Cushing's Shadow: An Overlooked Diagnosis in Endocrinology.

PURPOSE: The purpose of this study was to describe a novel heterozygous missense variant in the nuclear receptor subfamily 3 group C member 1 ligand-binding domain causing primary generalized glucocorticoid resistance and highlight diagnostic pitfalls that can mimic Cushing's disease. CLINICAL CASE: A 22-year-old woman was evaluated for suspected Cushing's disease after elevated cortisol levels and pituitary imaging findings. She underwent three transsphenoidal surgeries, but hypercortisolism and clinical symptoms including hirsutism, acne, menstrual irregularities, and weight gain persisted.

Biographical Discovery, Affirmation, and Disruption: Trans and Nonbinary Peoples' Experiences Negotiating Gender and Polycystic Ovary Syndrome.

Polycystic ovary syndrome (PCOS) is the most common endocrine irregularity in people who are assigned-female-at-birth and are of reproductive age. PCOS causes infertility, irregular periods and hirsutism. Cis women with PCOS describe these symptoms as 'masculinising,' which makes them feel inauthentic as women. However, trans and nonbinary people with PCOS may desire a more masculine appearance, but little attention has been given to how these people may experience PCOS. Using Apify, we collected 450 posts from three popular PCOS Reddit communities. We examined titles, posts, and comments for

Models of care on the management of women with polycystic ovary syndrome: A multicentre study.

PURPOSE: International recommendations on the treatment of polycystic ovary syndrome (PCOS) advocate comprehensive assessment, risk stratification, and patient-centred education at first presentation; however, implementation within real-world Models of Care (MoCs) remains poorly understood. This study aimed to characterise PCOS MoCs, assess alignment with international recommendations, and examine variability in investigations at first specialist consultation. METHODS: We conducted a multicentre observational mixed-methods evaluation across eight secondary-care centres in five countries. Quali

Dexamethasone therapy in adolescents with inadequately controlled congenital adrenal hyperplasia: effects on hormonal Suppression, Puberty, and gonadal outcomes.

PURPOSE: Puberty poses challenges for the management of congenital adrenal hyperplasia (CAH) due to changes in the endocrine milieu and non-compliance with treatment. Uncontrolled hyperandrogenemia is a significant problem, especially for CAH patients in this age group. Along with the attainment of final height, a therapeutic window emerges for employing more potent glucocorticoids (GCs) to improve disease management in the pediatric age group. Dexamethasone (Dex) is a long-acting and more potent GC, and offers a convenient dosing schedule for patients. In this study, we aimed to evaluate the

Dual case insights shaping a new protocol for post-menopausal hirsutism.

Hyperandrogenism in women typically presents with hirsutism and other virilizing features. When it presents with accelerated clinical features and/or high androgen levels (total testosterone > 5 nmol/L) (>145 ng/dL) (reference range, <2.4 nmol/L [SI: <70 ng/dL]), particularly in the post-menopausal state, an underlying ovarian or adrenal neoplasm should be considered. With ovarian pathology, androgen-secreting ovarian tumors and ovarian hyperthecosis are the key differential diagnoses. The recommended treatment for androgen-secreting ovarian tumors (ASOTs) is surgical removal. In contrast, ova

Testosterone Therapy in Women: A Literature Review of Clinical Indications, Dosing, Routes of Administration, and Safety.

Testosterone (T) therapy has been used to address symptoms in women such as low libido, fatigue, and mood disturbances. Despite nearly a century of global use and consistent patient demand, the Food and Drug Administration (FDA) has yet to approve a T formulation for women. This narrative review examines the physiological role of T in females, the clinical presentation of androgen insufficiency, and the evidence supporting T therapy in both pre-and post-menopausal women. Androgens are critical to female health, with T being the most abundant biologically active sex androgen in women. It is pro

Severe Hyperandrogenism Revealing a 46,XY Disorder of Sex Development Due to a Novel NR5A1 Mutation.

46,XY disorders of sex development (DSDs) are rare congenital conditions resulting from impaired gonadal differentiation and/or steroidogenesis. The NR5A1 gene encodes steroidogenic factor 1 (SF-1), a key regulator of gonadal development and endocrine function, and pathogenic variants are associated with a broad phenotypic spectrum that may present beyond the neonatal period. We report the case of a 12-year-and-6-month-old phenotypic female adolescent presenting with severe hirsutism, acne, primary amenorrhea, and progressive virilization. Hormonal evaluation demonstrated hyperandrogenism with

Hirsutism beyond PCOS: Genome-wide evidence for genetic factors.

Hirsutism is a condition characterized by excessive hair growth in women, with a prevalence of 5-10 %. The diagnostic criteria usually include hair appearing in male-pattern areas, such as the face, abdomen, and chest. The contributing factors for hirsutism symptoms are heterogeneous and vary from genetic and ethnic backgrounds to diverse endocrinological causes, most commonly polycystic ovary syndrome. In this cross-ancestry GWAS, including 4834 cases and 352,966 controls, we explored the genetic background of hirsutism and identified 7 loci associated with the condition, 4 of which have not,

Adjunctive effects of apple cider vinegar on metabolic syndrome and insulin resistance in polycystic ovary syndrome patients receiving metformin: a randomized clinical trial.

Polycystic ovary syndrome (PCOS) is a common endocrine disorder in reproductive-aged women, frequently associated with insulin resistance (IR) and metabolic syndrome (MetS). Given the limitations of conventional therapies, complementary interventions targeting metabolic pathways are needed. This randomized clinical trial investigated the adjunctive effects of apple cider vinegar (ACV) on MetS components and IR in PCOS patients receiving standard metformin treatment (1500 mg/day). In this 12-week randomized controlled trial, 94 women meeting the Rotterdam criteria for PCOS, NCEP ATP III criteri

Pregnancy-induced Cushing syndrome: diagnosis and management of a rare endocrine disorder.

Pregnancy-induced Cushing syndrome (PICS) is an exceptionally rare form of adrenocorticotropin (ACTH)-independent Cushing syndrome (CS) caused by aberrant expression of luteinizing hormone (LH) and/or human chorionic gonadotropin (hCG) receptors in adrenal cortical cells. To date, only a small number of cases have been reported, most with adrenal abnormalities on imaging. We describe a 25-year-old pregnant woman who presented at 26 weeks' gestation with new-onset hypertension and overt features of hypercortisolism, including severe acne, hirsutism, dorsocervical fat pad, and extensive violaceo

Nonclassic congenital adrenal hyperplasia and bilateral pheochromocytomas in a patient with a germline pathogenic MAX variant.

We present the case of a patient with bilateral pheochromocytomas and newly diagnosed nonclassical congenital adrenal hyperplasia (NCCAH). While NCCAH is relatively common, pheochromocytomas are extremely rare. The patient is a 29-year-old woman who was referred for investigation of hirsutism, with a past medical history of hypertension since the age of 18 and precocious pubarche. Magnetic resonance imaging (MRI) showed a right adrenal mass of 4.4 × 4.7 cm and a left adrenal mass of 2.9 × 2.9 cm. Biochemical testing confirmed NCCAH and also revealed elevated 24-hour urinary catecholamines, as

The psychosocial experiences of women diagnosed with PMOS in Trinidad and Tobago: a qualitative inquiry.

Polyendocrine Metabolic Ovarian Syndrome (PMOS) is a globally prevalent endocrine disorder affecting reproductive-aged women and associated with numerous long-term health risks. While growing international research on PMOS examines both medical and non-medical aspects of this condition, few studies exist within the Caribbean region, mostly focused on the clinical aspects. Thus, the current study explores the possible psychosocial impacts of women diagnosed with PMOS in Trinidad and Tobago. A qualitative study was conducted with 24 women aged 18-45 years with a physician-confirmed PMOS diagnosi

Rebound Thymic Hyperplasia after Recovery from Ectopic Cushing Syndrome: A Case Report and Literature Review.

INTRODUCTION: Rebound thymic hyperplasia (RTH) is a rare benign condition resulting from compensatory proliferation of thymocytes after remission from stressful conditions, including hypercortisolism. Its appearance can sometimes be misleading, making differential diagnosis with thymic epithelial or neuroendocrine tumor challenging. CASE PRESENTATION: We describe the case of a 31-year-old patient who was referred to our outpatient clinic for worsening hirsutism, acne, and fatigue. Biochemical and dynamic tests showed an ACTH-dependent hypercortisolism of ectopic origin (Ectopic Cushing syndro

Adaptation and exploratory factor analysis of a PCOS-specific HRQoL questionnaire in a South Indian population.

INTRODUCTION: Polycystic Ovary Syndrome (PCOS) is a multifaceted disorder with physical, emotional, and social implications. Culturally adapted tools for assessing Health-Related Quality of Life (HRQoL) in Indian women, particularly those under 18 and over 45 years of age, need to be developed. This study aimed to culturally adapt the Nasiri-Amiri PCOS-specific HRQoL questionnaire for Malayalam-speaking women, evaluate its reliability and factor structure using exploratory factor analysis (EFA), and identify key HRQoL domains in this population. METHODS: Between July 2024 and December 2025, a

Yoga as a complementary intervention for polycystic ovary Syndrome management: a systematic review.

BACKGROUND: Polycystic Ovary Syndrome (PCOS), a prevalent metabolic and reproductive disorder, significantly impacts women of reproductive age. The conventional approach offers various symptomatic pharmacological interventions for PCOS, but some of them have adverse effects too. In this context, yoga has emerged as a promising non-pharmacological complementary approach. AIM: This systematic review aims to explore the therapeutic potential of yoga for managing PCOS with a focus on anthropometric, metabolic, endocrine, and psychological outcomes. METHODS: A systematic search was conducted acro
