Marked exacerbation of resolving macrophage activation syndrome in a patient with adult-onset Still's disease following one dose of granulocyte colony-stimulating factor.

Macrophage activation syndrome (MAS) is a frequent complication of adult-onset Still's disease (AOSD) and is characterised by organ dysfunction, cytopenia and coagulopathy. There are few data regarding the use of granulocyte colony-stimulating factor (G-CSF) as a treatment for neutropenia in MAS due to AOSD. This case describes a previously healthy mid-20s patient who was diagnosed with AOSD and subsequently MAS. Although his clinical symptoms and ferritin levels responded well to steroids, interleukin-1 inhibition and Janus kinase inhibition, he developed prolonged and profound neutropenia fo

Salvage treatment of AOSD-associated macrophage activation syndrome with bariticinib following conventional immunosuppressive therapy.

OBJECTIVES: Adult-onset Still's disease (AOSD)-associated macrophage activation syndrome (MAS) is a highly inflammatory condition. Currently, the main treatment method for AOSD-associated MAS is high-dose glucocorticoid shock therapy, but the effect is not satisfactory, and new therapeutic methods or drugs are urgently needed. The aim of this study was to evaluate the efficacy and safety of salvage baricitinib therapy for AOSD-associated MAS following conventional immunosuppressive therapy. METHOD: Data from nine patients with MAS treated in the Department of Infection and the Department of R

Atypical Presentation of Adult-Onset Still's Disease with Predominant Hepatic Involvement and Monocytosis: A Case Report.

INTRODUCTION: Adult-Onset Still's Disease (AOSD) is a rare inflammatory disease. It usually causes high fevers, joint pain, a fleeting rash, and very high ferritin levels. The symptoms can mimic infections, autoimmune disorders, or cancers, so it can be hard to diagnose. Liver involvement is common but usually mild. Severe hepatitis is rare. CASE PRESENTATION: We report a 24-year-old woman who had 3 weeks of daily high fevers, a temporary salmon-pink rash, sore throat, muscle pain, and joint pain. Her blood tests showed high white blood cells with mostly neutrophils, persistent monocytosis, s

Adult-Onset Still's Disease Presenting as Aseptic Meningitis: A Case Report and Literature Review.

Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disease that usually presents with nonspecific symptoms, making early diagnosis difficult. It presents with high spiking fevers, salmon-colored rashes, and arthritis. However, atypical presentations of AOSD exist, as seen in this case, where AOSD manifested as aseptic meningitis, further complicating the diagnosis. Given its rarity and symptom overlap with infections and autoimmune diseases, AOSD poses significant diagnostic challenges. This is a case report of a 51-year-old Saudi Arabian female who suffered several weeks of fe

A rare cause of fever of unknown origin in India.

In India, tuberculosis, lymphoma and adult onset Still's disease are the commonly reported causes of fever of unknown origin (FUO). We describe a case of an immunocompetent adult male patient who presented with FUO and anemia. He developed lymphadenopathy after 8 days of admission, for which further workup was done. His bone marrow aspirate showed pure red cell hypoplasia, for the evaluation of which, parvovirus-B19 (B19V) antibodies were ordered which came back positive. He was treated with a short course of steroids and on follow-up showed complete resolution of symptoms.

Unmasking the Masquerade: A Case Report of Adult-Onset Still's Disease.

Adult-onset Still's disease (AOSD) is a rare, inflammatory condition commonly characterized by polyarthritis, rash, and fever. Its diagnosis is often missed entirely or delayed due to a lack of biomarkers and nonspecific symptoms. We report a case of a 40-year-old female presenting with an unresolving sore throat, small joint arthritis, worsening low-grade fever, and diffuse rash. After an extensive workup requiring multiple hospitalizations, she was ultimately diagnosed with AOSD. Significant symptom improvement was seen following the initiation of canakinumab and a brief course of steroids.

Tofacitinib in the Treatment of Refractory Adult-Onset Still's Disease Co-diagnosed With Celiac Disease: A Case Report.

Adult-onset Still's disease (AOSD) is an autoinflammatory disease characterized by systemic and musculoskeletal manifestations driven by dysregulated cytokine activity. Fevers, evanescent salmon-colored rash, and transaminitis are hallmark features of the systemic subset of AOSD. Macrophage activation syndrome (MAS) represents the most feared and potentially life-threatening complication of AOSD. Tofacitinib, a Janus kinase (JAK) inhibitor, is approved for the management of several autoimmune conditions. We present a case of a 31-year-old woman with AOSD co-diagnosed with celiac disease, who w

Adult-Onset Still's Disease Associated With Hyperimmunoglobulin-E: A Case Report.

Adult-onset Still's disease (AOSD) is a rare, chronic, multisystem inflammatory disorder of unknown etiology. Diagnosis is made clinically and can be challenging. AOSD typically affects young adults but can occur in older patients. The association between AOSD and very high immunoglobulin-E levels (also known as hyperimmunoglobulin-E) is not well studied. In this report, we present the case of a middle-aged woman with AOSD who presented with more than two weeks of fever, severe generalized weakness, and sore throat, and responded very well to prednisone. She was found to have hyperimmunoglobul

Adult-Onset Still's Disease With Normal Ferritin Levels and Severe Sulfasalazine-Induced Probable Case of Drug Reaction With Eosinophilia and Systemic Symptoms (DRESS) Syndrome: A Unique Presentation and Management Challenges.

Adult-onset Still's disease (AOSD) is an uncommon clinical condition with an uncertain cause, characterized by arthritis, fever, evanescent rash, and other systemic presentations. This case report describes a 26-year-old female who had a fever, arthralgia, vomiting, sore throat, bilateral distal extremities edema, hypertension, and normal ferritin. She was diagnosed with AOSD using the Yamaguchi criteria based on exclusion and was treated with hydroxychloroquine and sulfasalazine. The patient's arthralgia improved significantly. However, she returned with complaints of a diffuse erythematous b

Adult-Onset Still's Disease Presenting With Leukopenia and Pulmonary Infiltrates: A Mimicker of Infection and Malignancy.

Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disorder that often mimics infections, tickborne diseases, or malignancies, leading to diagnostic delays. We present a 26-year-old male with persistent fever, arthralgia, lymphadenopathy, and elevated inflammatory markers unresponsive to antibiotics. Extensive infectious and autoimmune workup was largely negative except for positive antinuclear antibody (ANA) and ribonucleoprotein (RNP). Development of a maculopapular rash with spiking fevers and elevated ferritin (1240 ng/mL) met Yamaguchi's criteria for AOSD. Initiation of co

The Risk Factors of Refractory Adult-Onset Still's Disease.

Background: Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disorder of unknown etiology and pathogenesis. Some patients fail to respond to conventional glucocorticoids and immunosuppressant therapies, a condition known as refractory AOSD. The prognosis for patients with refractory AOSD is typically poor, significantly impacting their quality of life and overall health. This study retrospectively analyzes the predictive factors for refractory AOSD to provide new strategies and insights for clinical diagnosis and treatment. Methods: Overall, 105 AOSD patients hospitalized bet

Macrophage Activation Syndrome in Adult-Onset Still's Disease: Challenges in Early Detection and Management.

Adult-onset Still's disease (AOSD) is an immunological disorder that manifests with fever, evanescent rash, leukocytosis, and arthralgia. One of the most severe complications of AOSD is macrophage activation syndrome, a life-threatening complication characterized by hyperactivation of the immune system and multiorgan dysfunction. This case report describes a 24-year-old female who developed macrophage activation syndrome in the setting of AOSD. Initially, she developed heterogeneous and nonspecific symptoms of fever, pharyngitis, rash, lymphadenopathy, and migratory arthralgia after a trip to

Superior Aortic Recess Mimicking Takayasu Arteritis in a Patient With Adult-Onset Still's Disease.

The superior aortic recess, a normal pericardial extension around the ascending aorta, can be misinterpreted as pathological findings on imaging studies, potentially leading to misdiagnosis of conditions such as Takayasu arteritis. We report a case of persistent fever and joint pain initially suspected of having Takayasu arteritis based on contrast-enhanced CT showing apparent aortic wall thickening. Laboratory tests showed elevated inflammatory markers (CRP: 7.07 mg/dL, WBC: 11,200/μL), microcytic anemia (hemoglobin: 7.8 g/dL), and thrombocytosis (platelets: 599,000/μL). Initial treatment wit

Adult-Onset Still's Disease With Suspected Steroid Hypersensitivity: A Case Report.

Adult-onset Still's disease (AOSD) is a rare, systemic auto-inflammatory disorder characterised by quotidian spiking fevers, arthralgia or arthritis, and a salmon-pink maculopapular rash. Corticosteroids are the mainstay of treatment, but rarely, hypersensitivity reactions to steroids can complicate management. Here, we report a complicated case of a 39-year-old woman who presented with recurrent high-grade fever, arthralgia, rash, sore throat, and lymphadenopathy. Laboratory investigations revealed elevated inflammatory markers and serum ferritin. She met Yamaguchi criteria for AOSD and respo

Hemophagocytic Lymphohistiocytosis: A Life-Threatening Hyperinflammatory Syndrome.

Hemophagocytic lymphohistiocytosis (HLH) is a rare, life-threatening hyperinflammatory syndrome resulting from uncontrolled immune activation. It can be triggered by infections, malignancies, or autoimmune conditions and is difficult to diagnose due to its nonspecific clinical presentation. A male patient around 30 years old was admitted to the ICU in 2016 with severe diarrhea, vomiting, and abdominal pain. He developed pancytopenia requiring granulocyte colony-stimulating factor (G-CSF) support. Bone marrow aspiration and trephine revealed hemophagocytosis, with markedly elevated serum ferrit

Coexistence of Adult-Onset Still's Disease and Graves' Disease: Coincidence or Continuum?

Adult-onset Still's disease (AOSD) is a rare systemic autoimmune disorder of unclear etiology, typically characterized by prolonged fever, arthralgia, transient rash, and leukocytosis. Graves' disease (GD) is a common autoimmune cause of hyperthyroidism in young adults. Although associations between autoimmune conditions have been reported, the coexistence of AOSD and GD remains uncommon. We describe the case of a 22-year-old patient presenting with clinical and biochemical hyperthyroidism, confirmed by thyroid scintigraphy. The clinical picture was further complicated by persistent inflammato

Successful re-administration of tocilizumab in a patient with adult-onset Still's disease after improvement of macrophage activation syndrome.

Tocilizumab (TCZ) is effective for inducing remission in adult-onset Still's disease (AOSD), but its use may occasionally trigger macrophage activation syndrome (MAS). The rationale for re-introducing TCZ in patients with a history of MAS is not well established. Here, we report a case of successful re-administration of TCZ for an AOSD relapse in a patient with a prior history of MAS during TCZ therapy. A 67-year-old woman, initially treated with TCZ for polyarthritis, developed MAS associated with AOSD. MAS was resolved with glucocorticoid pulse therapy, high-dose glucocorticoids, and cyclosp
