Investigation of Acoramidis-based ionic liquids as potential stabilizers of transthyretin using DFT calculations, molecular docking, and ADMET studies.

Transthyretin amyloidosis (ATTR) is a type of amyloidosis that characterize by abnormal deposition of the transthyretin protein in various organs. Acoramidis drug is considered a promising treatment for patients with ATTR. However, oral delivery of the insoluble Acoramidis is challenging due to its poor bioavailability and low solubility. In the present study, three Acoramidis-based ionic liquids (ILs) are reported to address this challenge. The biological activity and reactivity of these compounds evaluated using computational chemistry methods, including molecular DFT calculations, molecular

Percutaneous Left Atrial Appendage Occlusion for Atrial Fibrillation in Cardiac Amyloidosis.

BACKGROUND: Cardiac amyloidosis (CA) is frequently complicated by atrial fibrillation (AF), yet outcomes after left atrial appendage occlusion (LAAO) in this population remain poorly defined. METHODS: We conducted a retrospective TriNetX study of adults with AF undergoing LAAO, comparing patients with and without CA after 1:1 propensity matching. Outcomes were assessed using Kaplan-Meier analyses and Cox regression. RESULTS: Among 532 matched pairs, mortality and major adverse cardiovascular events were similar between groups, whereas major bleeding was higher in CA (HR 1.90). CONCLUSIONS:

Rationale, Objectives and Study Design of the Taiwan Registry of Hypertrophic Cardiomyopathy (THIC).

BACKGROUND: The Taiwan Registry of Hypertrophic Cardiomyopathy (THIC) is a multicenter national registry containing the clinical and imaging data of patients with hypertrophic cardiomyopathy (HCM) in Taiwan. The aim of the registry is to systematically evaluate the clinical, genetic and biochemical features, possible natural course, and outcomes of HCM and relevant rare diseases that mimic HCM such as Fabry disease (FD) and transthyretin amyloid cardiomyopathy in Taiwan, and to identify their specific "red-flag" signs, which are especially valuable from the perspective of unique genetic mutati

Core Needle Biopsy of the Abdominal Fat Pad for Diagnosis of Amyloidosis.

Amyloidosis is a rare systemic disorder that is underdiagnosed and whose incidence is increasing as the population ages. The reference standard for diagnosis is endomyocardial biopsy, though its utility as a screening tool is limited. Fine needle aspiration (FNA) and surgical excisional biopsy (SEB) of the abdominal fat pad have emerged as alternatives to endomyocardial biopsy as screening tools for amyloidosis. Given concerns about the variable sensitivity of FNA and the more invasive nature of SEB, our referring providers asked our group to perform core needle biopsies (CNB) of the abdominal

Glymphatic dysfunction links vascular pathology to Alzheimer's biomarkers and cognitive decline.

BACKGROUND: Vascular damage, including cerebral amyloid angiopathy (CAA) and non-amyloid cerebral small vessel disease (CSVD), has been linked to glymphatic dysfunction, which may contribute to Alzheimer's disease (AD) pathology and cognitive decline. We investigated the associations among vascular damage, glymphatic function measured by the DTI-ALPS (Diffusion Tensor Imaging-Analysis Along the Perivascular Space) index, AD plasma biomarkers, and cognitive decline. METHODS: This study includes 1,249 participants recruited from Samsung Medical Center. We performed linear regression analysis to

Trends and disparities in amyloidosis and cardiovascular disease mortality: a population-based retrospective study in the United States (1999-2020).

BACKGROUND: Amyloidosis is increasingly recognized as a contributor to heart failure, particularly among older adults and patients with heart failure with preserved ejection fraction (HFpEF). Despite advances in diagnostic imaging and disease-modifying therapies, amyloidosis remains underdiagnosed in many settings, and population-level data examining its co-occurrence with cardiovascular disease on death certificates are limited. This study examined two decades of national mortality data to evaluate deaths co-coded with amyloidosis and cardiovascular disease (CVD) in the United States and to a

Single-cell RNA sequencing reveals disease associated changes in brain endothelial cells in the 5XFAD mouse.

Vascular dysfunction is a key contributor to Alzheimer’s disease (AD) pathology, where changes to the endothelium and its crucial role in maintaining blood-brain barrier (BBB) integrity have been of particular emphasis. The transgenic 5XFAD (5X Familial Alzheimer’s Disease) mouse model, which exhibits AD-related amyloidosis through FAD associated mutations in amyloid precursor protein (APP) and presenilin-1 (PS1), has become a widely adopted preclinical model in AD-related research studies. The need for cross-study standardization, accessibility, and data reproducibility has led to the widespr

Recent advances in small molecules for amyloid fibril inhibition: chemical strategies and molecular mechanistic insights from lysozyme and insulin models.

Protein misfolding and aggregation are central pathological processes underlying amyloidosis, a group of disorders characterized by the deposition of insoluble amyloid fibrils. These toxic aggregates form due to conformational destabilization and exposure of hydrophobic protein regions, leading to aberrant interactions that disrupt normal cellular functions. Amyloid fibrillation is a key pathological event in both neurodegenerative and systemic disorders. In metabolic and systemic amyloidosis, such as lysozyme derived and insulin derived amyloidosis, peripheral fibril deposition leads to progr

Hospitalisation for acute heart failure and in-hospital mortality before, during and after the COVID-19 pandemic in France: a nationwide cohort study from 2013 to 2024.

INTRODUCTION: Healthcare systems were reorganised in 2020 to manage the COVID-19 pandemic. Despite their urgent status, hospital admissions for acute heart failure (AHF) were reported to decline from 9% to 66% worldwide between 2020 and 2021, with divergent findings regarding in-hospital mortality. This study aimed to investigate in detail the evolution of AHF hospitalisations and in-hospital mortality in France from 2013 to 2024. METHODS: Based on the 2.9 million AHF hospitalisations recorded in France from 2013 to 2024, yearly numbers of hospitalisations and deaths expected in years 2020-20

Two Sides of the Same Coin: Transthyretin (TTR) as a Target or Drug Carrier for Drug (Bio)conjugates.

Transthyretin (TTR) is a tetrameric protein present in plasma and cerebrospinal fluid that binds to thyroxine (T4) and retinol (vitamin A) to transport them across the blood-retina barrier and to the liver. Mutations on the TTR gene cause destabilization of the tetramer structure leading to misfolded monomers and aggregates, thus triggering several pathologies (i.e., cardiomyopathy and neurodegeneration). The stabilization of TTR tetramer architecture and the silencing of TTR gene expression represent viable therapeutic strategies for amyloidosis. Moreover, the TTR role as a delivery system us

Multiparametric CMR for detection of cardiac amyloidosis in patients with undifferentiated concentric left ventricular hypertrophy.

Multiparametric CMR can detect cardiac amyloidosis (CA) in patients with undifferentiated concentric left ventricular hypertrophy (LVH). A retrospective patient search was performed for patients ≥ 18 years of age who received 1.5T CMR from January 2018 to August 2022 to evaluate for myocardial infiltration in the setting of known concentric LVH at a tertiary medical center. Clinical records were reviewed for positive diagnosis or exclusion of CA. CMR were post-processed to evaluate ventricular volumes and function, native T1 mapping, and extracellular volume (ECV). Late gadolinium enhancement

A Spatial Proteomics Approach to Isolate and Analyze Amyloid Proteins Using Laser Capture Microdissection and Mass Spectrometry.

Amyloidosis comprises a heterogeneous group of disorders marked by the extracellular deposition of insoluble protein fibrils, known as amyloids, which can disrupt normal tissue architecture and lead to organ dysfunction. Accurate identification and subtyping of the amyloidogenic protein are critical for clinical management as treatment strategies vary significantly depending on the underlying protein species. While conventional diagnostic tools such as Congo red staining and immunohistochemistry are commonly used, they suffer from limited specificity and antibody availability, often resulting

[Amyloidosis in the elderly: a clinical enigma between frailty and diagnosis].

Cardiac amyloidosis is considered a rare disease but is increasingly frequent in elderly patients. In these patients, heterogeneous and peculiar clinical manifestations require specific approaches and dedicated diagnostic-therapeutic pathways. Early diagnosis is important because mortality is high, and the disease is often latent and underdiagnosed. In particular, the light chain form represents a real emergency, due to the rapidly fatal course in the absence of treatment. Specific programs based on a multispecialist approach should be implemented to reduce delays in diagnosis. Thanks to these
