INTRODUCTION: Perforated cholecystitis is a severe complication of gallbladder inflammation that is associated with substantial morbidity and mortality. Diagnosis is often challenging because clinical and radiographic findings overlap with those of uncomplicated cholecystitis and, in some cases, gallbladder malignancy. This review summarizes the current evidence regarding the diagnosis, imaging, management strategies, outcomes, and differentiation of perforated cholecystitis from gallbladder cancer.
METHODS: A comprehensive literature review was conducted using PubMed and Embase from inceptio

Bouveret syndrome is a rare complication of cholelithiasis in which a gallstone enters the duodenum via a cholecystoenteric fistula, causing gastric outlet obstruction. Because of chronic inflammation and nonspecific symptoms, the condition can easily be misdiagnosed as a gastrointestinal malignancy. An 85-year-old woman presented with severe weight loss (10 kg), night sweats, and non-bloody vomiting. CT imaging revealed massive aerobilia, a thickened gallbladder wall, and an ill-defined mass at the gastroduodenal junction. These findings raised strong suspicion for a malignant process in the

Cholecystocolonic fistula (CCF) is a rare type of cholecystoenteric fistula, which are sometimes diagnosed incidentally in patients undergoing cholecystectomy. They are usually a complication of chronic cholecystitis, often secondary to gallstones, and they are difficult to diagnose due to their rarity and vague clinical presentation. If untreated, they can lead to complications such as acute cholangitis, biliary peritonitis, and biliary cirrhosis. This highlights the importance of early recognition of CCF and timely treatment. While the generally accepted management is surgical, there are no

RATIONALE: We present the case of gallbladder adenomyomatosis (GA)-induced cholecystogastric fistula (CGF) that mimicked an invasive gastric gastrointestinal stromal tumor (GIST), leading to diagnostic and surgical challenges in an emergency setting.
PATIENT CONCERNS: A 71-year-old woman presented to the emergency department with acute abdominal pain and clinical signs of mechanical small-bowel obstruction (SBO).
DIAGNOSES: Contrast-enhanced abdominal computed tomography (CT) revealed a 6-cm mass along the gastric antrum with apparent hepatic invasion and concurrent features of SBO. The gall

Cholecystoduodenal fistula and cholecystocolonic fistula are rare complications of gallstone pathology. A 62-year-old female presented to our surgical assessment unit with abdominal pain and vomiting. A computed tomographic scan revealed complex biliary-enteric fistulas. The fistulas involved the gallbladder, duodenum, and colon, with evidence of partial gastric outlet obstruction secondary to a gallstone in the proximal duodenum. Furthermore, imaging revealed a large gallstone impacted proximal to the diverticular stricture in the sigmoid colon. A minimally invasive approach to resolve the fi

IMPORTANCE: Difficult cholecystectomies are associated with a higher risk of severe bilio-vascular injuries.
OBSERVATIONS: Obesity, cirrhosis, high American Society of Anesthesiologists score, previous abdominal operations, and presence of acute cholecystitis or common bile duct stones are associated with difficult cholecystectomies. On imaging, thickened gallbladder wall, pericholecystic fluid, and an impacted gallstone are associated with difficult cholecystectomies. In challenging operations, the use of imaging (intraoperative cholangiography, intraoperative ultrasound, near-infrared chola

A 60-year-old female with abdominal pain and nausea was diagnosed with gallbladder cancer (cT3aN2M1, cStage IVB). Distant metastases had disappeared after 10 courses of chemotherapy, followed by conversion surgery consisting of subtotal stomach-preserving pancreaticoduodenectomy, partial hepatectomy, and dissection of para-aortic and regional lymph nodes. On postoperative day (POD) 3, additional drainage was performed from the median incision because of bile leakage (BL) from the choledochojejunostomy. The drain tube was removed on POD 89, and the patient was discharged. On the 13th day after

Rapunzel syndrome is a rare form of gastric trichobezoar that extends into the small intestine, leading to intestinal obstruction. Biliary-enteric fistulas are abnormal communications between the biliary system and the gastrointestinal tract, generally occurring spontaneously. We report the case of a 26-year-old female patient with a history of cerebral palsy who presented to the emergency department with an acute abdomen. Computed tomography revealed a mass occupying the stomach and intestine. A laparotomy was performed, identifying a gastric and duodenal trichobezoar complicated by an acute

BACKGROUND: Cholecystoduodenal fistula (CDF) is a rare complication of recurrent gallstone disease, characterized by chronic inflammation, localized ischemia, and tissue necrosis. In this study, the clinical outcomes associated with laparoscopic manual suturing and stapler-assisted closure were compared in the context of CDF repair with accompanying video presentations. METHODS: Overall, 21 patients diagnosed with CDF who underwent treatment at Fujian Provincial Hospital between January 2010 and December 2024 were retrospectively analyzed. Participants were categorized into two groups: those w

Cholecystocolonic fistula (CCF) is a rare complication of chronic gallbladder disease, most commonly seen in elderly women aged 60-70 years. Although most cases are detected on imaging, some are discovered incidentally during surgery. If discovered preoperatively, management can be provided operatively or nonoperatively, depending on symptoms and severity. We present a case of a CCF that was discovered in an asymptomatic 72-year-old man during routine colonoscopy screening. Imaging confirmed the diagnosis, and surgical management was performed. This was initially attempted robotically, but was

INTRODUCTION AND IMPORTANCE: Pathology of the falciform ligament is rare in adulthood due to the closure of the communication of the umbilicus with the external environment during embryological development. Compared to the pediatric population, where pathologies arise from external communication, falciform ligament abscesses in adults occur as a sequelae of hepatopancreaticobiliary pathology through an unclear mechanism. Falciform ligament abscess in adults is an exceedingly rare complication, with only 19 cases published in English literature.

INTRODUCTION: Mirizzi syndrome (MS) is a rare complication of chronic cholelithiasis, resulting from an impacted gallstone in the cystic duct or gallbladder neck, leading to common hepatic duct obstruction. Advanced cases, such as Type IV, involve cholecystobiliary fistula formation, posing significant diagnostic and therapeutic challenges.
CASE PRESENTATION: We report a case of a 69-year-old woman presenting with jaundice, fever, and right upper quadrant pain. Laboratory tests revealed cholestasis and hepatic cytolysis. Imaging, including ultrasonography computed tomography, and magnetic res

BACKGROUND: Routine use of surgical drains after abdominal operations has largely been abandoned over the past decades. Studies have failed to demonstrate benefits of routine drainage following liver, gallbladder, gastric, and colorectal surgeries. Until recently, intraoperative placement of abdominal drains was the gold standard in pancreatoduodenectomies (PDs) due to concerns about uncontrolled postoperative pancreatic fistula (POPF). A large randomized trial in 2014 reported increased mortality in patients without postoperative drain placement. However, as the study did not stratify partici

Cholecystocolonic fistula (CCF) is a rare complication of chronic cholecystitis, characterised by an abnormal connection between the gallbladder and colon. It predominantly affects elderly patients with a history of gallstone disease and chronic inflammation. It is often difficult to diagnose preoperatively due to nonspecific symptoms, low sensitivity on current available imaging, and is often discovered intraoperatively. A high index of suspicion for a CCF should be maintained in patients with longstanding chronic cholecystitis, even when preoperative imaging is unrevealing. Surgeons should b

Gastrobiliary fistula represents a rare and serious complication arising from various etiologies, including gallbladder calculi, peptic ulcer disease, neoplasms, and foreign bodies. Incidents of gastrobiliary fistula secondary to gastric foreign bodies are exceedingly uncommon. This report details the case of a 79-year-old woman who presented with intermittent upper abdominal pain persisting for over 10 months, with exacerbation noted in the preceding 3 days. Radiological evaluation revealed cholelithiasis with gas accumulation within the gallbladder, alongside linear hyperdense shadows sugges

Cholecystogastric fistula (CGF) is a rare complication of chronic cholelithiasis characterized by an abnormal communication between the gallbladder and the stomach. We report the case of a 69-year-old female who underwent elective laparoscopic cholecystectomy for symptomatic gallstone disease. Intraoperatively, a CGF was unexpectedly identified due to dense adhesions between the gallbladder and the gastric antrum. The fistula was successfully managed laparoscopically using a linear endostapler for resection and primary closure of the gastric defect, followed by standard cholecystectomy. The pa

Hepatic artery pseudoaneurysm is a rare but potentially life-threatening vascular condition, with risks of rupture, fistula formation, and massive hemorrhage. It most commonly occurs secondary to trauma, infection, inflammation, or iatrogenic injury. In this report, the case of a 15-year-old boy with idiopathic juvenile arthritis who presented with chronic anemia, intermittent right upper quadrant and epigastric pain, low-grade fever, and occasional vomiting for two years is reported, with repetitive attacks of pancreatitis. Ultrasound of the abdomen showed sludge in the gallbladder. Magnetic

Hirai and colleagues reported delayed gastroduodenal fistula formation 6 months after EUS-guided gallbladder drainage with a double-pigtail plastic stent. Persistent stent pressure from gallbladder contraction may cause bowel injury even with double-pigtail stents. Careful stent selection and positioning, together with regular imaging follow-up, may reduce injury and detect delayed complications.
