BACKGROUND: It remains unclear whether active pulmonary tuberculosis risk is still high in rheumatoid arthritis patients in settings where tuberculosis infection screening is performed before the use of biologicals. Moreover, the impacts of seropositivity and disease-modifying antirheumatic drugs on active pulmonary tuberculosis risk should be elucidated.
METHODS: The incidence of active pulmonary tuberculosis was compared between patients with rheumatoid arthritis (n=59 577; 41 501 seropositive rheumatoid arthritis and 18 076 seronegative rheumatoid arthritis) and 1:5 age- and sex-matched co

OBJECTIVES: Fibromyalgia (FM) is a relevant comorbidity in immune-mediated rheumatic diseases (IMRD). Immune mechanisms have been postulated to trigger FM, potentially leading to distinct clinical features compared to FM occurring without IMRDs. This study aims to provide a comprehensive comparison of FM characteristics in patients with and without concomitant IMRD and to evaluate the differences in diagnostic FM criteria between the two groups.
METHODS: A comprehensive dataset of clinical, psychosocial, and sleep variables, along with validated questionnaires, was prospectively collected fro

A 79-year-old male patient on hemodialysis for 29 years was admitted to the hospital with unexplained fever (C-reactive protein, 16.1 mg/dL), anemia (hemoglobin 7.6 g/dL), and multiple joint swelling that began 7 months earlier. Infection was ruled out, both rheumatoid factor and anti-cyclic citrullinated peptide antibodies were negative. Positron emission tomography-computed tomography showed positive findings in bilateral shoulder joints, palmar region, femoral head area, and cervical spine. Magnetic resonance imaging showed low intensity on T1 and low-normal intensity on T2, especially in t

A 48-year-old man with longstanding seronegative rheumatoid arthritis (RA) developed a progressive myelopathy characterized by sensory disturbances and gait ataxia. His symptoms emerged after four years of adalimumab therapy, prompting magnetic resonance imaging (MRI). MRI revealed a non-enhancing cervical cord lesion extending from C2 to C7. Testing for aquaporin-4 (AQP4) and myelin oligodendrocyte glycoprotein (MOG) antibodies was negative, and other etiologies were excluded. He showed a partial response to corticosteroids, and adalimumab was discontinued in favor of rituximab. This case und

To investigate the supplementary value of the neutrophil-to-lymphocyte ratio (NLR) in diagnosing rheumatoid arthritis (RA) and assessing disease activity, particularly in seronegative RA. This retrospective single-center study included 304 patients from the Department of Rheumatology and Immunology at a tertiary hospital between February 2021 and February 2024. The RA group consisted of 201 patients, and the non-RA control group had 103 patients. Demographic characteristics, blood tests, inflammatory markers (C-reactive protein, erythrocyte sedimentation rate), autoantibodies (rheumatoid facto

Background: Rheumatoid arthritis is a relatively common rheumatic disease that can present with inflammatory arthritis and subcutaneous nodules. Multicentric reticulohistiocytosis and fibroblastic rheumatism are rarer entities that also present with these features. Methods: Two cases, one of each of fibroblastic rheumatism and multicentric reticulohistiocytosis, are described highlighting characteristic clinical, radiographic, and histologic findings. A narrative review of the literature on these rarer conditions, compared with rheumatoid arthritis, is provided with a focus on articular and cu

OBJECTIVE: Rheumatoid arthritis (RA) is classified into seropositive (SP-RA) and seronegative (SN-RA) types, reflecting distinct immunological profiles. This study aimed to identify the T cell phenotypes associated with each type, thereby enhancing our understanding of their unique pathophysiological mechanisms.
METHODS: We analyzed peripheral blood T cells from 50 participants, including 16 patients with untreated SP-RA, 17 patients with SN-RA, and 17 healthy controls, utilizing 25 T cell markers. For initial analysis, a dataset was established through manual T cell subset gating analysis. F

A 55-year-old woman presented to our hospital with arthritis of the hands. Tests for anti-cyclic citrullinated peptide antibodies and rheumatic factor were negative, and seronegative rheumatoid arthritis was diagnosed. The patient was treated with methotrexate and certolizumab (tumour-necrosis factor-α inhibitor). Disease activity was controlled with the drugs, but after 14 months, erythematous plaques with scaling appeared on the palms and feet. Skin biopsy revealed bullous lesions with aseptic abscess formation, leading to the diagnosis of palmoplantar pustulosis and pustulotic arthro-osteit

OBJECTIVE: To investigate whether patients with ACPA-negative (ACPA-) and ACPA-positive (ACPA+) rheumatoid arthritis (RA) exhibit distinct immune and metabolic profiles in blood, using integrative proteomic and metabolomic analyses. By uncovering subgroup-specific molecular signatures, we aim to improve the biological understanding of RA heterogeneity and support the development of more precise diagnostic and stratification strategies.
METHODS: We performed high-throughput proteomic and metabolomic profiling on plasma from a well-characterized cohort comprising 40 patients with ACPA- RA, 40 p

Background: Seronegative rheumatoid arthritis (SNRA), defined by the absence of rheumatoid factor (RF) and anti-citrullinated peptide antibodies (ACPA), represents 20-30% of rheumatoid arthritis cases. Once considered a milder phenotype, SNRA is now recognised as a heterogeneous entity in which a substantial subset of patients develops structural progression comparable to seropositive RA. The binary RF/ACPA-based definition is increasingly viewed as insufficient, as the broader anti-modified protein antibody (AMPA) family-including antibodies against carbamylated, acetylated and malondialdehyd

BACKGROUND: Rheumatoid arthritis (RA) is a chronic, progressive autoimmune disorder characterized by joint pain, stiffness, and deformities, leading to functional impairment and reduced quality of life (QoL). Disease-modifying anti-rheumatic drugs (DMARDs) are the cornerstone of therapy, effectively reducing disease activity and improving health-related QoL (HRQoL).
OBJECTIVE: To compare HRQoL outcomes between seropositive and seronegative RA patients and to assess the improvement in HRQoL following initiation of conventional synthetic DMARDs (csDMARDs) in both seropositive and seronegative R

OBJECTIVES: This study investigated the 5-year all-cause mortality trend for women and men with seropositive versus seronegative rheumatoid arthritis (RA) compared with the background population (BP).
METHODS: Using nationwide registers, a population-based cohort of incident patients with RA diagnosed in Denmark between 2000 and 2014 was identified. Based on immunoglobulin M-rheumatoid factor and/or anticitrullinated peptides, a prespecified algorithm was used to determine the serologic status. The pseudo-observation method was used to determine the cumulative incidence proportion, the age-ad

BACKGROUND: IgG4-related disease (IgG4-RD) is a systemic fibro-inflammatory condition characterized by elevated IgG4 serum levels and tissue infiltration by IgG4-positive plasma cells. While often presenting with organ-specific involvement, such as sialadenitis or pancreatitis, its rheumatologic manifestations are rare and poorly understood. IgG4-RD often overlaps with autoimmune diseases such as rheumatoid arthritis (RA), posing a diagnostic challenge, particularly in seronegative presentations.

OBJECTIVE: To analyze the association of anti-CarP levels with clinical activity, manifestations, and serological profile in rheumatoid arthritis (RA) patients.
METHODS: Cross-sectional study including RA patients and healthy controls (HC). Clinical and analytical data were collected, and anti-CarP levels were measured by ELISA.
RESULTS: A total of 101 patients (79% female, mean age 45), and 98 HC (67% female, mean age 55) were included. RA patients had significantly higher anti-CarP levels than HC (p<0.001), increasing with age (p=0.004). Anti-CarP correlated with RF and ACPA positivity (p<

Background and Clinical Significance: Antisynthetase syndrome (ASyS) is a rare autoimmune entity defined by the presence of anti-aminoacyl-t ribonucleic acid (RNA) synthetase autoantibodies and classically associated with a triad of interstitial lung disease (ILD), inflammatory myopathy, and arthritis. Additional clinical features may include Raynaud's phenomenon and "mechanic's hands". Among antisynthetase antibodies, anti-PL-12 is notably associated with predominant or isolated ILD and may occur in the absence of clinically evident myositis, thereby complicating timely diagnosis. Sjögren's syndrome is a chronic autoimmune disorder with limited therapeutic options. Glucagon-like peptide-1 (GLP-1) receptor agonists, primarily used for diabetes and obesity, have emerging evidence of anti-inflammatory and immunomodulatory effects. We present a case of a 37-year-old woman with primary Sjögren's syndrome and comorbid Hashimoto's thyroiditis and seronegative rheumatoid arthritis, who experienced frequent debilitating flares affecting her quality of life. After starting semaglutide for weight management and losing over 100 lbs, the patient reported a dramatic reduction in flar
