EGFR-mutated lung adenocarcinoma presenting as miliary intrapulmonary carcinomatosis: A diagnostic mimic of tuberculosis.

Miliary intrapulmonary carcinomatosis (MIPC) is a rare metastatic pattern of lung cancer that closely mimics disseminated tuberculosis (TB), posing a significant diagnostic challenge. We present a 53-year-old Filipino woman, a lifelong nonsmoker with prior latent TB, who presented with progressive cough and a diffuse bilateral miliary nodular pattern on chest imaging. An extensive infectious workup was unrevealing, and bronchoscopic biopsy revealed poorly differentiated adenocarcinoma (TTF-1+) harboring an EGFR exon 21 L858R mutation. Staging demonstrated brain, osseous, and pleural metastases

[The institutionalization of vaccination in Spain: from smallpox to the National Vaccination Schedule (1803-1975)].

Vaccination has played a central role in the historical development of public health in Spain and in the progressive institutionalization of state health intervention. From the late eighteenth century onwards, the fight against smallpox acted as a driving force for increasingly systematic preventive policies, evolving from variolation to jennerian vaccination and its administrative regulation. The Royal Philanthropic Vaccine Expedition (1803-1813) represented the first major milestone in this process, transforming vaccination into an organized, permanent practice under public authority. Granuloma-Like Structures in Tuberculosis A Critical Review of Their Biology, Quantitative Behavior, and Translational Use.

Tuberculosis (TB) granulomas are complex multicellular structures central to disease pathogenesis and treatment challenges, notably due to bacterial persistence and immune modulation. Existing animal and in vitro models do not fully capture granuloma heterogeneity and immune complexity. Here, we review the biology and applications of Granuloma-Like Structures (GLS) derived from human peripheral blood mononuclear cells infected with Mycobacterium tuberculosis, which recapitulate key features of early granuloma formation, including macrophage differentiation, lymphocyte involvement, and cytokine

Gene conversion is a key driver of diversity hotspots in Mycobacterium tuberculosis antigens and virulence-associated loci.

Despite the long-held view of Mycobacterium tuberculosis (Mtb) as a genetically conserved pathogen, many genomic regions remain poorly resolved due to high sequence homology and repetitive content. Using complete genome assemblies generated from long-read sequencing of 151 globally representative clinical isolates, we comprehensively analyzed genome-wide patterns of genetic diversity and evolution across the Mtb genome. Our analysis uncovers pronounced diversity hotspots within paralogous regions (PRs) generated by recurrent gene conversion between homologous genes. In many cases, these hotspo

Diverse infection models demonstrate robust resistance of Mycobacterium tuberculosis to innate immunity.

Mycobacterium tuberculosis (Mtb) is a robust activator of innate immunity. While innate immune activation of macrophages can impact Mtb replication in vitro, there is little in vivo evidence that the innate immune system controls Mtb before the onset of adaptive immunity. Prior work has generally used specific pathogen-free (SPF) mouse models and relatively large infectious doses, which may obscure the capacity of innate immunity to control Mtb. Here, we performed ultra-low dose Mtb infections and found that the initial innate immune response was unable to curb even minimal Mtb infectious dose

BCG vaccination mitigates tau pathology and restores cognitive function in PS19 mice.

Retrospective studies in patients with nonmuscle invasive bladder cancer have reported a significant reduction in Alzheimer's disease incidence (12 to 78%) among Bacille Calmette-Guérin (BCG) recipients versus controls. To investigate the underlying mechanisms, we evaluated BCG in the PS19 mouse model of tauopathy. We found that BCG administration reduced hippocampal phospho-tau and microgliosis while preserving neuronal markers. In vivo volumetric T2-magnetic resonance Imaging (MRI) demonstrated attenuation of brain atrophy accompanied by increased glutamate-weighted chemical exchange saturat

How do we monitor doxy-PEP impact and when should we consider revisiting guidelines?

The 2026 WHO guidelines recommending doxycycline post-exposure prophylaxis (doxy-PEP) represent the first global antibiotic prophylaxis recommendation for bacterial sexually transmitted infections (STIs), with provisions for antimicrobial resistance (AMR) monitoring.Doxy-PEP should be governed as an adaptive intervention subject to periodic, transparent review, but establishing decision rules faces challenges.Doxy-PEP is expected to select for resistance, including in Neisseria gonorrhoeae, Staphylococcus aureus, and the gut and skin microbiomes; the scope of clinical consequences is incomplet

Substrate Mimicry by Inhibitors of the Mycobacterium tuberculosis 20S Proteasome and Their Activity against Mycobacteria In Vitro and in Macrophages.

The remarkable and problematic capacity of Mycobacterium tuberculosis to persist in the host is dependent on a functional 20S proteasome that is structurally and functionally analogous to its mammalian counterpart. It has been proposed that this enzyme is critical for the bacterium's adaptive response to a variety of stressors in the host. Here, we report the structure of a rationally designed and species-selective inhibitor of the Mtb 20S proteasome in complex with the enzyme. The inhibitor was inspired by the syringolin natural products-covalent proteasome inhibitors that mimic a substrate.

Risk Markers for Anastomotic Complications After Tracheal Resection for Benign Stenosis.

BACKGROUND: Tracheal resection with primary end-to-end anastomosis is the definitive treatment for benign tracheal stenosis, yet anastomotic complications remain clinically important, and their risk markers are incompletely defined. We evaluated patient- and surgery-related variables associated with anastomotic outcomes and described these associations in primary versus surgically altered operative fields. METHODS: This retrospective single-center cohort study included all consecutive adults undergoing tracheal resection for benign tracheal stenosis between 2007 and 2024. Sixty-nine patients

Inflammatory Biomarkers (IL-6, IL-10, IL-17, TNF-α) in TB patients attending a tertiary care hospital in Bhubaneswar, Odisha : a cross - sectional study.

BACKGROUND: Tuberculosis exhibits a complicated immune-pathogenesis that involves pro-inflammatory cytokines which modulate the host's immune responses. Cytokines like IL-6, IL-10, IL-17 and TNF-α are crucial in managing inflammation, immune protection and the progression of the disease that affect diagnostic and treatment responses. AIM: In this study, the levels of cytokines IL-6, IL-10, IL-17 and TNF-α in the serum samples of pulmonary TB patients were estimated and their diagnostic significance was assessed and correlated with socio-demographic profile. METHODS: Sera samples were collect

Development and Internal Validation of a Clinical-Epidemiological Model for Active Pulmonary Tuberculosis Among Patients Already Evaluated for Suspected Disease in a Low-Incidence Hospital.

BACKGROUND: In low-incidence hospital settings, airborne isolation capacity is limited. We developed a clinical-epidemiological model to estimate active pulmonary tuberculosis (PTB) probability among adults already selected for PTB evaluation, using information available before definitive diagnostic testing. METHODS: We retrospectively analyzed consecutive adults who underwent microbiological testing for suspected PTB and were managed with airborne precautions at a university hospital in Rome from January 2016 through December 2021 (n=682 complete cases). A LASSO-penalized logistic regression

A hypothesis-generating study of collateral reactivation: a reanalysis of Mycobacterium tuberculosis genomes from 119 recurrent tuberculosis cases.

Recurrent tuberculosis is traditionally classified as exogenous reinfection or endogenous reactivation. We previously proposed "collateral reactivation," in which distinct Mycobacterium tuberculosis subclones derived from a hypothetical original clone are involved in separate disease episodes, in contrast to traditional endogenous ("direct-descent") reactivation. To explore whether collateral reactivation occurs beyond the single case we previously reported, we reanalyzed publicly available whole-genome sequencing data from 123 recurrent tuberculosis cases. After curation, 119 cases (pairwise

Refractions of tuberculosis and migration in the northern suburb of Paris: Exposure, adherence, and semantic recalcitrance.

France is overall a low endemic country for tuberculosis (TB); however, Seine-Saint-Denis-the poorest department of metropolitan France-has the highest incidence rate in the whole country, excluding overseas territories. At the national level, TB mainly concerns people coming from a country with a high endemicity of TB, people in conditions of absolute poverty, and people living in prison facilities. In this north-eastern Parisian suburb, we conducted qualitative research through a multi-sited ethnography in six university hospitals of the Assistance Publique - Hôpitaux de Paris (AP-HP), two i

Prevalence and determinants of metabolic abnormalities among adult tuberculosis patients at Gondar Town, Northwest Ethiopia: an institution-based comparative cross-sectional study.

BACKGROUND: Metabolic abnormalities, such as diabetes mellitus, hypertension, dyslipidemia, and central obesity, are significant contributors to the global burden of non-communicable diseases. Tuberculosis (TB) patients may be at increased risk of these conditions due to chronic inflammation, nutritional depletion, and anti-TB drug effects; however, locally grounded comparative evidence from resource-limited settings such as northwest Ethiopia remains scarce. This study aimed to assess the prevalence and determinants of metabolic abnormalities among TB patients in Gondar Town, Ethiopia. METHO

Quantitative assessment of high-pressure processing inactivation of foodborne bacteria and viruses in foods.

High-pressure processing (HPP) is a non-thermal food processing technology that can inactivate microbiological hazards while preserving product quality. This study compiled 2692 D-values across 15 microbial hazards pressurized at different pressures at a temperature ≤ 45 °C. The Bigelow model was used to estimate Dref values at Pref of 500 MPa. Based on these values, microbial hazards were grouped into three HPP resistance categories across all food matrices: 1) Sensitive (D500MPa ≤ 1 min): Vibrio spp., Aeromonas caviae, Campylobacter spp., Rotavirus, Cronobacter spp., Hepatitis A virus, Norov
