Anterior mini-bikini incision for surgical drainage in pediatric septic arthritis of the hip.

PURPOSE: Open arthrotomy is the standard treatment for septic arthritis of the hip (SAH) in pediatric patients. The anterior and medial approaches are commonly employed; however, these techniques result in substantial tissue trauma and unfavourable scarring. The bikini incision technique has also been documented in several studies. This study evaluated the efficacy of open drainage via a mini-bikini anterior approach for pediatric SAH patients. METHODS: This was a single-centre retrospective analysis. From January 2020 to March 2024, pediatric patients diagnosed with SAH were enrolled and und

Multifocal Osteoarticular Infection by Methicillin-Sensitive Staphylococcus aureus in the Neonatal Period: A Diagnostic Challenge.

We report the case of a 15-day-old neonate presenting with multifocal osteoarticular infection, initially manifesting as septic arthritis of the right wrist, which rapidly progressed to involve multiple sites, including the right deltoid and left hip. The etiological agent was identified as methicillin-sensitive Staphylococcus aureus (MSSA). This case is notable for its multifocality, the severity of the clinical course in the neonatal period, and the association with a prior history of neonatal admission for severe hypernatremic dehydration.

Bone and Joint Infections in Tropical Settings: High Prevalence of Gram-Negative Bacilli and Implications for Empirical Therapy.

BACKGROUND: Bone and joint infections (BJIs) are increasingly reported worldwide, but data on their epidemiology remain limited in tropical settings. We aimed to characterize the causative agents of BJIs and their resistance patterns, in order to inform empirical antibiotic in our tropical setting. METHODS: This 6-year retrospective study included all adults with a first microbiologically confirmed episode of BJI between January 2019 and December 2024 at the Guadeloupe University Hospital, a tertiary care center in the Caribbean. RESULTS: A total of 312 patients with BJI were included. Among

A Rare Case of Mycotic Common Femoral Artery Pseudoaneurysm from Septic Ankle Arthritis: Surgical and Endovascular Considerations.

PURPOSE: Mycotic aneurysms are rare, life-threatening vascular infections that typically result from direct vascular inoculation, including intravenous drug use or vascular access procedures. Hematogenous seeding from a remote musculoskeletal source is distinctly uncommon. This report describes the multidisciplinary management of a mycotic common femoral artery pseudoaneurysm arising from septic ankle arthritis, highlighting key open and endovascular considerations. CASE REPORT: A 78-year-old man presented with painful right-groin swelling and septic right-ankle arthritis. Computed tomographi

A Rare Case: Septic Arthritis of The Wrist.

INTRODUCTION: Septic arthritis of the wrist is an uncommon condition, representing only a small fraction of all septic arthritis cases. CASE REPORT: This report details the clinical course of a 63-year-old female patient who developed septic arthritis and subsequent osteomyelitis of the wrist following a traumatic laceration. Despite multiple courses of antibiotics and a delayed diagnosis, the patient ultimately required a two-stage surgical approach, including debridement, placement of an antibiotic spacer, and definitive arthrodesis with autologous bone grafting. Effects of antibiotics encapsulated in hyaluronic acid hydrogels on different osteogenic cells and bacteria.

Topical application of antibiotics in the treatment of orthopaedic implant-related infections can be achieved by using hyaluronic acid (HA)-based hydrogels as carriers. Our aim was to investigate potential toxic effects of a novel antibiotic-loaded hydrogel on osteogenic cells and its antibacterial effect against staphylococci. A covalently cross-linked hyaluronic acid (HA)-based hydrogel was loaded with increasing concentrations of cefuroxime and vancomycin and their release was examined by UV spectrometry. Primary human (HoBs), mouse (MoBs) osteoblasts, or SaoS-2 cells were either exposed to

Methicillin-resistant Staphylococcus aureus has phenotypic variation in mecA expression that alters antibiotic sensitivity.

Methicillin resistant Staphylococcus aureus (MRSA) bacteremia has a high rate of morbidity and mortality. Multiple clinical studies have demonstrated improved outcomes when MRSA bacteremia is treated with dual antibiotic therapy that includes a β-lactam antibiotic such as cefazolin. This is a paradox as MRSA should be inherently resistant to this class of antibiotics. We report on a serendipitous observation of a phenotype where MRSA became sensitive to cefazolin when cultured in a physiologic relevant media of fetal bovine serum as well as in synovial fluid. This could be observed across mult

Mettl3-modified lncRNA Snhg16 aggravates osteoarthritis via promoting chondrocyte ferroptosis by regulating Snd1/Gpx4 axis.

PURPOSE/AIM: Long noncoding RNA (lncRNA) small nucleolar RNA host gene 16 (Snhg16) has been confirmed to accelerate osteoarthritis (OA) progress. However, its regulatory mechanism has not been fully elucidated. MATERIALS AND METHODS: Interleukin-1β (IL-1β)-induced ATDC5 chondrocytes were used to mimic OA cell models, and destabilization of medial meniscus (DMM) surgery was performed to construct OA mice models. The expression levels of Snhg16, glutathione peroxidase 4 (Gpx4), staphylococcal nuclease domain-containing 1 (Snd1), methyltransferase-like 3 (Mettl3) and extracellular matrix (ECM) d

Drug-caged drugs enable photocatalytic dual decaging of nitric oxide and anesthetics for antibacterial analgesia.

Bacterial infections often result in significant pain, negatively impacting patient outcomes and quality of life. Conventional therapies primarily focus on pathogen eradication but frequently overlook the associated pain, thereby limiting their overall clinical effectiveness. Herein, we propose a 'drug-caged drug' strategy in which the secondary amine group of the local anesthetic tetracaine (TTC) is selectively caged by nitric oxide (NO), forming the TTC-NO prodrug. The TTC-NO prodrug is co-loaded with the photocatalyst fac-Ir(ppy)3 into poly(ethylene glycol)-b-poly(ε-caprolactone) (PEG-b-PCL

Skin androgens regulate Staphylococcus aureus pathogenicity via quorum sensing.

Skin cells secrete testosterone, with greater amounts secreted at the skin surface of males compared with females. Males are also more susceptible to skin infections than females. Here we report that mice engineered with testosterone-deficient skin are resistant to methicillin-resistant Staphylococcus aureus infections. Testosterone promoted the expression of S. aureus cytotoxic virulence factors by activating the accessory gene regulator (agr) quorum-sensing pathway in a concentration-dependent manner and independent of quorum-sensing-activating auto-inducing peptides. Septic Arthritis of the Hip in a Premature Neonate Caused by Staphylococcus epidermidis: A Case Report.

Neonatal septic arthritis is a rare but potentially destructive condition requiring prompt diagnosis and intervention to prevent long-term sequelae. We reported a premature neonate who presented on day of life 20 with refusal to move the left hip and was diagnosed with septic arthritis caused by Staphylococcus epidermidis, an uncommon pathogen in neonates without indwelling devices. Diagnostic imaging confirmed left hip septic arthritis, and surgical irrigation and drainage were performed. Operative cultures grew S. epidermidis, and the patient completed targeted intravenous antimicrobial ther

Surgical and Functional outcome of Infective Knee Operated with Arthrotomy.

INTRODUCTION: Septic arthritis of the knee represents a significant orthopedic emergency, necessitating timely and effective intervention to avert irreversible joint destruction and potentially life-threatening systemic sequelae. Arthrotomy, involving open surgical drainage and meticulous debridement, continues to be the standard of care in cases with advanced infection, dense purulent collections, or where minimally invasive approaches such as arthroscopy are not viable. While this procedure is widely practiced, especially in resource-constrained settings, there remains a paucity of data rega

Real-World Comparison of Intravenous vs. Oral Antimicrobial Therapy for Bone and Joint Infections.

Well-designed randomized controlled trials (RCTs) have demonstrated safe and effective use of oral antimicrobial therapy for bone and joint infections. Application of data for implementation into real-world practice, however, has inherent challenges. This retrospective cohort study compared real-world use of intravenous versus oral antimicrobial therapy in bone and joint infections within a large healthcare system comprising both academic and community medical centers. The primary outcome was the proportion of treatment failure. Key secondary outcomes included the proportion of patients with l

Daphnoretin as a potent sortase A inhibitor effectively disarms drug-resistant Staphylococcus aureus infections.

The rapid spread of drug-resistant Staphylococcus aureus (S. aureus) has severely undermined the efficacy of existing antibiotics, highlighting the urgent need to explore novel antivirulence strategies to address this global public health challenge. Sortase A (SrtA) is a critical virulence factor of S. aureus that covalently anchors multiple surface proteins to the bacterial cell wall, thereby mediating adhesion, invasion, immune evasion, and biofilm formation, playing a pivotal role in pathogenesis. In this study, potential inhibitors were screened using a fluorescence resonance energy transf
