Comparison of efficacy and safety of non-oral therapeutic interventions for zoster-associated pain: a systematic review and network meta-analysis.

BACKGROUND: Zoster-associated pain (ZAP) encompasses acute, subacute, and postherpetic neuralgia stages. It often results in persistent sensory abnormalities and substantial impairment of quality of life. Although oral pharmacotherapy remains the first-line and foundational approach, its effectiveness may be limited in some patients; accordingly, non-oral interventions are investigated as complementary or escalated strategies. However, high-quality evidence investigating the relative efficacy and safety of these interventions remains scarce. OBJECTIVE: This study intended to systematically ev

Pulmonary Cryptococcosis With Suspected Central Nervous System Involvement in a Patient With Sarcoidosis and CD4 Lymphopenia: A Case Report.

Sarcoidosis is a multisystem granulomatous disorder associated with immune dysregulation and impaired cell-mediated immunity, which may increase susceptibility to opportunistic infections, including cryptococcosis. The clinical and radiological overlap between sarcoidosis progression, neurosarcoidosis, and cryptococcal infection can create a significant diagnostic challenge, particularly when neurological symptoms are present. We present a 50-year-old male with a 15-year history of pulmonary sarcoidosis and CD4 lymphopenia who developed pulmonary cryptococcosis with subsequent symptoms concern

Neurologic Complications of Varicella-Zoster Virus Reactivation: Clinical and Imaging Features.

Varicella-zoster virus (VZV), a neurotropic α-herpesvirus, establishes lifelong latency in the nervous system after primary infection. Reactivation leads to herpes zoster (HZ), which can result in severe neurologic complications including postherpetic neuralgia, vasculopathy, encephalitis, meningitis, myelopathy, cranial neuropathy, and optic neuritis. These complications may occur even in the absence of a characteristic rash, making early recognition crucial for timely antiviral therapy to maximize the likelihood of a good outcome. Clinical manifestations of VZV-associated neurologic disorder

Neonatal Varicella-Zoster Virus Meningitis Without Pleocytosis: Diagnostic Value of Polymerase Chain Reaction.

Neonatal varicella is uncommon but may lead to severe complications, including central nervous system involvement. Cerebrospinal fluid findings may be misleading in neonates, and polymerase chain reaction-confirmed viral central nervous system infection may occur without pleocytosis, particularly when lumbar puncture is performed early in the course of illness. We report a 17-day-old term male neonate admitted with generalized vesicular rash and fever associated with irritability and poor feeding. Lumbar puncture performed on admission showed normal cerebrospinal fluid parameters without pleoc

Body Under Attack: Disseminated Varicella-Zoster Virus Infection.

Varicella-zoster virus (VZV) infection is typically benign and self-limiting. In adults, it most commonly manifests as a localized cutaneous infection. However, in immunocompromised individuals, VZV can undergo hematogenous dissemination and cause severe visceral involvement, including pulmonary, hepatic, pancreatic, cardiac, and central nervous system complications. Disseminated disease may be further complicated by multiorgan failure, acute respiratory distress syndrome (ARDS), and coagulopathy and is associated with significant mortality. We report a 64-year-old man who presented to the eme

Rashless varicella-zoster virus encephalitis diagnosed by metagenomic next-generation sequencing: two case reports.

BACKGROUND: Varicella-zoster virus (VZV) can cause a range of central nervous system (CNS) infections, but early diagnosis is difficult when typical skin rash is absent. Rashless VZV encephalitis may present with nonspecific clinical, cerebrospinal fluid (CSF), and neuroimaging findings and can mimic autoimmune encephalitis, primary central nervous system lymphoma, or other disorders. We report two cases of rashless VZV encephalitis diagnosed by CSF metagenomic next-generation sequencing (mNGS), with subsequent neurological complications. CASE PRESENTATION: Case 1 was a 68-year-old man admitt

Multimodal prognostication of autoimmune encephalitis: an Australian autoimmune encephalitis consortium study.

BACKGROUND AND OBJECTIVES: To identify factors predictive of a favourable modified Rankin score (mRS) at 12 months in patients with autoimmune encephalitis (AE). To evaluate predictors of a binary composite clinical-functional outcome measure, encompassing mRS, drug-resistant epilepsy (DRE) and memory impairment, at 12 months. METHODS: Univariable and multivariable logistic regression analyses for predictors of a favourable mRS (i.e. mRS ≤ 2) and a composite clinical-functional outcome at 12 months were used. RESULTS: A total of 231 patients with AE were recruited. Multivariable logistic reg

Can the herpes zoster vaccination be a strategy against dementia?

Herpes Zoster (HZ), caused by the reactivation of the varicella-zoster virus (VZV), is a common infectious disease. Recent studies suggest a potential association between HZ and the development of dementia, particularly Alzheimer's disease and other neurodegenerative disorders. Epidemiological evidence indicates that HZ infection, especially in individuals with central nervous system involvement, may increase the risk of dementia. Pathologically, VZV may contribute to neuronal dysfunction and degeneration by inducing chronic neuroinflammation, infection-related cerebrovascular lesions, and dir

Post-herpetic Abdominal Pseudohernia With Underrecognized Characteristics of Male Predominance and Co-occurring Fatal Visceral Herniation: A Case Report and a Literature Review.

Post-herpetic abdominal pseudohernia (PHAP) is a rare complication associated with herpes zoster (HZ), characterized by transient paresis of the myotome ipsilateral to the affected segmental nerves. Clinically, it manifests as asymptomatic laxity and protrusion of the abdominal musculature, mimicking abdominal wall herniation. While the varicella-zoster virus (VZV) primarily affects the sensory nervous system, the incidence and prognosis of motor involvement remain poorly estimated. Despite PHAP's self-limiting nature with a favorable prognosis in most cases, the disease requires a careful dif

Atypical Presentation of Varicella-Zoster Virus Encephalitis in an Elderly Immunocompetent Adult: Early Diagnosis and Positive Outcome Following Treatment.

Varicella encephalitis following herpes zoster ophthalmicus (HZO) is a rare but serious complication that can occur due to the reactivation of the varicella-zoster virus (VZV). HZO involves the ophthalmic division of the trigeminal nerve, typically presenting with a dermatomal rash, and can lead to various ocular complications. In some cases, this reactivation can extend to the central nervous system, resulting in encephalitis, which can lead to significant morbidity and mortality, particularly in immunocompromised or elderly individuals. This report describes a case of VZV encephalitis compli

The Interplay Between Varicella-Zoster Virus and Giant Cell Arteritis: An In-Depth Narrative Review.

Giant cell arteritis (GCA) is a large-vessel vasculitis that is commonly associated with inflammation of the blood vessels within the head and neck. The condition primarily affects older and immunocompromised individuals and is diagnosed through a combination of physical examination, laboratory tests, and, in some cases, biopsy. Most patients present with headaches, visual disturbances, and jaw claudication accompanied by low-grade fever, fatigue, and weight loss. Varicella-zoster virus (VZV) may play a role in the pathogenesis of GCA. VZV DNA has been identified within the cerebral arteries o

Rhombencephalitis associated with varicella-zoster virus masquerading as Guillain-Barré syndrome.

BACKGROUND: Although rare, rhombencephalitis or inflammation of the brainstem and cerebellum has significantly associated morbidity and mortality. CLINICAL FEATURES: We describe the case of a 64-year-old previously healthy, immunocompetent man who presented with acute bulbar symptoms (difficulty swallowing, change in phonation, and drooling). His symptoms progressed in severity and he ultimately required intubation due to declining lung function and inability to manage his secretions. Clinical examination revealed bulbar dysfunction without involvement of other cranial nerves. A lumbar punctu

Coagulation functions and factors correlated with central nervous system infection in herpes zoster patients.

OBJECTIVE: Reports on central nervous system (CNS) infection caused by varicella-zoster virus (VZV) reactivation are increasing, but its pathogenesis remains unclear, which causes delayed diagnosis and treatment. Some studies suggested that hypercoagulability is involved in the pathogenesis of CNS infection of VZV. This study investigated the coagulation parameters of herpes zoster (HZ) and their correlations with the VZV-related CNS infection, and provided a reference for the early diagnosis and treatment. METHODS: We selected 123 consecutive patients, including 95 HZ cases and 28 VZV mening

CSF Beta-Synuclein, SNAP-25, and Neurogranin in Infectious and Autoimmune Inflammatory Neurologic Diseases.

BACKGROUND AND OBJECTIVES: Beta-synuclein (beta-syn), synaptosomal-associated protein 25 (SNAP-25), and neurogranin are CSF biomarkers of synaptic damage, which have been poorly investigated in non-neurodegenerative neurologic diseases. In this study, we examined the diagnostic and prognostic role of these markers compared with the neuroaxonal damage marker neurofilament light chain protein (NfL) in infectious and autoimmune inflammatory neurologic diseases (IINDs and AINDs). METHODS: This cohort study included CSF samples from patients with different etiologies of IIND (varicella-zoster viru

Disseminated Nocardiosis: Poorly Controlled Diabetes as an Immunocompromising State.

Nocardia species are environmental pathogens that cause rare infections, primarily affecting immunocompromised individuals. These infections can present as pulmonary, cutaneous, or disseminated diseases, often involving the central nervous system. The most common portal of entry is via inhalation or skin breakdown; it is often found in soil, water, and vegetable matter. While typically associated with organ transplantation, malignancy, and immunosuppressive therapy, this case highlights how poorly controlled diabetes may also function as an immunocompromised state, therefore increasing suscept

Refractory acute retinal necrosis presenting as peripapillary choroiditis - A diagnostic and therapeutic challenge.

This report describes a challenging case of refractory acute retinal necrosis (ARN) with peripapillary choroiditis as the initial presenting sign. Imaging studies confirmed multifocal choroidal lesions and noted the novel sign of vertical hyper-reflective strips (VHRS) in the outer nuclear layer. Initial treatment with acyclovir and valacyclovir failed, and involvement of the other eye suggested a resistant variant of varicella zoster virus. High-dose oral famciclovir therapy resulted in rapid resolution of ARN in both eyes. This case highlights the rare occurrence of choroiditis, the novel fi

[Left mandibular osteonecrosis following herpes zoster of the third branch of left trigeminal nerve: A case report].

Herpes zoster of trigeminal nerve was a common skin disease caused by varicella-zoster virus infection. Simple involvement of the third branch of trigeminal nerve was rare, and so were oral complications such as pulpitis, periodontitis, spontaneous tooth loss, bone necrosis, etc. This article presented a case of herpes zoster on the third branch of the left trigeminal nerve complicated with left mandibular osteonecrosis. We reported the case of a 64-year-old man with sudden pain in the left half of the tongue 1 month ago, and then herpes on the left facial skin appeared following with acute pa
