Lofgren Syndrome in a 35-Year-Old Female: A Case Report.

Lofgren syndrome is an acute and pathognomonic form of sarcoidosis characterized by fever, hilar lymphadenopathy, arthritis, and erythema nodosum (EN). We report a 35-year-old female patient from Ethiopia who presented with a 4-day history of arthritis involving bilateral ankles, knees, and wrists associated with fever and skin lesions typical of EN. Chest imaging (X-ray and computed tomography (CT) scan) revealed bilateral hilar and mediastinal lymphadenopathy as well as perilymphatic nodules in the mid and upper zones of both lungs. Workup for alternative diagnoses, including tuberculosis, w

"Tuberous sclerosis in Greece: A national cohort study on clinical features and rare manifestations".

BACKGROUND: Tuberous Sclerosis Complex (TSC) is a rare, multisystem genetic disorder with highly variable clinical manifestations. While international registries such as TOSCA have provided large-scale data, national-level studies remain limited. This study represents the first national cohort analysis of TSC patients in Greece, providing comprehensive insights into clinical characteristics, genotype-phenotype correlations, and previously underreported rare manifestations. METHODS: A descriptive analysis was conducted on 115 TSC patients diagnosed based on the latest criteria. Clinical, genet

Spontaneous Idiopathic Non-venereal Cavernosal Abscess: A Rare Case Report and Review of Literature.

Penile abscesses are uncommon in urology and usually arise from trauma, intracavernous injections, systemic infections, and sexually transmitted infections (STIs). However, spontaneous cases with no definitive causes are sporadic. Here, we describe an unusual presentation in a 55-year-old man with no history of trauma, diabetes, immunosuppression, or recent sexual activity. During this time, he was being treated for erythema nodosum when he developed worsening penile swelling and pain. Imaging (ultrasound and MRI) showed a lesion within the corpora cavernosa; surgical exploration and biopsy co

The Risk of Developing Ocular Involvement Among Behçet's Disease Patients Presenting with Mucocutaneous Involvement at Disease Onset: Data from the International AIDA Network Behçet's Disease Registry.

INTRODUCTION: Behçet's disease (BD) frequently arises with exclusively mucocutaneous involvement, but some patients will develop major organ involvement, including ocular inflammation. This study aims to assess the patients' demographic and clinical characteristics that may be associated with the development of ocular involvement in patients with BD with exclusively mucocutaneous involvement in the early stages. METHODS: Patients' data were collected in the International AutoInflammatory Disease Alliance (AIDA) Network registry dedicated to BD. RESULTS: A total of 328 patients with BD were e

VEXAS Syndrome Presenting as Erythema Nodosum and Pancytopenia: Diagnostic Utility of 18 F-FDG PET/CT in Systemic Inflammation.

VEXAS syndrome (Vacuoles, E1 enzyme, X-linked, Auto-inflammatory, Somatic) is a recently identified adult-onset auto-inflammatory disorder caused by somatic mutations in the UBA1 gene. This syndrome can mimic infections, hematologic malignancies, and autoimmune diseases, making diagnosis challenging. We present a case involving a 47-year-old man with hyperthyroidism who experienced prolonged fever, painful nodules, and pancytopenia. Initial evaluations suggested erythema nodosum and vasculitis. An 18 F-FDG PET/CT scan revealed diffuse hypermetabolism in the lungs, bone marrow, spleen, and lowe

A Case of Fasciitis Following SARS-CoV-2 mRNA Vaccination: Case Review and Literature Overview.

Vaccines play a crucial role in preventing various infectious diseases, and their safety has been well established, with adverse effects generally being mild and transient. However, in rare instances, serious adverse events, including autoimmune diseases, have been reported following vaccination. Among these conditions, fasciitis is extremely rare. To our knowledge, the present report describes the first documented case of fasciitis occurring in combination with erythema nodosum after vaccination against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). A 29-year-old Japanese woman

Painless Appendicitis: A Peculiar Surgical Case of Acute Granulomatous Appendicitis.

Acute appendicitis is one of the most common surgical emergencies, typically presenting with right-sided abdominal pain, nausea, and anorexia. Painless appendicitis is very uncommon and poses a diagnostic challenge. This case report highlights an unusual presentation of painless granulomatous appendicitis. We present a female patient in her 20s with a history of erythema nodosum and systemic inflammatory symptoms who was incidentally diagnosed with acute retrocecal appendicitis on imaging. Despite radiological evidence, she denied abdominal pain or other classic symptoms. After a thorough rheu

Clinicopathological concordance across panniculitis: biopsy timing and laboratory parameters in predicting temporal variability and histopathological features in erythema nodosum.

The evolving pathological features of panniculitis make biopsy timing critical. This study explores clinicopathological concordance and the role of biopsy timing and non-invasive laboratory parameters in predicting histopathological findings in erythema nodosum (EN). Our retrospective study analysed the clinical and pathological features of all clinically suspected and/or histopathologically diagnosed panniculitis cases over a period of 3.5 years. The week of biopsy, total leukocyte count, neutrophil-to-lymphocyte ratio, neutrophil-to-monocyte ratio, erythrocyte sedimentation rate (ESR), and t

Male predominance in inflammatory bowel disease despite similar disease activity: A large retrospective cohort study.

Inflammatory bowel diseases (IBD), including ulcerative colitis and Crohn's disease (CD), exhibit clinical heterogeneity, with sex-related differences potentially influencing disease course and prognosis. This study investigates the impact of sex on IBD prognosis in a large cohort. Data were collected from patients diagnosed with IBD at our clinics between July 1993 and April 2025. Demographic, clinical, and treatment variables were analyzed, focusing on sex-based differences in disease onset, disease activity, extraintestinal manifestations (EIMs), treatment escalation, hospitalizations, and

Granulomatous mastitis, erythema nodosum, and arthritis syndrome: a narrative review.

INTRODUCTION: Idiopathic granulomatous mastitis (IGM) is a rare chronic inflammatory breast disease of unknown etiology, typically presenting with breast pain, swelling, and mass formation. Erythema nodosum (EN) is an inflammatory panniculitis that can occur as a systemic manifestation of various conditions, and arthritis has occasionally been observed alongside granulomatous mastitis (GM), suggesting an underlying autoimmune component. This narrative review synthesizes reported cases of GM with EN, with or without arthritis-termed "GMENA" syndrome-to characterize its clinical features and man

From bilateral pulmonary embolism to the final diagnosis of Behçet's disease: a rare case report.

BACKGROUND: Behçet’s disease (BD) is a chronic, relapsing multisystem vasculitis that most frequently presents with recurrent oral and genital ulcers, ocular involvement, and skin lesions. Vascular complications are well recognized, but pulmonary embolism (PE) is an uncommon manifestation, despite the high prevalence of venous thrombosis in BD. When it occurs, PE can be life-threatening and poses significant diagnostic and therapeutic challenges. CASE PRESENTATION: A 38-year-old male with no prior comorbidities presented with recurrent hemoptysis, fever, and systemic symptoms of several months

Unusual Presentation of Subcutaneous Panniculitis-Like T-Cell Lymphoma.

Subcutaneous panniculitis-like T-cell lymphoma (SPTCL) is a rare variant of primary cutaneous T-cell lymphoma primarily affecting subcutaneous adipose tissue and mimicking lobular panniculitis. It often presents with erythematous subcutaneous nodules or plaques, and diagnosis can be challenging due to its diverse clinical manifestations. We report an unusual presentation of SPTCL in a 40-year-old female with recurrent, painless ulcerated plaques on the thighs and lower legs, accompanied by systemic symptoms including weight loss, fatigue, and laboratory abnormalities. Initially misdiagnosed as

Real-World Insights Into the Clinical Characteristics and Survival Outcomes of Extranodal Peripheral T-Cell Lymphomas: A Multiethnic Asian Cohort Study.

BACKGROUND: Extranodal peripheral T-cell lymphomas (PTCLs) comprise a diverse group of lymphomas whose real-world presentation and course are infrequently studied. METHODS: We retrospectively analyzed the clinical characteristics and survival outcomes of 50 patients at a tertiary cancer center in Singapore between 2002 and 2024. RESULTS: The cohort comprised 26 males and 24 females, with a median age of 50 years (range 16-84 years). The 5-year progression-free survival (PFS) and overall survival (OS) were 20.2% and 66.6%, respectively. Male sex (p = 0.03) and lack of complete response to ini

Erythema Nodosum as the Primary Manifestation of Acute Histoplasmosis in a Pediatric Patient From an Endemic Region.

Erythema nodosum (EN) is a common form of septal panniculitis that typically presents with painful, erythematous nodules on the lower extremities. While most cases are idiopathic, EN can be triggered by infection, medications, autoimmune and inflammatory pathologies, recent immunization, or malignancy. Commonly, EN has been associated with fungal infections, including Histoplasma capsulatum, a dimorphic fungus endemic to the Ohio and Mississippi River Valleys. Although pulmonary involvement is the most common manifestation of the fungal infection, extrapulmonary presentations can occur. We pre
