The role of contrast-enhanced ultrasound in the diagnosis of gastroesophageal reflux.

OBJECTIVE: To assess the use of contrast-enhanced ultrasonography in the diagnosis of gastroesophageal reflux and hiatal hernia, avoiding ionizing radiation. MATERIAL AND METHODS: Seven contrast-enhanced esophagogastric ultrasound examinations were performed in children between 5 months and 3 years of age, 5 to evaluate gastroesophageal reflux and hiatal hernia, and 2 for gastroesophageal reflux. An ultrasound machine with specific software for contrast visualization, a multi-frequency probe, and sonographic contrast agent were used. Findings were correlated with upper gastrointestinal series

Paediatric necrotising pancreatitis.

Necrotising pancreatitis is a rare but serious complication of acute pancreatitis in children. We report a case of a toddler who presented with a 1 day history of persistent non-bilious vomiting, intermittent pallor and irritability, without fever or diarrhoea. Initial examination was non-specific, but progressive abdominal tenderness and new onset of fever prompted imaging and laboratory investigations. Elevated inflammatory markers and serum lipase, along with contrast-enhanced CT findings, confirmed a diagnosis of necrotising pancreatitis. The patient was transferred to a tertiary hospital,

High grade non-Hodgkin lymphoma as culprit of ileal perforation in a patient presenting with generalized peritonitis: a case report.

BACKGROUND: Non-Hodgkin lymphoma is one of the most common types of lymphoma affecting the gastrointestinal tract. This malignancy can originate from either B-lymphocytes or T-lymphocytes. Intestinal perforation due to non-Hodgkin lymphoma is a relatively rare occurrence, and when it does happen, it typically arises after the initiation of chemotherapy. In the reported case, the perforation happened prior to the chemotherapy, making the case unusual. We aim to highlight the importance of maintaining a high index of suspicion and the critical role of timely, multidisciplinary management in impr

Life-threatening upper gastrointestinal bleeding caused by pancreatic pseudocysts: A case report.

RATIONALE: Pancreatic pseudocysts are fluid-filled sacs that typically develop as a complication of pancreatitis or abdominal trauma. These lesions are relatively rare, with an estimated incidence of 0.5 to 1 case per 100,000 people annually. While asymptomatic pseudocysts often resolve with conservative management, symptomatic cases or those complicated by infection, hemorrhage, or obstruction generally require surgical intervention. PATIENT CONCERNS: A 45-year-old man had recurrent black stools and vomiting blood. Initially diagnosed with gastritis, his condition worsened despite treatment,

Is nasogastric intubation still necessary after pancreaticoduodenectomy? A case-control cohort study.

BACKGROUNDS/AIMS: The benefits of nasogastric intubation after pancreaticoduodenectomy are not well understood, and it remains unclear which patients may need nasogastric intubation in the immediate postoperative period. This study evaluated the effectiveness of nasogastric intubation following pancreaticoduodenectomy and identified factors influencing the reintubation rate. METHODS: We conducted a retrospective case-control cohort study involving adult patients who underwent pancreaticoduodenectomy for either benign or malignant periampullary disease, with a 90-day follow-up. Patients were d

Rare gastric bezoar in a school-aged child: a paediatric diagnostic conundrum.

A healthy toddler presented with a 1-month history of constipation and was started on laxatives. Four years later, the patient returned with an abdominal mass and pain, but no vomiting, diarrhoea or hard stools. An ultrasound showed a thickened hyperechoic wall with acoustic shadowing in the epigastric region and enlarged mesenteric lymph nodes. Routine laboratory tests were normal. Laxatives were reintroduced, and a follow-up ultrasound 3 months later still indicated an abdominal mass. Five months afterwards, the CT scan suggested a bezoar, leading to a referral to the paediatric surgical uni

Neurogenic Appendicopathy in Pediatric Appendectomies During the COVID-19 Era: A Retrospective Analysis of Histologically Negative Cases.

INTRODUCTION: In approximately 15% of cases undergoing surgery for acute appendicitis (AA), normal findings are observed in the histological examination of the appendix. Neurogenic appendicopathy (NA) is clinically indistinguishable from AA and is relatively less known. This retrospective study is designed to investigate the frequency of NA in patients operated on during the COVID-19 pandemic who were PCR-negative for COVID-19 and showed no histological evidence of appendicitis. MATERIALS AND METHODS: Pediatric patients presenting with classic symptoms of AA (right lower quadrant pain, tender

When Textbook Meets Reality: A Rare Case of Boerhaave's Syndrome With Mackler's Triad.

Boerhaave's syndrome is a rare and life-threatening form of spontaneous esophageal perforation, typically triggered by forceful vomiting and often misdiagnosed due to nonspecific clinical features. Although Mackler's triad (vomiting, chest pain, and subcutaneous emphysema) is classically associated with the condition, it is infrequently observed in full. We present the case of a 32-year-old man with a history of ulcerative colitis (UC) who presented to the emergency department with acute chest pain and repeated vomiting following dinner. He reported a sensation of food impaction and sought car

Metoclopramide combined with probiotics improves gastric retention in mechanically ventilated patients following craniocerebral surgery.

OBJECTIVES: This study aimed to evaluate the effect of the co-administration of metoclopramide and probiotics on enteral feeding tolerance in mechanically ventilated patients after cranial surgery. METHODS: From January 2023 to December 2024, a total of 88 patients presenting with acute brain injury and treated by craniocerebral surgery were screened. Of these, 32 were excluded and 56 patients were enrolled and assigned to either the intervention group (n = 32) or the control group (n = 24). Enteral nutrition was initiated within 24 h following nasogastric tube placement. The intervention gro

An unintended path: Foley catheter-induced biliary obstruction following gastrostomy tube replacement - A case report.

Percutaneous endoscopic gastrostomy (PEG) tubes are widely used for long-term enteral access. Accidental dislodgement is a relatively common occurrence, and in many settings a Foley catheter is inserted as a temporary measure to preserve tract patency until definitive replacement can be performed. Although this practice is routinely employed, Foley catheters lack an external flange and are therefore susceptible to distal migration and associated complications. We describe a case of a 97-year-old woman with advanced dementia who presented to the emergency department with recurrent vomiting and

Abdominal compartment syndrome following chemotherapy-induced gastrointestinal mucositis: a case report.

BACKGROUND: Abdominal compartment syndrome (ACS) is a life-threatening condition typically associated with trauma, major surgery, or acute abdominal pathology. Its occurrence in oncology patients, particularly as a complication of chemotherapy-induced gastrointestinal mucositis, is exceedingly rare. This report presents a unique case of ACS triggered by mucosal injury following chemotherapy, complicated by septic shock and multiorgan failure, in a patient with lung cancer. CASE PRESENTATION: A 64-year-old male with lung cancer underwent platinum-etoposide chemotherapy. Following his fourth cy

Combined Acupoint Massage and Abdominal Mirabilite Application for Accelerating Gastrointestinal Recovery in Pediatric Patients After Endoscopic Retrograde Cholangiopancreatography: Protocol for a Randomized Controlled Trial.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is crucial for managing pediatric hepatobiliary diseases but frequently results in postoperative gastrointestinal dysfunction such as delayed flatus and defecation, which can prolong recovery and increase the risk of complications. Nonpharmacological interventions such as acupoint massage and mirabilite application offer potential benefits, but evidence for their efficacy and synergy in children after ERCP is lacking. OBJECTIVE: This study aims to evaluate the individual and combined effects of acupoint massage and abdominal mi

Paraduodenal Hernia With Intestinal Obstruction During Pregnancy.

BACKGROUND Paraduodenal hernia (PDH) is a rare internal hernia, accounting for 50% to 55% of internal hernias but only 0.2% to 0.9% of intestinal obstructions. Right-sided PDH is less common, occurring in approximately 25% of cases. CASE REPORT This case describes a 33-year-old pregnant woman at 20 weeks 6 days gestation who was admitted with a 15-day history of abdominal distension accompanied by nausea and vomiting. The patient had postprandial exacerbation of intermittent abdominal pain, bilious vomiting, and significant weight loss (5 kg), with a history of similar symptoms during previous

ALK-positive inflammatory myofibroblastic tumor in the pelvis of a child: a case report and literature review.

Inflammatory myofibroblastic tumor (IMT) is a rare neoplasm that primarily affects children and young adults. While typically found in the lungs, liver, and gastrointestinal tract, pelvic involvement is recognized but occurs less frequently than intra-abdominal IMT, particularly in pediatric patients. Here we report a case of a 2-year-old boy who presented with a brief history of vomiting and decreased appetite. Imaging revealed a cystic-solid mass in the pelvis with progressive enhancement of the solid component, leading to suspicion of a vascular soft tissue neoplasm. Surgical exploration id

Emergency Laparoscopic Reversal of One‑Anastomosis Gastric Bypass Following Early Anastomotic Failure: Video Case Report of Management.

BACKGROUND: One anastomosis gastric bypass (OAGB) is the third most common metabolic bariatric surgery. Early anastomotic failure is uncommon and may necessitate re-operation. The most common re-operative strategy is peritoneal washout and drainage, while the preferred reconstructive approach is conversion to Roux-en-Y gastric bypass (RYGB). Reversal to normal anatomy in this emergency setting is extremely rare. We present a detailed single-patient emergency OAGB reversal for a leak with operative video, addressing a gap in the published literature. CASE PRESENTATION: A 28‑year‑old woman with

Gastric bronchogenic cyst: a case report and review of the literature.

INTRODUCTION AND IMPORTANCE: Bronchogenic cysts are rare congenital foregut malformations, usually asymptomatic in early life. Gastric bronchogenic cysts (GBCs) are exceedingly uncommon and often pose diagnostic challenges due to nonspecific symptoms and inconclusive imaging. Misdiagnosis is common, as GBCs can mimic solid tumors or other cystic lesions. CASE PRESENTATION: A 53-year-old asymptomatic man was found to have an incidental gastric mass on abdominal CT, revealing a 4.0 × 2.1 cm non-enhancing lesion along the lesser curvature. Gastroscopy was unremarkable. He reported no symptoms su

Omentopexy after laparoscopic sleeve gastrectomy in children and adolescents: is it effective in reducing post-operative complications?

Laparoscopic sleeve gastrectomy (LSG) is one of the most commonly performed restrictive bariatric procedures. Postoperative gastrointestinal complications are distressing in patients who have undergone this procedure. Most of these complications can be prevented by omentopexy as a modification on laparoscopic sleeve gastrectomy. The purpose of this study was to evaluate the effectiveness of omentopexy during laparoscopic sleeve gastrectomy in reducing postoperative complications. This prospective comparative study, conducted between January 2015 and October 2024 at Al-Azhar University Hospital

Abdominal Cocoon Syndrome With Greater Omental Hypoplasia: A Rare Anatomical Insight and Diagnostic Challenge.

Abdominal cocoon syndrome, or idiopathic sclerosing encapsulating peritonitis (SEP), is an uncommon cause of small bowel obstruction marked by the encasement of bowel loops within a dense fibrocollagenous membrane. Due to its rarity and nonspecific presentation, diagnosis is frequently delayed, particularly in patients without prior abdominal surgery or known risk factors. We report a diagnostically challenging case of a 53-year-old male with an 18-month history of recurrent gastrointestinal obstruction, including bilious vomiting, oral intolerance, and epigastric pain.
