Exploring shared genetic architecture in psoriasis and rheumatoid arthritis: a multi-method approach.

Psoriasis and rheumatoid arthritis (RA) are immune-mediated diseases that co-occur clinically, but their shared genetic architecture and the direction of shared effects remain debated. We integrated genome-wide association study (GWAS) summary statistics from FinnGen and UK Biobank using linkage disequilibrium score regression (LDSC), high-definition likelihood (HDL), ρ-HESS, MTAG, CPASSOC, colocalization, and MAGMA. The primary cross-population analysis showed a negative global genetic correlation (LDSC unconstrained: rg = - 0.3004, P = 6.70 × 10⁻³; constrained: rg = - 0.2251, P = 3.00 × 10⁻⁴

CD74 as a Potential Adjunctive Tissue Biomarker for Bullous Pemphigoid Lesions: A Retrospective Study Integrating Bioinformatics Analysis and Experimental Validation.

BACKGROUND: Bullous pemphigoid (BP), an autoimmune blistering disorder that primarily afflicts the geriatric population, poses diagnostic and therapeutic challenges, particularly in patients with comorbidities. This retrospective study aimed to identify and validate potential adjunctive tissue biomarkers for BP and explore their association with immune-related mechanisms through integrated bioinformatics analysis and experimental validation. METHODS: Transcriptomic data from the GEO dataset were analyzed to identify differentially expressed genes (DEGs), which were further cross-referenced wi

Question-Driven Selection of Psoriasis Animal Models: A Structured Narrative Review of 48 Model Records.

Animal models connect psoriasis mechanisms to preclinical testing, but no model is informative for every question. This structured narrative review used PubMed, supplemented by reference-list screening and manual checks, to identify 29 original studies contributing 48 model records. One reviewer conducted screening and extraction, with targeted verification by a second reviewer. Within the selected records, IMQ- and IL-23-induced models primarily addressed acute inflammatory pathways, genetically engineered and composite models tested defined perturbations, humanized models examined human immu

[Tissue resident memory cells in psoriasis].

Tissue resident memory cells (Trm-cells) function as both benign and pathogenic memory cells. Benign Trm-cells play an important role in adaptive immunity by protecting against external pathogens, whereas pathogenic Trm-cells can induce relapses in several chronic skin diseases, including psoriasis. Early and intensive treatment with biologic therapy can lead to effective, long-term disease control by limiting local memory formation in the skin. However, this review finds it important not to compromise the beneficial role of Trm-cells in adaptive immunity when pursuing long-term treatment stra

Clinical case series: Emergence of psoriatic arthritis during biologic IL-23 pathway inhibition therapy for psoriasis.

Psoriasis (PsO) and psoriatic arthritis (PsA) are increasingly recognized as manifestations within the spectrum of psoriatic disease, and emerging observational data suggest that biologic therapy targeting interleukin-23 (IL-23) may reduce the risk of incident PsA. This finding remains controversial given the potential for channeling and information bias. Here, we describe five patients with long-standing PsO who developed new-onset PsA while achieving excellent skin clearance with IL-23 or IL-12/23 inhibitor therapy (risankizumab in four patients and ustekinumab in one). The time from the ini

Incidence and Risk Factors of Treatment-Emergent Eczematous Reactions in Patients with Psoriasis Treated with Biologics: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVE: Treatment-emergent eczematous reactions have been reported in patients with psoriasis receiving biologic therapies, but their pooled incidence and associated factors remain unclear. We aimed to estimate the incidence and associated factors for treatment-emergent eczematous reactions in biologic-treated psoriasis patients. METHODS: We searched PubMed, EMBASE, and Web of Science (to Jan 2026) for studies reporting new-onset or exacerbated eczema in psoriasis patients treated with tumor necrosis factor (TNF), interleukin-17 (IL-17), interleukin-12/23 (IL-12/23), or IL-2

Psoriasis, Cardiovascular Risk, and Artificial Intelligence: Precision Risk Stratification Along the Skin-Vascular Axis-A Narrative Review.

Psoriasis functions as a systemic inflammatory disorder associated with accelerated atherosclerosis via the Skin-Vascular Axis, conferring a cardiovascular (CV) risk frequently underestimated by conventional calculators. In this narrative review, we elucidate the mechanistic role of the IL-23/IL-17 pathway in vascular remodeling and highlight a critical clinical challenge: cutaneous-vascular dissociation, wherein profound skin clearance may not reflect internal vascular homeostasis. We evaluate the systemic CV impact of targeted therapies, highlighting the vasculo-protective potential and real

Massive Upper Gastrointestinal Bleeding in an Adolescent Patient: A Case Report.

BACKGROUND: Gastrointestinal (GI) bleeding in pediatric patients arises from diverse etiologies that vary across age groups. Although it is an uncommon cause of life-threatening hemorrhagic shock, prompt recognition and management are essential. Owing to the infrequency of hemorrhagic shock from GI bleeding in pediatrics, emergency physicians may lack familiarity with its diagnosis and management. Furthermore, multiple etiologies may coexist and may not be detected during the initial esophagogastroduodenoscopy, increasing the risk of premature diagnostic closure. CASE REPORT: A 17-year-old bo

Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 1.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that usually starts in the second or third decade of life and has a profound negative impact on quality of life of those who are affected. HS presents with both inflammatory and cicatricial lesions that are often markedly symptomatic, including pain and purulent discharge, and commonly affect areas that are intertriginous, resulting in additional frictional and mechanical discomfort. The pathophysiology is complex and incompletely understood and results in changes that are transcutaneous, involving the epidermis, superf

Advancing Our Understanding of Hidradenitis Suppurativa: A Comprehensive Guide for Clinicians Designed to Improve Therapeutic Outcomes for Patients-Part 2.

Hidradenitis suppurativa (HS) is a chronic inflammatory cutaneous disorder that often starts in the second or third decade of life, induces both inflammatory and cicatricial cutaneous lesions, and frequently involves intertriginous and anogenital anatomic sites. Abscesses, nodules, and draining tunnels are reversible inflammatory lesions that cycle over time through unpredictable periods of exacerbation; cicatricial lesions are permanent and persist as fibrotic scars and nondraining tunnels, the latter reflecting the shells of prior draining tunnels. HS has a profound adverse impact on overall

Non-Invasive Transcriptomic Profiling of Inflammatory Skin Diseases Using Four Tape Strips.

Tape strip-based transcriptomics shows promise for precision medicine in inflammatory skin diseases, but methodological variation limits reproducibility and clinical use. This study investigates whether four or eight tape strips are sufficient for transcriptomic profiling across three inflammatory skin diseases. As part of the Danish cohort study BIOSKIN, patients with atopic dermatitis (AD), contact dermatitis (CD), psoriasis, and healthy controls (n = 8 per group) were enrolled. Two sets of eight consecutive tape strips were obtained from each participant from lesional skin (patients) or hea

Dermatological Diseases and Household Behavioral Spillovers: A Conceptual Framework.

Background: Chronic diseases may generate behavioral responses extending beyond the affected individual. Although family health effects and behavioral spillovers have been examined in health economics and behavioral research, their relevance to dermatological disease management has received limited conceptual attention. Objective: To develop a conceptual framework distinguishing individual behavioral adaptation from household behavioral spillovers and identifying the conditions under which disease management behaviors may extend beyond the patient. Methods: This conceptual study integrates the

Immunomodulatory Therapy in Patients Living With HIV and Inflammatory Dermatoses: A Comprehensive Review of Current Protocols and Emerging Evidence.

Despite effective antiretroviral therapy (ART), HIV infection drives progressive immune dysfunction, complicating the management of comorbid inflammatory and autoimmune dermatologic conditions such as psoriasis, atopic dermatitis, and cutaneous lupus erythematosus. These conditions often require immunomodulatory therapies, but their use in people living with HIV (PLWH) raises concerns about heightened infection risks, impacts on viral load and CD4 counts, and interactions with ART. The advent of targeted biologics, including tumor necrosis factor, interleukin (IL)-17, IL-23, and Janus kinase i

Associated Factors and Quality of Life in Self-Reported Psoriasis Among Adults in Saudi Arabia: An Exploratory Cross-Sectional Study.

Background and Objectives: Psoriasis is a chronic immune-mediated inflammatory disease whose burden extends beyond the skin. Community-based data from Saudi Arabia are scarce, as most local studies recruit from dermatology clinics and miss people who never reach specialist care. This study examined its frequency in a multi-regional sample of adults in Saudi Arabia, described its clinical profile, identified factors associated with it, and examined its association with quality of life. Methods: An exploratory cross-sectional study recruited 1619 adults residing in Saudi Arabia by online conveni

TRPA1 as the Intersection of Sensory Perception, Innate Immunity, and Adaptive Immune Regulation in Cutaneous Immunity.

Transient receptor potential ankyrin 1 (TRPA1) is a polymodal ion channel best known for its role in nociception and environmental sensing. However, TRPA1 is now recognized to be expressed in several non-neuronal cell types, including keratinocytes and immune cells, suggesting a broader role in the regulation of cutaneous immune responses. This comprehensive review examines the current evidence linking TRPA1 to cutaneous immunity under both physiological and pathological conditions with particular focus on inflammatory skin disorders such as atopic dermatitis, allergic contact dermatitis and p

Glycome remodeling programs pathogenic γδ T17 cell responses in autoimmunity.

Glycans play fundamental roles in T cell biology; however, the glycome landscape of γδ T cells remains to be unlocked. Despite the growing importance of γδ T cells in host defense, tissue homeostasis, and diseases, the glycosylation mechanisms guiding their activation, commitment, and function remain poorly defined. By performing single-cell RNA sequencing, we revealed that type 3/γδ T17/interleukin-17A (IL-17A)-committed γδ T cells rely on (N-acetylglucosaminyltransferase 1) MGAT1-MGAT5-mediated branched N-glycosylation. We further demonstrated that the absence of branched N-glycans on γδ T c
