Amphicrine Carcinomas of the Pancreas Showing Neuroendocrine and Acinar Cell Differentiation: A Case Report and Literature Review.

This study clarifies the distinct clinicopathological and molecular features of amphicrine pancreatic carcinoma (AmpCA) through a case analysis and literature review. We present a case of AmpCA in a 73-year-old female, characterized by co-expression of acinar cell markers and neuroendocrine markers within individual tumor cells. Molecular profiling revealed high-risk mutations in KRAS G12V, TP53, and RB1, along with a high Ki-67 index (85%). Literature comparison confirms that AmpCA exhibits a unique bidirectional, single-cell differentiation pattern, fundamentally distinguishing it from the b

Natural polysaccharides in pancreatic cancer therapy: From immunomodulation and gut microbiota regulation to metabolic intervention.

Natural polysaccharides derived from plants, fungi, macroalgae, cyanobacteria, and other marine microorganisms have attracted increasing interest as adjuvant candidates for pancreatic cancer management due to their biocompatibility, structural diversity, and multi-target activities. This review consolidates current knowledge on their anti-pancreatic cancer activities, integrating findings from in vitro, in vivo, and mechanistic studies. We delineate the multi-level regulatory networks through which polysaccharides exert anti-tumor effects via four interconnected mechanisms: direct actions on c

Risk Factors for Early Intestinal Resection in Pediatric Crohn's Disease: A Nationwide Cohort Study.

INTRODUCTION: Complicated disease affects a substantial proportion of pediatric Crohn's disease (CD) patients and leads to surgery. A subset of children requires intestinal resection shortly after diagnosis. Several risk factors for surgery are known; however, the predictors of these early resections have not been well defined. We aimed to characterize this patient group and identify risk factors associated with early intestinal resections. METHODS: We performed a retrospective analysis of patients under 18 y diagnosed with CD between 2010 and 2019, based on the Hungarian Pediatric inflammato

Next-Generation CAR-Macrophage Therapies: Design Principles and Advances in Cancer Treatment.

Macrophages possess intrinsic tumor-homing and tissue-infiltrating abilities, performing various effector functions, including phagocytosis and antigen presentation. These properties make chimeric antigen receptor macrophages (CAR-M) a promising approach to overcoming major barriers in cellular therapy for solid tumors. This review systematically compares and critically evaluates key design variables that determine the therapeutic activity and clinical feasibility of CAR-M, including CAR architecture, macrophage source, gene-delivery method, and manufacturing strategy. We also discuss emerging

Association of Prophylactic Abdominal Drain Placement With Short-Term Postoperative Outcomes After Hepatectomy: A Nationwide Inpatient Database Study in Japan.

AIM: Although randomized trials have discouraged routine prophylactic abdominal drainage after uncomplicated hepatectomy, these drains are still widely used in clinical practice. This study evaluated the association between prophylactic abdominal drain placement and short-term outcomes after hepatic resection using a nationwide inpatient database. METHODS: We identified patients who underwent initial hepatic resection for liver tumors from the Japanese Diagnosis Procedure Combination database. Patients were classified into drain and no-drain groups according to whether a prophylactic abdomina

Periodontal diseases and coronary artery disease: epidemiological links, pathogenic mechanisms and prospects for comorbidity integrated management.

Periodontal disease (PD) and coronary artery disease (CAD) are two highly prevalent chronic systemic disorders with overlapping population risk profiles. Accumulating observational, cellular, and genetic studies have suggested potential positive correlations between PD severity, periodontal pathogen load, and CAD incidence, coronary lesion severity, and major adverse cardiac events, whereas Mendelian randomization analyses may not support a direct causal link between the two illnesses. Several synergistic pathogenic pathways may mediate their association: periodontal pathogens may translocate

From CAF subtypes to clinically relevant states: cancer-associated fibroblasts in pancreatic ductal adenocarcinoma.

Pancreatic ductal adenocarcinoma (PDAC) develops within a dense, poorly perfused tumor microenvironment (TME) in which cancer-associated fibroblasts (CAFs) are prominent cellular and structural components. Once viewed as a uniform population of activated matrix-producing cells, CAFs now include myofibroblastic, inflammatory, antigen-presenting and other populations identified by single-cell and spatial studies. These categories sharpen experimental resolution but do not form a stable hierarchy. CAF phenotypes overlap across spatial niches and disease stages, and they change under metabolic str

Landscape of Endocytic Effectors for Extracellular Targeted Protein Degradation.

Extracellular targeted protein degradation (eTPD) has emerged as a new strategy for regulating the abundance and activity of extracellular and membrane proteins, with prominent potential to address traditionally undruggable targets in oncology, autoimmune diseases, and neurodegenerative disorders. Most eTPD platforms employ heterobifunctional chimeras that link proteins of interest (POIs) to specific endocytic effectors (EEs), either lysosome-targeting receptors (LTRs) or membrane-bound E3 ubiquitin ligases, to direct POIs to lysosomes for degradation. Since EE engagement dictates cellular upt

Host-pathogen pyrimidine metabolism drives cystic fibrosis lung disease.

Cystic fibrosis (CF), caused by CFTR mutations, is characterized by progressive pulmonary remodeling, particularly during bacterial infection. This pathological environment, exacerbated by altered epithelial growth, leads to thickened airway walls, inducing permanent narrowing and long-term breathing difficulties. However, what drives this adverse milieu remains unclear. Here, we identify activation of the de novo pathway of pyrimidine synthesis (DNPPS) as a central contributor of CF lung remodeling. In response to impaired mitochondrial bioenergetics, CFTR-mutant epithelial cells engage compe

Structure and functional divergence of the non-canonical BoNT-like toxin PG1 and PG2.

Botulinum neurotoxins (BoNTs) are potent bacterial toxins. Distinct BoNT-like toxins, including BoNT/X, BoNT/En, and PMP1, form a distinct lineage. Here we characterize two additional members of this group, PG-toxin 1 and 2 (PG1 and PG2). Their light-chain (LC) and heavy-chain (HC) domains are encoded by adjacent genes. PG1 and PG2 LCs cleave Drosophila SNAP-25 at distinct sites (N196-E197 and R191-K193, respectively); PG1 is more potent, while PG2 activity depends on pH and ionic strength. Both LCs enter Sf9 cells via clathrin-mediated endocytosis. PG1 LC is toxic to Drosophila and northern a

Toripalimab-induced isolated ACTH deficiency associated with gastroparesis: a case report.

Immune checkpoint inhibitors can cause isolated ACTH deficiency, a rare condition often misdiagnosed due to nonspecific symptoms. We report a cholangiocarcinoma patient on toripalimab and lenvatinib who experienced recurrent gastrointestinal symptom and precordial discomfort for 2 years, mistakenly attributed to drug toxicity or functional disorders. Following surgery, the patient developed refractory delayed gastric emptying, during which IAD was identified through negative imaging examinations, low ACTH and cortisol levels. Glucocorticoid replacement rapidly resolved symptoms. This case high

Clinical benefit of locoregional therapy for oligometastasis after surgery for pancreatic cancer: a single center retrospective study.

BACKGROUND: The prognosis of patients with pancreatic cancer (PC) is generally poor. However, favorable outcomes have recently been reported in selected patients with metastatic PC who receive multidisciplinary treatment. Oligometastasis (OM) represents a limited metastatic state and may be associated with better prognosis than multiple metastases (MM). The clinical role of locoregional therapy (LT) for postoperative OM remains unclear. METHODS: We included all consecutive patients with pancreatic malignancies who underwent macroscopically curative resection between 2013 and 2018. Patients wi

Mechanisms of synergistic action between olaparib and KPT-330 in pancreatic cancer.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) is a highly aggressive and prognostically poor cancer driven by multifaceted mechanisms, notably immune evasion and drug resistance. The majority of PDAC patients present with unresectable locally advanced or metastatic disease, emphasizing a critical unmet need for effective therapies against advanced-stage pancreatic cancer. Despite the proven efficacy of the PARP inhibitor Olaparib against pancreatic cancer in patients carrying BRCA mutations, its practical use is limited by the development of acquired resistance. The clinical application

Determinants of Non-Receipt of Guideline-Concordant Treatment in Pancreatic Cancer: A Classification Tree Analysis.

BACKGROUND: Non-receipt of guideline-concordant treatment (GCT) in pancreatic ductal adenocarcinoma (PDAC) is associated with worse outcomes. Studies have identified associated factors, but less is known about how these factors co-occur within patient subgroups at the time of presentation to oncology care. METHODS: We conducted a retrospective cohort study of patients with PDAC treated at two academic centers. Candidate variables available at presentation were selected a priori across demographic, clinical, socioeconomic, and healthcare utilization domains. Classification and regression tree
