Tocilizumab as a treatment tool for ROSAH syndrome: a case report.

BACKGROUND: ROSAH syndrome (Retinal dystrophy, Optic nerve edema, Splenomegaly, Anhidrosis, and Headache) is a rare autosomal dominant disorder caused by heterozygous missense pathogenic variants in ALPK1. This gene encodes alpha-kinase 1, a key regulator of the inflammatory NF-κB pathway. Pathogenic variants in ALPK1 are associated with increased NF-κB activation and chronic inflammation. Clinically, ROSAH syndrome presents with early-onset retinal dystrophy and optic nerve edema, alongside systemic inflammatory and non-inflammatory features. CASE PRESENTATION: We present a 16-year-old femal

Partial Horner's Syndrome Secondary to Carotid Dissection With an Associated Folate Deficiency.

Horner's syndrome is an uncommon clinical finding characterized by the triad of ptosis, miosis, and anhidrosis. It typically results from disruption along the oculosympathetic pathway. When anhidrosis is absent, the partial Horner's syndrome suggests a lesion distal to the superior cervical ganglion and is an important clinical clue. One of the most important causes in young patients is carotid artery dissection, a potentially serious but often under-recognized condition. We report the case of a 34-year-old woman who presented with a left-sided headache and new-onset ptosis. Examination reveal

A rare case of LORICRIN gene c.684dup mutation associated with Vohwinkel syndrome in a Turkish patient, in silico analysis and literature review.

BACKGROUND: Vohwinkel Syndrome (VS) is a rare autosomal dominant skin disorder with two main mutation types: the classic form caused by GJB2 gene mutations and loricrin keratoderma (LK) linked to LOR gene mutations. LK typically lacks hearing loss and often presents at birth as collodion baby syndrome. METHODS: A 7-year-old male proband presenting with congenital thickening of the palms and soles was admitted to Başakşehir Çam and Sakura City Hospital. Whole-exome sequencing was performed from a whole blood sample using the MGI-400 platform, and common or benign mutations were excluded. Segre

Horner's Syndrome after Carotid Endarterectomy: A Case Report and Review of Literature.

Horner's syndrome, characterized by ptosis, miosis, and anhidrosis, results from oculosympathetic complex injury, often due to trauma affecting the superior cervical ganglion. Although rare following carotid surgery, we present a case of Horner's syndrome after elective carotid endarterectomy (CEA). This report explores potential mechanism, including prolonged surgical retraction and hematoma formation, while reviewing similar cases in the literature. A 45-year-old woman presented with recurrent dizziness and progressive left-sided hearing impairment over 5 years. She also reported neck discom

Idiopathic Harlequin Syndrome: A Case Report of an Uncommon Disease.

Harlequin syndrome is a rare autonomic disorder presenting with unilateral facial flushing and sweating due to sympathetic dysfunction, often idiopathic but occasionally secondary to an underlying pathology. We report a 52-year-old woman with a three-year history of left-sided hemifacial flushing and hyperhidrosis, most noticeable on exertion or heat exposure. She had comorbid diabetes, hypertension, and dyslipidemia. Neurological examination was normal. Exercise provocation reproduced the symptoms, and the iodine-starch test confirmed hyperhidrosis on the left side with anhidrosis on the righ

Beyond the Headache: A Subtle Horner's Syndrome Revealing Carotid Artery Dissection.

Partial Horner's syndrome, characterized by ptosis and miosis without anhidrosis, is a classic but often underrecognized manifestation of internal carotid artery (ICA) dissection and may represent the sole clinical indicator of an evolving vascular pathology. Horner's syndrome is a neurological disorder caused by interruption of the sympathetic nerve pathways that supply the eye and face. Despite its clearly defined clinical features, Horner's syndrome is relatively uncommon, and standardized management guidelines are limited. Early detection relies on CT angiography (CTA), as the initial non-

Clinical and Genetic Characterization of Hereditary Sensory and Autonomic Neuropathy Type IV in a Consanguineous Population: Identification of Novel NTRK1 Variants and Expansion of Phenotypic Spectrum.

Hereditary sensory and autonomic neuropathy type IV (HSAN4) is a rare neurological disorder characterized by anhidrosis and congenital insensitivity to pain caused by biallelic pathogenic variants in NTRK1. The condition develops because of dorsal root and autonomic ganglion neurodegeneration, which ultimately results in reduced sensation and autonomic neurological dysfunction. We ascertained several neuropathic patients and performed genetic testing using gene panels and exome sequencing (ES). Genetic variants were confirmed by Sanger sequencing. Thirteen families, each with a single affected

Congenital insensitivity to pain with anhidrosis: a case report.

Although it is very rare, the syndrome of Congenital Insensitivity to Pain with Anhidrosis (CIPA) should be known to anaesthetic practitioners due to its perioperative implications. We report the first case of this in the British Isles, in a 5-year-old girl, whose osteomyelitis required surgical intervention. The genetics, the cellular basis and pathophysiology of the condition are outlined, and the relevance of analgesia in this challenging setting is considered in this report.

A Novel Inflammatory Autoimmune-Like NTRK1-Associated Phenotype in an Adult Man.

INTRODUCTION: Neurotrophic tyrosine receptor kinase 1 (NTRK1) encodes a 796 amino-acid-long transmembrane nerve growth factor (NGF) receptor, which is abundantly expressed in neuromuscular tissues. Deficiency of NTRK1 is typically clinically presented as autosomal recessive infantile congenital insensitivity to pain with anhidrosis (CIPA), characterized by decreased pain and temperature perception, anhidrosis, and, sometimes an intellectual disability and a premature death. So far, over 170 different NTRK1 mutations have been reported in the literature, including the missense disease-causing v

ACR Appropriateness Criteria® Horner Syndrome.

Horner syndrome is a combination of signs and symptoms caused by abnormalities in the oculosympathetic pathway. Typically, patients present with ipsilateral miosis, ptosis, and anhidrosis. Imaging the oculosympathetic pathway is complex because of its extensive course, need for targeted protocols, and the wide range of pathology that can cause it. For this document, we focus our attention on workup guidelines based on clinical features: 1) acuity (acute, nonacute including chronic, insidious, and unknown onset), 2) painless or painful (neck pain, occipital region pain, or headaches), 3) whethe

Prolonged Horner's Syndrome After Interscalene Nerve Block: A Case Report and Summary of the Literature.

Horner's syndrome is a well-known complication of interscalene nerve blocks and is caused by a disruption along the oculosympathetic pathway that results in the classic triad of ipsilateral miosis, ptosis, and anhidrosis, although the presence of anhidrosis may vary based on anatomy and location of injury. While symptoms typically subside promptly with the resolution of anesthesia, long-lasting Horner's syndrome is a documented side effect. The authors report the case of a 43-year-old woman with prolonged Horner's syndrome following an interscalene nerve block and shoulder surgery. The authors

Erythroderma, Alopecia, Anhidrosis, and Vitiligo as Complications of a Red Ink Tattoo-A Case Report.

Background: Adverse reactions to tattoo pigments are increasingly recognized, yet severe systemic complications remain rare and poorly characterized. Red tattoo ink, in particular, is associated with delayed hypersensitivity reactions, but widespread manifestations affecting multiple organ systems have not been documented. This case report aims to describe an unusual constellation of erythroderma, alopecia universalis, anhidrosis, and vitiligo triggered by red tattoo ink and to highlight the diagnostic and therapeutic challenges associated with such reactions. Case presentation: This case repo

A case study of a novel homozygous EDAR splice site variant in hypohidrotic ectodermal dysplasia with tooth agenesis: molecular dynamics insights.

Hypohidrotic ectodermal dysplasia (HED) is a genetic disorder that can caused by mutations in the EDAR gene, which encodes the Ectodysplasin A receptor, leading to defective ectodermal structure development. This study investigates the molecular impact of a novel homozygous c.730 + 1G > T splice site variant in the EDAR gene, identified in a consanguineous Iranian family with HED. The 10-year-old proband presented with a classic, severe HED phenotype, including anhidrosis (impaired sweating) leading to recurrent hyperthermia, sparse hair, dry skin, and severe oligodontia with only three teeth

Horner's Syndrome as a Rare Complication of Thyroidectomy With Central Neck Dissection: A Case Report and Review of Potential Mechanisms.

Horner's syndrome (HS) is characterized by miosis, ptosis, enophthalmos, anhidrosis, and vascular dilation on one side of the face, resulting from an interruption in the oculosympathetic pathway. Here, we present a 55-year-old female patient diagnosed with thyroid carcinoma metastasis via fine-needle aspiration biopsy of the right supraclavicular lymph node. She underwent total thyroidectomy and right central lymph node dissection. On the first postoperative day, the patient developed right-sided miosis and upper eyelid ptosis, leading to a diagnosis of HS. Although this rare complication foll

Hippo signaling in mammalian reproduction.

ABSTRACT: Chronic endometritis, defined by chronic inflammation of the endometrium, remains a clinical and biologic challenge even using hysteroscopy allowing a direct vision of the uterine cavity without anesthesia, and conventional histology using Hematoxylin and Eosin staining. Our primary objectives were to evaluate the relevance of hysteroscopy and conventional histology compared to immunohistochemical expression of syndecan-1 (CD138, a marker of plasma cells), which is a heparan sulfate proteoglycan involved in inflammation and enables diagnosis of chronic endometritis. The second object

The role of fetal pancreatic islet cell transplantation in the treatment of type 2 diabetes mellitus.

BACKGROUND: There is growing evidence that the development of allergic sensitization can be influenced by environmental co-factors. Studies showed that growing up on a farm can protect children against allergic sensitization. However, little is known whether this 'farming effect' can only be observed in early lifetime or whether it also plays a role in later childhood. OBJECTIVE: The aim of our study was to test whether a farming environment is negatively associated with a new occurrence of skin prick test (SPT) positivity in school children. As a secondary outcome we investigated whether chi
