Interim Update on Reactive Infectious Mucocutaneous Eruption (RIME).

Reactive infectious mucocutaneous eruption (RIME), a term describing blistering skin and mucosal lesions, is a term coined by a group of Pediatric Dermatology Research Alliance (PeDRA) researchers and first published in 2021. Since that time, the literature has grown with reports of diverse infectious triggers, management strategies, and complications. This expert opinion PeDRA perspective, developed by a group conducting a consensus process on management of RIME, is an interim update on the current state of diagnosis and management of RIME with a focus on practical information for the busy cl

Histopathological Patterns in Bullous Dermatitis Artefacta: A Retrospective Case Series.

BACKGROUND: Dermatitis artefacta (DA) is associated with self-harming behaviors, which can be conscious or unconscious. It is characterized by lesions with atypical distributions and geometric patterns, produced by manipulation of the skin through friction, suction, heat, cold, electric current, or chemicals. Diagnosis is challenging, and there are no confirmatory tests, but histology can provide important clues for identifying artificial causes. OBJECTIVE: Our aim was to retrospectively review histological characteristics and anatomopathological patterns in patients with bullous DA. METHODS

Lamotrigine-associated SJS/TEN overlap during valproate co-therapy managed with rhTNFR-Fc and therapeutic plasma exchange: A case report.

RATIONALE: Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is a rare, life-threatening severe cutaneous adverse reaction. Evidence for escalation therapy in relapsing disease remains limited. PATIENT CONCERNS: A 49-year-old man developed fever and a rapidly progressive generalized erythematous maculopapular eruption after lamotrigine was initiated directly at 50 mg once daily during concomitant valproate therapy. DIAGNOSES: At admission on August 3, 2023, no epidermal detachment was observed and SJS/TEN was not clinically recognized. On August 9 (hospital day 7), epidermal frag

Acute Hemorrhagic Edema in Infants: A Continuing Clinical Challenge.

We report a case of acute hemorrhagic edema in a four-month-old boy, which occurred following a viral respiratory infection seven days prior to his admission. The clinical presentation began with the appearance of erythematous macules initially localized to both feet, which then spread to both thighs. All of this occurred against a background of a fever of 38.5 °C, while the child's general condition remained stable. Initially treated as purpura fulminans, but given the child's stable general condition and normal laboratory findings, the diagnosis was revised to acute hemorrhagic edema of infa

Reactive Infectious Mucocutaneous Eruption Following a Respiratory Infection in an Adolescent: A Case Report.

Reactive infectious mucocutaneous eruption (RIME) is a recently defined mucocutaneous manifestation associated with respiratory infections, most commonly reported secondary to Mycoplasma pneumoniae. The core diagnostic criteria of RIME involve greater than or equal to two mucosal areas with erosive mucositis, minimal cutaneous lesions, a recent infectious prodrome, and exclusion of drug-induced etiologies. RIME presents with acute mucositis involving the oral, ocular, and genital mucosa, with presentations similar to other mucocutaneous disorders such as Stevens-Johnson syndrome (SJS), erythem

Systemic Therapy for Paediatric Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis: A Systematic Review of Current Evidence and Limitations.

AIM: The role of systemic therapy in paediatric Stevens-Johnson Syndrome (SJS) and Toxic Epidermal Necrolysis (TEN) remains undefined. Management is primarily guided by observational data and variable practice patterns. This systematic review aims to integrate recent evidence and evaluate clinical outcomes associated with systemic therapies in paediatric SJS/TEN. METHODS: A systematic search of PubMed, Embase and Scopus was conducted from January 1990 to February 2025. Studies were eligible if they included patients (< 18 years) with SJS, SJS-TEN overlap or TEN managed with supportive care al

Summary of Best Evidence for Wound Management in Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare but life-threate-ning mucocutaneous reactions characterized by extensive epidermal detachment and systemic complications. Wound management is central to survival and long-term outcomes, yet standardiz-ed care pathways remain controversial and in-consistently implemented. This study aimed to systematically synthesize the best available evidence on wound management for patients with SJS/TEN to support clinical decision-making and optimize clinical outcomes. A comprehensive search of major international and Chinese datab

Data augmentation method for computer-aided diagnosis using specular reflection.

UNLABELLED: Drug-resistant Plasmodium falciparum has been on the rise, mainly due to point mutations in the pfDHFR or dihydrofolate reductase gene. This has heightened urgency to discover and develop new antimalarial drugs. This research utilized Pharmacophore Modeling, Virtual Screening, Molecular Docking and Molecular Dynamics Simulation techniques to identify potential pfDHFR inhibitors from virtual databases. Ligand-based pharmacophore hypothesis (H1) (r = 0.949, r2 = 0.883) was formulated using a training set of 28 compounds that demonstrated pfDHFR inhibitory activity across four orders

A cross-sectional analysis of inflammatory markers as a severity and prognostic indicator in severe cutaneous adverse drug reactions.

Background The inflammatory markers (IMs) are a set of biomarkers used clinically to assess a patient for the presence of an active inflammatory disease or disease activity. They are elevated in conditions like infections, malignancies, mental illness and drug reactions among others. Aim The study aimed at identifying the IMs that reflect the severity and recovery in severe cutaneous adverse drug reactions (SCARs). Methods A cross-sectional observational study was conducted among patients with SCARs (Stevens Johnson syndrome [SJS]/ toxic epidermal necrolysis [TEN] and drug reaction with eosino

An Uncommon Reaction to a Common Medication: Stevens-Johnson Syndrome Associated With Azithromycin.

Stevens-Johnson Syndrome (SJS) is a rare but potentially fatal skin reaction characterized by extensive involvement of the epidermal and mucosal tissue. Although medications are the typical culprits, infectious diseases and chronic illnesses may also trigger SJS or toxic epidermal necrolysis (TEN). We present a rapid, severe SJS/TEN case in response to azithromycin administration, underscoring the critical need for pharmacovigilance and discontinuation of suspected medications, even if the potential causative agent is not commonly associated with these severe cutaneous adverse reactions.

Recovering From Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis.

IMPORTANCE: Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS/TEN) survivors experience substantial long-term sequelae. Research on physical symptoms experienced during acute hospitalization is well documented, but limited studies have been completed on the long-term biopsychosocial effects of SJS/TEN, particularly from the patient's perspective. OBJECTIVE: To increase the understanding of the long-term complications of SJS/TEN. DESIGN, SETTING, AND PARTICIPANTS: This qualitative investigation was completed from within a community-based study, the SJS Survivors Study, using a semi

Blistering Drug Eruptions: Diagnostic Challenges and Management in Clinical Practice: Unmasking the Cause: A Grand Rounds Review of Blistering Drug Eruption.

Blistering drug eruptions represent a spectrum of severe cutaneous adverse reactions characterized by epidermal and mucosal detachment, which present clinically as vesiculobullous lesions. These conditions, including Stevens-Johnson syndrome, toxic epidermal necrolysis, bullous fixed drug eruptions, and other blistering mimickers, are rare but potentially life-threatening. Prompt recognition and management are critical to improving outcomes. This Grand Rounds Review describes two clinical cases of patients presenting with a blistering eruption, guiding the reader through the diagnostic approac

Clinical Features and Treatment Response in Chronic Recurrent Erythema Multiforme: Difference Based on the Etiology Related to Herpes Simplex Virus.

BACKGROUND: Erythema multiforme (EM) is typically a self-limited, acute hypersensitivity reaction. However, a subset of patients experiences chronic, recurrent episodes, for which clinical features and treatment strategies differ depending on the underlying etiology, especially in herpes simplex virus (HSV)-associated cases. OBJECTIVE: To investigate the clinical and phenotypic features of chronic recurrent EM and assess treatment responses, with a focus on differences based on HSV association. METHODS: This retrospective study included pathology-confirmed cases of suspected EM from 2010 to

A Case of Concurrent Behçet Disease and Erythema Multiforme in a Young Adult.

Behçet disease (BD) and erythema multiforme (EM) are rare, immune-mediated, clinically diagnosed conditions that can present with overlapping mucocutaneous findings and symptoms. We report a diagnostically challenging presentation of concurrent BD and EM characterized by oral ulcers, genital lesions, and targetoid bullous acral lesions in a 23-year-old African American man. This case highlights the risk of premature diagnostic closure and emphasizes that histopathology is supportive rather than determinative in diagnosing either of these conditions. Management required multidisciplinary input
