Teduglutide in pediatric patients under 10 kg with short bowel syndrome on parenteral support: An open-label study.

BACKGROUND: Patients with short bowel syndrome (SBS)-associated intestinal failure (SBS-IF) are dependent on parenteral support (PS). In Japan, teduglutide is the only GLP-2 analog medicine indicated for these patients. There are limited data for pediatric patients, especially those with a low body weight. This study evaluated the safety and efficacy of teduglutide in Japanese pediatric patients with SBS-IF (dependence on PS to provide ≥30% of fluid or caloric intake needs) who weighed less than 10 kg. METHODS: This phase 3, open-label study enrolled Japanese pediatric patients with SBS-IF. P

Causal associations between autoimmune diseases and cognitive impairment: A Mendelian randomization study.

Epidemiological evidence suggests that there is an association between systemic lupus erythematosus (SLE), type 1 diabetes (T1D), ankylosing spondylitis (AS), celiac disease (CD), and cognitive impairment (CI). However, the causal relationship between SLE, T1D, AS, CD, and CI remains unclear. The causal effects of the instrumental variables were analyzed using the random-effects inverse variance weighted method. Horizontal pleiotropy was examined by sensitivity analyses applying the weighted median method and the Mendelian randomization-Egger method. In order to avoid bias resulting from singl

m6A RNA methylation modulates antiviral response in celiac disease.

N6-methyladenosine (m6A) RNA modifications and reovirus infections have recently been implicated in the development of celiac disease. While viral infections are known to alter the RNA modification machinery of the host, their interplay in autoimmune tissue damage remains unexplored. In this study we employed an in vitro model combining a viral mimic with gliadin peptides to simulate celiac disease conditions, alongside the analysis of serum and intestinal biopsies from controls and patients to investigate the link between m6A methylation and viral infections in inducing autoimmune inflammatio

Proteomics Analysis of Celiac Disease-Active Peptides in Food Products with Partially Hydrolyzed Gluten.

Foods with partially hydrolyzed gluten may still trigger immune responses in celiac disease (CeD) patients due to residual CeD-active peptides. We applied nanoliquid chromatography-tandem mass spectrometry (nanoLC-MS/MS) to identify 54 CeD-active peptides in 15 barley malt extracts (BME), seven barley malt vinegars (BMV), and 15 soy sauces. Eight common CeD-active peptides were quantitated with targeted nanoLC-MS/MS and stable isotope dilution assay. Total peptide concentrations ranged from 481 mg/kg to 1305 mg/kg in BMEs, whereas three samples showed peptide concentrations near LOQ. Six BMVs

Autoimmunity to mental health: Risk of depression in type 1 diabetes and celiac disease patients - A systematic review, meta-analysis, and bias assessment.

BACKGROUND: Depression is expected to become the world's largest disease burden by 2030. However, the incidence of depression in individuals with Type 1 diabetes mellitus (T1DM) or Celiac disease (CD) remains poorly studied. To address this gap, a systematic review and meta-analysis was carried out. METHODS: This systematic review was approved at PROSPERO on February 9, 2025, and followed the PRISMA and MOOSE guidelines. The literature search considered all peer-reviewed quantitative studies from relevant databases up to February 7, 2025. Study-specific risk of bias was assessed using the ROB

Enhancing gluten modification for celiac patients: harnessing microbial transglutaminase, ε-poly-l-lysine, and prolyl endopeptidase.

The immunogenicity of gluten poses a significant challenge for individuals with celiac disease. This study aimed to reduce gluten immunogenicity using a dual treatment strategy: microbial transglutaminase and ε-poly-l-lysine (ε-PL) for structural modification, followed by enzymatic degradation with prolyl endopeptidase. Structural characterization of gluten proteins treated with transglutaminase and ε-PL revealed substantial modification of gliadins, with higher ε-PL concentrations showing more pronounced effects. Low-molecular-weight glutenins were less affected, and no deamidation or impact

Qualitative Research for the Development of Three New Quality of Life and Treatment Impact Questionnaires for Adults with Short Bowel Syndrome with Intestinal Failure (SBS-IF).

PURPOSE: Short bowel syndrome with intestinal failure (SBS-IF) requires lengthy parenteral support (PS) infusions to maintain nutrition and hydration, and may also involve treatment with glucagon-like peptide 2 (GLP-2) analog injections. SBS-IF and its treatment have significant health-related quality-of-life (HRQoL) impacts on patients. While existing patient-reported outcome (PRO) questionnaires measure HRQoL impacts of SBS, differences in intestinal anatomy or treatment type have their own impacts to patients that warrant assessment. This qualitative research supports the content validity o

Pharmacokinetics and Safety of Single-Dose Apraglutide in Individuals with Normal and Impaired Hepatic Function: A Phase 1, Open-Label Trial.

Intestinal failure-associated liver disease occurs in 20% to 30% of patients with short bowel syndrome and intestinal failure (SBS-IF). Apraglutide is a glucagon-like peptide-2 (GLP-2) analog in clinical development for the treatment of patients with SBS-IF. This study assessed the potential for changes in exposure of apraglutide in individuals with impaired hepatic function versus healthy volunteers. In this Phase 1, open-label, nonrandomized, single-dose trial, apraglutide 3.5 mg was administered to participants with moderate hepatic impairment (Child-Pugh B) or normal hepatic function. Prim

Beyond the Gut: thyroxine malabsorption, pharmacokinetics, and delivery.

Refractory primary hypothyroidism is defined by persistently elevated thyroid-stimulating hormone despite supraphysiological oral levothyroxine dosing. The distinction between true malabsorption and pseudomalabsorption (nonadherence) is critical for management planning. In 2023, Quiroz-Aldave et al proposed a comprehensive framework for the evaluation and management of refractory primary hypothyroidism, while Liu et al provided an updated overview of both conventional and novel levothyroxine formulations. This mini-review synthesizes recent evidence published since 2023 and presents 2 illustra

Unraveling the Complexity of Celiac Disease: A Narrative Review of Its Multisystem Nature.

Celiac disease is a chronic immune-mediated disorder triggered by the ingestion of gluten in genetically susceptible individuals, primarily those carrying HLA-DQ2 or HLA-DQ8 and, in rare cases, HLA DQ7 alleles. Although traditionally regarded as a gastrointestinal condition, celiac disease is now recognized as a multisystem disorder with a wide range of clinical presentations. It has been described as a "clinical chameleon" due to its variable manifestations, which may include non-specific symptoms, extraintestinal involvement, or even an asymptomatic course, often identified only through scre

Nutritional Strategies for Intestinal Rehabilitation in Children with Short Bowel Syndrome: A Narrative Review.

Background/Objectives: Nutritional management is fundamental to intestinal rehabilitation in children with short bowel syndrome (SBS), yet clinical practice remains heterogeneous and largely guided by expert opinion. Enteral nutrition (EN) is the main driver of intestinal adaptation and progression toward enteral autonomy, but optimal strategies vary according to residual bowel anatomy, postoperative phase, and feeding tolerance. This review aimed to synthesize available evidence on nutritional strategies for pediatric SBS, with a focus on EN initiation, advancement, composition, and outcomes.

Impact of standardized duodenal biopsy sampling on the diagnosis of ultrashort celiac disease.

Ultrashort celiac disease (UCD) is a subtype of celiac disease characterized by villous atrophy confined to the duodenal bulb. Its diagnosis remains challenging due to the histologic particularities of the bulb and the low adherence to recommended biopsy protocols. In a series of 728 adult patients, the prevalence of UCD was 4.5%. Patients presented digestive symptoms comparable to those of conventional celiac disease (CCD), but showed a lower frequency of extraintestinal manifestations, lower levels of IgA anti-tissue transglutaminase antibodies, and a lower prevalence of the HLA-DQ2.5 haplot

Multiplex autoantibody screening in pediatric type 1 diabetes: POC Blot vs. ELISA for gluten-related disorders.

BACKGROUND: Type 1 diabetes (T1D) is frequently associated with various autoimmune disorders, including gluten-related disorders (GRDs). Early screening for autoantibodies can provide insights into the co-occurrence of these conditions in pediatric patients. This study aims to evaluate the performance of a Point-of-Care Test (POC) BLOT based assay in comparison to the conventional enzyme-linked immunosorbent assay (ELISA) for multiplex autoantibody screening for gluten-related disorders in children with T1D. METHODS: A cohort of pediatric and adolescent patients diagnosed with T1D was recruit

T cell receptors for antigen on intraepithelial cytolytic T lymphocytes in celiac disease engage enterocyte HLA-E and HLA-B.

We compared duodenal biopsies showing active celiac disease (CeD) to normal controls using single-cell RNA sequencing, cyclic immunofluorescence, RNAScope, and proximity ligation assays. There is increased infiltration of villous but not crypt epithelium T cells bearing either αβ or γδ T cell receptors (TCRs) in CeD. Both T cell subsets are activated cytotoxic T lymphocytes (CTLs) and are the predominant mucosal source of IFNγ. In response to this IFNγ, villous but not crypt enterocytes show an IFNγ signature, including nuclear phospho-STAT1 protein, class II HLA molecules and IFNγ-inducible c

Favorable seizure control with a gluten-free diet in a patient with drug-resistant epilepsy and celiac disease: a case report.

INTRODUCTION: Celiac disease (CD) is a chronic autoimmune disorder triggered by gluten sensitivity. Beyond its gastrointestinal presentation, CD is increasingly associated with neurological manifestations, including epilepsy. CASE PRESENTATION: To report the case of a 21-year-old female with a 15-year history of medically resistant epilepsy. Despite multiple antiseizure medications, seizure control remained inadequate. Further evaluation led to a diagnosis of biopsy-confirmed CD. The patient was initiated on a strict gluten-free diet, which resulted in significant clinical improvement and bett
