Cost-effectiveness of the hepatitis B birth-dose vaccine in South Africa: a mathematical modelling analysis.

BACKGROUND: In 2022, South Africa had an estimated hepatitis B prevalence of 4·7-6·0%. In an effort to eliminate viral hepatitis as a public health threat, a targeted (ie, selective) hepatitis B birth-dose vaccination (HepB-BD) policy was introduced in 2023. Under this policy, only infants born to mothers with confirmed hepatitis B positivity are eligible for HepB-BD, which is given in addition to peripartum antiviral prophylaxis (PAP) for mothers from the second or third trimester of pregnancy. This policy differs from the WHO recommendation that all newborns should receive a HepB-BD vaccine

Efficacy of direct-acting antiviral regimens for acute HCV infection: a systematic review and meta-analysis.

BACKGROUND: Acute and recently acquired hepatitis C virus (HCV) infection clusters in men who have sex with men living with HIV and in people who inject drugs, where early cure benefits both the individual and transmission control. The best interferon-free dual direct-acting antiviral (DAA) regimen and treatment duration, including short courses, remain undefined. METHODS: MEDLINE, Scopus, and the Cochrane Central Register of Controlled Trials were searched to 1 July 2026 for studies of interferon-free dual-DAA therapy in acute or recently acquired HCV reporting SVR12. Each study was appraise

Acute Hepatitis C Infection in a MSM Living With HIV in Trinidad.

Hepatitis C virus (HCV) infection is widespread globally, however the data on HCV is sparse in the anglophone Caribbean particularly among persons living with HIV. Acute HCV infection is frequently asymptomatic and in resource limited countries, there are numerous challenges with diagnosis and treatment. We present the case of a 45 year old man who has sex with men (MSM) living with HIV in Trinidad with sexually acquired HCV infection in whom there were elevated liver transaminases and low HCV RNA levels that signified a strong immune mediated response in an attempt to clear the virus, but thi

Detection of Acute Hepatitis C Outbreaks in California Using SaTScan: Statewide Spatiotemporal Analysis.

BACKGROUND: Acute hepatitis C virus outbreaks in California are identified by local health jurisdictions through the investigation of cases reported by laboratories and health care providers to the state's public health surveillance system. However, acute hepatitis C cases are widely underreported, limiting timely outbreak detection and early intervention. SaTScan (Space and Time Scan Statistics) has been proposed as a valuable tool to better detect disease clusters that may be missed by traditional surveillance, even when the disease is underreported, but it is not routinely used for acute he

Seroprevalence and Clinical Characteristics of Hepatitis D Virus Infection in Korean Patients with Chronic Hepatitis B, Liver Cirrhosis, and Hepatocellular Carcinoma.

BACKGROUND/AIMS: Hepatitis delta virus (HDV) coinfection exacerbates clinical outcomes in chronic hepatitis B (CHB) patients. With emerging HDV-targeted therapeutics, accurate prevalence and clinical impact data are essential. In this study, the seroprevalence, clinical significance, and molecular characteristics of HDV were assessed in South Korean patients across the spectrum of liver disease. METHODS: This two-center retrospective study included 526 patients with CHB: non-cirrhotic CHB (n=100), cirrhotic CHB (liver cirrhosis, n=100), and hepatocellular carcinoma (HCC; n=326). Serum samples

The HBV basal core promoter mutation confers a replicative advantage and transcriptionally reprograms hepatocytes toward HCC subtypes.

BACKGROUND: Hepatitis B virus (HBV) basal core promoter (BCP) and precore (PC) mutations occur during chronic HBV infection with the BCP-mutant being associated with increased hepatocellular carcinoma (HCC) risk. OBJECTIVES: The effects of these mutants on viral replication, hepatocyte biology, and carcinogenesis remain poorly defined. To address this, we characterized BCP- and PC-mutants using human hepatocyte chimeric mice and asked whether the resulting infection-induced transcriptomes correspond to subsets of human HBV-related hepatocellular carcinomas (HBV-HCCs). DESIGN: Isogenic wild-t

Changing epidemiology and etiology of hepatocellular carcinoma in Europe: implications for clinical practice and health policy.

Primary liver cancer, of which hepatocellular carcinoma (HCC) represents the vast majority, is on the rise globally. Despite an expected decrease in viral hepatitis-related HCCs, the number of new cases and deaths from liver cancer is predicted to increase by over 30% in 2050 in Europe. This is largely driven by an increase in metabolic dysfunction-associated steatotic liver disease-related HCC and an aging population. Since HCC usually develops in patients with underlying chronic liver disease, over half of all cases appear to be preventable by targeting modifiable risk factors. This series p

THE ROLE OF CELL THERAPY IN THE COMPREHENSIVE TREATMENT OF CHRONIC HEPATITIS C-RELATED LIVER CIRRHOSIS.

BACKGROUND: Despite the high efficacy of direct-acting antiviral (DAA) therapy for chronic hepatitis C virus (HCV) infection, a considerable proportion of patients continue to exhibit significant structural liver damage, including advanced fibrosis and cirrhosis. Therefore, the search for additional therapeutic approaches aimed at enhancing liver regeneration and improving hepatic functional status remains highly relevant. OBJECTIVE: To evaluate the clinical efficacy of fetal stem hepatocyte transplantation as part of a comprehensive treatment strategy for liver cirrhosis associated with chro

Integrated computational approaches for identifying and evaluating phytochemical inhibitors against HCV NS3/4A protease.

Hepatitis C virus (HCV) remains a major global health concern, causing chronic hepatitis, cirrhosis, and hepatocellular carcinoma (HCC), with an estimated 185 million people affected worldwide. In the absence of an FDA-approved vaccine, the development of effective antiviral strategies remains essential. The HCV NS3-4A protease, which mediates cleavage of the viral polyprotein at specific downstream sites of NS3 and is essential for replication-complex formation, represents a key therapeutic target. In this study, approximately 1,480 phytocompounds were compiled from the IMPPAT database and pr

Hepatic Steatosis in People With Chronic Hepatitis B in Africa: Prevalence, Correlates, and Clinical Significance.

To address data gaps around metabolic liver disease in Africa, within the Hepatitis B in Africa Collaborative Network (HEPSANET), we evaluated participants with chronic hepatitis B virus infection (CHB) for steatotic liver disease (SLD) using transient elastography with controlled attenuation parameter (CAP). Using multivariable logistic regression, we identified sociodemographic, anthropometric, and clinical associations with SLD (CAP ≥ 248 dB/m), and evaluated whether SLD severity was independently associated with fibrosis. Short-term impacts of SLD on mortality were also assessed with Cox p

RESONANCE: Protocol for a Nationwide Cohort Study of Chronic Liver Diseases Using the French National Health Data System.

BACKGROUND: Chronic liver diseases (CLDs) are frequent in Europe, including in France, and are mainly driven by alcohol, metabolic dysfunction (diabetes and obesity), and viral hepatitis. Although risk factors are well established and easy to identify, CLDs are frequently diagnosed at an advanced stage, translating into poor prognosis. Descriptive data on CLD burden in France remain scarce, and health trajectories of these patients are uncharted. However, such data are crucial to guide clinical practice, to determine the target population for personalized public health policies, and to develop

Trends in respiratory virus positivity across urban and rural hospitals in Ontario over three seasons.

OBJECTIVES: Respiratory viruses are a global health burden. While influenza has been a large focus of surveillance, non-influenza viruses remain understudied in Canada, leaving gaps in understanding their seasonal patterns and epidemiology. The coronavirus disease 2019 pandemic further disrupted typical virus seasonality, highlighting the urgent need for expanded surveillance to track evolving trends. METHODS: A multicenter retrospective analysis of molecular testing data for 15 respiratory viruses was conducted on tests performed over three consecutive respiratory virus seasons. Hospital-lev

Gut microbiome-enteric virus interactions: mechanisms, clinical implications, and translational directions.

The gut microbiome influences epithelial barrier integrity, metabolite availability, mucosal immune tone, and vaccine responsiveness, all of which are relevant to enterically transmitted viral infection. Norovirus and rotavirus primarily replicate in the intestinal mucosa and cause acute gastroenteritis, whereas poliovirus replicates in the gut and may disseminate systemically. Hepatitis A virus and hepatitis E virus are acquired through the intestine and shed fecally, but their dominant clinical manifestation is hepatitis. This review synthesizes evidence linking the bacteriome, phageome/viro

Acute autoimmune hepatitis diagnosed concurrently with human immunodeficiency virus.

We report a woman in her 50s who presented with jaundice and severe hepatocellular injury, concurrently found to have newly diagnosed advanced HIV infection (CD4 count 17, viral load 195 600 copies/mL). In this context, infectious hepatitis or opportunistic infections were the important initial consideration, but viral serology, imaging and liver biopsy excluded opportunistic and viral causes. Markedly raised immunoglobulin G, positive anti-nuclear and smooth muscle antibodies, typical histology and rapid response to corticosteroids confirmed acute autoimmune hepatitis (AIH). She improved on p

Becoming Physically Active and Risks of Mortality and Liver-Related Events in Adults With Chronic Hepatitis B: A Nationwide Cohort Study.

Physical activity (PA) confers several health benefits; however, whether increasing PA among physically inactive adults with chronic hepatitis B (CHB) is associated with lower risks of adverse clinical outcomes remains unclear. We used the nationwide Korean National Health Insurance database. We included 318,131 adults with CHB who were physically inactive at the initial health screening (Exam 1) and completed a subsequent screening within 4 years (Exam 2). Among them, 58,703 initiated moderate-to-vigorous PA (MVPA), defined as vigorous activity ≥ 20 min/day on ≥ 3 days/week or moderate activi

Direct-Acting Antiviral Therapy in Patients With Hepatocellular Carcinoma Receiving Non-Curative Treatment: High Sustained Virological Response Rates and Preservation of Liver Functional Reserve.

Direct-acting antivirals (DAAs) cure most chronic hepatitis C virus (HCV) infections, but their impact on liver functional reserve in patients with a history of hepatocellular carcinoma (HCC), particularly after non-curative HCC treatment, remains unclear. We evaluated antiviral efficacy and liver functional reserve after DAA therapy according to HCC history and timing of DAA initiation. In this retrospective single-center cohort, 716 patients initiated DAA therapy, and 690 with evaluable sustained virological response at 12 weeks after treatment (SVR12) were analysed. Patients were classified

DETECTION OF HEPATITIS B AND C VIRAL INFECTION AMONG CHILDREN WITH ACUTE LEUKAEMIA.

BACKGROUND: Viral hepatitis, particularly Hepatitis B (HBV) and Hepatitis C (HCV), represents a significant health complication in immunocompromised patients, including children diagnosed with acute leukaemia. These patients are at increased risk of getting infections due to frequent blood transfusions, invasive procedures, and immunosuppressive therapy. The study aimed to evaluate the occurrence of hepatitis B and C virus infection among children with acute leukaemia treated in the pediatric haematology unit at Ibn Al-Ather Teaching Hospital in Mosul city, compare them with a control group an

Trends in Hepatitis D Virus Prevalence Among HBsAg-Positive Individuals With and Without HIV in Italy: A Converging Epidemiology.

BACKGROUND AND AIMS: Hepatitis D virus (HDV) infection has historically been more prevalent among people with HIV (PWH) due to shared transmission routes. Whether HIV status still identifies a subgroup at higher risk of HDV in contemporary settings remains unclear. METHODS: We performed two cross-sectional analyses assessing anti-HDV prevalence among HBsAg-positive PWH enrolled in the ICONA cohort in 2009-2012 and 2019-2023, compared with HIV-negative individuals from the MASTER and PITER cohorts. RESULTS: Anti-HDV prevalence among PWH declined from 19.1% to 12.5%, converging with that obser
