Establishment of In Vivo Mouse Models to Study Salmonella-Induced Chronic Infection and Gastrointestinal Cancer.

Salmonella infection increases the risk of chronic diseases, e.g., inflammatory bowel diseases, colon cancer, and gallbladder cancer. However, the mechanisms leading to the development of these conditions are still not completely understood. Here, we describe the methods to establish in vivo mouse models to study Salmonella-induced chronic infection and colon cancer. These models will allow us to investigate the initiation and progression of diseases.

Efficacy of prophylactic ursodeoxycholic acid in preventing gallstone formation after metabolic bariatric surgery: an updated systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Rapid weight loss following Metabolic bariatric surgery (MBS) is associated with an increased risk of developing de novo gallstone formation. We aimed to evaluate the efficacy of prophylactic ursodeoxycholic acid (UDCA) in preventing gallstone formation and reducing gallstone-related outcomes after MBS. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) following the PRISMA guidelines through PubMed/MEDLINE, Cochrane, Embase, Scopus, Google Scholar, and clinicaltrials.gov. The primary outcome was gallstone formation at 12-24 months,

Cholecystocolonic fistula with ruptured cystic artery pseudoaneurysm presenting as massive per rectal bleeding: a case report.

INTRODUCTION AND IMPORTANCE: Cholecystocolonic fistula and cystic artery pseudoaneurysm are rare complications of gallbladder disease. Their coexistence with massive lower gastrointestinal bleeding is extremely rare and potentially fatal. CASE PRESENTATION: A 47-year-old male presented with severe acute cholecystitis, followed by massive per rectal bleeding and hypovolemic shock. Contrast enhanced CT revealed a cholecystocolonic fistula with a cystic pseudoaneurysm. The patient underwent emergency open cholecystectomy, primary colon repair, and diversion ileostomy. Postoperative histopatholog

Robotic Cholecystectomy Remains Safe and Effective After Regular Hours: A 3-Year Update.

BACKGROUND AND OBJECTIVES: Despite growing popularity in robotic-assisted surgery, access during evenings and weekends for nonelective operations may be restricted. A prior smaller comparison study in 2024 found no basis for safety concerns in managing acute cholecystitis in this setting. We sought to provide additional data to further evaluate whether restriction of robotic cholecystectomy during "after-hours" is justified. METHODS: We performed a retrospective analysis of 161 patients undergoing robotic cholecystectomy for acute cholecystitis according to 2018 Tokyo criteria by a single sur

PSMC2 Promotes Gallbladder Carcinoma Progression by Regulating the AKT/Cyclin D1/CDK6 Signaling Axis.

Gallbladder cancer (GBC) is a prevalent and aggressive malignancy of the biliary system. Studies have revealed that Proteasome 26S subunit ATPase 2 (PSMC2) is overexpressed and associated with the development process of several cancers. However, the specific role in GBC remains unknown. This study aims to explore the association between PSMC2 and GBC pathogenesis. We assessed PSMC2 expression from normal and GBC patient samples. Using Cell Counting Kit-8 (CCK-8), colony formation, scratch, and Transwell migration assays, we investigated the effects of PSMC2 on the proliferation and migration o

Perforated and Complicated Cholecystitis: A Narrative Review of Diagnosis, Management, and Differentiation from Gallbladder Cancer.

INTRODUCTION: Perforated cholecystitis is a severe complication of gallbladder inflammation that is associated with substantial morbidity and mortality. Diagnosis is often challenging because clinical and radiographic findings overlap with those of uncomplicated cholecystitis and, in some cases, gallbladder malignancy. This review summarizes the current evidence regarding the diagnosis, imaging, management strategies, outcomes, and differentiation of perforated cholecystitis from gallbladder cancer. METHODS: A comprehensive literature review was conducted using PubMed and Embase from inceptio

Cholesterolosis mimicking gallbladder adenomyomatosis on imaging in an asymptomatic patient with concurrent cholelithiasis: a case report.

BACKGROUND: Gallbladder cholesterolosis and adenomyomatosis (GA) share overlapping cross-sectional imaging features on CT enterography and MRCP, creating a significant diagnostic pitfall. Histopathology remains the only definitive means of distinction. CASE PRESENTATION: A 44-year-old asymptomatic female was incidentally found to have gallbladder wall thickening and cholelithiasis on CT enterography. MRI/MRCP demonstrated focal fundal wall thickening with intramural high-signal foci, which was interpreted as fundal-type GA with a coexisting gallstone. Elective laparoscopic cholecystectomy was

Cost distribution and Monte Carlo simulation of financial risk under DRG-based prospective payment for laparoscopic cholecystectomy in patients with gallbladder polyps: a single-center retrospective study.

BACKGROUND: Under China's Diagnosis-Related Group (DRG) prospective payment system, hospitals bear the financial risk of costs that exceed the fixed reimbursement standard, making case-level cost distribution and the probability of loss critical to departmental sustainability for high-volume, low-complexity procedures such as laparoscopic cholecystectomy. OBJECTIVE: However, the probability distribution of hospitalization costs and the quantitative financial risk faced by a single institution operating under a fixed DRG payment standard for the group HS10B (laparoscopic cholecystectomy withou

Biliary microbiota and metabolic reprogramming in gallstone-associated gallbladder cancer: evidence from bile, stones, mucosa and tumor tissue.

Gallbladder cancer (GBC) commonly arises in gallbladders affected by cholelithiasis and chronic cholecystitis, although malignancy develops in only a small minority of patients with stones. This disparity has directed attention to the local conditions created by concentrated bile, stone surfaces, persistent microbes and altered bile acid and lipid metabolism. Evidence in this field is difficult to compare because bile, stones, mucosa and tumor tissue measure different biological processes, while human associations, animal experiments and cross-sectional omics studies are often discussed togeth

Bouveret Syndrome Mimicking Gallbladder Malignancy: A Diagnostic Challenge in an 85-Year-Old Woman.

Bouveret syndrome is a rare complication of cholelithiasis in which a gallstone enters the duodenum via a cholecystoenteric fistula, causing gastric outlet obstruction. Because of chronic inflammation and nonspecific symptoms, the condition can easily be misdiagnosed as a gastrointestinal malignancy. An 85-year-old woman presented with severe weight loss (10 kg), night sweats, and non-bloody vomiting. CT imaging revealed massive aerobilia, a thickened gallbladder wall, and an ill-defined mass at the gastroduodenal junction. These findings raised strong suspicion for a malignant process in the

Ultrathin endoscope-assisted pancreatobiliary interventions: from direct transpapillary access to created access routes.

BACKGROUND: ERCP is the standard endoscopic approach for pancreatobiliary diseases, but it relies mainly on fluoroscopic assessment and cannot provide direct intraductal visualization. Ultrathin upper gastrointestinal endoscopes, originally developed for transnasal or pediatric endoscopy, have therefore been used as forward-viewing platforms for direct observation and therapy in the bile duct, pancreatic duct, gallbladder, and peripancreatic cavities. This review summarizes ultrathin endoscope-assisted interventions through both natural and created access routes, excluding dedicated mother-bab

Lactobacillus plantarum may prevent cholesterol gallstone formation by restoring gallbladder motility through preserving Cajal-like cells in mice.

BACKGROUND: Gallstone disease is closely linked to gallbladder dysmotility and gut microbiota dysbiosis; however, the specific role of Lactobacillus depletion in this pathogenesis remains unclear. This study investigated whether lithogenic diet (LD) induced cholesterol gallstone formation is associated with Lactobacillus deficiency and evaluated the potential of Lactobacillus plantarum (L. plantarum) supplementation to mitigate stone formation by restoring gallbladder motility. METHODS AND RESULTS: Here, we used a murine cholelithiasis model using C57BL/6 mice fed an HFD to characterize wheth

Incidental Gallbladder Cancer After Laparoscopic Cholecystectomy: A Comparison Between Patients With and Without Acute Cholecystitis at a High-Volume Center.

INTRODUCTION: Incidental gallbladder cancer (iGBC) after laparoscopic cholecystectomy (LC) remains clinically relevant, and preoperative recognition may be especially difficult in acute cholecystitis (AC). We reviewed iGBC cases diagnosed after LC and descriptively compared cases with and without AC. METHODS: We retrospectively reviewed 2572 consecutive LCs performed between January 2013 and November 2025. After excluding patients with preoperatively detected gallbladder polyps or suspected malignancy, 2224 patients were analyzed (AC, n = 663; non-AC, n = 1561). The denominator cohort was use

Short-term prophylactic biliary stenting after complete common bile duct stone removal to prevent post-ERCP cholangitis in Japan (STEP-ERCP study): study protocol for a confirmatory multicentre randomised controlled trial.

INTRODUCTION: Endoscopic retrograde cholangiopancreatography (ERCP) is the standard treatment for common bile duct stones. Although complete stone removal can be achieved in most cases, post-ERCP cholangitis still occurs in a subset of patients, even in the absence of preprocedural cholangitis. Temporary prophylactic biliary stenting after complete stone extraction may reduce post-ERCP cholangitis by preventing transient biliary obstruction caused by papillary oedema, impaired bile drainage or residual biliary sludge. However, evidence supporting this strategy is currently limited to retrospec
