Tofacitinib treatment in a girl with localized scleroderma of the head and face: a case-based review.

En coup de sabre scleroderma (ECDS) and progressive hemifacial atrophy (PHA) are classified as forms of linear scleroderma of the head and face, a subset of juvenile localized scleroderma (JLoS). In ECDS, sclerotic lesions primarily localize to the forehead and frontoparietal scalp, typically unilaterally. PHA involves the atrophy of facial soft tissues and adjacent cartilage-bone structures on one side, leading to significant deformities. Both forms may involve the eye and central nervous system (CNS), potentially presenting with hemiplegia in PHA. These conditions can coexist and require imm

[Filgotinib treatment of calcinosis in centromere-antibody-positive systemic sclerosis].

Centromere-antibody-positive limited cutaneous systemic sclerosis (LCSSc) is a common disease. It generally takes a benign course, but can still considerably reduce quality of life. Our patient-a woman born in 1987 who was first diagnosis 2017-suffered from pain and declining hand function due to sclerodactyly and calcinosis. Methotrexate (MTX) and analgesics were not effective. After commencing off-label treatment with filgotinib, the patient reported rapid improvement. An X-ray after 8 months showed a reduction in calcinosis load. Filgotinib is well tolerated. Sclerodactyly has resolved, and

Linear scleroderma 'en coup de sabre'.

Linear scleroderma 'en coup de sabre' is a rare condition characterised by inflammation and fibrosis of the skin and underlying tissues; it rarely presents in adults. The adult-onset form seldom has extracutaneous manifestations, but those that occur are usually neurological. A 43-year-old woman with epilepsy had a scar-like lesion on her forehead. The MR brain scan showed focal cortical atrophy and white matter changes on the same side as the skin lesion. Skin biopsy identified hyaline sclerosis of the reticular dermis consistent with scleroderma. Her clinical condition stabilised on mycophen

Maternal and fetal outcomes in pregnancies affected by mixed connective tissue disease.

INTRODUCTION: Mixed connective tissue disease (MCTD) is an autoimmune condition with overlapping features of lupus, systemic sclerosis, and polymyositis. Maternal-fetal outcomes specific to MCTD remain poorly characterized; a scoping review was conducted. METHODS: Five databases were queried. Extracted data included demographics, antibody profiles, symptoms, treatments, and maternal-fetal outcomes. Descriptive analysis was performed. RESULTS: Of 269 articles, 33 met inclusion criteria, including 375 pregnancies. Common symptoms and disease manifestations included Raynaud's phenomenon, arthra

Clinical Associations of Anti-RNPC3 Autoantibodies in Mixed Connective Tissue Disease.

OBJECTIVE: The RNA-binding region-containing protein 3 (RNPC3) protein acts as a molecular bridge, promoting U11/U12 RNP complex formation. In previous reports, patients with systemic sclerosis (SSc) with anti-RNPC3 autoantibodies had an increased risk of interstitial lung disease (ILD), severe gastrointestinal (GI) disease, and cancer. The aim of this study was to compare phenotypic features of patients with mixed connective tissue disease (MCTD) with and without anti-RNPC3 autoantibodies. METHODS: A retrospective MCTD cohort was studied. Addressable laser bead immunoassay was used to detect

JAK inhibitors for the treatment of juvenile localized scleroderma: a case report and literature review.

BACKGROUND: Juvenile localized scleroderma (JLS) is a chronic autoimmune and fibrosing disease that can cause significant damage and impairments. Currently, management of JLS is challenging due to the paucity of antifibrotic drugs. In patients with severe JLS, systemic glucocorticoids(GCs) combined with methotrexate(MTX) are recommended, but some patients show intolerance or poor response. Janus kinase (JAK) inhibitors have demonstrated efficacy and tolerability in treating cutaneous thickening in vivo and in vitro. We aim to describe a JLS patient treated with upadacitinib(UPA) in our hospita

Physical rehabilitation interventions for hand function in people with systemic sclerosis.

Systemic sclerosis (SSc) is a complex systemic autoimmune rheumatic disease with marked clinical heterogeneity. Cutaneous manifestations affecting the hands and wrists include vascular insufficiency, early edema (puffy fingers), followed by progressive skin fibrosis and atrophy (sclerodactyly). This progressive skin tightening results in joint stiffness, deformity, functional impairment and reduced quality of life. Beyond skin changes, hand involvement may also include inflammatory arthritis, joint contractures, tendon friction rubs, Raynaud phenomenon, digital ulcers, acro-osteolysis, and cal

Cervical Spine Involvement in Morphea Patients With Subclinical Neurologic Signs.

Morphea is a rare skin disease that involves the fibrosis of subcutaneous tissues. Several subtypes of morphea, such as linear and deep variants, may affect musculoskeletal structures and produce neurologic manifestations. However, morphea is differentiated from systemic scleroderma by the absence of internal organ damage. Although the prognosis of a morphea diagnosis is generally favorable, systemic involvement can be severe. However, early detection may improve disease management. The relationship between morphea and its involvement in the cervical spine remains incompletely characterized. A

E114G de Novo Mutation in GJB2 Gene in a Chinese Patient with Classical Vohwinkel Syndrome.

Vohwinkel syndrome (VS) is a rare autosomal dominant form of palmoplantar keratoderma (PPK), characterized by diffuse hyperkeratosis of the palms and soles, starfish-shaped keratotic lesions, and pseudo-ainhum. Mutations in GJB2 have been implicated in VS pathogenesis. In this study, Sanger sequencing of the GJB2 coding exons and exon-intron boundaries was performed in a Chinese patient with VS. A missense mutation c.341A>G (E114G) was also identified. This finding expands the mutational spectrum of VS and provides further insight into its genetic basis, which may facilitate improved genetic s

Castleman-Like Lymphadenopathy in a Patient with Mixed Connective Tissue Disease: A Case Report and Review of the Literature.

Differentiating reactive lymphadenopathies in the context of autoimmune disease from Idiopathic Multicentric Castleman Disease (iMCD) poses a significant diagnostic challenge. Castleman-like histological features have been described in various autoimmune disorders, necessitating a strict and comprehensive integration of clinical and laboratory findings to reach the correct diagnosis. Although the Castleman Disease Collaborative Network (CDCN) consensus guidelines list several autoimmune conditions as exclusion criteria for an iMCD diagnosis, mixed connective tissue disease (MCTD) is not curren

Morphea En Coup De Sabre Treated With Combination Hyaluronic Acid Filler and Neuromodulator: A Case Report.

Morphea en coup de sabre is a localized scleroderma variant that often leaves permanent, disfiguring frontoparietal atrophy after disease stabilization. We report a 27-year-old woman with four stable, linear, atrophic forehead plaques consistent with burned-out morphea en coup de sabre who sought cosmetic improvement. Targeted prabotulinumtoxin-A was administered to hyperfunctional temporalis, corrugator supercilii, and depressor supercilii muscles to reduce dynamic deforming forces. Two weeks later, hyaluronic acid filler was delivered via a blunt-tipped microcannula on the periosteal plane i

Rash-Negative Dermatomyositis-Spectrum Overlap Myositis With PM/Scl Positivity Presenting With Progressive Pharyngeal Dysphagia.

Dermatomyositis-spectrum inflammatory myopathy may present without characteristic cutaneous findings, and overlap phenotypes can delay recognition when early neuromuscular examination is not yet diagnostic. We describe the case of a patient with rheumatology-diagnosed dermatomyositis without rash and PM/Scl-positive overlap myositis with systemic sclerosis features whose course was marked by interstitial lung disease, progressive weakness, and objectively confirmed pharyngeal dysphagia. A 64-year-old female with interstitial lung disease presented with several weeks of polyarthralgia, severe f

Anti-PM/Scl Antibody-Positive Dermatomyositis With Rapidly Progressive Interstitial Lung Disease in a 19-Year-Old Male: Clinical Implications of a Presumptive Diagnosis Based on Line Immunoassay Positivity.

Anti-PM/Scl antibodies are classically linked to systemic sclerosis-polymyositis overlap, but a dermatomyositis phenotype with interstitial lung disease has also been described. However, the clinical course and therapeutic approach for short-term progressive interstitial lung disease in anti-PM/Scl-positive dermatomyositis remain incompletely defined. A 19-year-old Japanese male developed cough and exertional dyspnea after several weeks of progressive myalgia and proximal muscle weakness. Isolated Sclerodactyly Associated with Silica Exposure in a Stonemason: A Case Report.

INTRODUCTION: Sclerodactyly is commonly associated with systemic sclerosis (SSc), occurring alongside Raynaud's phenomenon and disease-specific autoantibodies. The combination of occupational silica exposure and SSc is known as Erasmus syndrome. However, isolated digital fibrosis in silica-exposed individuals without systemic features or SSc-specific autoantibody positivity has not yet been described. CASE PRESENTATION: A 40-year-old stonemason presented with a 6-month history of progressive fingertip sclerosis, fissuring, and pain, in combination with hand dermatitis, which was initially dia

Treatment for progressive hemifacial atrophy: paradigm shift from 2D photograph to 3D stereophotogrammetry.

BACKGROUND: Progressive hemifacial atrophy is a rare craniofacial disease that commonly involves withered skin, soft tissue, cartilage, or bone on one side of the face. The author proposed a therapeutic algorithm tailored to PHA's severity, which revealed a paradigm shift from subjective evaluation on 2D photographs to more objective assessment using 3D stereophotogrammetry. METHODS: This retrospective observational study comprised all patients diagnosed with PHA between February 2000 and August 2025. Patients were categorized into three tiers according to deficits in soft tissue, facial skel
