The history of entamoebiasis.

This review article summarizes the history of amoebic dysentery (entamoebiasis) caused by Entamoeba histolytica. Initially, Entamoeba species were thought to be the most primitive extant eukaryotes, but more recent research revealed that they emerged relatively late in evolutionary history. Paleoparasitological data suggest that E. histolytica has been a parasite of humans since ancient times and was probably spread throughout the world by man during early human migration. By the end of the 19th century, it was established that E. histolytica was the etiological agent of amoebic dysentery and

Multidisciplinary Management and Clinical Outcomes in Amebic Liver Abscess: A Prospective Study From a Tertiary Care Center.

Background An amebic liver abscess (ALA) remains a major public health concern in endemic regions, with alcohol consumption as a key predisposing factor. Despite advances in diagnostics and therapeutics, delayed diagnosis and inadequate management continue to contribute to patient morbidity. This study evaluates the clinical features, hepatobiliary involvement, and outcomes associated with a structured treatment approach in ALA. Methods This prospective observational study included 50 patients diagnosed with ALA at a tertiary care center between 2022 and 2024. Demographic data, clinical presen

Amoebic liver abscess co-infected with bacterial liver abscess: A rare case in an immunocompromised patient.

We present a complex case of a 43-year-old HIV-positive Chinese male with co-infection of amoebic liver abscess (ALA) and bacterial liver abscess caused by Salmonella enterica subsp. enterica serotype Typhi (abbreviated as Salmonella Typhi). The patient presented with fever and abdominal pain. Initial bacterial cultures identified Salmonella Typhi, but targeted antibiotic therapy failed to resolve his symptoms, prompting to further investigation. Metagenomic next-generation sequencing (mNGS) of pleural and liver abscess drainage fluids revealed sequences of Entamoeba histolytica, confirming a

Amebic Liver Abscess with Inferior Vena Cava Thrombosis-Case Report and Review of Literature.

The clinical evolution and resolution of amebic liver abscess (ALA) is usually unremarkable. However, factors such as malnutrition, alcoholism, and smoking increase the risk of complications that may be seen with ALA. We present a case of inferior vena cava thrombosis as a complication of ALA. Timely anticoagulation therapy along with antibiotics was given which led to rapid resolution of symptoms.

Unmasking Amoebiasis: An Unexpected Cause of Colitis in a Non-endemic Region.

Amoebic colitis is a common mimic of inflammatory bowel disease (IBD), primarily encountered in developing countries. We present a case of a 73-year-old British male with no travel history to any amoebic endemic regions, who presented with a three-month history of per rectal (PR) bleeding, diarrhoea, and a positive faecal immunochemical test (FIT). Prior to this, he reported no history of experiencing any gastrointestinal symptoms. Colonoscopy revealed patchy pan-colitis, most marked in the ascending colon, and histology confirmed chronic inflammation. A diagnosis of IBD was made, and the pati

Three Autochthonous Cases of Amoebic Liver Abscess Clustered in a Small Village of Tuscany (Central Italy), a Non-Endemic Area.

Amebiasis is a rare condition in industrialised countries but is epidemiologically growing. Clinical manifestations may range from asymptomatic to invasive disease. An amebic abscess can be the result of extraintestinal amebiasis, and it is associated with relatively high morbidity and mortality. We present three indigenous cases of amoebic liver abscesses observed within a few weeks (October-November 2023) in patients living in a small area near Lucca in Tuscany, Central Italy. Fever accompanied by abdominal pain and liver abscess was observed in all three patients, and one of them presented

Amebic liver abscess rupture into inferior vena cava with pulmonary thrombo embolism: a case report.

BACKGROUND: Vascular complications such as hepatic vein and inferior vena cava thrombus, phlebitis, and extrahepatic biliary obstruction are rare in amebic liver abscesses. Many pathophysiological mechanisms are proposed, but the exact pathogenesis is still not known. These complications are believed to be multifactorial, with local and systemic factors acting synergistically. Here, we present a case of amebic abscess rupture into the inferior vena cava, with a thrombus extending up to the right atrium and pulmonary thromboembolism treated successfully. CASE PRESENTATION: A 45-year-old Indian

Metronidazole-induced cerebellar dysfunction: An underdiagnosed identity.

Metronidazole is a commonly used antibiotic in clinical practice. It is associated with neurological toxicity if used for a longer duration or at a higher dose. In our case, the patient was prescribed metronidazole for the treatment of amoebic liver abscess for 7 weeks and he developed metronidazole-induced cerebellar dysfunction and small fibre peripheral neuropathy. Characteristic MRI findings, temporal association of symptoms with drug and resolution on drug withdrawal helped to make the diagnosis. Our case highlights the need of avoidance of prolonged administration of metronidazole and ad

Case Report: Percutaneous catheter drainage in the treatment of amebic liver abscess with syphilis infections - a case study and literature review.

INTRODUCTION: Amebic liver abscesses (ALAs) result from Entamoeba histolytica, a protozoan parasite transmitted through contaminated food or water. Diagnosis relies on imaging and serology, and treatment typically involves antibiotics such as metronidazole. CASE PRESENTATION: Herein, we present a case of a 47-year-old man who presented with hepatitis B virus and syphilis infections during admission. Laboratory tests and computed tomography scan revealed a liver abscess. Percutaneous catheter drainage (PCD) was conducted on the day of admission. Positive amebiasis titers confirmed ALAs. Althou

High-density lipoprotein cholesterol at admission reflects disease course, severity, and influences management decisions in patients with amoebic liver abscess.

Amoebic liver abscess (ALA) remains a major health burden in tropical regions. Our observational study evaluated the association between serum lipid profiles and disease severity in 118 patients with ALA. Patients were classified into uncomplicated and complicated ALA groups. Serum high-density lipoprotein cholesterol (HDL-C) levels were significantly lower in ALA patients compared to controls and further reduced in complicated cases. HDL-C emerged as an independent predictor of complicated ALA and demonstrated better predictive value than conventional inflammatory markers such as total leucoc

Amoebic Liver Abscesses in Returning Travelers: Lessons to Keep in Mind From a Nonendemic Area.

Despite being relatively common in endemic settings, amoebic liver abscesses are rarely diagnosed in high-resource countries, where they are usually imported by migrants or travelers to endemic areas. Between 2010 and 2024, we observed two cases of amoebic liver abscess that were successfully managed in a tertiary-care center in Italy. In both cases, the infection was contracted in Colombia. Given the high morbidity and mortality associated with nontreated amoebic liver abscesses, this report aims at underlying the importance of travel history and of considering amoebic liver abscess among the

Imaging insights into pediatric liver masses: A comprehensive minireview for hepatology practice.

Pediatric liver masses encompass a diverse spectrum of benign and malignant lesions, with distinct patterns based on patient age. Optimal imaging is critical for timely diagnosis, management, and prognosis. This pictorial minireview categorizes pediatric liver masses by age group to guide hepatology and radiology practice, with an emphasis on imaging characteristics. In children from birth to six years of age, the most common liver masses include hepatoblastoma, the most common primary hepatic malignancy in this age group; infantile hemangioma, a benign vascular tumor with a characteristic app

Management of a Pregnant Woman With Malaria, Stillbirth, and a 12 × 11-cm Amoebic Liver Abscess in a Burundian Hospital: A Case Report.

Background: Malaria and amoebic infections are considered risk factors for stillbirth and preterm labor, but their coexistence during pregnancy has not been previously reported. Case: We describe the first case of averted maternal mortality with fetal death in utero at 22 weeks' gestation, complicated by both falciparum malaria and hepatic amoebic abscess, in a rural hospital in Burundi. Conclusion: Amoebic liver abscesses are rarely described in pregnancy and, as far as we are aware, never in conjunction with severe malaria: two parasitic infections requiring completely different treatments.

Entamoeba histolytica - binding of the lipopeptidophosphoglycan - specific monoclonal antibody EH5 to dextrans.

The human intestinal parasite Entamoeba histolytica remains the cause of significant morbidity and mortality. Glycans often play an important role in its interactions with the host. A major surface component of E. histolytica, the lipopeptidophosphoglycan (LPPG), is a glycosylphosphatidylinositol- (GPI)- linked glycoprotein with a large amount of O-glycosylation with dextran-like oligoglucosyl side chains. We had previously isolated an LPPG-specific monoclonal antibody named EH5. This antibody protected SCID (severe combined immunodeficient) mice from amoebic liver abscess. To date, the sequen

A case of giant amebic liver abscess persisting for years after treatment.

A 64-year-old male was admitted with a giant liver abscess 5 days after the onset of fever. Upon admission, he received intravenous antibiotics, including metronidazole (MNZ), and underwent percutaneous liver abscess drainage. Despite early intervention, he developed severe respiratory failure requiring mechanical ventilation, and the abscess rapidly enlarged to 15.1 cm in diameter by clinical day 9. His general condition and systemic inflammation improved after 4 weeks of antibiotic therapy with MNZ, meropenem, and daptomycin. Entamoeba histolytica was later identified by polymerase chain rea

Clinical Predictors for Early Aspiration in Patients With Liver Abscesses: Insights From a Combined-Risk Model.

BACKGROUND: Liver abscesses represent a significant contributor to surgical morbidity, particularly in developing nations, with amoebic liver abscess (ALA) being the most common type. The debate surrounding the role of early aspiration in preventing complications makes this study particularly important. METHODS: This prospective observational study included 30 consecutive adult patients diagnosed with liver abscesses through radiological imaging over a two-year period at a tertiary care hospital in India. Patients who had ruptured abscesses necessitating surgical intervention were excluded to

18 F-PSMA Expression in Amoebic Liver Abscesses Mimics as Metastatic Prostate Cancer.

Prostate-specific membrane antigen (PSMA) PET/CT imaging has revolutionized the management of prostate cancer. Increased PSMA expression is also reported in many benign pathologies and can lead to diagnostic pitfalls if not carefully considered. The knowledge of these conditions is crucial in clinical decision-making in prostate cancer. Here, we highlight a case of liver amoebic abscesses interpreted as necrotic hepatic metastases on ultrasonography showing PSMA expression in periphery of the early hepatic amoebic lesions.

Cardiac tamponade secondary to ruptured amoebic liver abscess.

A young man in his 20s presented with shortness of breath for 2 hours, with a history of fever and abdominal pain for 10 days. On preliminary examination, he was visibly tachypnoeic and hypotensive, with a raised jugular venous pulse and muffled heart sounds. Additionally, there was profound tenderness in the right hypochondrium. Transthoracic echocardiography revealed a dilated, non-collapsing inferior vena cava and collapsed right-sided chambers of the heart, confirming the diagnosis of cardiac tamponade due to a massive, circumferential pericardial effusion (2.8 cm in maximum dimension). Entamoeba histolytica carbonic anhydrase.

Amoebiasis, caused by Entamoeba histolytica, remains a major public health issue, particularly in developing countries with poor sanitation. It is also a significant challenge among those who travel to endemic areas, causing, in many cases, so-called traveler diarrhea. Approximately 10 percent of the global population is estimated to be affected by this parasitic infection. The primary route of transmission is the consumption of food or water contaminated with E. histolytica cysts. While most infected individuals may remain asymptomatic, some develop severe complications, including hemorrhagic
