Introducing A Stepwise Anatomic Framework for Urogenital Reconstruction Derived From Orofacial Plastic Surgery Principles: A Case Report.

We present a novel labial reconstruction technique in a 48-year-old woman with extensive bilateral labial defects caused by steatocystoma multiplex. An anatomy-guided, stepwise approach integrating scalp and orofacial reconstructive principles enabled successful restoration using a rhomboid mucocutaneous flap, minimizing tissue tension while preserving neurovascular continuity, function, and aesthetics.

A Rare Presentation of a Pilar (Trichilemmal) Cyst of the Upper Arm in an Elderly Male.

Pilar cysts, also known as trichilemmal cysts, are benign keratin-filled lesions that arise from the outer root sheath of the hair follicle. Pilar cysts account for a small minority of cutaneous cysts and are most commonly found on the scalp, particularly in middle-aged women. Because they overwhelmingly occur on the scalp, presentation in other anatomical locations is uncommon and can create diagnostic uncertainty. We present the case of an 81-year-old male with a slowly enlarging, painless mass on the left upper arm that had been present for nearly eight years. The lesion was well-circumscri

Endoscopic repair of large dural defects in transsphenoidal surgery by suturing acellular dermal matrix graft with the dura: a technical note.

OBJECTIVE: To introduce acellular dermal matrix (ADM) suturing as a novel repair method for intraoperative high-flow cerebrospinal fluid (CSF) leakage during transsphenoidal surgery (TSA), aiming to replace autologous fat/fascia lata grafts and reduce surgical morbidity. METHODS: Five patients (2 invasive pituitary adenomas, 2 craniopharyngiomas, 1 epidermoid cyst) underwent total sellar/suprasellar tumor resection and presented intraoperative high-flow CSF leakage. ADM grafts were tailored to dural defects and sutured directly to native dural edges. Outcomes included repair integrity, operat

Unusual variants of trichilemmal tumors - Report of two cases with review of literature.

Trichilemmal tumors are rare dermal neoplasms derived from the outer root sheath of hair follicles, showing a follicular differentiation on histology and are categorized into benign and malignant entities. Usually presenting as solitary lesions, they have a predilection for women in the fourth to eighth decades of life, with the scalp being the most common site of occurrence. In this article, we report two extremely rare cases, one diagnosed as malignant proliferating trichilemmal tumor with sarcomatous differentiation (MPTT-S) and the other as proliferating trichilemmal cyst with foci of carc

Superimposed intraoperative neuromonitoring for epidermoid cyst: Two-dimensional operative video.

BACKGROUND: Intraoperative neuromonitoring (IONM) is critical for preserving cranial nerve function during cerebellopontine angle (CPA) tumor surgery, particularly when multiple cranial nerves are involved. We report a case of an epidermoid cyst resection in which IONM waveforms were projected in real time onto the surgical microscope display. CASE DESCRIPTION: A 36-year-old woman presented with the left facial spasm and numbness. Magnetic resonance imaging (MRI) revealed a left CPA lesion extending from the medulla to the midbrain, compressing the brainstem and involving cranial nerves III-X

Sweat gland carcinoma with neuroendocrine differentiation (SCAND) arising in the axilla: A case report highlighting diagnostic challenges and surgical management.

BACKGROUND: Sweat gland carcinoma with neuroendocrine differentiation (SCAND) is a rare, newly recognized cutaneous adnexal tumor arising from sweat glands and characterized by neuroendocrine features. Given its rarity and resemblance to benign lesions, clinical diagnosis can be challenging. CASE PRESENTATION: A 73-year-old man presented with a 40-year history of intermittent discharge from a right axillary mass, which had initially been diagnosed as an epidermal cyst. Following lesion excision along the tumor margin at a local clinic, histopathological analysis revealed apocrine carcinoma, a

An inflamed epidermal cyst treated with a vacuum-assisted biopsy device.

Inflamed epidermal cysts (ECs) are typically excised once inflammation subsides. Although incision and drainage are usually performed to reduce inflammation, we herein report a case of an inflamed EC, which was treated using a vacuum-assisted biopsy device (VABD). A 93-year-old woman presented with pain and redness in the left breast. We treated using VABD to achieve maximal lesion removal, and conducted a pathological examination of the obtained tissue. The lesion was pathologically diagnosed as an EC. Although the pain and inflammation had subsided within a week of treatment, a red nodule pe

Recurrent secondary milia after full-thickness skin graft using retroauricular donor skin for dog-bite defect: a case report.

Full-thickness skin grafting (FTSG) is frequently used to reconstruct facial soft tissue defects because it provides favorable color and texture matching. Secondary cystic lesions, including milia or epidermal cysts, that develop after FTSG are rare. A 29-year-old woman sustained a dog-bite injury resulting in a 4.5× 2.5 cm defect involving the philtrum and upper lip. The philtrum was reconstructed using a full-thickness skin graft harvested from the retroauricular area, while the upper lip was repaired using a mucosal V-Y advancement flap. Ten months later, hypertrophic scarring developed, an

Clinical presentation, imaging features and surgical outcomes of pineal region epidermoid cysts: A scoping review.

Epidermoid cysts comprise about 1% of intracranial tumors, they are benign and extra-axial, arising from ectopic ectodermal tissue. Pineal region localization is rare. Surgical treatment can be challenging due to critical structures encountered along surgical corridors. We aimed to synthesize the literature compiled on this disease and present an illustrative case. We performed a scoping review according to PRISMA-Sc guidelines. Studies describing pineal region epidermoid cysts reporting clinical, surgical, or radiological data were included. A total of 33 patients from 31 studies were include

Parietal Intradiploic Epidermoid Cyst Invading the Dura Mater.

The authors present a 16-year-old male patient who developed an epidermoid cyst located at the right parietal bone, initially noticed after trauma at age 8. Over time, the cyst gradually increased in size without neurological symptoms. However, after a second trauma at age 16, he developed muscle weakness and numbness in the left upper limb. Imaging revealed an epidermoid cyst invading the dura mater, causing brain tissue compression and hemorrhage. The patient underwent craniotomy and total excision of the lesion, and pathology confirmed the diagnosis. Postoperatively, the patient showed no n

Spontaneous Expulsion of a Sebaceous Cyst: A Case Report of a Rare Surgical Outcome.

Sebaceous cysts are lesions commonly found on the skin. Although they can occasionally get infected and rarely have other complications, they seldom cause any serious problems to the patient. Traditional treatment is through an elliptical skin incision, which can be delayed if the cyst is infected. Other minimally invasive methods of excision/treatment have also been described, which result in less scarring, better cosmetic outcome, and low morbidity. The present case is about an unusual outcome that occurred during the management of a sebaceous cyst. The patient presented with an inflamed seb

Primary Intracranial Squamous Cell Carcinoma Arising from an Epidermoid Cyst: Successful Management with Subtotal Resection and Gamma Knife Radiosurgery in an Elderly Patient.

Primary intracranial squamous cell carcinoma (SCC) arising from an epidermoid cyst is an exceptionally rare and aggressive malignancy with a dismal prognosis. Conventional management typically involves gross total resection followed by wide-field radiotherapy; however, this intensive approach is often unfeasible for elderly or frail patients. We present a case of primary intracranial SCC in a 75-year-old woman who presented with rapid cochleovestibular deterioration. Imaging revealed subtle enlargement of a long-standing cerebellopontine angle epidermoid cyst. Subtotal resection was performed

Characterization of dermatofibromas and epidermoid cysts using dermoscopy-guided high-frequency ultrasound imaging.

PURPOSE: Epidermoid cysts and dermatofibromas may resemble malignant lesions, requiring accurate non-invasive differentiation. Our objective was to assess dermoscopy-guided high-frequency ultrasound (DG-HFUS) in distinguishing these lesions. METHODS: Nine lesions (four epidermoid cysts, five dermatofibromas) were examined using DG-HFUS with a 33 MHz linear probe in B-mode. The dermoscopic modality guided probe placement. For each lesion, a minimum of 10 consecutive frames were acquired to ensure measurement reproducibility. Lesions were evaluated based on predetermined characteristics. RESUL

[A Case of Testicular Epidermoid Cyst Treated by Tumor Enucleation].

Testicular epidermoid cysts are relatively rare benign testicular tumors that are primarily observed in young adults. Most testicular tumors in adults tend to be malignant, and high inguinal orchiectomy is often performed to treat testicular epidermoid cysts. However, accurate preoperative and intraoperative diagnoses can make testis-sparing surgery a viable option. A 25-year-old man presented with a painless mass in the right scrotum. Ultrasonography revealed a hypoechoic lesion with an echogenic rim and an onion ring sign. Magnetic resonance imaging showed the target sign in the right testis

Common Vulvar Neoplasms-Benign and Malignant.

Common vulvar neoplasms include benign lesions such as epidermal inclusion cysts, fibroepithelial polyps, and Bartholin gland tumors, as well as malignant entities including squamous cell carcinoma, melanoma, extramammary Paget disease, and sarcomas. Benign growths can closely resemble malignant processes, creating diagnostic challenges, while malignant tumors are associated with high morbidity and mortality. Advances in histopathology, imaging, and molecular techniques have improved diagnostic precision and informed more individualized therapeutic strategies. Comprehensive understanding of be

Management of Finegoldia magna-associated non-puerperal breast abscess in an adolescent: Integration of surgical source control and targeted antimicrobial therapy.

Finegoldia magna is an opportunistic anaerobic pathogen rarely causing non-puerperal breast abscess (NPBA) in adolescents. Its biofilm-forming capacity often complicates standard treatments. We report a 17-year-old female who presented with NPBA following influenza A and the rupture of an epidermal cyst. Anaerobic culture identified F. magna alongside Staphylococcus simulans. Guided by the patient's penicillin allergy and anaerobic resistance trends, systemic cefmetazole was administered. Concurrently, ultrasound-guided mechanical cavity debridement was performed, followed by percutaneous indw
