Epidemiology of Syphilis, Diagnosis, Differential Diagnosis and Treatment: Up-to-Date Knowledge.

Syphilis is an infectious sexually transmitted disease that has re-emerged as a major global public health concern, with a steadily increasing incidence in many regions of the world. It is primarily spread through sexual contact, but can also be transmitted from mother to foetus during pregnancy and, rarely, through blood transfusion. It is estimated that millions of people are affected by syphilis globally, although the exact number of infectious individuals varies by region and is difficult to determine precisely. Despite advances in diagnostic methods and the availability of effective antib

Molecular Biomarkers Linking Psoriasis and Cardiovascular Disease: Current Evidence and Clinical Perspectives.

Psoriasis is a chronic, immune-mediated inflammatory disease affecting the skin, nails, and joints. Its complex pathogenesis results from interactions between genetic susceptibility and environmental factors and is accompanied by an increased risk of multiple systemic comorbidities, particularly cardiovascular disease. Growing evidence indicates that both conditions share common molecular pathways driven by chronic inflammation, endothelial dysfunction, metabolic disturbances, and immune dysregulation. Among the emerging molecular biomarkers, chemerin, endocan, and growth differentiation facto

Callicarpa nudiflora-Derived Extracellular Vesicle-Based Hydrogel Loaded with Apremilast for Enhanced Therapy of Psoriasis.

Apremilast (APST) is an effective medication for psoriasis, but its clinical application is limited by its exclusive availability as an oral tablet formulation and the associated systemic adverse effects. Hydrogel-based transdermal delivery offers a promising alternative for localized APST administration. To address this, Callicarpa nudiflora-derived extracellular vesicle-based hydrogel loaded with APST (APST-EV-Gel) was developed. APST was first loaded into Callicarpa nudiflora-derived extracellular vesicles (APST-EVs) with a drug loading of 17.14% ± 0.13% and an encapsulation efficiency of 4

Targeting Cannabinoid 1 Receptor in Macrophages Alleviates Psoriatic Skin Inflammation Through the AUF1-NLRP3 Axis.

Psoriasis is a chronic inflammatory skin disease associated with systemic metabolic dysregulation; however, the molecular pathways linking metabolic signaling to skin inflammation remain poorly understood. Here, we identify cannabinoid 1 receptor (CB1R) in macrophages as a key immunometabolic regulator of psoriatic inflammation and a potential therapeutic target. CB1R expression was increased in macrophages infiltrating psoriatic lesional skin in both humans and mice, and both pharmacological inhibition and genetic deletion of CB1R markedly attenuated psoriasis-like pathology in mouse models b

Phenotype Switching in Pediatric Inflammatory Skin Diseases During Biologic Therapy: A Literature Review Focused on Atopic Dermatitis and Psoriasis.

Biologic therapies are highly efficacious for pediatric atopic dermatitis (AD) and psoriasis (PsO); however, their expanding use has led to increased reports of phenotype switching-a paradoxical condition where biologics induce new-onset immune-mediated dermatoses. To address clinical uncertainty regarding these complex reactions, this review evaluates real-world case reports, case series, and observational studies of pediatric AD and PsO patients who developed phenotypic switching during biologic therapy. Dupilumab is the most frequently implicated agent, commonly inducing psoriasiform, pustu

Research Progress on Targeted Therapies for Pemphigus Vulgaris and Bullous Pemphigoid: Mechanisms, Opportunities and Challenges.

Autoimmune bullous diseases (AIBDs) are a group of severe disorders characterized by autoantibodies that attack structural proteins within the skin/mucosa, leading to blister and bullae formation. Pemphigus vulgaris (PV) and bullous pemphigoid (BP) are among the most prevalent representative AIBDs. Traditional treatments involving corticosteroids and immunosuppressive agents are often associated with substantial adverse effects and a high rate of relapse. In recent years, the therapeutic landscape has evolved toward precision targeted therapies, encompassing biological agents, small-molecule d

Dietary and Nutritional Considerations for Metabolic Dysfunction-Associated Steatotic Liver Disease in Psoriasis and Hidradenitis Suppurativa: A Narrative Review.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognised in patients with chronic inflammatory skin disease, particularly psoriasis, for which epidemiological evidence of increased MASLD prevalence is relatively strong; corresponding data in hidradenitis suppurativa (HS) remain more limited. This narrative review examines the epidemiological and biological links between psoriasis, HS and MASLD, and evaluates dietary and nutritional strategies relevant to their multidisciplinary management. Particular emphasis is placed on distinguishing direct evidence in pat

A Multi-Axis Plantar Load Measurement Platform for the Assessment of Foot Disorders: Comparison of Patients With Psoriatic Arthritis to a Healthy Control Cohort.

BACKGROUND: Accurate assessment of plantar loading, including shear forces, is crucial for understanding foot pathologies such as dactylitis in psoriatic arthritis (PsA). Existing devices predominantly measure plantar pressures, neglecting important shear stresses which may initiate dactylitis. This study presents a novel multi-axis plantar load measurement platform to address this gap. METHODS: The study utilised a newly developed plantar load measurement system, comprising a Hall-effect-based magnetic sensing array, custom data logging electronics, and tailored data processing software. IL-23 and monocyte interactions redirect human Th17.1 cells to an IL-17AhighIFN-γhigh potentially pathogenic profile.

INTRODUCTION: IL-23-targeted therapies have demonstrated clinical efficacy in psoriatic arthritis (PsA), primarily through the reduction of IL-17A-driven inflammation. Consequently, most studies have focused on classical, high IL-17A-producing, CCR6+CXCR3-CCR4+ Th17 cells. In contrast, the impact of IL‑23 signaling on CCR6+CXCR3+CCR4- Th17.1 cells -traditionally characterized by high IFN-γ and low IL-17A production-remains largely unexplored, despite their recognized pathogenicity in autoimmune disease. The aim of this study was to characterize the effects of IL-23 on human memory Th17.1 cells

Galectin-1 deficiency attenuates imiquimod-induced psoriasiform skin inflammation and is associated with reduced macrophage recruitment.

INTRODUCTION: Psoriasis is an immune-mediated inflammatory skin disease in which innate immune activation and the IL-23/IL-17 axis coordinate keratinocyte hyperproliferation and leukocyte recruitment. Galectin-1 (Gal-1) is a glycanbinding protein with context-dependent immunoregulatory effects; however, its function in psoriasiform skin inflammation remains incompletely defined. Methods: Here, we evaluated Gal-1 expression in human psoriasis lesions and investigated its contribution to imiquimod (IMQ)-induced psoriasiform inflammation in mice. RESULTS: Gal-1 immunoreactivity was increased in

Serum NF-κB p105 and IL-17 in psoriasis: associations with disease status and severity.

BACKGROUND: Psoriasis vulgaris is a chronic immune-mediated inflammatory disease recognized as a systemic disorder with cardiometabolic comorbidities. Although the IL-23/Th17 axis is central to pathogenesis, the clinical relevance of circulating inflammatory mediators, particularly nuclear factor kappa B (NF-κB) related proteins, remains incompletely defined. OBJECTIVES: This study aimed to evaluate serum NF-κB p105 and interleukin-17 (IL-17) levels in patients with psoriasis vulgaris, assess their association with disease severity, and explore their discrimination between patients with psori

T cells form distinct spatially restricted immune responses in synovium of patients with psoriasis arthritis and rheumatoid arthritis.

INTRODUCTION: Psoriatic arthritis (PsA) and rheumatoid arthritis (RA) are chronic, inflammatory joint diseases, causing progressive structural damage. Non-immune cells, such as stromal fibroblasts promote inflammation, cartilage degradation and bone resorption in the synovium. Here, we hypothesized that lining fibroblasts have distinct immune cell neighborhoods in PsA and RA. METHODS: The spatial distribution of structural and immune cell populations was determined using high-multiplex immunohistochemistry in synovial biopsies from 9 PsA and 6 RA patients without previous methotrexate (MTX) t

Keratinocyte-derived CXCL-CXCR2 ligand-receptor axis by MED1 orchestrates neutrophil phenotypes during psoriasis.

BACKGROUND: Psoriasis is a chronic inflammatory skin disease characterized by aberrant keratinocyte proliferation and dysregulated immune responses. Increasing evidence suggests that reciprocal communication between tissue-resident keratinocytes and infiltrating immune cells shapes the inflammatory microenvironment and governs disease progression. However, the molecular mechanisms underlying this immune-parenchymal crosstalk remain incompletely understood. METHODS: Here, we generate epidermal-specific Med1 conditional knockout (cKO) mice using the Cre-loxP system and induce psoriasis model wi

Inflammatory memory as a potential driver of rosacea pathogenesis: integrating mechanisms of chronic relapse and perspectives on disease modification.

Inflammatory memory refers to functional changes retained by a tissue after an inflammatory episode that allow faster or stronger responses to subsequent challenges. It encompasses epigenetic memory in epithelial and stem cells, trained immunity, and tissue-resident memory T cells. Rosacea is a chronic inflammatory skin disease characterized by recurrent flushing, persistent erythema, and papulopustular lesions, and relapse after treatment cessation is common; the mechanisms that drive recurrence remain unclear. In this hypothesis-generating critical review, we use inflammatory memory as an in

Curcumin (Curcuma longa): Applications in Clinical, Regenerative, and Aesthetic Dermatology.

BACKGROUND: Curcumin (CUR), the principal polyphenol in Curcuma longa, has gained increasing interest in dermatology because of its anti-oxidant, anti-inflammatory, antimicrobial, photoprotective, and regenerative properties. Despite versatile biological activity, its clinical use has been limited by poor bioavailability and significant product variability across formulations. METHODS: A targeted PubMed search was conducted using a combination of the following keywords: "curcumin," "turmeric," and "dermatology." Only articles in English and relevant to this review were included. Additional ar

Obesity and Early Biologic Failure in Psoriasis: An Italian Multicentre Retrospective Cohort Study.

INTRODUCTION: Obesity may reduce biologic effectiveness in psoriasis, but its effect on specific reasons for treatment discontinuation is less well defined. We evaluated the association between obesity and biologic drug survival, with particular attention to primary failure and therapeutic class. METHODS: This retrospective multicentre cohort study included patients with plaque psoriasis treated with biologics at five Italian dermatology centres. Analyses were conducted at the treatment-line level using Kaplan-Meier methods and Cox proportional hazards models. Obesity was defined as body mass

Psoriasis Area and Severity Index Use in Pediatric Psoriasis.

Psoriasis Area and Severity Index (PASI) score is the primary instrument used to assess psoriasis severity and determine eligibility for clinical trials. The PASI scoring system assumes fixed adult body surface area (BSA) proportions for the head (10%), upper extremities (20%), trunk (30%), and lower extremities (40%); however, these proportions differ in pediatric populations. We conducted a systematic review of articles in Embase, PubMed, and Scopus, identifying more than 300 records. After quality appraisal using the Literature Review Scoring Rubric and manual relevance screening, 5 high-qu
