Histological and Genetic Markers of Cellular Senescence in Keratinocyte Cancers and Actinic Keratosis: A Systematic Review.

Cellular senescence is a stress-induced cell-cycle arrest that constrains expansion of ultraviolet-damaged keratinocytes yet can remodel the microenvironment. This systematic review evaluated histological and genetic or epigenetic senescence markers in actinic keratosis (AK), cutaneous squamous cell carcinoma (cSCC), and basal cell carcinoma (BCC). PubMed, Scopus, and Web of Science were searched (January 2005-May 2025); 34 human studies were included. AK showed an early senescent signature with frequent cyclin-dependent kinase inhibitor p21 (p21CIP1) expression (82.1%) and DNA damage signalin

Diameter, height, and volume as macroscopic predictors of photocarcinogenesis in UV-induced SKH1 papules.

BACKGROUND: The albino hairless SKH1 mouse is the most widely used model for UV-induced skin carcinogenesis, yet standardized macroscopic thresholds for classifying papules as cancer are lacking. Prior studies have used diameter alone, with variable cutoffs and limited dermatopathology. OBJECTIVE: To determine how papule diameter, height, and volume correlate with histopathology in SKH1 mice and to identify practical cutoffs beyond which papules can be considered cancer. METHODS: Forty-four SKH1 mice were irradiated with solar-simulated UV (290-400 nm; 90 mJ/cm2 UVB; 5 days/week for 10-18 we

Lack of Rb and p53 Expression in a Case of CM2B4 Negative Merkel Cell Carcinoma In Situ Associated With Squamous Dysplasia.

Merkel cell carcinoma (MCC) is a dermal neuroendocrine malignancy driven by inactivation of the tumor suppressors Rb and p53 either through Merkel cell polyomavirus (MCPyV) infection or ultraviolet (UV)-induced mutations. Virus-positive MCC exhibits pure neuroendocrine morphology, low UV-induced mutational burden, retained Rb protein, and low p53 expression. Viral-negative MCC includes pure neuroendocrine carcinomas and combined tumors with squamous and neuroendocrine components. These tumors typically exhibit loss of Rb expression and either overexpression of mutated p53 or a complete absence

Actinic Keratosis at the Crossroads of Inflammation and Cutaneous Dysbiosis.

Actinic keratosis (AK) represents an early stage of keratinocyte carcinogenesis and has long been attributed primarily to cumulative ultraviolet (UV) radiation-induced genetic damage. However, increasing evidence suggests that AK arises within a complex cutaneous microenvironment, in which chronic inflammation, oxidative stress, immune dysregulation, and alterations of the skin microbiota interact to promote and sustain field cancerization. UV exposure not only drives mutational events in epidermal keratinocytes, but also disrupts barrier integrity and local immune surveillance, reshaping micr

Non-Invasive Assessment of Treatment Response in Actinic Keratosis: A Clinically Oriented Multimodal Review.

BACKGROUND: In actinic keratosis (AK), clinical clearance after field-directed therapies does not necessarily correspond to histological resolution, resulting in subclinical persistence and risk of recurrence. OBJECTIVE: To provide a practical, up-to-date framework for non-invasive monitoring of treatment response in AK, integrating clinical assessment and dermoscopy with high-resolution imaging techniques, reflectance confocal microscopy (RCM), line-field confocal optical coherence tomography (LC-OCT), and high-frequency ultrasound (HFUS), and to discuss emerging optical biomarkers based on

DermaCalibra: A Robust and Explainable Multimodal Framework for Skin Lesion Diagnosis via Bayesian Uncertainty and Dynamic Modulation.

Background: Accurate and timely diagnosis of skin lesions, including Melanoma (MEL), Basal Cell Carcinoma (BCC), Squamous Cell Carcinoma (SCC), Actinic Keratosis (ACK), Seborrheic Keratosis (SEK), and Nevus (NEV), is often hindered by the severe class imbalance and high morphological similarity among pathologies in clinical practice. Although multimodal learning has shown potential in resolving these issues, existing approaches often fail to address predictive uncertainty or effectively integrate heterogeneous clinical metadata. Therefore, this study proposes DermaCalibra, a robust and explain

Longitudinal Assessment of Vascularity in Actinic Keratosis Using Dynamic Optical Coherence Tomography: Practical Considerations and Theoretical Insights.

Longitudinal monitoring of actinic keratosis (AK) is a part of patient management, scientific studies, and the development of new treatment strategies. Dynamic optical coherence tomography (D-OCT), an imaging method that enables noninvasive assessment of microvasculature, in conjunction with automated quantitative analysis, shows promise as an objective and robust method for monitoring AKs. Critical to quantitative longitudinal assessments is a dataset in which good-quality images are available at all time points. Yet, to date, few resources exist to train clinical researchers on how to ensure

[(Chemo-)Preventive strategies for skin cancer: current state of knowledge and implications for daily practice].

In view of rising incidence rates worldwide, the prevention of skin cancer is a central pillar of public health. While UV protection forms the basis of primary prevention, pharmacological strategies (chemoprevention) are becoming increasingly important. This paper provides a practice-oriented synthesis of current evidence, ranging from behavioural prevention and systemic approaches with nicotinamide and retinoids to experimental strategies such as immune checkpoint inhibitors and DNA repair enzymes.

Advances in the application of photodynamic diagnosis in Skin Tumors.

Photodynamic Diagnosis (PDD) is a non-invasive imaging technique. It relies on a photosensitizer that, when activated by a specific light source, causes metabolically active tissues like tumors to emit visible red fluorescence. PDD offers high sensitivity, high specificity, and real-time visualization. This article reviews advances in the use of PDD for common cutaneous tumors, including Keratinocyte Carcinoma and extramammary Paget disease (EMPD). In managing skin tumors, PDD can be used during preoperative, intraoperative, and postoperative stages. This helps optimize diagnosis and treatment

Novel abrasive and oscillating skin preparation device as pre-treatment of actinic keratosis in photodynamic therapy: A Single-center, prospective, open-label, randomized, split-site trial.

BACKGROUND: Photodynamic therapy (PDT) is recommended as a first-line treatment for multiple actinic keratoses (AK) and field cancerized skin. When using PDT, skin preparation is recommended before application of photosensitizer cream to enhance absorption. Different physical methods exist to remove crusts and scales from the AK lesions. However, currently widely used methods can be time-consuming, unpleasant to the patient, or require expensive equipment. OBJECTIVES: To investigate the performance, safety, and comfort of a novel skin preparation device consisting of a soft biocompatible abra

Enhanced skin cancer classification for minority classes using Conditional GAN pipeline and CNN-ViT ensemble.

Although Deep learning (DL) methodologies have progressed substantially, the classification of skin cancer remains a challenging task. There are several reasons for this. Some of them are: artifacts like Hair; severe class imbalance in dermatoscopic datasets; and difficulty in extracting both fine-grained local features (details within small area of lesion) like texture, color, pigment network, vascular patterns and long range global features like overall shape, border irregularity, asymmetry. To overcome these, this study presents a novel, two-stage framework. At first, C’GAN (Conditional Gen

Skin physiology during daylight photodynamic therapy with additional fractional laser therapy.

Daylight photodynamic therapy is currently the preferred treatment for actinic keratosis due to its greater effectiveness and a lower level of pain compared to conventional photodynamic therapy. To enhance the effectiveness of the therapy, especially for hyperkeratotic forms, various pre-treatments are currently available, primarily fractional laser therapy. Until now, there have been few studies investigating skin physiology after photodynamic therapy and fractional laser treatment. The aim of the study was to investigate the skin physiological changes in daylight photodynamic therapy with an

A deep learning model for histopathological diagnosis of actinic keratosis: a diagnostic case control accuracy study.

BACKGROUND: Actinic keratosis (AK) is a precancerous skin lesion with the potential to progress into squamous cell carcinoma (SCC), with an overall prevalence of 14%. Although AK is not routinely biopsied, it represents a significant portion of dermatopathology cases. Advances in histopathology now allow for the digitization of slides using whole slide imaging (WSI). While deep learning models (DLMs) have shown promise in various medical fields, their application in dermatopathology remains limited. METHODS: Building on prior research, we developed a U-Net-based DLM to detect AK in histopatho
