A de novo INSR variant in Type A insulin resistance syndrome: familial investigation and genetic implications.

Type A insulin resistance syndrome (TAIRS) is a rare autosomal dominant disorder associated with variants in the Insulin Receptor (INSR) gene. It is characterized by insulin resistance, hyperandrogenism, and acanthosis nigricans. The severity of the condition may be influenced by homozygosity or heterozygosity, with some female patients being misdiagnosed with polycystic ovary syndrome (PCOS). A 13-year-old female proband from a family was identified with hyperinsulinemia, hyperandrogenism, acanthosis nigricans, hirsutism, acne, oligomenorrhea, and masculinization. Exome sequencing and Sanger

Cutaneous signs of insulin resistance with central obesity: insights into adipocentric metabolic dysfunction in South Asians.

AIM AND OBJECTIVES: To investigate the Cutaneous Signs of Insulin Resistance, namely acanthosis nigricans (AN) and acrochordon (AC), in individuals with Central Obesity (CO-CSIR) as physical predictors of metabolic syndrome (MetS), underlying adipose tissue pathology, and consequent pathophysiological traits in South Asians. METHODS: In this study, 371 participants were recruited in a tertiary care facility and grouped based on the presence of cutaneous signs (AC and/or AN) and central obesity. Each participant was assessed for MetS, T2D, as well as other demographic, biochemical, and radiolo

Clinical and Dermoscopic Evaluation of the Effectiveness of Metformin versus Pioglitazone in Acanthosis Nigricans: A Randomised Controlled Trial.

BACKGROUND: Acanthosis nigricans (AN) features thick, hyperpigmented skin in flexural areas, often indicating insulin resistance. Dermoscopy of AN demonstrates sulci cutis and crista cutis, and differentiates it from other pigmentary dermatoses. Comparative efficacy studies are lacking in the literature. This study evaluates the clinical and dermoscopic changes in AN treated with metformin and pioglitazone. PATIENTS AND METHODS: A randomised open-label trial was conducted on consenting AN patients aged 18 years or older. Patients were randomized to receive either metformin 500mg twice a day o

Population-based Characterization of PTEN Hamartoma Tumor Syndrome.

PTEN hamartoma tumor syndrome (PHTS) is a cancer predisposition disorder caused by germline PTEN variants, yet its full clinical spectrum remains poorly defined due to reliance on highly selected cohorts. Accordingly, PHTS is underrecognized and its prevalence underestimated. Leveraging genomic and electronic health record data from 414,830 participants in the All of Us (AoU) Research Program, we identified 55 individuals with pathogenic or likely pathogenic PTEN variants, the majority of whom lacked a prior PHTS diagnosis, underscoring underrecognition in the general population. PHTS affects

Skin manifestations in diabetes-what is new?

Diabetes is a chronic disease with a continuously increasing prevalence worldwide. Chronic hyperglycaemia results from elevated blood glucose levels due to disturbed insulin secretion and/or action. Diabetes adversely affects the structure and function of micro- and macrovasculature, leading to the failure of various organs and tissues. Diabetes complications affect the kidneys, retina, peripheral nerves, heart, brain, muscle, and skin. Approximately 30% of diabetic patients have cutaneous manifestations, which may be the first sign of metabolic derangement. Skin manifestations strongly associ

Assessing the efficacy and safety profiles of 0.025% tretinoin in treating axillary hyperpigmentation with acanthosis nigricans: a randomized double-blinded study.

Acanthosis nigricans (AN) is a dermatological condition, marked by hyperpigmentation and skin thickening, frequently affecting body folds like the axillae. Treatment options for axillary hyperpigmentation remain underexplored. This study evaluated the efficacy of 0.025% tretinoin cream in treating axillary hyperpigmentation associated with AN. In a randomized, intra-individual, split-side design study, participants applied 0.025% tretinoin cream on one axilla and a cream-based on the other. The study spanned 12 weeks, with topical application for the first 8 weeks, followed by a 4-week cessati

Clinical dermatoendocrinology: saving lives by looking at the skin.

The separation of the interior from the exterior environment through the skin was fundamental for the evolutionary progression from prevertebrates into vertebrates. The development of the skin also established an internal environment controlled by hormones. The skin is influenced by different hormones; it is also the largest endocrine organ, producing several hormones. Skin inspections often save lives from skin cancer but can also diagnose potentially deadly endocrine diseases. The objectives of this review were to describe the emergence and definition of dermatoendocrinology and to focus on

Paraneoplastic Dermatoses: A Clue for Underlying Malignancies.

Paraneoplastic dermatoses (PDs) belong to a group of rare and polymorphous dermatoses, and they can often be the first sign of underlying malignancies. Therefore, dermatologists should be able to identify skin features to identify earlier underlying neoplasms. Indeed, lack of familiarity with cutaneous clues of internal malignancies can lead to a delay in the diagnosis and an impairment of the prognosis of the patients. In this review, we described several PDs, including more common and rarer PD. Indeed, while malignant acanthosis nigricans, characterized by velvety, verrucous, hyperpigmented

Paraneoplastic Syndromes in Gallbladder Cancer: A Systematic Review.

Background and Objectives: Gallbladder cancer (GBC) is a biologically aggressive malignancy characterised by poor survival outcomes often attributed to delayed diagnosis due to nonspecific clinical presentations. Paraneoplastic syndromes (PNSs), atypical symptoms caused by cancer itself, may serve as valuable indicators for timely diagnosis, particularly in malignancies with nonspecific features. Understanding the manifestations of PNSs in GBC is, therefore, critical. This systematic review collates case studies documenting the association of PNS with GBC, including subsequent management and c

The Impact of Bariatric Surgery on the Development and Progression of Dermatologic Diseases: A Narrative Review.

Obesity is a major global health concern characterized by excessive fat accumulation, which significantly increases the risk of numerous comorbidities. While lifestyle modifications and pharmacotherapy are commonly employed, bariatric surgery is recognized as a highly effective treatment option. These procedures alter gastrointestinal anatomy, restricting food intake and modifying nutrient absorption, thereby reducing hunger and increasing satiety. Beyond weight reduction, bariatric surgery can improve or resolve obesity-related conditions, including type 2 diabetes, hypertension, sleep apnea,
