Initial Results from a Novel Video Cholangiopancreatoscope: An International Multicenter Study.

Background Novel 8.9-Fr and 11-Fr digital cholangiopancreatoscopes (Scivita Medical) have recently become available in Europe, but data on technical performance and safety remain limited. We report initial real-world experience with the Scivita platform. Methods This international, multicenter, retrospective cohort study included consecutive patients undergoing digital single-operator cholangioscopy (dSOC) or pancreatoscopy (dSOP) between March and October 2025 at 10 European centers. The primary outcome was technical success, defined as achievement of the intended procedural goal. Secondary o

Difficult biliary cannulation during ERCP: techniques and an evidence-informed practical algorithm.

Difficult biliary cannulation remains the pivotal technical barrier in therapeutic endoscopic retrograde cholangiopancreatography (ERCP) and is a major modifiable driver of post-ERCP pancreatitis (PEP). Contemporary practice has moved from prolonged standard attempts toward earlier recognition of difficult access, guidewire-based primary cannulation, targeted rescue techniques, and routine pancreatitis prophylaxis. This review summarizes current evidence and proposes a practical, evidence-informed algorithm for native papilla ERCP. Difficult cannulation is best recognized using objective proce

Between Fire and Restraint: Context-Dependent Roles of Interleukins 27, 33, 35 and 37 in Acute Pancreatitis.

BACKGROUND: Interleukins 27 (IL-27), 33 (IL-33), 35 (IL-35) and 37 (IL-37) occupy potentially important but biologically non-equivalent positions at the interface between inflammatory amplification, tissue protection and immune restraint in acute pancreatitis (AP). This narrative review critically synthesizes AP-specific evidence, distinguishing mechanistic participation from biomarker association and defining translational priorities. METHODS: PubMed/MEDLINE, Scopus and Google Scholar were searched through July 2026. The review was structured with reference to the Scale for the Assessment of

Covered Stent Placement for a Splenic Artery Pseudoaneurysm in a Patient With Inflammatory Vascular Disease.

Splenic artery pseudoaneurysms (SAPs) are potentially life-threatening vascular lesions with a high risk of rupture that may be associated with inflammatory vascular pathologies. While transcatheter embolization is the most common endovascular treatment for SAPs, covered stent placement preserves arterial patency and splenic perfusion but is limited by anatomic constraints, such as inadequate landing zones and excessive vessel tortuosity. This case report discusses the clinical and technical considerations in successfully stenting a SAP in a patient with a history of systemic lupus erythematos

Spontaneous fistulization of a giant pancreatic pseudocyst into the duodenum following acute pancreatitis: a case report.

INTRODUCTION: Pancreatic pseudocysts are well-recognized sequelae of pancreatitis, but giant pseudocysts (>10 cm) are uncommon and rarely resolve through spontaneous fistulization. Spontaneous internal rupture into the gastrointestinal tract happens in less than 3% of cases and typically requires surgical or endoscopic intervention. CASE PRESENTATION: We report the case of a 40-year-old male with a history of alcohol use who developed a giant pancreatic pseudocyst (22 × 12 cm) following acute pancreatitis. He presented with abdominal discomfort, early satiety, and vomiting. Imaging revealed a

Impact of postoperative hyperamylasemia and post-pancreatectomy acute pancreatitis following pancreaticoduodenectomy in a single-center cohort.

BACKGROUND: The clinical significance of postoperative hyperamylasemia (POH) and post-pancreatectomy acute pancreatitis (PPAP) following pancreaticoduodenectomy (PD) is understudied. Hence, we aimed to determine the incidence, risk factors, and short-term clinical impact of post-PD POH and PPAP. METHODS: A retrospective, single-center, observational study was conducted with consecutive adult patients who underwent surgery between January 2012 and September 2025. POH and PPAP were defined and graded using the 2022 International Study Group of Pancreatic Surgery (ISGPS) consensus criteria. RES

The endocannabinoid signaling system in acute and chronic pancreatitis.

The endocannabinoid system (ECS) is a ubiquitous lipid signaling network that regulates tissue homeostasis. Increasing evidence indicates that the ECS undergoes profound remodeling during pancreatic injury and may influence key pathological processes underlying acute pancreatitis (AP) and chronic pancreatitis (CP). Interest in this field has grown in parallel with increasing cannabis use among individuals with pancreatic disorders and the urgent need for alternatives to opioid-based pain management. Components of the ECS, including cannabinoid receptors, the endogenous ligands, and enzymes tha

A rare case of familial chylomicronemia in infancy due to homozygous LMF1 variant.

Familial chylomicronemia syndrome (FCS) is a rare autosomal recessive disorder of triglyceride (TG) metabolism, with an estimated prevalence of 1 in 100 000 to 1 in 1 000 000. It typically presents in childhood or adolescence, while presentation in infancy is exceedingly rare. Loss-of-function variants in lipoprotein lipase gene account for 60-80% of FCS cases. Other causes of FCS include loss-of-function variants in glycosylphosphatidylinositol-anchored high-density lipoprotein-binding protein 1, apolipoprotein A-V, apolipoprotein C-II, and lipase maturation factor 1 (LMF1) genes. LMF1 varian

Endoscopic resolution of esophageal varices in a patient with IgG4-related disease: a case report.

IgG4-related pancreatitis is a complex and multiorgan disease that can mimic pancreatic malignancy. Corticosteroids are the first-line therapy, but their use may not be an option for patients with metabolic comorbidities, such as diabetes mellitus. We report a rare case of a 58-year-old man who presented with portal hypertension and esophageal varices. The patient underwent pancreaticoduodenectomy for suspected pancreatic malignancy. However, a surgical pathology report retrieved during revision confirmed IgG4-associated autoimmune pancreatitis and documented diffuse sclerosing lymphoplasmacyt

Single-cell RNA sequencing reveals PANoptosis signatures of acinar cells, macrophages, and neutrophils during injury and repair in acute pancreatitis.

BACKGROUND: Acute pancreatitis (AP) involves acinar cell injury, inflammation, and tissue remodeling. Although regulated cell death pathways contribute to AP progression, the cell type-specific features and dynamics of PANoptosis remain unclear. This study aimed to characterize PANoptosis-associated signatures in pancreatic cell populations at single-cell resolution. METHODS: A caerulein plus lipopolysaccharide-induced mouse AP model was established. Pancreatic tissues from wild-type, AP, and early recovery groups were analyzed by single-cell RNA sequencing. Major cell types and subclusters w

Biphasic hypertriglyceridemia in pregnancy with apolipoprotein C-II deficiency.

Apolipoprotein C-II (ApoC-II) deficiency is a rare genetic cause of familial chylomicronemia syndrome (FCS) that predisposes affected individuals to severe hypertriglyceridemia and acute pancreatitis. Evidence guiding management during pregnancy is limited. We report a woman with ApoC-II deficiency (ApoC-II Wakayama mutation) who showed a biphasic triglyceride (TG) elevation. TG increased markedly in the second trimester, improved with intensified dietary restriction, and rose again to 1400 mg/dL (15.8 mmol/L) in the third trimester. Because she remained asymptomatic and TG had responded to di

Serial Calcium Monitoring Revealed Primary Hyperparathyroidism in Severe Acute Pancreatitis: A Case Report.

BACKGROUND: Hypocalcemia is commonly monitored as a severity marker in acute pancreatitis, whereas hypercalcemia is an uncommon but important clue to primary hyperparathyroidism (PHPT). CASE PRESENTATION: A 42-year-old man without gallstones, tumor, or alcohol use developed severe acute pancreatitis, with organ failure persisting beyond 48 h. His corrected calcium level was at the lower reference limit on admission but rose from Day 3, exceeded the upper reference limit on Day 4, and reached a peak of 18 mg/dL on Day 21. Intact parathyroid hormone was elevated, and cervical ultrasonography sh

Effectiveness and Safety of Endoscopic Retrograde Cholangiopancreatography for Post-Transplant Biliary Strictures in Children: Experience From Two Centers in Southern Iran.

BACKGROUND: Biliary strictures are among the most frequent complications following pediatric liver transplantation and can compromise graft function if not promptly managed. Endoscopic retrograde cholangiopancreatography (ERCP) is increasingly recognized as a safe, minimally invasive first-line therapy, though pediatric data remain limited. METHODS: We retrospectively reviewed children under 18 years who developed post-transplant biliary strictures between 2017 and 2025 at two tertiary centers in Shiraz, Iran. Demographic, clinical, and procedural data were analyzed. Procedural success was de

GLP-1 receptor agonists and serious adverse events: Evidence, uncertainty and clinical implications.

Glucagon-like peptide-1 (GLP-1) receptor agonists provide benefits in weight reduction, glycaemic control in Type 2 diabetes, and cardiovascular and renal outcomes. Their widespread use has given rise to concerns regarding rare but potentially serious adverse events. This narrative review appraises evidence regarding seven serious adverse events linked to GLP-1 receptor agonists: thyroid cancer, non-arteritic anterior ischaemic optic neuropathy (NAION), suicidal behaviours and self-harm, diabetic retinopathy complications, acute pancreatitis, gallbladder and biliary disease and intestinal obst

High-fat diet accelerates pancreas pathology in the Cel-HYB1 mouse model.

INTRODUCTION: The CEL gene encodes the pancreatic enzyme carboxyl ester lipase. The human CEL-HYB1 allele, a hybrid variant of CEL and its adjacent pseudogene CELP, is associated with a five-fold increased risk for chronic pancreatitis in European cohorts. Humanized Cel-HYB1 mice develop chronic pancreatitis spontaneously albeit with a relatively mild presentation. The aim of this study was to further characterize the pancreatic phenotype in Cel-HYB1 mice. METHODS: We investigated Cel-HYB1 animals by two complementary approaches. First, we analyzed compound heterozygous Cel-16R/HYB1 mice wher
