Emerging topical nonsteroidal therapies in pediatric dermatology: atopic dermatitis, psoriasis, alopecia areata, vitiligo, and seborrheic dermatitis.

PURPOSE OF REVIEW: To review the current literature for updates on pediatric nonsteroidal topical therapies (NSTT) for the treatment of atopic dermatitis (AD), psoriasis, alopecia areata (AA), vitiligo, and seborrheic dermatitis (SD). RECENT FINDINGS: The FDA approved difamilast and expanded the approved age ranges for roflumilast, tapinarof, and ruxolitinib to treat AD. Recent studies also show positive results with delgocitinib. In the field of psoriasis, current research shows promise for tapinarof and roflumilast in an expanded age range. Among new AA treatments, relevant literature suppo

Clinical Efficacy and Scalp Microbiome Changes Induced by AMPamide-Containing Shampoo in Patients With Seborrheic Dermatitis.

BACKGROUND: Seborrheic dermatitis (SD) is a chronic inflammatory scalp disorder associated with Malassezia dysbiosis and increased sebum production. AMPamide has been suggested to have anti-inflammatory and sebum-regulating effects, but its clinical efficacy and microbiome-modulating effects in SD remain unclear. OBJECTIVE: To evaluate the clinical efficacy and scalp microbiome changes following 4 weeks of use of an AMPamide-containing shampoo in patients with SD. METHODS: In this observational study, 30 patients with SD applied an AMPamide-containing shampoo for 4 consecutive weeks. Clinica

The Role of the Vagus Nerve and Its Stimulation in Modulating Inflammatory Skin Diseases and Other Dermatologic Conditions: From Mechanisms to Therapeutics.

BACKGROUND: Inflammatory skin diseases typically exhibit autonomic imbalance characterized by sympathetic dominance and parasympathetic hypofunction. The vagus nerve plays a fundamental role in modulating inflammatory responses through the cholinergic anti-inflammatory pathway, a neural circuit in which vagal efferent signals lead to acetylcholine release that suppresses pro-inflammatory cytokine production, and hypothalamic-pituitary-adrenal axis (HPA). SUMMARY: This review analyzes emerging evidence supporting vagus nerve stimulation (VNS) as a therapeutic approach for inflammatory skin dis

A Novel IFIH1 Mutation Causing Hyper-IgE Syndrome Presenting as Childhood Erythroderma: Case Report.

Erythroderma in the pediatric population is an uncommon yet challenging condition. The etiologies encompass a wide variety of illnesses, including seborrheic dermatitis, infections, inborn errors of immunity, ichthyosiform disorders and metabolic abnormalities. Hyper-immunoglobulin E syndrome (HIES) occurs due to various known genetic variants, e.g., STAT3 (signal transducer and activator of transcription-3), IL6R (Interleukin-6 receptor) deficiency and others, with an extensive list of features including erythroderma in childhood. Our case describes association of a possible novel mutation in

The follicular microbiome in hair-follicle-associated inflammatory diseases: A systematic review.

BACKGROUND: Hair follicles represent distinct cutaneous microenvironments that host microbial communities. Dysbiosis has been increasingly associated with inflammatory skin and hair disorders, but available studies were limited in scope and sample size, limiting confidence in the conclusions that can be drawn. OBJECTIVES: To assess the bacterial microbiome of hair follicles across inflammatory hair-follicle-associated skin and hair diseases and identify recurrent alterations. METHODS: We searched six databases, including MEDLINE and Embase, for studies published from 2007 to August 2024. Stu

Icariin Regulates Acne Pathophysiology and Extracellular Matrix Protein Synthesis: Implications for Acne Vulgaris and Atrophic Acne Scar Management.

Icariin, the principal active compound of Epimedium species used in traditional East Asian medicine, exhibits anti-inflammatory and lipid-regulating effects. However, its effects on human sebocytes and dermal fibroblasts, which could be implicated in the management of acne and atrophic scarring, remain unexplored. This study aimed to evaluate the effects of icariin on human sebocytes, keratinocytes and dermal fibroblasts. Icariin's effects on cell viability, lipid synthesis, inflammation, abnormal keratinization and extracellular matrix (ECM) synthesis were assessed using human SEB-1 sebocytes

Perifollicular fibrosis and inflammation in androgenetic alopecia and seborrheic dermatitis: diagnostic challenges in differentiation from fibrosing alopecia in a pattern distribution.

BACKGROUND: Androgenetic Alopecia (AGA) and Seborrheic Dermatitis (SD) are scalp conditions that may show histopathological features overlapping with Fibrosing Alopecia in a Pattern Distribution (FAPD), potentially leading to diagnostic confusion between non-scarring and scarring alopecias. OBJECTIVE: To identify histopathological features that distinguish AGA from SD and to evaluate findings overlapping with those described in FAPD, focusing on perifollicular fibrosis and inflammation. METHODS: Transverse scalp biopsy sections from 56 Caucasian male patients (21 AGA, 35 SD) were blindly eva
