A Case of Palmoplantar Keratodermas Complicated by Pseudoainhum and Literature Review.

BACKGROUND: Lichen planopilaris (LPP) is an autoimmune disorder leading to lymphocytic cicatricial alopecia. Different agents such as hydroxychloroquine, methotrexate, cyclosporine, and mycophenolate mofetil have been tried to control hair loss with limited efficacy. JAK inhibitors are immune-modulating drugs which interfere with the JAK-STAT signaling pathway in lymphocytes and are used in treatment of inflammatory conditions such as rheumatoid arthritis and alopecia areata. SUMMARY: Our aim was to determine effectiveness of JAK inhibitors in LPP and its clinical variant, frontal fibrosing a

Epidermal γδ T cells, CD8 T cells and macrophages are increased in number in alopecia areata and express BST2 as part of an interferon-driven antiviral gene signature.

Alopecia areata is an autoimmune disorder affecting approximately 2% of the global population, characterized by immune-mediated disruption of hair follicle immune privilege and unpredictable hair loss. While CD4 and CD8 T cells are established drivers of pathogenesis, the roles of other immune cell populations remain incompletely defined. Here, we reaffirm the pathogenic role of CD8 T cells and identify a contribution of epidermal γδ T cells and macrophages to alopecia areata. Using publicly available single-cell RNA sequencing (scRNAseq) data from the skin of C3H/HeJ mice with alopecia areata

‌Global burden of alopecia areata from 1990 to 2019 and emerging treatment trends analyzed through GBD 2019 and bibliometric data.

The global prevalence of alopecia areata (AA) is estimated to be around 2%. However, the global disease burden of AA and current research hotspots in this field have received limited attention. The disability adjusted life years (DALYs), incidence rates, and prevalence rates of AA from 1990 to 2019 were collected from the Global burden of disease (GBD) 2019 database and analyzed using R software to elucidate the temporal trend in disease burden associated with AA. VOSviewer software was employed to cluster keywords within the field of AA for identification of research hotspots. On a global sca

Stigma, Bullying, and Resilience: Psychosocial Outcomes in Marginalized Adolescents With Visible Dermatologic Conditions.

Visible skin diseases such as acne, eczema, vitiligo, and keloids exert a disproportionate psychosocial burden on marginalized adolescents, particularly among Indigenous, Latinx, and migrant youth. This review illuminates how the cycle of stigma, bullying (both in-person and online), and discrimination is amplified by intersecting cultural, racial, and socioeconomic factors, leading to elevated rates of depression, anxiety, self-harm, and suicide risk. However, despite the gravity of these challenges, psychosocial distress remains routinely underdiagnosed and undertreated in these populations,

Safety Assessment of Ritlecitinib Based on the FDA Adverse Event Reporting System (FAERS) Database: A Real-World Pharmacovigilance Study.

Ritlecitinib has been approved by the U.S. Food and Drug Administration (FDA) for the treatment of severe alopecia areata in adults and adolescents 12 years and older, which has become a promising treatment. Given their widespread clinical use, a comprehensive real-world study of their adverse events (AEs) is warranted. This study aims to offer insights into the characteristics of ritlecitinib's association with AEs. Disproportionality metrics, including the reporting odds ratio, proportional reporting ratio, information component, and empirical Bayesian geometric mean, were employed in our st

Alopecia Areata Associated with Dupilumab: National Database Study.

Background: Alopecia areata (AA), an autoimmune condition causing non-scarring hair loss, often coexists with atopic dermatitis (AD) due to shared T-helper cell type 2 (Th2)-mediated pathways. Dupilumab, a monoclonal antibody inhibiting IL-4 and IL-13 signaling, is a cornerstone treatment for AD but has conflicting reports regarding its impact on AA, with some suggesting therapeutic benefits and others indicating AA induction. Methods: This retrospective study, utilizing the TriNetX Research Network's de-identified data from over 300 million patient records, investigates the association betwee

Is Intralesional Methotrexate an Effective Alternative to Intralesional Triamcinolone in Alopecia Areata? Findings From a Randomized Controlled Trial.

BACKGROUND: Alopecia areata (AA) is an autoimmune condition resulting in hair loss, sometimes just in small patches but occasionally across larger areas like the entire scalp. For localized AA, treatments often involve injecting corticosteroids, such as triamcinolone acetonide (TrA), directly into the affected areas. Methotrexate (MTX), a drug known for its ability to suppress immune responses, has also been considered as an alternative. However, there has not been much research directly comparing these two treatments. METHODS: This study involved 40 individuals with localized AA. These patie

Evaluating Current and Emergent JAK Inhibitors for Alopecia Areata: A Narrative Review.

Alopecia areata (AA) is an autoimmune condition characterized by non-scarring hair loss on the scalp, face, and body, affecting approximately 2% of the global population. Current treatments, including topical corticosteroids, topical immunotherapies, and systemic immunosuppressants, often demonstrate inconsistent efficacy and raise concerns about long-term safety, emphasizing the need for safer and more effective therapies. Janus kinase (JAK) inhibitors have emerged as a promising treatment option, offering a targeted approach by addressing the immune-mediated mechanisms driving hair follicle

Skin Reactions and Other Underappreciated Dermatologic Side Effects of Cancer Therapies.

Cutaneous adverse events (cAEs) are among the most common toxicities associated with modern cancer therapies, which are particularly heightened among immunotherapies and targeted agents. Despite being frequently immune-mediated, the majority of cAEs are mild to moderate and can be effectively managed without interruption of anticancer treatment. In immunotherapies-receiving patients, common phenotypes of cAEs include eczema-like, lichenoid, psoriasiform, vitiligo-like, and bullous eruptions. Targeted therapies including epidermal growth factor receptor inhibitors, BRAF inhibitors/MEK inhibitor

Tofacitinib in alopecia areata and HIV: A curious intersection of immunomodulation, hair regrowth, and hypertrichosis.

Tofacitinib is a Janus kinase 3 (JAK3) inhibitor effective in treating alopecia areata (AA) by promoting hair regrowth. Its mechanism may involve upregulation of vascular endothelial growth factor (VEGF) and reduction of inflammation. However, the range of effects, particularly on hair growth in specific populations, remains under-investigated. We describe a patient with HIV, well-controlled on antiretroviral therapy, who developed hirsutism after starting tofacitinib for AA. This unexpected side effect of hypertrichosis was completely resolved after discontinuing the medication and undergoing

Understanding the Intricate Pathophysiology of Psoriasis and Related Skin Disorders.

Psoriasis is a chronic inflammatory condition that is polygenic and multisystemic, impacting approximately 2-3% of the global population. The onset of this disease is influenced by an intricate interplay of genetic and environmental factors, predisposing individuals to the psoriasis phenotype. The complex pathogenesis of psoriasis contains certain key aspects found in other autoinflammatory and autoimmune dermatological diseases. Among these, vitiligo, alopecia areata, hidradenitis suppurativa, vitiligo, connective tissue diseases, bullous dermatoses, and atopic dermatitis are conditions that

A Scoping Review of Exosome Delivery Applications in Hair Loss.

The objective of this scoping review was to understand the extent and type of evidence found in the current literature on the delivery mechanisms of exosome therapeutics and how these methods can work synergistically with existing treatments for alopecia. Alopecia is primarily characterized as non-scarring or scarring (cicatricial). In cicatricial alopecia, the hair follicles are irreversibly destroyed, causing permanent hair loss. In non-cicatricial alopecia, the hair follicles are undamaged, allowing for possible hair regeneration. Non-scarring alopecia includes androgenetic alopecia, teloge

Addressing the Root Causes of Female Hair Loss and Non-Pharmaceutical Interventions.

Female hair loss affects over 50% of postmenopausal women, with up to 40% of healthy women experiencing frontal/parietal hair loss, often beginning around reproductive age. Androgenetic alopecia is the most prevalent form of hair loss. Other types include alopecia areata, telogen effluvium, and scarring alopecias, such as lichen planopilaris and central centrifugal cicatricial alopecia. Hair loss can severely impact a woman's psychological well-being, yet treatments approved by the US Food and Drug Administration (FDA) remain limited to topical minoxidil and oral Janus kinase inhibitors.
