Pyoderma Gangrenosum Masquerading as Chronic Infective Ulcers: Two Case Reports Across Age Extremes.

Pyoderma gangrenosum (PG) is a rare, ulcerative neutrophilic dermatosis that presents significant diagnostic challenges due to its clinical heterogeneity and lack of definitive laboratory markers. Misdiagnosis often leads to inappropriate treatments, delayed recovery, and increased morbidity. We report two cases of PG at opposite ends of the age spectrum-a 90-year-old male and a 10-year-old boy-each initially misdiagnosed as chronic infective ulcers. Both patients demonstrated classical features of PG, including rapid ulcer progression, pathergy phenomenon, undermined violaceous borders, and n

A rare case report of pyodermatitis vegetans linked with inflammatory bowel disease in juvenile population.

Pyodermatitis vegetans (PDV) is a rare, chronic inflammatory dermatosis associated with inflammatory bowel disease (IBD), particularly ulcerative colitis. Though PDV has been documented globally, it remains rare in pediatric populations and under-reported in Pakistan. This case highlights an adolescent male with suspected IBD presenting with extensive cutaneous manifestations consistent with PDV. A 17-year-old male presented with a 10-year history of recurrent vegetative skin lesions, which initially started behind the left ear and then involved the face, axillae, back, chest, buttocks, glutea

Metastatic Crohn Disease Manifesting as Granulomatous Cheilitis in a Patient With Well-Controlled Crohn Disease.

Crohn disease (CD) can affect any organ system outside the gastrointestinal tract, and these conditions are classified as extraintestinal manifestations and include arthritis, uveitis, and cutaneous manifestations. Cutaneous involvement includes erythema nodosum, pyoderma gangrenosum, and metastatic CD. The latter can be defined as a granulomatous skin lesion that is noncontiguous to the intestinal disease. In this article, we report a rare case of a 48-year-old woman with CD in endoscopic and clinical remission with chronic persistent orofacial granulomatosis of the lip secondary to CD, despi

Isolation of a Methicillin-Resistant Staphylococcus pseudintermedius Strain from a Domestic Dog with Vulvovaginitis: A Potential Public Health Concern?

Background:Staphylococcus pseudintermedius is a major opportunistic pathogen of dogs and the primary cause of canine pyoderma and other infections. The global emergence of methicillin-resistant S. pseudintermedius (MRSP) represents a significant challenge in veterinary medicine due to its frequent multidrug-resistant phenotype and limited therapeutic options. Methods: We describe the phenotypic and genomic characterization of an MRSP isolate recovered from a vaginal swab of an 11-year-old dog diagnosed with vulvovaginitis in southern Italy. Antimicrobial susceptibility testing was performed us

Primary Cutaneous Anaplastic Large Cell Lymphoma Mimicking Pyoderma Gangrenosum: An Atypical Presentation and Diagnostic Challenge.

Primary cutaneous anaplastic large cell lymphoma (PC-ALCL) is a rare subtype of cutaneous T-cell lymphoma that is known for its CD30-positive atypical large lymphoid cells. PC-ALCL usually presents as solitary nodules and has both clinical and histological features that could mimic other cutaneous lymphomas, which can lead to misdiagnosis. We report the case of a 62-year-old patient presenting with chronic ulcers whose condition at first mimicked pyoderma gangrenosum (PG), but eventually turned out to be PC-ALCL following immunohistochemical investigations. Radiological assessment demonstrated

Rapid Remission through Multimodal Therapy in a Severe Form of PASH Syndrome: A Case Report.

INTRODUCTION: PASH syndrome is a rare autoinflammatory dermatosis characterized by the triad of pyoderma gangrenosum (PG), acne conglobata, and hidradenitis suppurativa (HS), driven by innate immune dysregulation and IL-1-mediated neutrophilic inflammation. CASE PRESENTATION: We report a 38-year-old woman presenting with severe HS (Hurley stage III), chronic nodulocystic facial acne, and PG-like ulcers, consistent with PASH syndrome. Her condition was marked by debilitating pain (VAS 8/10), malodorous discharge, and extensive ulceration, necessitating examination under general anesthesia. Pre

Clinicopathological Challenge: A Rapidly Progressive Bullous Ulcer in a Patient Receiving Gilteritinib.

Neutrophilic dermatoses are an emerging complication of targeted leukemia therapies and can present diagnostic challenges in immunocompromised patients. We report an 80-year-old woman with therapy-related acute myeloid leukemia who developed a painful violaceous papulonodule on the right shin that rapidly became bullous and ulcerated with intense surrounding hemorrhage 5 months after initiating the FLT3 inhibitor gilteritinib. Histopathology demonstrated a deep neutrophilic infiltrate with negative infectious workup, supporting FLT3 inhibitor-associated bullous pyoderma gangrenosum. The lesion

Pyoderma gangrenosum is associated with excess incident major atherothrombotic events.

BACKGROUND AND AIMS: Systemic inflammation contributes to atherosclerosis and its thrombotic complications, yet population-level data for neutrophil-predominant dermatoses are limited. Pyoderma gangrenosum (PG) is a prototypical neutrophilic inflammatory dermatosis, we quantified its association with incident atherothrombotic events and MACE over 5 years. METHODS: Adults with PG were compared with non-PG controls. To limit reverse causation, we excluded individuals with pre-index cardiovascular events and selected vascular procedures. Cohorts were 1:1 propensity-score matched (greedy nearest

Pyoderma Gangrenosum with Pseudoepitheliomatous Hyperplasia Histopathologically Misdiagnosed as Squamous Cell Carcinoma: A Case Report and Brief Literature Review.

Pseudoepitheliomatous hyperplasia (PEH), a rare histopathological manifestation of pyoderma gangrenosum (PG), closely resembles squamous cell carcinoma (SCC) and leads to misdiagnosis, yet relevant reported cases remain limited. We report a 71-year-old female who developed abdominal pain and persistent fever after tooth extraction. Subsequent erythema, ulcerative necrotic lesions occurred on her left dorsal hand following frequent intravenous infusion, and lesions paradoxically expanded after surgical debridement. Initial biopsy and pathological consultation suggested SCC with positive surgica

Pyoderma gangrenosum caused by the molecular uncoupling of OTULIN catalytic activity and LUBAC binding.

The pathogenic mechanisms underlying pyoderma gangrenosum (PG) remain unclear. Here we report three patients with PG from two unrelated kindreds with homozygous R57C mutation of the linear deubiquitinase OTULIN. The patients have isolated, pediatric-onset, OTULIN-related PG (ORP). In contrast to OTULIN-related autoinflammatory syndrome (ORAS), caused by mutations affecting the catalytic domain, R57C affects the PUB-interacting motif with distinct biochemical, immunological and clinical consequences. OTULIN-R57C is catalytically active but is unable to bind the linear ubiquitin assembly complex

Infliximab-Responsive Severe Ulcerative Colitis With Pyoderma Gangrenosum After IL-6 and IGF-1 Blockade.

Interleukin-6 (IL-6) and insulin-like growth factor-1 (IGF-1) are involved in immune regulation and tissue repair, but their combined effects on gastrointestinal mucosa remain unclear. We report a case of new-onset ulcerative colitis with pyoderma gangrenosum after sequential IL-6 and IGF-1 receptor inhibition. The patient developed severe ulcerative colitis and pyoderma gangrenosum refractory to corticosteroids but showed rapid improvement after infliximab therapy, with endoscopic evidence of mucosal improvement. This case highlights a potential role for tumor necrosis factor-α (TNF-α) blocka

Disproportionality analysis of pyoderma gangrenosum reporting to the FDA Adverse Event Reporting System in association with antirheumatic biologics.

BACKGROUND: Several cases of pyoderma gangrenosum, a rare neutrophilic dermatosis, have been reported in patients initiating biological therapies for rheumatic health conditions. Despite this, systematic analysis across available therapies is lacking. This study investigated pyoderma gangrenosum reporting in association with antirheumatic biologics to the US Food and Drug Administration Adverse Event Reporting System (FAERS). METHODS: Reports from FAERS (2016-second quarter of 2025) were complied, deduplicated, and standardized. Pyoderma gangrenosum cases were classified according to the Medi
