Whole-genome analysis of CVA4 in children with herpangina in two cities of Shandong, China.

Sporadic epidemics of herpangina caused by Coxsackievirus A4 (CVA4) have been reported globally, however, research on CVA4 remains relatively scarce. In this study, 60 Coxsackievirus A4 (CVA4) strains were successfully isolated from 256 throat swab specimens and 50 high-quality complete genome sequences were generated using high-throughput sequencing technology. Phylogenetic analysis based on the VP1 region demonstrated that all CVA4 were classified into the C2 genotype. Whole-genome analysis revealed that the strains in this study clustered within the same major evolutionary clade as CVA4 str

Full-genome analysis of emerging Coxsackievirus B4 genotype D strains associated with herpangina in eastern China.

In 2024 herpangina surveillance in Yuhuan City, China, two Coxsackievirus B4 strains (Z168, Z296) were identified. Full-length genome sequencing revealed that both strains belonged to genotype D that was first documented in eastern China. The two strains showed the highest nucleotide similarity to Coxsackievirus B4 strains isolated in the UK (2017). In the recombination events, both major parental strains were identified as Coxsackievirus B4 strains isolated in France (2015 and 2017). Echovirus 11 and Coxsackievirus B5 were detected as minor parental strains, indicating intertypic recombinatio

A randomised trial of a bioreactor-produced EV-A71 vaccine for endemic control in children.

Enterovirus (EV) A71 causes endemic outbreaks of hand-foot-mouth disease (HFMD) and herpangina in young children, occasionally leading to severe complications and fatal outcomes. We evaluated EnVAX-A71, a bioreactor-produced, aluminum-adjuvanted inactivated EV-A71 (B4/E59) vaccine, in a randomized, double-blind, placebo-controlled phase 3 trial in Taiwan and Vietnam. A total of 4011 children aged 2-71 months were randomized 3:2 to vaccine or placebo; the primary endpoint was laboratory-confirmed EV-A71-associated HFMD/herpangina, evaluated from 28 days after dose two over a follow-up of 1 to 2

Regional and facility-type variations in infectious diseases in childcare and early childhood education facilities in Japan during the COVID-19 pandemic: A nationwide cross-sectional questionnaire survey.

Young children are susceptible to infectious diseases due to their developing immune systems and close contact in group care settings. During the coronavirus disease 2019 (COVID-19) pandemic, infection prevention measures may have altered the epidemiology of common childhood infections, yet evidence on variations by facility type and region remains limited. In this study, the occurrence of COVID-19 and child-specific infectious diseases in childcare and early childhood education facilities in Japan was examined with particular focus on facility type and regional population density. Global Evolution and Phylodynamics of Coxsackievirus A4.

Coxsackievirus A4 (CVA4) is an enterovirus associated with diverse clinical syndromes, yet its global evolution and transmission dynamics remain poorly defined. This study aimed to characterize the molecular epidemiology, evolutionary history, and phylodynamic patterns of CVA4 at both local and global scales. In 2024, 34 CVA4 infections were identified during a localized herpangina outbreak in Beijing, primarily affecting children under 5 years of age, with a pronounced peak in July. High detection and isolation rates, together with temporal clustering in mid-2024, indicate a genuine short-ter

Enterovirus Testing in Hand, Foot, and Mouth Disease and Herpangina: A Highly Sensitive Single-Round VP4-VP2 Reverse-Transcription Polymerase Chain Reaction Assay with a Redesigned Reverse Primer.

Molecular typing of enteroviruses (EVs) is essential for surveillance of hand, foot, and mouth disease (HFMD). Conventional reverse-transcription polymerase chain reaction (RT-PCR) targeting the VP4-VP2 region can be insufficiently sensitive, reducing the detectability of Enterovirus A (EV-A). We developed a single-round RT-PCR assay using a modified reverse primer design (C3R) for rapid EV detection and genotyping. Sensitivity was evaluated using EV-A71 and poliovirus type 1 reference strains, across 60 EV-positive clinical specimens. The C3R-based assay showed ~1000-fold higher sensitivity f

Clinical and epidemiological characteristics of hospitalized children with enterovirus infection: a retrospective study from 2016 to 2025 at Beijing children's hospital.

BACKGROUND: Enteroviruses (EVs) are important pathogens that cause neurological and systemic infections in children. Currently, only hand, foot, and mouth disease (HFMD) is monitored in China, indicating a lack of systematic surveillance for other EV-related diseases. Additionally, long-term epidemiological data on pediatric EV infections in Beijing are scarce. The aim of this study was to describe the clinical and epidemiological characteristics of children with EV infection in Beijing over a ten-year period. METHODS: In this retrospective study, all hospitalized patients who underwent EV RNA

Enterovirus infections in children.

Enteroviruses (EVs), which include numerous types, are among the most common pathogens causing infections in children. EVs can cause diverse diseases, including herpangina, hand-foot-and-mouth disease, myocarditis, pneumonia, meningitis, encephalitis, and acute flaccid paralysis. Severe cases may result in pediatric mortality, posing substantial threats to child health and public health. The diversity of EV types combined with rapid viral evolution and antigenic variation presents formidable challenges for EV prevention and control, with dominant genotypes being continuously replaced by emergi

Modelling the impacts of meteorological factors on herpangina: a Bayesian spatiotemporal analysis.

BACKGROUND: Herpangina is a common enteroviral illness in young children with recurrent seasonal surges. Although meteorological conditions influence transmission, evidence capturing non-linear and delayed effects across space remains limited. We aimed to quantify the exposure-lag-response associations of temperature, precipitation, and relative humidity with herpangina risk in Japan using a Bayesian spatiotemporal model. METHODS: Weekly sentinel surveillance herpangina cases and meteorological variables were obtained for the 47 prefectures from January 2001 to December 2024. We applied a Bay

Genome Analysis of Coxsackievirus A4 Identified from Herpangina Children in Northern China.

Sporadic epidemics of coxsackievirus A4 (CVA4) have been reported worldwide. However, the lack of the whole genome sequence has restricted the study of the gene characterization and evolution of CVA4. In this study, four whole genome sequences and 17 VP1 sequences of CVA4 identified from Linyi, northern China, in summer 2024 were used for genetic characterization and phylogenetic analysis. Four genotypes (A, B, C, and D) and five subgenotypes (C1-C5) were identified based on VP1 sequences. The Linyi CVA4 strains belong to subgenotype C2, which has also been the main prevalent subgenotype in Ch

Association between short-term heat exposure and herpangina incidence in Japan: a nationwide space-time-stratified case-crossover study.

This study aims to quantify the impact of ambient temperature on the risk of herpangina in Japan. We compiled weekly time-series data on the incidence of herpangina and meteorological factors in all 47 Japanese prefectures (2005-2019). A space-time-stratified case-crossover design, combined with a distributed lag nonlinear model, was employed to estimate the relative risk (RR) of heat exposure across prefectures. A multivariate meta-analysis was conducted to obtain pooled estimates at the national level. Our analysis revealed a nonlinear, inverted U-shaped association between ambient temperatu

A lethal juvenile mouse model for the evaluation of antiviral reagents against coxsackievirus A4.

Coxsackievirus A4 (CVA4) as a pathogen causing severe herpangina has become global outbreak epidemics during the past 5 years. However, there is currently no effective vaccines and drugs available. Here, one mouse-adapted CVA4 virus (YZ08) was cultivated by repeatedly cross-passaging between human RD cells and 11-day-old juvenile mice. Our results revealed the YZ08 virus was highly lethal to 13-day-old juvenile mice. Virus titer assay indicated that the YZ08 virus effectively replicated in juvenile mice. This model was successfully used to evaluate the therapeutic efficacy of herb medicine, an
