Unusual Abdominal Wall Complication After a Bilateral Transversus Abdominis Plane Block for Postoperative Analgesia.

Transversus abdominis plane (TAP) blocks are widely regarded as safe techniques for postoperative analgesia. We describe an unusual postoperative complication characterized by rapidly progressive bilateral abdominal wall ulceration following bilateral TAP block and infiltration of laparoscopic port sites with local anesthetics. The multifocal distribution, poor response to antibiotics, negative cultures, and association with procedural trauma raised strong concern for a sterile autoinflammatory dermatosis consistent with pyoderma gangrenosum or a related neutrophilic process, although a defini

Exploring cheminformatics tools to combat prostate cancer: diagnostics, therapeutics, and mechanistic studies.

INTRODUCTION: Stroke leaves many people with upper limb impairments which significantly affect function and Quality Of Life (QOL). Most studies report outcomes at the impairment and activity level. Less attention has been paid to real-life use of the affected upper limb in daily activities. Self-reported measures of performance of activity in daily life using the affected upper limb is important to obtain as people may have minimal deficits on impairment and activity level outcomes yet still have significant difficulties performing self-care and other Activities of Daily Living (ADLs). These d

Peristomal pyoderma gangrenosum: a review of nursing research progress.

Peristomal pyoderma gangrenosum (PPG) is a rare and therapeutically challenging inflammatory dermatosis predominantly affecting individuals with stoma surgery. This review provides a comprehensive overview of PPG, encompassing its etiology, clinical manifestations, diagnostic methods, treatment strategies, and nursing management. PPG's multifactorial etiology involves immune dysfunction, genetic predisposition, surgical trauma, and demographic factors, which contribute to the formation of painful ulcers that significantly impair patients' quality of life. Accurate diagnosis requires a multidis

The utility of biopsy in pyoderma gangrenosum: a retrospective cohort study.

BACKGROUND: Pyoderma gangrenosum (PG) is a rare skin condition characterized by chronic, painful ulcerations. It is considered a diagnosis of exclusion, often resulting in diagnostic delay. Histological results in PG, even when biopsied despite a risk of pathergy, are nonspecific. OBJECTIVES: To examine retrospectively the utility of biopsies in the diagnosis of 58 patients treated for PG. METHODS: Medical records were reviewed to compare biopsy results versus the efficacy of diagnostic rating systems (Delphi, Su, PARACELSUS). RESULTS: Among 58 patients, 26 (45%) underwent biopsies, with on

Pyoderma Gangrenosum of the Eyelid: A Challenging Case.

BACKGROUND Pyoderma gangrenosum (PG) involving the eyelid is rare and can closely mimic other differential diagnoses, posing a diagnostic challenge. This case highlights the diagnostic approach and management of pyoderma gangrenosum-related eyelid lesions. CASE REPORT A 29-year-old man with a history of ulcerative colitis developed a persistent ulcerative lesion on his left upper eyelid, which failed to respond to conventional treatment for chalazion. Examination revealed a pinkish-yellow ulcerative lesion with ill-defined borders, causing distortion of the eyelid margin contour. Due to the po

Adalimumab and Tofacitinib Combination Therapy for Pyoderma Gangrenosum: A Case Report.

INTRODUCTION: Pyoderma gangrenosum (PG) is a rare inflammatory skin disease causing progressive necrotizing ulcers with limited treatment options due to adverse effects and high recurrence rates. CASE PRESENTATION: We present a case of a 70-year-old elderly patient diagnosed as PG, which was unresponsive to initial prednisolone treatment. After approximately 3 weeks of an individualized treatment regimen of adalimumab combined with tofacitinib, disease progression was halted, accompanied by a significant reduction in ulcer size, reduced exudation, and substantial pain relief. Subsequent taper

Carotid Pseudoaneurysm Repair in a Patient with Myeloproliferative Disorder Complicated by Contralateral Stroke, Graft Thrombosis, Pyoderma Gangrenosum, and Superinfection: A Case Report.

Carotid pseudoaneurysms are rare and potentially life-threatening, often necessitating urgent surgical intervention. Patients with myeloproliferative disorders (MPD) are predisposed to thrombotic and inflammatory complications. Pyoderma gangrenosum (PG), a rare neutrophilic dermatosis, is often misdiagnosed in postoperative settings. In the following article, we present a case of a 58-year-old woman with Philadelphia-negative MPD, neutrophilic leukocytosis, thrombocytosis, osteoporosis, and hypothyroidism, who presented with a giant left common carotid artery pseudoaneurysm. She underwent urge

Multidisciplinary Approach to Complex Lower Extremity Limb Salvage in Pyogenic Arthritis, Pyoderma Gangrenosum, and Acne (PAPA) Syndrome: A Case Report.

Limb salvage using free tissue transfer in patients with rare autoinflammatory syndromes is poorly described, and the safety of microvascular reconstruction in this population remains uncertain. Hereditary pyogenic arthritis, pyoderma gangrenosum, and acne (PAPA) syndrome is characterized by dysregulated inflammation, pathergy, and impaired wound healing, raising concern for flap compromise and postoperative complications. We report a rare case of lower extremity free tissue transfer in a patient with PAPA syndrome. A 66-year-old woman with rheumatoid arthritis and PAPA syndrome presented with

Convolutional Neural Networks in Chronic Wound Segmentation and Tissue Classification Using Real-World Images.

Chronic wounds cause a significant burden to affected patients and to society. Effective and objective diagnostic and monitoring methods are needed in wound care, and artificial intelligence offers one promising alternative. In this study, real-world wound images were used to train a convolutional neural network to automatically segment wound area and wound tissues on an image. The study included altogether 362 images of venous, arterial, vasculitis and pyoderma gangrenosum wounds. The model was based on a convolutional neural network architecture U-Net, and fully supervised learning was utili

Selective IL-23 Inhibition in Conventional Treatment-Refractory Pyoderma Gangrenosum: A Multicenter, Retrospective Study.

BACKGROUND: The therapeutic management of pyoderma gangrenosum (PG) is challenging. Conventional therapies, including systemic corticosteroids and cyclosporine, are limited by incomplete responses, frequent relapses, and cumulative toxicity. Emerging evidence supports a pathogenic role of the interleukin (IL)-23/IL-17 axis in PG, suggesting selective IL-23 inhibition as an appealing therapeutic approach. This study assessed the effectiveness, safety, steroid-sparing potential, and predictors of response to selective IL-23 inhibitors in conventional treatment-refractory PG. METHODS: This multi

Autoinflammatory disease and severe neutropenia due to de novo variant of PSTPIP1 with increased binding to pyrin.

Mutations in the gene PSTPIP1 may cause several different autoinflammatory syndromes, but the mechanisms by which distinct PSTPIP1 mutations lead to these differing phenotypes are not fully understood. The two best characterized autoinflammatory conditions resulting from PSTPIP1 mutation are pyogenic arthritis, pyoderma gangrenosum, and acne (PAPA) syndrome and PSTPIP1-associated myeloid-related proteinemia inflammatory (PAMI) syndrome. Here, we report a novel gain-of-function PSTPIP1 mutation (p.N236K) causing PAMI syndrome in a patient with systemic autoinflammation and severe neutropenia. T

Unusual Cutaneous and Mucosal Manifestations of Neonatal Inflammatory Skin and Bowel Disease Type 1 due to a Novel Homozygous ADAM17 Mutation.

Neonatal inflammatory skin and bowel isease type 1 (NISBD1) is a rare autosomal recessive genodermatosis caused by pathogenic variants in ADAM17, a gene essential for epithelial development and cytokine regulation. Affected individuals typically present during infancy with severe cutaneous inflammation, recurrent infections, fragile hair, and variable gastrointestinal involvement. We report a 4-year-old girl with an unusual constellation of mucocutaneous manifestations associated with a novel homozygous ADAM17 mutation (c.436G>A; p.Gln146Lys), thereby expanding the known phenotypic spectrum of

Secondary Microbial Infection in Pyoderma Gangrenosum: Comparable Rates to Venous Ulcers in a Retrospective Cohort.

Pyoderma gangrenosum (PG) is a neutrophilic dermatosis traditionally regarded as a sterile inflammatory ulcer; however, positive wound cultures are frequently encountered in clinical practice and may complicate or delay diagnosis and management. Data regarding bacterial superinfection in PG remain limited. The objective of this study was to determine the frequency and spectrum of microbial involvement in PG ulcers compared with venous ulcers. We performed a retrospective cohort study of patients evaluated at the University of Utah between June 2012 and June 2025. Electronic medical records wer

Evaluation of Acellular Intact Fish Skin Grafts for Treating Acute and Chronic Wounds.

Acute, chronic, and hard-to-heal wounds pose a growing challenge due to reduced patient quality-of-life, higher cost and increased hospital admissions. Although there is no gold standard for wound care, there is a trend toward simple, outpatient-compatible treatment options. Acellular dermal fish skin derived from Gadus morhua (Atlantic cod) is a promising alternative. This retrospective study evaluated the surgical procedures and outcomes associated with fish-skin graft transplantation for difficult-to-treat wounds. Forty-four patients (33 male and 11 female; mean age: 72.4 years; mean wound
