NAIL PLATE DAMAGE INDEX IN DERMATOSCOPIC EXAMINATION OF PATIENTS WITH ONYCHOMYCOSIS.

INTRODUCTION: The lack of uniform criteria for assessing the degree of damage to the nail plates of patients with onychomycosis does not allow for an objective assessment of the effectiveness of therapy and a comparative assessment of the results of treatment using different methods. In modern medicine, the use of instrumental methods of diagnosis and treatment is becoming more widespread. In particular, one of the options for obtaining information about the state of lesions is nail plate damage index (NPDI). However, the correspondence of the macroscopic appearance to the actual condition rem

Anthropogenic tritium as a continental-scale tracer in river-derived recharge.

BACKGROUND: Environmental hazards like floods, droughts and landslides pose serious public health and wellbeing consequences for populations, especially vulnerable communities in countries with low adaptive capacity. Empirical evidence from peer-reviewed literature and official climate change assessments indicates that, alongside high interannual climate variability, Uganda has experienced observable climate change over the past 50 years. This changing and increasingly variable climate poses challenges to population health in diverse ways, including through the transmission of infectious disea

Atypical fingernail onychomycosis due to Aspergillus niger in a healthy adult: an unusual presentation of a non-dermatophyte mold infection.

BACKGROUND: Onychomycosis is commonly caused by dermatophytes, but nondermatophytic molds (NDMs) are increasingly recognized, even in immunocompetent individuals. These infections often present atypically, leading to delayed diagnosis. NDMs require accurate identification, as they may not respond to standard antifungal treatments used for dermatophyte infections. CASE REPORT: An immunocompetent man in his 40s presented with discoloration of a single right index fingernail. He reported no trauma, systemic illness, or immunosuppressive therapy. Examination revealed brown-black nail discoloration

Management of Nail Toxicities From Fibroblast Growth Factor Receptor Inhibitors.

BACKGROUND: Alterations in fibroblast growth factor receptor (FGFR) signaling are present in many malignancies, including urothelial carcinoma, cholangiocarcinoma, and gastrointestinal cancers, and FGFR inhibitors (FGFRi) play an increasing role in the treatment of these malignancies. Nail toxicities, such as onycholysis, paronychia, and nail fragility are an important part of the adverse effect profile of FGFRi that remain underrecognized and poorly characterized. METHODS: We conducted a systematic literature review using PubMed and Google through March 2025, including case reports, trials,

Nail dystrophy successfully treated with dupilumab in pediatric atopic dermatitis: case series and literature review.

Atopic dermatitis (AD) is a common chronic, recurrent inflammatory disease, yet its accompanying nail abnormalities have long received insufficient attention. The clinical characteristics, underlying mechanisms, and assessment systems for nail dystrophy in AD remain unclear. AD-associated nail dystrophy can manifest in various forms, including Beau's lines, nail pitting, koilongchia, trachyonychia, leukonychia, brachyonychia, melanoychia, onychomadesis, onychoschizia, onycholysis, and paronychia. Current treatments face limitations such as slow onset of action and uncertain efficacy with tradi

Pseudomonas aeruginosa and Burkholderia cepacia complex co-infection in green nail syndrome: a case report.

Green nail syndrome (GNS) is classically characterized by a triad of green nail discoloration, paronychia, and onycholysis. Although Pseudomonas aeruginosa (P. aeruginosa) is the primary etiologic agent, infections involving co-pathogens are uncommon and pose considerable diagnostic and therapeutic challenges due to the absence of standardized management guidelines. We herein report a rare case of GNS caused by a co-infection with P. aeruginosa and the Burkholderia cepacia complex (BCC) in a woman with a history of manicures and frequent domestic exposure to water and detergents. The diagnosis

Pharmacovigilance analysis of the FGFR inhibitor futibatinib using the FDA adverse event reporting system (FAERS) database.

BackgroundFutibatinib, a novel FGFR inhibitor, is a promising therapy for cancers with FGFR alterations. However, its safety profile remains under investigation. This study evaluates futibatinib-associated adverse events using the FDA Adverse Event Reporting System (FAERS).MethodsA retrospective pharmacovigilance analysis was conducted using FAERS data from January 1, 2013, to March 9, 2025. Disproportionality analysis was performed via OpenVigil 2.1, with signals defined by a reporting odds ratio (ROR) > 2.00 and a 95% confidence interval lower bound >1.00. Events with at least two reports we

Distal Nail Canal Visible on Onychoscopy as a Diagnostic Sign of Dermatophytoma.

BACKGROUND: Early recognition of dermatophytoma is essential to initiating appropriate therapy and avoiding treatment failure. Dermatophytoma is a variant of distal subungual onychomycosis, characterized by an accumulation of hyphae and scales trapped under the nail plate, making antifungal treatment challenging. Its hallmark feature is a yellow-white mass under the nail with a distal band of onycholysis or "canal". OBJECTIVES: This study aimed to describe the clinical and onychoscopic features of dermatophytoma to improve diagnostic accuracy. METHODS: We conducted a retrospective study of 1

Beyond the Plate: Nail Bed Dermoscopy for Differentiation of Psoriatic, Fungal, and Traumatic Fingernail Onycholysis.

BACKGROUND: Onycholysis is a common but etiologically heterogeneous nail sign, most frequently due to psoriasis, onychomycosis, and trauma. Since the diagnosis of the underlying cause can be misleading, we investigated whether dermoscopy (onychoscopy) pre- and post-clipping of the onycholytic nail plate can provide reproducible patterns to distinguish these causes without the need for a biopsy. MATERIALS AND METHODS: In a multicenter retrospective image-based study, 60 onycholytic fingernails were examined (20 patients per diagnosis: psoriasis, onychomycosis, trauma). Onychoscopy of the intac

Pregnancy Associated Atypical Onychopapilloma: A Novel Presentation of an Emerging Entity in Dermatopathology.

Onychopapilloma (OP) is a benign nail unit tumor of the distal nail matrix and nail bed that typically presents as solitary longitudinal erythronychia with distal subungual hyperkeratosis and focal onycholysis. Atypical OP is a newly recognized and evolving concept in dermatopathology. Herein, we present a histopathologically-proven atypical OP with a classic clinical appearance that developed during pregnancy. A 37-year-old Gravida two Para one woman presented with a red longitudinal band on the left thumbnail that appeared during the first trimester of an uncomplicated pregnancy. The lesion

Nail Langerhans Cell Histiocytosis: A Rare Case Series of Two Patients With Single-System and Multisystem Disease Presenting With Nail Lesions.

Langerhans cell histiocytosis (LCH) is a rare myeloid neoplastic disorder driven by somatic MAPK pathway mutations, most commonly BRAFV600E, characterized by clonal proliferation of CD1a+/CD207+ dendritic cells forming granulomatous lesions in various tissues. Nail involvement is an uncommon but clinically significant manifestation, often associated with multisystem disease and a potentially poorer prognosis. We report 2 pediatric cases of LCH presenting with nail changes as the initial clinical feature. Case 1, a 2-year-old boy, was presented with longitudinal grooving, onycholysis, subungual

Editor's Highlights-August 2026.

Noninvasive imaging techniques are playing an increasingly significant role in dermatologic diagnosis, risk evaluation, and the monitoring of therapeutic outcomes. In the August issue of the Journal, we highlight how dermoscopy, onychoscopy, trichoscopy, and line-field confocal optical coherence tomography (LC-OCT) can refine clinical decision-making across acral melanocytic lesions, onycholysis, primary cicatricial alopecia, and hyperkeratotic actinic keratoses. These papers show that imaging is most useful when tailored to anatomical and clinical context, advancing standardized and clinicall

Clinical assessment of nail and skin lesions in indoor climbers: A clinic-based cross-sectional study.

BACKGROUND: Indoor climbing has grown substantially in popularity; however, clinically assessed data on climbing-related nail and skin conditions remain limited. Tight, performance-oriented climbing shoes and repetitive training exposure may contribute to mechanical stress affecting the toes and forefoot, but evidence derived from direct clinical examination is scarce. AIM: To characterize the frequency, typology, and anatomical distribution of nail and skin lesions in a clinic-based sample of indoor climbers, and to assess associations with footwear compression and training exposure. METHOD

Treatment of Isolated Nail Psoriasis with JAK Inhibitor Tofacitinib: A Case Report.

BACKGROUND: Isolated nail psoriasis is clinically rare, characterized by nail involvement in the absence of cutaneous lesions, and is often misdiagnosed as onychomycosis. Although tofacitinib, a JAK1/JAK3 inhibitor, is approved for psoriatic arthritis, evidence regarding its efficacy and safety in isolated nail psoriasis remains limited. CASE PRESENTATION: We present a 39-year-old female with a 6-year history of refractory isolated nail psoriasis, previously unresponsive to itraconazole, acitretin, topical antifungals, and corticosteroids. The patient received oral tofacitinib 5 mg twice dail
