Off-Label Uses of Nemolizumab: A Narrative Review.

BACKGROUND: Nemolizumab, a humanized monoclonal antibody targeting interleukin-31 receptor α (IL-31Rα), is approved for atopic dermatitis (AD) and prurigo nodularis (PN) in adults and pediatric patients 12 years and older, following phase 3 trials demonstrating significant reductions in pruritus and disease severity in patients inadequately controlled with topical therapies. Emerging evidence has demonstrated rapid and sustained pruritus reduction in diverse pruritic conditions beyond its approved indications. METHODS: We reviewed the literature from 2021 to 2025 examining off lab

Dermoscopy of Prurigo Nodularis/Lichen Simplex Chronicus of the Scalp: A Comparative Observational Study in Fair and Dark Skin.

INTRODUCTION: Although recognition of prurigo nodularis (PN) and lichen simplex chronicus (LSC) of the scalp is usually straightforward, they may sometimes pose difficulties in terms of differential diagnosis with other similar dermatoses. The aim of this observational retrospective study was to assess dermoscopic features of PN and LSC of the scalp across fair- and dark-skinned individuals and compare them with those of clinical mimickers. METHODS: Fair-skinned (Fitzpatrick phototypes I-III) and dark-skinned (Fitzpatrick phototypes IV-VI) patients with a histological diagnosis of PN/LSC of t

Clinical spectrum and quality of life burden of vulvar lichenoid dermatoses: a cross-sectional study of 1177 patients.

BACKGROUND: Vulvar lichen sclerosus (VLS), vulvar lichen simplex chronicus (VLSC) and erosive vulvar lichen planus (EVLP) are the most common vulvar lichenoid dermatoses. They are frequently misdiagnosed in clinical practice, and comparative studies of these diseases remain limited. OBJECTIVES: To analyse the clinical characteristics and quality of life (QoL) burden of these three distressing vulvar diseases and to raise awareness of them. METHODS: This retrospective cross-sectional study analysed clinical registration data of adult patients diagnosed with VLS, VLSC or EVLP at a single-centr

Fractional CO2 Laser Combined With Topical Corticosteroids for Vulvar Lichen Simplex Chronicus: An Observational Study From an 8-Year Single-Center Experience.

BACKGROUND: Vulvar lichen simplex chronicus (VLSC) is a chronic inflammatory dermatosis that remains difficult to manage long term. Fractional CO2 laser (FxCO2) combined with topical corticosteroids (TCs) has emerged as a potential therapeutic strategy; however, high-quality long-term evidence remains limited. OBJECTIVE: To evaluate the long-term efficacy and safety of FxCO2 combined with TC in patients with VLSC. STUDY DESIGN: This retrospective single-center study included 172 patients with biopsy-confirmed VLSC treated between 2016 and 2024. All patients received at least one FxCO2 + TC t

Retrospective Study of the Risk of Progression to Squamous Cell Carcinoma of Vulvar Lichen Planus Forms Isolated or Associated With Other Dermatoses.

AIMS: The aim of the study was to examine the potential risk of vulvar lichen planus (LP), alone or associated with lichen sclerosus (LS) or lichen simplex chronicus (LSC), to evolve towards vulvar cancer (VSCC). METHODS: We retrospectively investigated the incidence of vulvar cancer in women diagnosed with LP, alone or associated with LS and LSC, between 2007 and 2022. RESULTS: We retrieved the data of 77 women with LP with a mean age of 63.5 ± 11.9 years old. Of these, 53 had LP, 19 had LP+LS, and 5 had LP+LSC. Ultrapotent topical corticosteroids were the first-line treatment. The mean fol

Vulvar Dermatitis: Diagnosis and Management.

Candidiasis, herpes simplex virus, bacterial vaginosis, irritant contact dermatitis, allergic contact dermatitis, psoriasis, and eczema/lichen simplex chronicus are often misdiagnosed, mistreated, and underrecognized causes of vulvar discomfort. Vulvar discomfort is frequently encountered in dermatology, gynecology, urology, and primary practice. A lack of ownership among specialties often leads to significant knowledge gaps among these providers and delays in care. Clinical symptoms of vulvar dermatitis can overlap, posing a diagnostic challenge. Herein, we provide a tool to guide the diagnos

BACKGROUND: Vulvar lichen sclerosus (VLS), vulvar lichen planus (VLP), and vulvar lichen simplex chronicus (VLSC) are inflammatory dermatoses with overlapping clinical features, posing diagnostic challenges. VLS and VLP carry malignant potential, unlike VLSC. Comparative studies are limited despite frequent misclassification. OBJECTIVE: To compare the clinical characteristics of VLS, VLP, and VLSC and identify distinguishing diagnostic features. METHODS: A retrospective study was conducted on clinical data from patients diagnosed with VLS, VLP, or VLSC at the Beijing Hospital Vulvar Clinic (

Adjunctive Habit Reversal Therapy in Treatment of Lichen Simplex Chronicus (Neurodermatitis): A Comparative, Pilot Study.

BACKGROUND AND OBJECTIVES: Lichen simplex chronicus (LSC) is a common dermatosis with a habitual itch-scratch cycle and associated psychological elements. Therefore, psychological methods, such as habit reversal, to break the scratching behaviour may have a therapeutic benefit in this condition. Our objective in this study was to assess additional clinical effects of habit reversal therapy when combined with standard therapeutic measures (i.e. topical corticosteroids, antihistamines, and emollients). PATIENTS AND METHODS: Thirty-four patients with LSC were divided into two groups: group A (st

Beyond the Scalp: Bilateral Hand Trichoteiromania Revealed by Trichoscopy - A Case Report.

INTRODUCTION: Trichoteiromania is a friction-induced hair shaft disorder caused by repetitive rubbing/scratching and is often considered the hair shaft expression of lichen simplex chronicus ("scratching-pseudo-alopecia"). It is classically described on the scalp and presents with lichenified plaques and characteristic trichoscopy such as broom hairs and V-hairs. Pruritus or an urge to rub is common and may help distinguish it from trichotillomania (hair pulling), which is often asymptomatic. To date, the condition has been described almost exclusively as a scalp disorder. CASE PRESENTATION:

Discoid Lupus Erythematosus in Genital Skin: A Case Report and Review of the Literature.

BACKGROUND: Discoid lupus erythematosus (DLE) represents the most common variant of chronic cutaneous lupus erythematosus, a skin-specific autoimmune disorder within the broader lupus erythematosus spectrum Clinically, DLE is characterized by erythematous, scaly plaques with areas of hyperpigmentation. These lesions predominantly occur on sun-exposed areas, particularly the scalp and cheeks. Involvement of the genital skin is rare and poses a significant diagnostic challenge due to its clinical overlap with other inflammatory and infectious dermatoses. To date, only 14 cases involving genital

Not All That Itches Is DRESS: A Case Report of Lichenified Dermatosis and Hypereosinophilia Associated With Tocilizumab.

Drug-induced hypereosinophilic syndromes (HESs) have been associated with a variety of medications, including biologic agents. We present a case of an 85-year-old female with a history of giant cell arteritis (GCA) complicated by ascending aortic aneurysm and aortitis, who developed marked peripheral eosinophilia (peaking at 3710/µL) and a persistent pruritic papular rash during long-term tocilizumab therapy. Extensive infectious, hematologic, and immunologic evaluations were unrevealing. While the clinical picture raised concern for a drug-induced hypersensitivity reaction, histopathology was

Targeting type 2 inflammation in dermatology: mechanisms and clinical implications.

Type 2 inflammation is a fundamental immunologic pathway underlying a growing spectrum of dermatologic diseases. While classically associated with atopic dermatitis, type 2 immune responses are increasingly recognized as key contributors to prurigo nodularis, chronic spontaneous urticaria, bullous pemphigoid, lichen simplex chronicus, and other chronic pruritic disorders. Barrier disruption and epithelial injury initiate the release of alarmins, including thymic stromal lymphopoietin (TSLP), interleukin (IL)-33, and IL-25, which activate innate and adaptive immune pathways that promote type 2

Intraepithelial Penile Lesions.

Intraepithelial penile lesions encompass non-neoplastic and preneoplastic lesions. Non-neoplastic lesions comprise condyloma acuminatum, including giant condyloma acuminatum, also known as Buschke-Löwenstein tumor. It is usually the low-risk human papillomavirus (HPV) types 6 and 11 that are most prevalent in penile condylomas. However, high-risk HPV types can be detected along with low-risk types in a subset of patients. Penile intraepithelial neoplasia (PeIN) is a preneoplastic lesion that is either HPV-associated or HPV-independent. HPV-associated PeIN represents the majority of PeIN in reg
