Awareness Level of Herpes Zoster Infection Among the Residents of Jeddah, Saudi Arabia.

INTRODUCTION: Herpes zoster (HZ), also known as shingles, is a viral infection caused by the reactivation of the varicella-zoster virus. Awareness and knowledge of HZ and its associated complications vary across countries. Limited data are available on the awareness and knowledge of HZ among the general population in Saudi Arabia. Therefore, this study aims to measure awareness of HZ infection and its vaccine among the citizens of Jeddah, Saudi Arabia. METHODS: An online questionnaire was distributed to the citizens of Jeddah, Saudi Arabia, to assess their awareness and knowledge of HZ. The q

Herpes zoster ophthalmicus with atypical occipital lobe and splenial lesions: A case report.

A 32-year-old male with past medical history of migraine headaches (last episode 15 years prior) presented with persistent headache lasting 2 days and with associated nausea, photophobia and phonophobia concerning for migraine. Subsequent visits to outside facilities suggested a cortical/subcortical infarct of the left medial occipital lobe secondary to migraines. However, after continued symptoms and the subsequent development of a maculopapular rash along the right orbit and eyelid, suspicion grew that this was not a simple case of migraine induced infarct. Cerebral spinal fluid analysis sho

A self-assembling ferritin nanoparticle-based trivalent vaccine of gB/gE/gH elicits potent immune responses against varicella-zoster virus.

The causes varicella and herpes zoster, posing a significant global health burden. ZOSTAVAX (live attenuated) and Shingrix (protein subunit) are the main vaccines against varicella-zoster virus (VZV). However, ZOSTAVAX was linked to three recent deaths in Australia, and AS01b-adjuvanted Shingrix often causes side effects like fever and inflammation. Therefore, developing a second recombinant VZV vaccine is highly significant. Here, we developed a trivalent VZV vaccine by displaying glycoproteins gB, gE, and gH on 24-mer ferritin nanoparticles, enhancing immunogenicity and broadening epitope co

Varicella zoster virus and the central nervous system.

Varicella zoster virus (VZV) causes varicella, also known as chickenpox, after which VZV remains latent in neural ganglia. VZV reactivation can result in herpes zoster, also known as shingles. In addition to its well-known effects on the peripheral nervous system, reports increasingly suggest that VZV can have potentially devastating effects on the central nervous system (CNS). Several epidemiological studies indicate that VZV reactivation is associated with stroke and interest is growing in potential associations of VZV with dementia. In the past 5-6 years, vaccination against herpes zoster h

Impact of CD19 CAR T-Cell Therapy on Pathogen-Specific Antibody Titers in Lymphoma Patients.

BACKGROUND: CD19-directed chimeric antigen receptor (CAR) T-cell therapy has exhibited efficacy in treating relapsed/refractory high-grade B-cell malignancies, but off-tumor effects cause prolonged hypogammaglobulinemia and B-cell aplasia. Limited data exist on pathogen-specific IgG titers post-CD19 CAR T-cell therapy. METHODS: We evaluated the impact of CD19 CAR T-cell therapy on vaccine-preventable infectious disease IgG titers in 20 patients to assess humoral immunity. IgG titers for measles virus (MeV), mumps virus (MuV), rubella virus (RuV), varicella zoster virus (VZV), pneumococcal cap

Towards personalized vaccine repurposing for Alzheimer's prevention: genotype-specific protective association of the shingles vaccine with odds of Alzheimer's disease in participants of two large cohort studies.

BACKGROUND: Our earlier study found that adult vaccination against shingles (herpes zoster infection caused by the reactivation of the varicella-zoster virus) was associated with significantly reduced risk of Alzheimer's disease (AD) later in life. Here, we investigated genotype-specific associations between the shingles vaccine and the odds of AD using data from the Health and Retirement Study (HRS) and the UK Biobank (UKB). METHODS: This prospective case–control study included genotyped participants from the HRS (N = 9,188) and UKB (N = 66,748) cohorts, who survived beyond age 75. Multivaria

Sequential Varicella and Herpes Zoster in an Immunocompetent Adult.

Varicella and herpes zoster, caused by primary infection and reactivation of varicella-zoster virus (VZV) respectively, can present with a wide spectrum of cutaneous and systemic complications. This case highlights an uncommon sequential presentation of varicella and zoster in an immunocompetent adult in noncontiguous dermatomes and emphasizes the importance of clinicopathological correlation in atypical presentations.

Herpes Zoster and Frailty in Older Adults: a Systematic Review.

BACKGROUND: Herpes zoster (HZ) and its complications, postherpetic neuralgia (PHN), are common in adults, particularly the frail. Frailty may affect infection outcomes and vaccine efficacy. This study evaluated the association between frailty and HZ/PHN risk, and examined vaccine uptake, immunogenicity, and efficacy in frail versus non-frail individuals. METHODS: We systematically reviewed PubMed/MEDLINE, the Cochrane Library, Embase, and grey literature for studies published from January 2015 to January 2025. Eligible studies included observational and randomized controlled trials evaluating

Virus-induced onychomadesis: Exploring the role of viral infection in nail shedding.

Onychomadesis, characterized by proximal detachment of the nail plate due to temporary arrest of matrix proliferation, has been increasingly recognized as a complication following viral infections. Enterovirus-associated hand-foot-and-mouth disease (HFMD) is the most frequently reported cause. Recent studies demonstrate that some enteroviruses, including Coxsackievirus A10, utilize the host receptor KREMEN1 (KRM1) to impair Wnt/β-catenin signaling and suppress nail stem cell differentiation, thereby providing a molecular basis for infection-induced nail shedding. Additionally, cases of onychom

Review of Herpes Zoster Occurrence in Patients With Atopic Dermatitis Treated With JAK Inhibitors.

Atopic dermatitis (AD) is a chronic inflammatory skin disease that markedly impairs quality of life. Janus kinase inhibitors (upadacitinib, abrocitinib, and baricitinib) are effective therapies for moderate-to-severe AD but are associated with viral reactivation, particularly herpes zoster (HZ), due to immunosuppressive effects on interferon signaling and natural killer cell activity. The objective of this study was to systematically evaluate the risk of HZ among AD patients treated with JAK inhibitors (JAKis), identify key risk factors, and assess the clinical outcomes and management of HZ in

Bispecific Antibodies in B-Cell Lymphomas: Mechanisms, Efficacy, Toxicity, and Management.

Bispecific antibodies represent a pivotal advancement in treating relapsed/refractory B-cell lymphomas, addressing unmet needs for patients with limited conventional options. This review examines CD20 × CD3 bispecific antibodies (BsAbs) like mosunetuzumab, epcoritamab, odronextamab, and glofitamab, which link malignant B-cells and T-cells, thus inducing targeted tumor lysis. These IgG-like molecules activate T-cells, triggering proliferation and cytotoxic molecule release, bypassing MHC presentation. These agents have received regulatory approval for the treatment of various B-cell lymphomas a

Varicella-Zoster Virus and the Eye: Clinical Spectrum, Management, and Vaccination.

Varicella-Zoster Virus (VZV) is one of the most important pathogens in ophthalmology. Reactivation may involve the adnexa (blepharoconjunctivitis, pseudomembranous conjunctivitis), cornea (dendritic keratitis, nummular and necrotizing stromal keratitis, disciform endotheliitis, neurotrophic ulcers, mucous-plaque keratitis) and sclera (episcleritis, anterior scleritis). Uveal inflammation ranges from anterior uveitis-with iris atrophy, trabeculitis-induced glaucoma and complicated cataract-to posterior necrotizing syndromes: acute retinal necrosis in immunocompetent hosts and progressive outer

Amoxicillin-Clavulanate-Induced Pityriasis Rosea: A Pharmacovigilance Case Report.

Adverse drug reactions (ADRs) are a significant cause of morbidity and represent an important public health concern. Cutaneous adverse drug reactions (CADRs) are the most frequently encountered form of ADRs, with antibiotics being among the leading causative drug classes. Although amoxicillin-clavulanate is widely regarded as a safe and commonly prescribed antibiotic, rare and atypical cutaneous reactions may occur and pose diagnostic challenges. We report a rare case of amoxicillin-clavulanate-induced pityriasis rosea in a 34-year-old male who presented with a progressive pruritic and painful

Intravenous methylprednisolone as add-on induction therapy for chronic inflammatory demyelinating polyradiculoneuropathy: a randomised controlled trial.

BACKGROUND: Most patients with chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) require maintenance treatment. We aimed to determine whether combined treatment with intravenous immunoglobulin (IVIg) and intravenous methylprednisolone (IVMP) increases remission rates compared with IVIg and placebo. METHODS: We performed a randomised, double-blind, placebo-controlled study. Adults with CIDP who were treatment naive, relapsed after a remission or required continued treatment after a single IVIg dose were included. Participants were treated with IVMP (1000 mg) or placebo (sodium c

Effectiveness of adult zoster vaccination in preventing a first episode of zoster keratitis: A large-scale database study.

PURPOSE: The adult recombinant zoster vaccine (Shingrix) has demonstrated effectiveness in preventing herpes zoster ophthalmicus (HZO); however, its effectiveness in preventing zoster keratitis has not been directly evaluated. Given the wide variability in reported keratitis rates among patients with HZO and the potentially sight-threatening nature of this complication, this study was conducted to address this knowledge gap. METHODS: This retrospective, multicenter cohort study was conducted using a global federated electronic health record network. All patients were aged ≥50 years and had no

Post Varicella lung fibrosis: a lingering scar of viral pneumonia.

Varicella pneumonia occurs in 5%-15% of adults with primary varicella infection. While acute pulmonary complications of varicella pneumonia are well described, there are limited data on its long-term sequelae. We report a rare case of post varicella lung fibrosis that presented to us 2½ years after an uneventful recovery from varicella pneumonia. What makes this case unique is the demonstration of characteristic inclusion bodies in the histology specimen and the positive PCR test for varicella DNA, which substantiated the diagnosis and elucidated the possibility of long-term viral persistence.

Herpes Zoster Induced by the Shingrix Vaccine: A Case Report and Literature Review.

Herpes zoster (HZ) is a dermatological condition that is caused by the reactivation of the varicella zoster virus (VZV). The recombinant zoster vaccine (RZV; Shingrix, GlaxoSmithKline Biologicals, Rixensart, Belgium) is approved in the United States for the prevention of HZ reactivation in adults aged >50 years. We present a case of a 50-year-old immunocompetent female patient who developed HZ within the first 72 hours post Shingrix vaccination. The patient was evaluated by their primary care provider for a painful, vesicular rash, along with headache and fatigue. A diagnosis of Shingrix vacci
