National variation in acute kidney injury diagnosis and monitoring across adult intensive care units in china: a nationwide cross-sectional survey.

BACKGROUND: Acute kidney injury (AKI) is common in critically ill patients and strongly associated with adverse outcomes, yet national ICU-level evidence on AKI diagnosis and monitoring in China remains limited. METHODS: We conducted a nationwide anonymous cross-sectional web survey of adult ICUs in China from June 25 to September 26, 2025. The survey assessed AKI diagnosis and staging, UO and SCr monitoring across clinical scenarios, biomarker use, early warning systems, and changes in UO monitoring after AKI. We used multivariable logistic regression to identify factors associated with UO-m

Preventive and post-exposure effects of apigenin against ochratoxin A-induced nephrotoxicity in rats: biochemical and histopathological evidence.

Ochratoxin A (OTA) is a potent nephrotoxin commonly found in foods and feeds, with the kidney as its primary target organ. Apigenin (APG), a plant-derived flavonoid with antioxidant and anti-inflammatory properties, may counteract OTA-induced renal damage. This study evaluated the preventive and post-exposure recovery effects of APG against OTA-induced nephrotoxicity in rats. Sixty adult male Sprague-Dawley rats were assigned to five groups (n = 12): control; APG (20 mg/kg bw/day); OTA-contaminated diet (10 mg OTA/kg diet); APG + OTA preventive group; and OTA + APG post-exposure recovery group

The Mechanism of S-adenosylhomocysteine Hydrolase in Regulating Podocyte Apoptosis and Diabetic Nephropathy via Moap1.

Elevated levels of S-adenosylhomocysteine (SAH) in patients with diabetes are closely associated with renal dysfunction. Inhibition of SAH hydrolase (SAHH) increases SAH levels, promotes apoptosis, and exacerbates streptozotocin (STZ)-induced diabetic nephropathy. However, the precise mechanism by which SAHH inhibition induces podocyte apoptosis remains unclear. This study aimed to investigate how SAHH inhibition specifically induces podocyte apoptosis and to elucidate the underlying molecular mechanisms. We established an STZ-induced podocyte-specific SAHH knockout mouse model and an in vitro

Paeoniflorin Promotes Mitophagy Against Acute Kidney Injury via PRDX5-Dependent Inhibition of RACK1.

Impaired mitophagy is a core driver of acute kidney injury (AKI), yet effective pharmacological strategies to restore this mitochondrial quality control mechanism remain limited. Paeoniflorin (Pae), a natural monoterpene glycoside, has shown renoprotective potential, but whether and how it regulates mitophagy remain unclear.. This study investigated the protective effects of Pae in mice subjected to bilateral renal ischaemia-reperfusion injury (IRI) and in cisplatin-treated TCMK-1 cells, with renal tubular-specific Prdx5 conditional knockout (Prdx5-CKO) mice used to determine the requirement f

Imbalance of renin-angiotensin system in calcium oxalate crystals-induced renal oxidative stress and autophagy.

BACKGROUND: Calcium oxalate (CaOx) stones are the most prevalent urological calculi with high incidence and recurrence. Effective preventive drugs remain lacking because of unclear pathogenesis. Although the imbalance of the dual renin-angiotensin system (RAS) axes is closely linked to oxidative stress and autophagy, its role in CaOx stone formation remains undefined. METHODS: In vitro, human renal tubular epithelial cells were stimulated with CaOx crystals, with siRNA-mediated AT1R or MasR knockdown or pretreatment with Ang Ⅱ, Ang-(1-7), antioxidant Nacetylcysteine (NAC) or autophagy inhibit

Integrated Bedside Hemodynamic Assessment and Monitoring of Venous Congestion With Venous Excess Ultrasound (VExUS): A Narrative Review.

This review evaluates the venous excess ultrasound (VExUS) score as a bedside tool for quantifying systemic venous congestion and predicting congestive end-organ injury, particularly acute kidney injury (AKI). By assessing flow patterns in the hepatic, portal, and renal veins alongside inferior vena cava (IVC) size, VExUS enables serial monitoring of congestion trajectories. While highly prognostic in heart failure, its predictive performance varies in more heterogeneous populations. Ultimately, integrating VExUS with cardiac output indices may help clinicians identify specific hemodynamic phe

Multi-Modal Biometric Fusion for Athletic Performance Optimization: Clinical Validation of the Dynamic Multi-Sensor Fusion Classifier (DMSFC).

Integrating real-time biometric monitoring into sports science is increasingly important for optimizing athletic performance and reducing injury risk. However, many existing systems have limited capacity to accurately combine data from multiple biometric sources. This study evaluated the Dynamic Multisensor Fusion (DMSFC) classifier for analyzing physiological and functional data collected from multiple biometric modalities, including electrocardiography (ECG), fingerprint measurements, and facial recognition. A multimodal biometric framework based on DMSFC was developed to monitor and classif

Cardiac magnetic resonance with myocardial strain as a predictor of anthracycline-induced cardiotoxicity in patients with lymphoma: a prospective multimodality study.

BACKGROUND: Cancer therapy-related cardiac dysfunction (CTRCD) is a frequent and clinically relevant complication in patients with lymphoma treated with anthracyclines. Early identification of subclinical myocardial injury is critical to prevent progression to overt heart failure and to avoid chemotherapy dose reductions that may compromise oncologic outcomes. We aimed to evaluate the role of cardiac magnetic resonance (CMR) with T1 mapping and feature-tracking myocardial strain, in comparison with transthoracic echocardiography (TTE) and circulating biomarkers, for the early detection of CTRC

Renal artery imaging through long-term follow-up after radiofrequency renal denervation: Results from the SPYRAL Clinical Program.

Background: Renal denervation (RDN) safely reduces blood pressure (BP) in patients with uncontrolled hypertension. A potential long-term concern is the development of de novo renal artery stenosis (RAS) secondary to vascular injury. This study assesses the long-term vascular safety and incidence of RAS in patients treated with radiofrequency RDN from the SPYRAL HTN-OFF MED and -ON MED trials. Patients and methods: All patients underwent renal angiograms for assessment of the renal anatomy prior to randomization. Patients with RAS >50% were excluded from enrollment. At long-term follow-up (439+

Integrating gut microbiome and serum metabolomes to investigate the hepatoprotective effects of Lactiplantibacillus Plantarum PA01 against deoxynivalenol-induced liver injury in broilers.

Deoxynivalenol (DON) contamination is a significant challenge in broilers, which has a negative impact on liver function and health, endangering broiler production. This study aimed to evaluate the protective effects of L. plantarum PA01 in mitigating DON-induced liver injury and to elucidate the underlying mechanisms. A total of 144 one-day-old Arbor Acres broilers were divided into three groups: the control group receiving only a basal diet, the DON group (5 mg DON/kg diet), and the DP group (1 × 109 cfu L. plantarum PA01/g diet + 5 mg DON/kg diet). The results showed that L. plantarum PA01

Cardio-hepatic injury phenotypes and short-term outcomes in acute decompensated heart failure.

BACKGROUND: Abnormal liver biochemistry is common in acute decompensated heart failure (ADHF), yet most studies evaluate individual markers rather than clinically meaningful patterns of hepatic involvement. We examined whether cardiohepatic injury phenotypes derived from routine liver tests identify distinct short-term prognostic profiles in patients hospitalized with ADHF. METHODS: In this retrospective cohort study, we included consecutive adults hospitalized with ADHF between December 2006 and 2017. Using peak in-hospital values of aspartate aminotransferase, alanine aminotransferase, alka

Biofilm-host immunometabolic crosstalk in Staphylococcus aureus orthopedic implant infection: mechanisms and translational constraints.

Orthopedic implant biofilms are not only antibiotic-tolerant bacterial communities but metabolically active niches that reshape host immunity. Compared with planktonic infection, mature biofilms impose spatial and metabolic constraints that can uncouple leukocyte recruitment from sterilizing activity. Neutrophils can penetrate and phagocytose portions of Staphylococcus aureus biofilms, but clearance becomes incomplete as biofilms mature and may coexist with degranulation and neutrophil extracellular trap-associated tissue injury. Macrophages show impaired access and bactericidal programming, a

Autophagy as an adaptive resistance mechanism to epigenetic therapy in cancer.

Epigenetic therapies can reverse malignant transcriptional programs, but their activity is often constrained by adaptive stress responses that allow cancer cells to survive and recover. Autophagy is a central component of this adaptation. By sustaining metabolic homeostasis, mitochondrial fitness, proteostasis, and lysosomal recycling, autophagy can buffer the cellular injury produced by inhibitors of DNA methyltransferases, histone deacetylases, and bromodomain proteins. These observations provide a strong rationale for combining epigenetic therapies with autophagy inhibition. However, the fi
