A Study Proposing a Pattern-Based Dermoscopic Algorithm for Diagnosis of Nail Psoriasis.

INTRODUCTION: Nail psoriasis (NP) presents with various morphological patterns. Identifying these patterns enhances the recognition and diagnosis of NP. Dermoscopy is the only modality capable of providing accurate information regarding these patterns. OBJECTIVE: This study aimed to identify and classify NP into distinct dermoscopic patterns and evaluate their relationships with disease severity and the risk of psoriatic arthritis. A secondary objective was to propose a pattern-based dermoscopic algorithm for NP diagnosis. METHODS: This prospective study included 208 patients diagnosed with

Onychoscopy in Nail Psoriasis: A Narrative Review of Diagnostic Utility, Disease Severity Assessment, Musculoskeletal Associations, and Treatment Monitoring.

Nail psoriasis is a common and clinically significant manifestation of psoriasis that may be difficult to recognize, quantify, and differentiate from other nail disorders. Onychoscopy has emerged as a non-invasive imaging technique that enables detailed visualization of nail matrix, nail-bed, and vascular abnormalities. This narrative review critically synthesized the current evidence on the diagnostic and clinical applications of onychoscopy in nail psoriasis. Literature search of PubMed/MEDLINE, Scopus, and Google Scholar was conducted, and the available evidence indicates that pitting, onyc

Isolated nail psoriasis: diagnostic challenges and therapeutic advances.

Isolated nail psoriasis, defined as psoriatic nail disease with no or little involvement of the skin, is a clinically significant yet frequently underrecognized condition. Its presentation often mimics other nail disorders such as onychomycosis or lichen planus which can lead to delayed diagnosis and inappropriate treatment. Patients may experience substantial functional impairment and psychosocial distress. This review highlights current knowledge of the epidemiology, clinical manifestations, and diagnostic challenges of isolated NP, with emphasis on populations where disease recognition is p

Marked improvement of isolated acrodermatitis continua of hallopeau with upadacitinib: a case report and literature review.

Acrodermatitis continua of Hallopeau (ACH) is a rare and chronic variant of pustular psoriasis characterized by sterile pustules, periungual inflammation, and progressive nail destruction. Due to its rarity and overlapping clinical features with other nail disorders, ACH is frequently underrecognized and remains difficult to treat, with no standardized therapeutic guidelines. Here, we report a case of long-standing, isolated ACH showing marked improvement after treatment with the selective Janus kinase 1 (JAK1) inhibitor upadacitinib. A 54-year-old man presented with a 13-year history of recur

Nail Involvement in Pemphigus: A Clinical and Dermoscopic Prospective Study.

INTRODUCTION: Nail involvement in pemphigus (NIP) was considered as a rare finding and a predictor factor of the severity of pemphigus. The diagnosis of NIP can be challenging, as it can be mistaken for other nail disorders. The aim of our study was to provide an overview of NIP, with a focus on the dermoscopy role to detect these nail changes. METHODS: A monocentric prospective study was performed from January 2018 to December 2022, investigating clinically and with a dermoscope NIP. RESULTS: Our study included 32 patients, among them 6 patients with NIP. The mean age was 53 years. The aver

True leukonychia: case reports and review of the literature.

Leukonychia refers to the whitening of the nail plate. We identified 22 cases reported in the literature. This study presents two cases of idiopathic leukonychia from Saudi Arabia, which, to our knowledge, are the first reported cases in the region. The first case involves a 26-year-old male with a decade-long history of white discoloration affecting all fingernails. He exhibited low vitamin B12 and elevated thyroid-stimulating hormone levels. The second case involves a 23-year-old female with asymptomatic white discoloration on five fingernails and a positive family history of leukonychia in

BACKGROUND: Vitamins have gained popularity among physicians and patients for purported benefits to hair, skin, and nail health. Safe and efficacious therapies for nail disorders, many of which are chronic conditions, are needed. SUMMARY: We conducted a literature review of studies assessing the efficacy of oral, topical, and intralesional vitamin/vitamin derivatives for the treatment of nail disorders, including yellow nail syndrome, brittle nail syndrome, onychomycosis, habit-tic nail deformity, periungual/subungual verruca, and nail psoriasis. Bacterial and Viral Infections of the Nail Unit.

Bacterial and viral infections of the nail unit are very common as primary infections, especially bacterial paronychia and warts, but they can also be superinfections complicating other nail disorders. In many nail unit infections, the clinical presentation is nonspecific: in these cases, diagnostic tests are mandatory before treatment, to avoid spread of the infection and drug resistance. The most common forms of bacterial and viral infections that may affect the nail unit are herein described in detail, with diagnostic and treatment options provided.

Retronychia of the toenails: a review with emphasis on pathogenesis, new diagnostic and management trends.

Retronychia is a condition, first described in 1999, characterized by the embedding of the proximal nail plate into the proximal nail fold and the stacking of a multiple generations of nail plates beneath the proximal nail fold. The disease affects frequently the toenails and is associated with stress-relevant situations including repetitive trauma, ischemic etiologies, postpartum, and compartment syndrome. Predisposing factors including static disorder of the feet may be underestimated. The paucity of data regarding the predisposing factors is because existing studies are limited to small cas

Examining the importance of laboratory and diagnostic testing when treating and diagnosing onychomycosis.

Onychomycosis is a fungal nail infection caused primarily by dermatophytes. Several other nail disorders, including psoriasis, can simulate onychomycosis. Accurate diagnosis is therefore vital for the ongoing treatment and management of onychomycosis and to avoid misdiagnosis and treatment delay, which can be both lengthy and costly. Often, a combination of histologic and laboratory techniques is used to obtain an accurate diagnosis. The potential diagnostic challenges associated with the differential diagnosis of onychomycosis caused by dermatophytes and the most common techniques used to con

Molecular diagnosis of onychomycosis.

Onychomycosis is a frequent cause of nail infections due to dermatophytes. Molds and yeast may also be responsible of these pathologies. Antifungal treatments are frequently given without a mycological diagnosis, partly because of the requisite time for obtaining the biological results. The mycological diagnosis requires a direct microscopic examination and a culture in order to accurately identify the fungal genus and species. Nevertheless, this conventional diagnosis is often time consuming due to the delay of fungal cultures and presents disadvantages that make it not sufficient enough to g

Development of topical therapeutics for management of onychomycosis and other nail disorders: a pharmaceutical perspective.

The human nail plate is a formidable barrier to drug permeation. Development of therapeutics for management of nail diseases thus remains a challenge. This article reviews the current knowledge and recent advances in the field of transungual drug delivery and provides guidance on development of topical/ungual therapeutics for management of nail diseases, with special emphasis on management of onychomycosis, the most common nail disease. Selection of drug candidates, drug delivery approaches, and evaluation of formulations are among the topics discussed. A comprehensive mathematical description

The impact of nail disorders on quality of life.

BACKGROUND: Nail disorders have considerable psychological impact and may limit activity by impairing functionality of both fingers and toes. OBJECTIVE: To evaluate the impact of nails disorders on quality of life (QoL). METHODS: 1063 patients with nail disorders completed an anonymous nail-specific QoL questionnaire consisting of 24 and 16 questions, respectively, for fingernails and toenails with five possible responses to each question. A score of 1-5 was given to each response and the final score was adjusted on a percentile scale. The subjects were classified in groups according to nail

Onychomycosis: Diagnosis and management.

Onychomycosis is a common nail ailment associated with significant physical and psychological morbidity. Increased prevalence in the recent years is attributed to enhanced longevity, comorbid conditions such as diabetes, avid sports participation, and emergence of HIV. Dermatophytes are the most commonly implicated etiologic agents, particularly Trichophyton rubrum and Trichophyton mentagrophytes var. interdigitale, followed by Candida species and non dermatophytic molds (NDMs). Several clinical variants have been recognized. Candida onychomycosis affects fingernails more often and is accompan

An ex vivo toe model used to assess applicators for the iontophoretic ungual delivery of terbinafine.

PURPOSE: An ex vivo intact toe model was developed to assess two different applicator designs for iontophoretic delivery of terbinafine into the nail only or the nail and surrounding skin. METHODS: Iontophoretic permeation studies were carried out on intact cadaver toes using nail-only and nail/skin applicators with a current dose of 10 mA*min (0.5 mA for 20 min). RESULTS: Iontophoresis enhanced drug permeation and tissue loading with both applicators tested. Greater drug delivery was observed with the nail/skin applicator due to the additional terbinafine being delivered directly through th

Health-related quality of life and nail disorders.

Health-related quality of life (HRQoL) measurements in clinical practice ensure that treatment and evaluations focus on the patient rather than the disease. To verify whether these measurements are also relevant in a minor dermatological disease and whether a generic routine assessment is sufficient to capture different aspects of quality of life, we studied patients affected by nail disorders. The Skindex-29, the 12-item General Health Questionnaire and the 36-item Short Form of the Medical Outcomes Study questionnaires were used. A total of 114 patients were enrolled: 31% men aged 48 +/- 15

Skin diseases of the lower extremities in the elderly.

Routine care of the legs and feet in the elderly is important in helping to prevent infections, malignancies, and further deterioration. The lower extremities are of particular interest because of the increasing amount of diseases and disorders involving the legs and feet. Foot disorders in the elderly are associated with poor choices in footwear, structural changes, brought on by aging, and inadequate knowledge about prevention and treatment. Conservative treatments along with gait modification provide positive long-term results, although sometimes surgery is necessary for severe foot disorde
