Non-Candida fungal cholecystitis - an uncommon occurrence even in the right host.

Invasive fungal infections (IFI) are a significant cause of morbidity and mortality in hematologic malignancy patients and hematopoietic cell transplant (HCT) recipients. Mold cholecystitis has rarely been described. We present a case of disseminated Lomentospora prolificans, that had recrudescence of symptoms with acute cholecystitis. Treatment included a temporary cholecystostomy and administration of the novel antifungal fosmanogepix. At 6 weeks post-HCT, the patient underwent cholecystectomy and completed a course of fosmanogepix, leading to resolution of her infection. Here we review the

Association of intra-operative adverse events with gall bladder wall thickness in patients undergoing laparoscopic cholecystectomy for acute calculous cholecystitis: A prospective study from a low middle income country.

OBJECTIVE: Acute calculous cholecystitis is one of the most common surgical infectious disease emergencies worldwide. Early laparoscopic cholecystectomy (LC) is the definitive treatment, but severe inflammation often leads to technical difficulty and increased intraoperative adverse events. This study evaluated preoperative gallbladder wall thickness (GWT) on ultrasound as a simple, objective predictor of difficult LC in acute calculous cholecystitis. METHODOLOGY: A prospective cross-sectional study was conducted in the Department of Surgery, Aga Khan University Hospital, Karachi, from May to

Comparison of Clinical Efficacy of Early Versus Delayed Laparoscopic Cholecystectomy Following Ultrasound-Guided Percutaneous Transhepatic Gallbladder Drainage.

AIM: To investigate the clinical value of early (≤7 days) versus delayed (>6 weeks) laparoscopic cholecystectomy (LC) after ultrasound-guided percutaneous transhepatic gallbladder drainage (PTGBD) in high-risk acute cholecystitis (AC), with PTGBD serving as a bridging intervention. METHODS: A retrospective analysis was performed on 110 high-risk AC patients admitted between August 2021 and January 2025. Patients were divided into an early LC group (52 cases) and a delayed LC group (58 cases). Surgical indicators, inflammatory response control, hospitalization duration, treatment costs, qualit

Immune Checkpoint Inhibitor-Associated Hepatobiliary Toxicity: A Case of Concurrent Immune-Mediated Hepatitis and Acalculous Cholecystitis.

Immune checkpoint inhibitors are associated with immune-related adverse events, including immune-mediated hepatitis, while acute acalculous cholecystitis remains exceedingly rare. We report a 52-year-old man with metastatic melanoma who developed concurrent grade 3 immune-mediated hepatitis and acute acalculous cholecystitis 2 weeks after combination ipilimumab-nivolumab therapy. He presented with right upper quadrant pain, jaundice, and marked transaminitis. Imaging showed gallbladder wall thickening with pericholecystic fluid without cholelithiasis, and alternative etiologies were excluded.

Comparative evaluation of ultrasound and computed tomography in the detection of imaging findings and complications of acute cholecystitis: a retrospective cohort study.

BACKGROUND: Gallstones (cholelithiasis) are common, with a significant portion of affected individuals developing acute cholecystitis, which can lead to severe complications. While ultrasound (US) is typically the first imaging method used, multidetector computed tomography (MDCT) may be better at detecting more complex scenarios. This study aimed to compare the detection of imaging features and complications of acute cholecystitis between US and MDCT in a cohort of confirmed cases, and to evaluate their complementary roles in clinical practice. METHODS: A retrospective study was conducted in

Floating gallbladder identified on preoperative computed tomography and managed laparoscopically: a case report.

Floating gallbladder is a rare anatomical variant characterized by absence or marked attenuation of its attachment to the hepatic bed, allowing increased mobility within the peritoneal cavity. Although this condition is considered a predisposing factor for gallbladder torsion, it may remain clinically silent and is often identified incidentally during surgery or preoperatively with computed tomography. Preoperative diagnosis remains challenging despite advances in imaging techniques. We report the case of a 73-year-old male patient admitted for elective laparoscopic cholecystectomy due to symp

The Clinical Impact of Team Triage vs Conventional Triage on Patients With Acute Surgical Appendicitis or Cholecystitis.

INTRODUCTION: Team triage incorporates collaborative evaluation by physicians and nurses during initial assessment, differing from conventional nurse-only triage models. Although previous studies have demonstrated improvements in wait times and patient satisfaction, diagnosis-specific clinical outcomes remain less well characterized. This study examined the impact of team triage on time-based performance metrics in patients diagnosed as having acute appendicitis or acute cholecystitis. METHODS: This retrospective, quasi-experimental study included 948 adult patients diagnosed as having append

Haemorrhagic Cholecystitis: A Rare Form of Haemorrhagic Shock in a Patient With a Double Risk.

Haemorrhagic cholecystitis is an uncommon condition with varied clinical presentations and several risk factors, such as anticoagulation, coagulopathy, malignancy, and trauma. We describe the clinical case of a 43-year-old man with two risk factors, anticoagulation and haemophilia B, who presented with atypical symptoms that progressed to shock. Diagnosis and therapeutic approach made this case particularly challenging. This condition should be a diagnosis to consider in a patient with relevant risk factors.

Outcomes and timing of laparoscopic cholecystectomy in gallstone disease: A single-center retrospective cohort study.

The optimal timing of laparoscopic cholecystectomy for gallstone disease remains debated. This study aimed to evaluate the outcomes of laparoscopic cholecystectomy and assess the impact of surgical timing on patient outcomes. We conducted a retrospective cohort study of 329 patients who underwent laparoscopic cholecystectomy at our institution. Demographic data, clinical presentations, surgical details, and outcomes were analyzed. The cohort was predominantly female (80.55%) with a mean age of 41.3 years. Most procedures (89.36%) were elective, with 43.47% of patients waiting over 4 months for

Granulomatous Biliary Stricture Mimicking Malignancy Managed by Laparoscopy: A Diagnostic and Therapeutic Challenge.

Diagnosing biliary strictures remains challenging, as benign inflammatory conditions may closely simulate malignancy on clinical and radiological evaluation. With proper patient selection and surgical expertise, minimally invasive surgery (MIS) provides safe staging and definitive management without compromising oncologic principles for suspicious biliary strictures. We report a 30-year-old male presenting with painless progressive jaundice, weight loss, and cholestatic jaundice with imaging features suspicious for malignancy. Despite the endoscopic decompression, the cross-sectional imaging a

Feasibility and safety of endoscopic ultrasound-guided gallbladder drainage using an electrocautery-enhanced lumen-apposing metal stent for inoperable acute cholecystitis with Niemeier type II perforation.

BACKGROUND AND AIMS: Gallbladder perforation has been considered a contraindication to EUS-guided gallbladder drainage (EUS-GBD). We evaluated the feasibility of EUS-GBD using an electrocautery-enhanced lumen-apposing metal stent (EC-LAMS) in inoperable acute cholecystitis (AC) complicated by Niemeier type II perforation. METHODS: We evaluated 19 consecutive high-risk surgical patients with moderate-to-severe AC and radiologic evidence of Niemeier type II perforation. The primary outcome was technical success. Secondary outcomes included clinical success and adverse events (AEs). RESULTS: Af

A retrospective observational study: are gallstone patients aging, or are the elderly more susceptible to gallstones?

BACKGROUND: Gallstone disease is a common clinical condition worldwide, with a global incidence ranging from less than 10% to over 40% and affecting approximately 3-11% of the population in China. This high and increasing prevalence places a significant burden on healthcare resources due to the costs associated with diagnosis, treatment, and long-term follow-up. More importantly, gallstones can lead to severe complications, such as acute cholecystitis, cholangitis, and pancreatitis, establishing the disease as a major public health concern. Although traditionally associated with a higher incid

Diagnostic utility of systemic inflammatory markers for acute cholecystitis in hemodialysis patients with end-stage renal disease.

Diagnosing acute cholecystitis (AC) in patients with end-stage renal disease (ESRD) is challenging because chronic systemic inflammation and immune dysregulation may obscure acute inflammatory responses. Conventional inflammatory markers may therefore have limited diagnostic specificity in this population. This study aimed to evaluate the diagnostic utility of systemic inflammatory indices in ESRD patients undergoing cholecystectomy. This retrospective single-center study included 48 ESRD patients who underwent cholecystectomy either for clinically suspected AC (AC group, n = 31) or prophylact
