Bullous Pemphigoid Possibly Induced by Deucravacitinib in a Patient with Psoriasis.

While bullous pemphigoid (BP) can be induced by various drugs, cases involving small-molecule inhibitors remain rare. We report the first case of BP occurring in a patient with psoriasis shortly after receiving deucravacitinib, a TYK2 inhibitor. Following treatment with methylprednisolone, minocycline, and ixekizumab, the lesions resolved within one month. Although the precise mechanism remains unclear, it may involve immunological drift and remodeling of skin immune microenvironment, which raises the need for clinical awareness regarding such reactions when using novel targeted immunomodulato

Increased risk of atrial fibrillation among patients with bullous pemphigoid: a nationwide cohort study in Taiwan.

BACKGROUND: Bullous Pemphigoid (BP) and Atrial Fibrillation (AF) share common underlying mechanisms involving chronic inflammation, but their relationship was unclear. OBJECTIVE: To assess the risk of incident AF in individuals with BP. METHODS: This nationwide cohort study utilized Taiwan's National Health Insurance Research Database from 2011 to 2019. Adults newly diagnosed with BP were identified and initially matched by age and sex to controls without BP. Stabilized inverse probability weighting was applied to balance baseline characteristics between cohorts. Cox proportional hazards mod

Extracellular vesicle-derived mir-148a-3p promotes plasmablast differentiation in bullous pemphigoid via BACH2 repression and mTOR activation.

Bullous pemphigoid (BP) is an autoimmune blistering disease characterized by the production of autoantibodies; however, the molecular mechanisms regulating plasma cell differentiation remain unclear. This study investigated the role of plasma-derived extracellular vesicles (EVs) and their microRNA cargo in regulating this process. EVs isolated from patients with BP and healthy controls were functionally assessed. BP-derived EVs significantly enhanced plasmablast differentiation, increased pathogenic autoantibody production, and elevated IL-6 secretion. High-throughput sequencing identified miR

Common Skin Diseases and Metabolic Syndrome: A Proinflammatory Chemokine Perspective.

Skin diseases frequently coexist with other disorders, such as metabolic syndrome, diabetes mellitus, depression, psoriatic arthritis, and cardiovascular disease. Altered levels of distinct chemokines, like CCL5/RANTES, CXCL12/SDF-1a, CCL7/MCP-3, CCL2/MCP-1, CXCL1/GROa, and the eotaxin family, contribute to the development and/or exacerbation of inflammation, which is a common feature of numerous skin diseases as well as metabolic syndrome. The pathological and molecular connections between chronic inflammatory skin diseases and metabolic syndrome are increasingly recognized as being driven by

Patient Perspectives on Treatment Outcomes and Priorities in Autoimmune Bullous Diseases: An Exploratory Survey among Dutch Patients.

INTRODUCTION: Autoimmune bullous diseases (AIBDs), comprising pemphigoid and pemphigus diseases, have seen limited therapeutic advances beyond rituximab for pemphigus vulgaris. As novel therapies are evaluated in clinical trials, well-defined, uniform, and relevant outcomes with patient involvement are essential. To date, however, patient-reported outcomes remain underrepresented, leaving uncertainty about whether trial results genuinely reflect patients' expectations. This study examines perspectives of patients with AIBD on treatment outcomes and priorities to ensure that future research foc

Bullous Pemphigoid Mimicking Infection in a Diabetic Foot: A Diagnostic Challenge.

Bullous pemphigoid (BP) is an autoimmune subepidermal blistering disorder that classically presents with tense bullae. In patients with diabetes and chronic wounds, atypical BP may mimic postoperative infection or ischemia, resulting in delayed diagnosis and unnecessary antimicrobial or surgical escalation. We report a 51-year-old man with poorly controlled type 2 diabetes and advanced chronic kidney disease who underwent partial first-ray amputation for wet gangrene. During postoperative wound healing, he developed widespread, sterile tense bullae on the feet and extremities. Clinical reasses

Risk of inflammatory dermatoses following COVID-19 infection: a cohort study of 204 241 patients.

BACKGROUND: The long-term impact of SARS-CoV-2 infection on inflammatory dermatoses remains incompletely defined. OBJECTIVES: To evaluate the association between COVID-19 infection and a broad range of inflammatory dermatoses. METHODS: In this cohort study, electronic health-record data were analysed for 204 241 patients seen at a tertiary medical centre between March 2020 and May 2023, including 22 368 with confirmed COVID-19 and 181 873 controls with negative testing. Diagnoses of 16 inflammatory dermatoses were extracted from standardized documentation fields and verified diagnostic codes

Cutaneous Toxicities of Small Molecules in Targeted Cancer Therapy Part II - Approach to Management and Treatment.

With the rapid expansion of small molecule inhibitors (SMIs) in oncology, the incidence and complexity of dermatologic adverse events (dAEs) have significantly risen1. While SMIs target essential oncogenic pathways, they frequently result in dermatologic toxicities that can impact patient quality of life and lead to dose reduction, treatment interruption, or discontinuation of life-prolonging therapy1. Effective management of these toxicities is critical not only for preserving skin integrity but also for ensuring ongoing oncologic treatment. Part II of this CME discusses guidance on oncoderma

Risk of Bias and Reporting Quality in BP180/BP230 ELISA Diagnostic Studies of Bullous Pemphigoid: A Meta-Epidemiological Study.

BACKGROUND: BP180 and BP230 enzyme-linked immunosorbent assays (ELISA) are widely used to support the diagnosis of bullous pemphigoid (BP), yet published sensitivity and specificity vary. We assessed whether methodological limitations and incomplete reporting may contribute to optimistic "headline" accuracy estimates. METHODS: We performed a meta-epidemiological methodological study of 18 published diagnostic accuracy studies (2004-2025) contributing 42 ELISA index tests (23 BP180, 19 BP230). Risk of bias was assessed with QUADAS-2 and reporting quality with a 16-item STARD-based checklist. W

Cutaneous Toxicities of Small Molecules in Targeted Cancer Therapy Part I - Mechanisms of Toxicity.

Advances in the understanding of cancer biology have led to a shift toward more precise, targeted anticancer therapies. Traditional chemotherapy acts as an intracellular poison to disrupt cell division or induce DNA damage resulting in cytotoxicity1. However, the effects of traditional chemotherapy are not relegated to neoplastic tissue, resulting in significant off-target cytotoxicity2. Targeted therapies were developed to provide a more specific approach to cancer treatment, focused on inhibiting the pathways involved in carcinogenesis and tumor growth. Small molecule inhibitors (SMIs) and a
