Idiopathic Atrophoderma of Pasini and Pierini: Response to Mycophenolic Acid Treatment.

Atrophoderma of Pasini and Pierini (APP) is a rare skin condition of unknown etiology. It is characterized by dermal atrophy with single or multiple hyperpigmented, non-indurated, and slightly depressed plaques on the trunk or limbs; unlike morphea, this condition does not cause induration. In this report, we analyze the case of a patient diagnosed with APP with a 19-year history of progression, aiming to highlight the few cases published in the literature. The patient responded favorably to treatment with mycophenolic acid (MPA). Therapeutic approaches for this condition are controversial due

[Disseminated atrophoderma of Pasini and Pierini with blaschkoid appearance].

The rare atrophoderma of Pasini and Pierini (APP) is most commonly attributed to the disease spectrum of morphea, alongside atrophoderma Moulin. We report the case of a young woman with an unusually extensive presentation of APP, which partially exhibited a blaschkoid pattern. The clinically unobservable atrophy of the corium could be confirmed using an additive diagnostic procedure with 12 MHz ultrasound.

A Case of Multiple Acquired Smooth Muscle Hamartoma With Depressed Appearance.

Smooth muscle hamartoma (SMH) is a benign cutaneous tumor that typically presents at birth as a solitary lesion in the lumbosacral region. Acquired cases with multiple lesions and subtle depression are exceedingly rare. We report a 40-year-old male with a seven-year history of multiple pigmented macules with slight depression on the trunk and extremities. Histopathology of a back lesion revealed irregularly distributed bundles of pale eosinophilic smooth muscle fibers in the upper dermis, positive for α-smooth muscle actin (α-SMA). Basal hyperpigmentation was observed, without swelling of coll

Molecular Subtypes of Balanopreputial and Urethral Male Genital Lichen Sclerosus: Distinct Transcriptomic and Clinicopathological Profiles.

Male genital lichen sclerosus (MGLSc) is a heterogeneous and aggressive disease characterized by varying severities of balanopreputial and urethral disease (MGLSc-US) and outcomes, including stricture. This study aims to elucidate the transcriptomic heterogeneity of MGLSc and explore its associations with histological and clinical features. We collected 40 preputial samples and 14 urethral tissue samples from patients with MGLSc-US, non-MGLSc urethral strictures, and redundant prepuce. Bulk RNA sequencing was performed to comprehensively profile the transcriptome. Molecular subtypes, functiona

Multi-omics analysis reveals the host-microbe interactions on the dysbiosis of tissue microbiota in male genital lichen sclerosus-induced urethral strictures.

UNLABELLED: Male genital lichen sclerosus-induced urethral stricture is a chronic inflammatory disease with significant microbiota dysbiosis. However, dysbiosis inside lesion tissue and its correlation with gene expression in male genital lichen sclerosus (MGLSc) remain elusive. This study investigated the influence of host-microbe interactions on dysbiosis and differential gene expression in MGLSc. Microbiome and transcriptome sequencing were conducted using prepuce samples from 27 MGLSc patients and 17 controls. We also performed immunohistochemistry staining of bacterial markers on prepuce

Atypical manifestation of psoriasis in the elderly: possible expression of Mal de Meleda diagnosis review and case report.

Mal de Meleda (MDM) is a rare hereditary skin disorder classified as an autosomal recessive palmoplantar keratoderma, with an estimated prevalence of 1 in 100,000 individuals. It is commonly linked to consanguineous parentage and typically presents during early childhood. The hallmark features of the condition include transient thickening of the palms and soles, skin sclerosis and atrophy, scleroatrophic erythematous lesions, pseudoainhum formation around the digits, and erythema around the mouth. Due to its rarity, many dermatologists may lack clinical familiarity with MDM. Moreover, there is

Systemic sclerosis: pathogenic mechanisms and their implications for treatment.

Systemic sclerosis (SSc) is a rare systemic autoimmune disease characterized by a triad of pathogenic mechanisms, including: a) microvascular hyperreactivity secondary to endothelial dysfunction, b) dysregulated immune activation of both innate and adaptive immunity, with the production of autoantibodies targeting nuclear antigens (e.g., anticentromere antibodies, anti-RNA polymerase III antibodies, and anti-topoisomerase I antibodies), and c) fibrosis of the skin and internal organs, such as the lungs, due to excessive extracellular matrix deposits produced by dysregulated myofibroblasts. The role of the mineralocorticoid receptor in skin.

The mineralocorticoid receptor (MR) plays a pivotal role in skin homeostasis, inflammation, and repair, interacting closely with the glucocorticoid receptor (GR) to regulate various physiological and pathological processes. Dysregulation of MR signaling has been implicated in several skin disorders, including psoriasis, atopic dermatitis, and impaired wound healing. Furthermore, studies have shown that patients with primary hyperaldosteronism exhibit epidermal hyperplasia, impaired differentiation, increased immune cell infiltrates, and elevated pro-inflammatory cytokines due to MR overactivat

The POLG Variant c.678G>C; p.(Gln226His) Is Associated with Mitochondrial Abnormalities in Fibroblasts Derived from a Patient Compared to a First-Degree Relative.

BACKGROUND: The nuclear-encoded enzyme polymerase gamma (Pol-γ) is crucial in the replication of the mitochondrial genome (mtDNA), which in turn is vital for mitochondria and hence numerous metabolic processes and energy production in eukaryotic cells. Variants in the POLG gene, which encodes the catalytic subunit of Pol-γ, can significantly impair Pol-γ enzyme function. Pol-γ-associated disorders are referred to as POLG-spectrum disorders (POLG-SDs) and are mainly autosomal-recessively inherited. Clinical manifestations include muscle weakness and fatigue, and severe forms of the disease can

Atrophic Pityriasis Versicolor: 3 Case Reports.

INTRODUCTION: Pityriasis versicolor is a common epidermal mycotic infection, well known in its classic hypo- or hyperpigmented macular form. However, its atrophic variant, first described in 1971, remains rare and poorly documented. The underlying cause of this variant has yet to be determined. CASE PRESENTATION: We report 3 cases of patients treated with topical steroids for inflammatory skin diseases (atopic dermatitis, cutaneous scleroderma, and mycosis fungoides) who developed erythematous and squamous atrophic lesions on their back during follow-up. Complete resolution of both patches an

Papular acantholytic dyskeratosis of the genitocrural area with positive direct immunofluorescence in a man affected by lichen sclerosus et atrophicus: a clinicopathological challenge.

Dear Editor, Papular acantholytic dyskeratosis (PAD) is a rare, sporadic, persistent disease of adulthood, localized to the genitocrural and anogenital regions. It was proposed as a new entity by Chorzelski et al. in 1984, based on the case of a 23-year-old woman with multiple, dome-shaped, white papules grouped on her vulva, which revealed histopathologically an epidermal proliferation with acantholysis and dyskeratosis, resembling both Haley-Haley (HHD) and Darier diseases (DD). Since then, PAD was reported as an asymptomatic/slightly itchy eruption of multiple white to skin-colored papules,
