent KRAS inhibitors often fail due to adaptive resistance mechanisms that emerge under pharmacologic pressure. Previous studies combining MAP2K4 inhibition with KRASG12C-targeted therapy determined that partial JNK suppression can enhance anticancer responses. Here, we extend this concept to PDAC, which is predominantly driven by KRASG12D mutations, and demonstrate that full JNK pathway inactivation restores sensitivity to

A Dual-Action CXCR4-Targeted Peptide-Drug Conjugate Engineered to Enhance Antitumor Efficacy and Mitigate Myelosuppression in Pancreatic Cancer.

Gemcitabine (GEM) is a first-line therapeutic option for pancreatic cancer; however, it has low efficacy due to rapidly developed drug resistance and severe dose-limiting myelosuppression. To enhance its therapeutic effect, this study developed a novel peptide-drug conjugate using a CXCR4 antagonistic peptide as the targeting head and gemcitabine as the drug payload, based on the characteristics of pancreatic tumor cells highly expressing CXCR4 that mediates immunosuppression and tumor progression through interaction with its specific ligand CXCL12. The therapeutic effect of the conjugate (P12

Amphicrine Carcinomas of the Pancreas Showing Neuroendocrine and Acinar Cell Differentiation: A Case Report and Literature Review.

This study clarifies the distinct clinicopathological and molecular features of amphicrine pancreatic carcinoma (AmpCA) through a case analysis and literature review. We present a case of AmpCA in a 73-year-old female, characterized by co-expression of acinar cell markers and neuroendocrine markers within individual tumor cells. Molecular profiling revealed high-risk mutations in KRAS G12V, TP53, and RB1, along with a high Ki-67 index (85%). Literature comparison confirms that AmpCA exhibits a unique bidirectional, single-cell differentiation pattern, fundamentally distinguishing it from the b

Targeting indocyanine green where it matters most: a CRP-stratified cohort study in acute cholecystitis.

BACKGROUND: Intraoperative indocyanine green (ICG) fluorescence cholangiography has been linked to fewer conversions to open surgery in emergency cholecystectomy, but it adds cost, and evidence for its effect on conversion and operative time is graded moderate to low even by the consensus statement recommending routine use. We asked whether preoperative C-reactive protein (CRP), a marker of surgical difficulty and conversion risk, identifies patients in whom ICG's benefit is greatest. METHODS: Prospective cohort of 221 patients undergoing emergency laparoscopic cholecystectomy for acute chole
