Early-Onset Multiple Necrobiosis Lipoidica in a Child with Type 1 Diabetes Mellitus.

Necrobiosis lipoidica is uncommon in childhood, and a multifocal presentation early in the course of type 1 diabetes mellitus appears to be infrequently reported. In our patient, multiple lesions developed within 3 years of diabetes diagnosis, despite the absence of overt microvascular complications. The persistence of lesions alongside recurrent episodes of ketoacidosis may suggest a potential association between suboptimal metabolic control and cutaneous microangiopathic changes; however, causality cannot be established on the basis of a single case. Early recognition of such lesions may be

Progression of diabetic retinopathy during pregnancy in a woman with ABCC8-MODY.

SUMMARY: Activating mutations in the ABCC8 gene are extremely rare and cause ABCC8-MODY. The phenotype is variable with onset of diabetes in childhood/early adulthood. Retinopathy is the most common reported complication. We describe a 31-year-old primigravida woman referred to and seen at our antenatal ambulatory diabetes clinic at 6 weeks plus 5 days gestation. She had a strong family history of diabetes and was diagnosed at the age of 11 years. Genetic testing revealed an activating pathogenic c.4139G>A variant in the ABCC8 gene. She was managed with glibenclamide, sitagliptin, and dapaglif

Necrobiosis Lipoidica in Pregnancy Complicated by Gestational Diabetes Mellitus: A Case Report.

Necrobiosis lipoidica (NL) is a rare chronic granulomatous skin disease that usually presents as well-defined, yellowish-brown atrophic plaques with telangiectasia on the lower extremities. Its pathogenesis is not fully understood, and it is commonly associated with diabetes mellitus. A 39-year-old gravida 4, para 3 woman at 36 + 5 weeks' gestation presented with painful, erythematous to yellow-brown atrophic plaques with mild telangiectasia on the lower limbs and lower abdomen. Her pregnancy was complicated by insulin-requiring gestational diabetes and late-onset pre-eclampsia. Lesions progre

Association between palisaded and interstitial granulomatous dermatoses and haematological neoplasms: evidence beyond coincidence?

BACKGROUND: Palisaded and interstitial granulomatous dermatoses (PIGDs) are a spectrum of presumed reactive cutaneous disorders sometimes associated with underlying diseases, notably malignant haemopathies. Specific associations between some of these dermatoses and particular haematological neoplasms may exist. OBJECTIVES: To describe the spectrum of haematological neoplasms associated with PIGDs and to establish whether specific associations exist. METHODS: We conducted a retrospective monocentric study of all cases of patients with PIGD associated with haematological neoplasms seen in our

Orthopaedic Implant Surgery or Trauma as a Trigger for Necrobiosis Lipoidica: A Manifestation of an Immunocompromised Cutaneous District?

Necrobiosis lipoidica (NL) is a rare chronic granulomatous skin disorder most commonly associated with diabetes mellitus, typically presenting in middle-aged women. We report an unusual case of NL in a young, non-diabetic male following orthopedic surgery involving a metallic implant. The patient developed characteristic well demarcated, atrophic, violaceous-brown plaques with violaceous borders localized over shin area of his left lower leg. Histopathological examination confirmed the diagnosis of NL, showing palisading granulomas with necrobiosis and lipid-laden macrophages. Notably, there w

Deep Inferior Epigastric Perforator Flap Reconstruction of Breast Necrobiosis Lipoidica.

Necrobiosis lipoidica (NL) is a granulomatous disease presenting as plaques with potential for progression to chronic ulcerating lesions. Breast involvement of the disease is rare. There are currently no FDA-approved medical therapies, and surgical treatment is controversial due to concerns for disease recurrence. Limited historical cases of surgical management demonstrate success when excision is taken to deep fascia. No prior reconstruction of NL manifesting on the breast has been documented in the literature. In this report, we present a patient with isolated ulcerated NL of the right breas

Necrobiosis Lipoidica Mimicking Chronic Infectious Ulceration: A Case Report and Literature Review.

Necrobiosis lipoidica (NL) is a rare, granulating autoimmune disease that can be quite difficult to diagnose. It has a higher incidence in diabetic patients than in the general population. Diagnosis often depends upon both clinical and histopathologic examinations. It has a frequent association with diabetes, metabolic syndrome (obesity, hypertension, high cholesterol), and thyroid diseases. This is the case of a 50-year-old male patient who developed ulcerative skin lesions that were refractory to antibiotic therapy. He underwent antibiotic treatments for about two years, as well as multiple

Cutaneous granulomatous disorders and associations with malignancy.

Granulomatous inflammation of the skin can arise in patients with inflammatory disorders, malignancy, or infections and in reactions to medications or foreign bodies. Granulomatous disorders describe a group of diseases marked by the presence of granulomatous inflammation in the dermis and can be categorized into infectious and noninfectious causes. Inflammatory, noninfectious granulomatous disorders, including granuloma annulare (GA), necrobiosis lipoidica, rheumatic nodules, cutaneous sarcoidosis, reactive granulomatous dermatitis (RGD), and necrobiotic xanthogranuloma (NXG), can occur in th

Burn injury is often a devastating event with long-term physical and psychosocial effects. Burn scars after deep dermal injury are cosmetically disfiguring and force the scarred person to deal with an alteration in body appearance. In addition, the traumatic nature of the burn accident and the painful treatment may induce psychopathological responses. Depression and post-traumatic stress disorder (PTSD), which are prevalent in 13-23% and 13-45% of cases, respectively, have been the most common areas of research in burn patients. Risk factors related to depression are pre-burn depression and fe

Subcutaneous Granuloma Annulare in a Middle-Aged Patient: A Case Report.

Subcutaneous granuloma annulare (SGA) is a rare, benign variant of granuloma annulare, typically seen in children, which presents as asymptomatic subcutaneous nodules with normal overlying skin. The etiology remains unclear despite associations with systemic disease such as diabetes mellitus. Clinicopathologic correlation is crucial to accurate diagnosis. In this report, we present a case of a 53-year-old female patient with tender, bilateral subcutaneous nodules on her elbows. Punch biopsy confirmed the diagnosis of granuloma annulare. The primary management for her condition at the time was

Review of the European Society for Photodynamic Therapy (Euro-PDT) Annual Congress 2025.

This article reviews novel data presented at the 2025 Annual Congress of the European Society for Photodynamic Therapy (Euro-PDT). Photodynamic therapy (PDT) has been investigated for the treatment of a broad spectrum of neoplastic, infectious and inflammatory dermatoses. PDT remains an effective treatment for actinic keratoses/field cancerization with increased interest in delivery of treatment indoors using artificial daylight sources as well as in home-administered daylight PDT. Sustained clearance has remained an important marker of the success of PDT for superficial and thin nodular basal

Red wine antioxidant properties implications in rheumatic diseases: exploring clonal variations in resveratrol and other bioactive compounds.

Resveratrol (RS), a non-flavonoid polyphenol, is a well-recognised anti-inflammatory compound of red wine. This narrative review aims to explore the mechanisms underlying its potential antioxidant properties in osteoarthritis (OA), rheumatoid arthritis (RA), spondyloarthritis (SpA), and osteoporosis (OP), as well as its clonal variation in red wine and future perspectives for clinical applications.Although human data remain limited and sometimes controversial, recent studies in animal models have demonstrated that RS can reduce inflammation by interacting with various cellular pathways, includ

SPP1hi macrophages, NKG7 T cells, CCL5hi fibroblasts, and IgM plasma cells are dominant features of necrobiosis.

Necrobiosis is a histologic term used to describe abnormal deposits of "degenerating" collagen within the skin. It can be found as an incidental finding in various granulomatous conditions, but is a hallmark of necrobiosis lipoidica (NL) and necrobiotic xanthogranuloma (NXG). There is limited prior research on necrobiosis. Here, we employed single-cell analysis of lesional and nonlesional skin to study the pathophysiology of necrobiosis. Our findings demonstrate that necrobiotic lesional skin is characterized by SPP1hi macrophages expressing MARCO; NKG7-expressing effector CD8+ T cells coexpre
