Pneumococcal sternoclavicular septic arthritis: A rare manifestation of invasive pneumococcal disease.

Septic arthritis of the sternoclavicular joint comprises less than 1 % of all septic arthritis cases. While Streptococcus species are common causative organisms, Streptococcus pneumoniae rarely causes this infection. We present a case of pneumococcal sternoclavicular septic arthritis in a 58-year-old male with a history of alcohol use disorder and no prior pneumococcal vaccination. Despite the introduction of pneumococcal vaccines, unvaccinated individuals remain at risk, especially those with comorbidities. This case highlights the need for vaccination and early intervention in high-risk popu

Safety and Efficacy of Simultaneous Vaccination with Polysaccharide Conjugate Vaccines Against Pneumococcal (13-Valent Vaccine) and Haemophilus Type B Infections in Children with Systemic Juvenile Idiopathic Arthritis: Prospective Cohort Study.

Background: The introduction of biological drugs into clinical practice for the treatment of children with systemic juvenile idiopathic arthritis (sJIA) allows disease control but increases the risk of infectious events. Infectious events cause immunosuppressive therapy interruptions, leading to disease flare and life-threatening complications, namely macrophage activation syndrome. Our study aimed to evaluate the efficacy and safety of simultaneous vaccination against pneumococcal and Haemophilus influenzae type b (Hib) in children with sJIA. Methods: This study included 100 sJIA patients rec

Pneumococcal and influenza vaccination coverage and impact on COVID-19 infection severity in patients with inflammatory rheumatic diseases: A French National Healthcare Database analysis.

INTRODUCTION: It has been suggested that vaccinations could induce a trained immunity able to decrease COVID-19 severity. Our primary aim was to evaluate the COVID-19 severity among patients with inflammatory rheumatic diseases (IRD) vaccinated against pneumococcus and influenza compared to those not vaccinated. A secondary objective was also to determine vaccination coverage within real-life population of IRD patients in France. METHODS: We conducted a longitudinal study within the French administrative and medical data base (SNDS). We have identified patients with one following of these IRD

Effect of a 1-month methotrexate delay on pneumococcal vaccine immunogenicity and disease control in patients with early rheumatoid arthritis (VACIMRA): an open-label randomised trial.

BACKGROUND: Pneumococcal vaccination is recommended for patients with rheumatoid arthritis. Because immunosuppressant therapies for rheumatoid arthritis hinder vaccine efficacy, vaccination should be administered before initiating immunosuppressive drugs. We aimed to compare humoral responses in patients with rheumatoid arthritis receiving the pneumococcal 13-valent conjugate vaccine (PCV13) before methotrexate initiation or simultaneously. METHODS: In this randomised, multicentre, open-label trial, patients were recruited from 26 rheumatology departments in 22 university hospitals and four g

Towards a minimal core dataset for systemic lupus erythematosus studies.

OBJECTIVE: SLE is a complex, heterogenous autoimmune disease. SLE researchers do not always collect the same data, making comparative studies difficult. We aimed to ascertain what variables SLE clinical researchers commonly collect for SLE research. Our ultimate goal is to generate a minimal core dataset for future SLE studies. METHODS: In 2020, we designed and distributed a questionnaire to members of the Systemic Lupus Erythematosus International Collaborating Clinics (SLICC) as well as additional active research centres in China. Our survey included 26 questions about the types of data tha

Multisite infection with Austrian syndrome caused by non-vaccine pneumococcal serotype-case report.

BACKGROUND: Although rare, Austrian syndrome remains a life-threatening triad consisting of pneumonia, endocarditis, and meningitis caused by Streptococcus pneumoniae. CASE SUMMARY: We present a case of Austrian syndrome, further complicated by spondylodiscitis and endophthalmitis, in an immunocompromised male receiving methotrexate treatment for rheumatoid arthritis. The infection was caused by a non-vaccine pneumococcal serotype (23B). Susceptibility testing showed reduced susceptibility towards benzylpenicillin, an uncommon finding in Denmark. CONCLUSION: This case illustrates some of the

When Fever Strikes Twice: A Case Report of Streptococcus pneumoniae Myelitis with Delayed-Onset Reactive Arthritis.

Background:Streptococcus pneumoniae is a well-known pathogen responsible for respiratory and invasive diseases; however, central nervous system (CNS) involvement in the form of bacterial myelitis is exceedingly rare, particularly in immunocompetent adults. Moreover, the association between pneumococcal infections and reactive arthritis is scarcely documented. We report an unusual case of pneumococcal myelitis complicated by reactive arthritis in an elderly patient with no evident immunosuppression. Case Presentation: A 68-year-old man with a medical history of hypertension, benign prostatic hy

A Rare Case of Invasive Pneumococcal Disease Presenting With Pneumonia, Meningitis, Septic Arthritis, and Cerebral Infarcts.

Streptococcus pneumoniae (SP) is a common pathogen responsible for a broad spectrum of infections and the leading cause of community-acquired pneumonia; however, invasive and life-threatening disease is rare in immunocompetent individuals. We describe the case of a 60-year-old male who presented with a constellation of neurological and systemic symptoms involving the respiratory, nervous, and musculoskeletal systems, and was subsequently diagnosed with invasive pneumococcal disease (IPD). Despite the severity of his illness, the patient made a remarkable recovery following prompt initiation of

Inflammatory Knee Reaction Mimicking Septic Arthritis Following Closely Spaced Pneumococcal Vaccines in a Pediatric Patient Undergoing Immunodeficiency Evaluation.

Vaccines in the pediatric population are crucial in preventing infectious diseases, but rare inflammatory responses can arise, particularly in patients with suspected immunodeficiency. This case highlights a unique inflammatory reaction mimicking septic arthritis following closely spaced pneumococcal vaccines (PPSV23) in a child undergoing immunodeficiency work-up. A previously healthy five-year-old Caucasian female with a history of recurrent bacterial infections presented with a two-day history of left knee swelling and pain after receiving two pneumococcal vaccines within a five-week interv

Surviving the Deadly Triad: Two Cases of Austrian Syndrome in Southern Switzerland, Associated With Disseminated Streptococcus pneumoniae Infection.

Austrian syndrome is a rare and life-threatening condition. It is more severe in immunocompromised patients, those with comorbidities, or harmful alcohol consumption. The etiopathogenesis involves hematogenous seeding of Streptococcus pneumoniae and local spread in the infected tissues, presenting with pneumonia, endocarditis, and meningitis. A 68-year-old man was hospitalized for impaired consciousness, left hemiparesis, and aphasia, consistent with meningoencephalitis. A CT scan showed otomastoiditis and bilateral pulmonary consolidations with respiratory failure. A transesophageal echocardi

The type of infections and the use of antibiotics among patients with rheumatoid arthritis: A review.

Patients diagnosed with rheumatoid arthritis have a heightened susceptibility to infections, which may lead to higher rates of illness and death. The heightened susceptibility may arise from the illness itself, which causes changes in the body's innate cellular defense mechanisms, or from the medications used to manage the condition. The precise level of risk for infections associated with traditional disease-modifying anti-rheumatic drugs has not been fully elucidated. This review aimed To investigate the type of infections and the use of antibiotics among patients with rheumatoid arthritis.

Low influenza, pneumococcal and herpes zoster vaccination coverage in Australian patients commencing a biologic or targeted synthetic disease modifying anti-rheumatic drug for inflammatory arthritis: a nationwide cross-sectional data linkage study.

Vaccine-preventable infections, such as herpes zoster (HZ), influenza, and pneumococcal disease are major contributors to morbidity and mortality in patients with inflammatory arthritis. To determine national vaccination coverage against influenza, S. pneumoniae and herpes zoster (HZ) in Australian patients dispensed the first script of a b/tsDMARD for inflammatory arthritis (IA). National data was used to determine the number of patients who received vaccination against S. pneumoniae and HZ within 6 months of commencement of the first Pharmaceutical Benefits Scheme (PBS)-subsidised b/tsDMARD

Coverage and Drivers of Vaccinations in Patients with Autoimmune Rheumatic Diseases: An Italian Multicentric Study.

BACKGROUND: Patients with autoimmune rheumatic diseases (ARDs) such as systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA) are at increased risk of infections due to immune dysregulation and immunosuppressive therapy. Vaccination is a cornerstone of infection prevention, but uptake is still inadequate. METHODS: We conducted an observational, multicenter study at four Italian rheumatology centers. Adult patients with RA or SLE on immunosuppressive therapy completed a standardized questionnaire assessing demographics, disease activity, treatments, vaccination status for influenza,

Compliance with recommended pneumococcal vaccination schedule in patients treated for rheumatoid arthritis: A retrospective cohort study in the Veterans Affairs population.

INTRODUCTION: Invasive pneumococcal disease (IPD) is a leading cause of death. Rheumatoid arthritis (RA) patients are at risk of IPD due to immunosuppressant medications. Up until 2022, two pneumococcal vaccines, the 13-valent Pneumococcal conjugate vaccine (PCV13) and pneumococcal polysaccharide vaccine (PPSV23), were recommended. Despite the recommendation change to give a single 20-valent PCV vaccine (PCV20), some still require multiple vaccinations. There is a need to identify barriers to vaccine uptake. METHODS: We conducted a retrospective cohort study to assess the on-time vaccination

Invasive pneumococcal disease serotype 23B1 causing multifocal septic arthritis, myositis and retroperitoneal abscess.

We describe a case of a previously healthy unvaccinated man in his 70s who developed penicillin-susceptible bacteraemic invasive pneumococcal disease due to non-vaccine serotype 23B with the unusual manifestations of multifocal myositis, intramuscular abscesses, polyarticular septic arthritis and synovitis. Blood cultures drawn prior to antibiotic therapy and culture of iliopsoas collection were helpful in making the diagnosis. At follow-up, he had persistent hip pain attributed to avascular necrosis of the head of femur, a possible late complication of his pyomyositis.
