Here, we report an exceptionally rare event of malignant transformation of an intracranial epidermoid cyst into squamous cell carcinoma (SCC), diagnosed through cerebrospinal fluid cytology. A 61-year-old male with a prior history of left retrosigmoid craniotomy and partial resection of a large cerebellopontine angle epidermoid tumor 13 years ago presented with acute mental status changes and weakness. Imaging revealed recurrent epidermoid cyst elements with associated obstructive hydrocephalus. The patient underwent ventriculoperitoneal shunt placement, and cerebrospinal fluid cytology reveal

Background/Objectives: Vascular malformations form masses in subcutaneous and muscular tissues throughout the body and are occasionally misdiagnosed as subcutaneous nonvascular tumors. Understanding and differentiating their clinical and imaging features are crucial due to their different treatments and prognoses. This study aimed to report cases of nonvascular tumors that were initially misdiagnosed and treated as vascular malformations. Methods: In this retrospective observational study, we enrolled 14 (1.8%) patients with pathologically diagnosed nonvascular tumors from among 536 patients w

OBJECTIVE: This study aims to investigate the imaging features of epidermoid cysts within intrapancreatic accessory spleen (ECIPAS) to benefit the differential diagnosis of pancreatic cystic lesions.
METHODS: We retrospectively reviewed the clinical, radiological and pathological data of 24 patients with pathologically confirmed ECIPAS. All cases underwent abdominal contrast-enhanced computed tomography (CE-CT) scans, with 4 cases further undergoing magnetic resonance cholangiopancreatography (MRCP). The imaging features of the lesions were analyzed.
RESULTS: The majority of the patients wer

Cystic neoplasms are frequently observed on the skin. These can be classified into three categories: (1) malformative/dysembryoplastic cysts, (2) acquired cysts (constantly cystic tumors) of epidermal or folliculo-sebaceous differentiation, (3) acquired cysts of sweat differentiation, which must be distinguished from sweat tumors, sometimes harboring cystic changes. The aim of this session is to present the most frequent cystic lesions, those associated with a genetic syndrome (notably cancer predisposition syndromes), and malignant tumors that may simulate cysts.

Eccrine hidrocystomas are rare, benign cystic lesions that usually affect the scalp, cheeks, and eyelids. They are thought to originate from the sweat glands. These lesions can be single or many in nature and frequently worsen in the summer from increased perspiration. They are caused by dilated ducts of eccrine sweat glands. Clinically, they seem like small, transparent cystic lesions that are painless, and they usually affect middle-aged or older people. However, a histological study is necessary to make a final diagnosis. We present a case of a 35-year-old woman who had an epidermal inclusi

Epidermoid cysts originating from the upper urinary tract are extremely rare, and only five cases have been reported previously. We report a rare case of epidermoid cyst that originated from the right ureter : A 76-year-old man presented with asymptomatic gross hematuria. Computed tomography showed a right ureteral tumor (14×8 mm in size). Retrograde pyelography showed a defect (14 mm diameter) of the contrast medium in the same region. Urinary cytological examination indicated class III. A ureteral tumor was clinically indicated, and retroperitoneoscopic-assisted radical nephroureterectomy wa

Epidermal cysts are common benign cutaneous tumors that usually appear as slowly growing lumps of up to a few centimeters in diameter. They rarely grow >5 cm in diameter and are referred to as giant epidermal cysts. Although soft tissue tumors >5 cm are recommended for imaging examination and biopsy preoperatively, biopsy of giant epidermal cysts has rarely been performed because of the complications that may arise when non-absorbable materials are released from the cyst. Here, I report a case of a giant epidermal cyst in which a core needle biopsy was performed, and the cyst was preoperativel

Proliferating trichilemmal tumor (PTT) is a rare, benign adnexal tumor of hair follicles that commonly mimics malignancy. Excellent outcomes can be achieved with early surgical excision. Delayed presentations-as in this giant shoulder PTT-are made possible, in part, by patient education and healthcare accessibility in low-resource settings.

INTRODUCTION AND IMPORTANCE: Proliferative trichilemmal tumor (PTT) is an exceedingly rare tumor arising from the hair follicle's isthmic portion. While most commonly found on the scalp of elderly women, its occurrence in the clavicular region and transformation to a malignant phenotype are seldom observed.
CASE PRESENTATION: We report a 67-year-old woman with squamous cell carcinoma degenerated from a trichilemmal proliferating tumor located in the clavicular region. The extension assessment did not reveal metastases, she benefited from surgical resection of the mass with an adenectomy sus c

Bone lesions of the lateral orbit are commonly indicated for surgical resection. While surgeons have a plethora of techniques to choose from, the transpalpebral approach offers a minimally invasive corridor with favourable outcomes. In this report, we describe this approach in further detail for the treatment of an epidermoid cyst arising from the sphenoid wing and lateral orbit in a female in her 50s. This patient underwent a successful transpalpebral lateral orbital approach for their sphenoid wing and orbital rim bone lesion at a tertiary medical centre. The surgical length was 110 min and

Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is an uncommon, low-grade, malignant tumor that predominantly affects older patients and is often located around the eyelids. Diagnosing EMPSGC is challenging because of its clinical and imaging similarities to benign lesions such as epidermal cysts. This report presents two cases of EMPSGC with ultrasound features resembling benign cystic lesions that were initially considered in the differential diagnosis. For both cases, ultrasound imaging revealed hypoechoic masses with posterior acoustic enhancement and peritumoral vascularity, whic

OBJECTIVE: To summarize the ultrasonography (US) and magnetic resonance imaging (MRI) findings of lymphoepithelial cysts (LECs) and dermoid cysts (DCs) and to discuss the hyperinternal echo in LECs based on histopathological findings.
METHODS: Patients who underwent US and MRI before resection and whose lesions were pathologically diagnosed as LECs or DCs were included (January 2010 to December 2023). Internal echo, convective flow, and Doppler signals were evaluated on US, while on MRI, we measured the apparent diffusion coefficient (ADC).
RESULTS: Surprisingly, all lesions (4 LECs and 7 DC

This study presents a congenital cranial epidermoid cyst with an unprecedented 70-year latency period, elucidating its distinct clinicoradiological features. A 72-year-old woman presented with a slowly enlarging, painless mass in the left frontoparietal region. Imaging revealed skull erosion, restricted diffusion on diffusion-weighted imaging, and hypoperfusion on perfusion-weighted imaging. Cinematic rendering was innovatively applied, providing three-dimensional visualization of intracystic cholesterol deposits and calcifications, which guided precise surgical excision (pathologically confir

Trichilemmal cysts (pilar cysts) are common adnexal tumors that mostly affect the scalp. They are usually benign, but can sometimes be malignant. Despite being common, there is limited data on what factors influence lesion size, multiplicity and risk of infection. The present retrospective observational study evaluated 27 patients who underwent excision of trichilemmal or related cystic tumous between January 2020 and June 2025 at a tertiary plastic surgery centre. The clinical nkvariables encompassed age, sex, diabetes mellitus (DM), hypertension (HT), tobacco use, Fitzpatrick skin type, and,

Epidermal inclusion cysts are defined as dermal or subcutaneous epithelial cysts which are filled with keratin and lined with true epidermis. Although exceedingly rare, they can lead to pathological conditions, such as nerve compression syndrome. To the best of our knowledge, this is the first documented case of a 59-year-old woman with ulnar nerve compression at the wrist due to dog bite hand injury. An epidermal inclusion cyst was located adjacent to the ulnar nerve, resulting in significant compression. Notably, no direct connection was observed between the cyst and ulnar nerve. Surgical re

RATIONALE: A natural malignant transformation of intracranial epidermoid cysts (IECs) into squamous cell carcinomas (SCCs) is very rare, and the mechanism remains unclear. Due to its rarity, there is currently no consensus on the management of primary intracranial SCC (PISCC) at present.
PATIENT CONCERNS: A 55-year-old woman was with an IEC in the right cerebellopontine angle region in our report. Her neurological symptom deteriorated rapidly in the 1 month before the admission.
DIAGNOSES: The preoperative diagnosis was IEC. Postoperative pathological examination confirmed the presence of SC
