Allergic contact dermatitis (ACD) to personal care products (PCPs) represents a significant and growing clinical challenge, with increasing use of diverse PCPs and their expanding range of ingredients. Common allergenic components include fragrances, botanicals, preservatives, surfactants, emollients, emulsifiers, vitamins, and ultraviolet filters. Patch testing remains the gold standard for diagnosing ACD and identifying causative allergens, but traditional panels often fail to capture emerging PCP-related allergens. This review highlights key established and emerging PCP allergens, such as l

BACKGROUND: Fragrance and preservative chemicals in cosmetic and household cleaning products are recognised causes of allergic contact dermatitis.
OBJECTIVES: To identify fragrance and preservative ingredients with potential contact allergenicity in Turkish market products.
METHODS: Labels of 245 products, including predominantly rinse-off cosmetic products and household cleaning products obtained during on-site visits to chain supermarkets were reviewed. Fragrance and preservative ingredients were scored (0-2) for reported allergen potential and summed as an 'allergen load' per product.

The use of cosmetic products is ubiquitous around the world. This article reviews the medical literature documenting adverse effects associated with cosmetic product use. An extensive, nonexhaustive literature search was conducted using the PubMed database. Studies reporting adverse effects from cosmetic products were included, encompassing case reports, case series, clinical trials and epidemiological studies. Contact dermatitis is the most common adverse effect associated with cosmetic use, affecting approximately 1-5% of the general population. Additional effects, including respiratory sens

BACKGROUND: Propylene glycol (PG) is widely used in cosmetic and pharmaceutical products, yet data on the prevalence of contact allergy to PG and related glycols, and their potential cross-reactivity, remain limited.
OBJECTIVES: To determine the prevalence of glycol contact allergy, describe clinical characteristics and assess potential cross-reactivity.
METHODS: We retrospectively analysed 1979 patients patch tested at Antwerp University Hospital between January 2021 and December 2024 who were tested with at least one glycol.
RESULTS: Fifty-five patients (2.79%) were allergic to PG. Detect

I investigated the key causes and mechanisms of skin sensitization, evaluated existing diagnostic methods, and proposed new strategies to reduce the risk for allergic reactions. My research focused on sensitization to cosmetic components, including synthetic fragrances, preservatives (i.e., methylisothiazolinone, chlorphenesin), and natural extracts. I used patch testing and molecular diagnostics based on pro-inflammatory markers (i.e., interleukin-6, tumor necrosis factor-alpha). Positive reactions occurred in 69.2% of participants, with synthetic fragrances (51.7%) and methylisothiazolinone

Carmine is a natural red dye derived from cochineal insects and is widely used in cosmetics and food products. Since carmine is derived from insects, individuals who follow vegan or vegetarian lifestyles and those who have religious beliefs may wish to ascertain its presence in the products they use. Additionally, carmine causes angioedema, urticaria, contact dermatitis, systemic anaphylaxis and even asthma in hypersensitive individuals. Conventional methods for detecting the presence of carmine require expensive equipment and skilled personnel; thus, robust, low-cost and rapid detection is ne

Background Pigmented contact dermatitis (PCD) is a pigmentary variant of contact dermatitis characterized by gradually developing facial hyperpigmentation with minimal or absent overt dermatitis. Owing to its close resemblance to other causes of facial melanosis, PCD is frequently underdiagnosed. Patch testing plays a crucial role in identifying causative allergens and guiding avoidance-based management. Objectives To evaluate the clinical profile, dermoscopic features, and patch test allergen spectrum in patients with PCD attending a tertiary care hospital. Methods This prospective observatio

Disseminated superficial actinic porokeratosis (DSAP) is a rare, UV-associated keratinization disorder that causes significant cosmetic impairment. In recent years, a new pathogenesis-based therapeutic approach using topical formulations containing statins and cholesterol has been introduced. We report on a 56-year-old woman who had DSAP on both forearms and lower legs for 6 years. Clinically, she presented with multiple, extensor-surfaced, pale erythematous, flat plaques with collarette-like scaling. Previous treatments with topical tretinoin and photodynamic therapy had not resulted in any s

BACKGROUND: Isothiazolinones are responsible for an epidemic of allergic contact dermatitis across Europe over the past 2 decades. Regulatory restrictions on methylisothiazolinone (MI) in cosmetics have been followed by declining sensitization rates in surveillance networks. Benzisothiazolinone (BIT) and octylisothiazolinone (OIT) have emerged as sensitizers of growing clinical relevance.
OBJECTIVE: To characterize temporal trends in sensitization to MCI/MI, MI, BIT, and OIT over more than 2 decades at a single center in Gran Canaria, Spain.
METHODS: Cross-sectional study of patients patch-t

Animal tests like the guinea pig maximization test (GPMT) and murine local lymph node assay (LLNA) were historically used to assess skin sensitization in cosmetics. The EU banned animal testing for cosmetics in 2013, prompting the development of alternative in vitro methods and new approach methodologies (NAMs) based on adverse outcome pathways (AOPs). These NAMs are integrated into Defined Approaches (DAs) to evaluate skin sensitization hazards and potency, which challenge is incorporating them into next-generation risk assessment (NGRA) frameworks. To address this challenge, we aim to achiev

BACKGROUND: Glove occlusion increases skin susceptibility to irritants ('occlusion effect'). Previous experimental data showed that applying protective creams (PCs) before glove occlusion does not prevent this effect and may even aggravate irritant responses.
OBJECTIVES: To assess whether the application of PCs after glove occlusion modifies the occlusion effect.
METHODS: In a randomized, single-blind (participants were blinded) study, 68 healthy volunteers were examined over 10 days. Two commercially available PCs (A, B) and one standardized model cream (C) were applied after occlusion for

BACKGROUND: Epidermal Langerhans cells (LCs) play a critical role in the induction of contact hypersensitivity. The LCs leave the skin, move to the regional lymph nodes and present the allergens embedded in the HLA-DR molecule to naive T-lymphocytes. To allow LC emigration from the epidermis, E-cadherin must be downregulated. In this study, we have examined the early events that occur in the human epidermis after exposure to three strong contact sensitizers and two commonly used fragrances by examining alterations of E-cadherin and HLA-DR expression.
METHODS: To determine whether E-cadherin a

PURPOSE OF REVIEW: Clinical management of allergic contact dermatitis (ACD) is increasingly challenging due to the growing number of allergens, complex exposure patterns, and evolving regulatory frameworks. This review aims to provide clinicians with an updated overview of well-known as well as new and emerging allergens relevant to the diagnosis and management of ACD. RECENT FINDINGS: Recent literature highlights changing exposure patterns and the identification of several new and emerging allergens, particularly in rapidly expanding consumer markets such as cosmetics and products associated

Benvitimod is a non-steroidal tyrosine protein kinase inhibitor commonly used to treat psoriasis. However, it can cause adverse reactions such as contact dermatitis and itching, which can seriously impact consumer health. This study involved designing and synthesising a novel fluorescent labeling reagent, 2-((7-nitrobenzo[c][1,2,5]oxadiazol-4-yl)oxy)benzoic acid (NBD-SA). This reagent is used to detect benvidimod in cosmetics using high-performance liquid chromatography with fluorescence. With the help of 1-ethyl-3-(3-dimethylaminopropyl)carbodiimide hydrochloride/4-dimethylaminopyridine catal

Contact allergy related to use of cosmetic products remains a considerable problem in the European Union population. The objectives of this study were to identify pitfalls in EU risk assessment methodologies and regulation(s) for cosmetic products and identify areas of potential improvements in future regulatory strategies. Two significant contact allergens, methylisothiazolinone and hydroxyisohexyl 3-cyclohexene carboxaldehyde, were investigated as examples. Other contact allergens were used to provide additional context. Opinions from the European Commission's scientific committees and regul
