Effusion Unveiled: Impending Cardiac Tamponade as the Initial Manifestation of Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized primarily by inflammatory polyarthritis, although extra-articular manifestations, including pericardial disease, are well recognized. While pericardial effusions are relatively common, large symptomatic effusions causing cardiac tamponade as the initial manifestation of RA are exceedingly rare. We report a 75-year-old woman with hypertension and dyslipidemia who presented with progressive dyspnea and fatigue one month after an episode of right-hand monoarthritis with an initially unrevealing rheumatologic evaluation. Steroid-Refractory Adult-Onset Still's Disease: A Case Report.

Adult-onset Still's disease (AOSD) is one of the systemic autoinflammatory disorders characterised by an evanescent salmon-pink rash, high spiking fevers, and inflammatory arthritis. AOSD is driven by dysregulated immune activation involving excess pro-inflammatory cytokines, specifically IL-1β and IL-18. Current consensus recognises AOSD and systemic juvenile idiopathic arthritis as a continuous disease spectrum. Because it lacks a definitive biomarker and mimics other severe conditions, AOSD remains a diagnosis of exclusion; early recognition is paramount to prevent complications including m

Outcomes of Patients With Schizophrenia and Bipolar Disorder Undergoing Lumbar Fusion: A Retrospective National Database Study.

BACKGROUND: Studies of mental health comorbidities in spine surgery have primarily focused on the relationship between anxiety or depression and postoperative outcomes. The purpose of this study was to compare rates of preoperative comorbidities and 90-day to 2-year outcomes between patients who underwent posterior lumbar fusion (PLF) with or without a diagnosis of schizophrenia (SCZ) or bipolar disorder (BD). METHODS: A retrospective review of the PearlDiver Mariner-170 database from 2010 to 2023 was performed. All included patients underwent 1-level PLF and had ≥2 year follow-up. Patients w

Open synovectomy for treatment of tenosynovial giant cell tumors of the knee in children.

We present seven pediatric cases of open knee synovectomy for tenosynovial giant cell tumor (TGCT)-four diffuse (D-TGCT) and three nodular (N-TGCT). The mean patient age at surgery was 11.3 ± 4.77 years. Five cases were diagnosed and treated within the past 2 years, indicating an unusually high incidence of TGCT in our institution. Subtotal synovectomy was performed through an anterior parapatellar approach, while posterior lesions were accessed between the semitendinosus and medial gastrocnemius muscles. The mean follow-up period was 39 ± 62.3 months (minimum 12 months). No recurrences were d

Anti-citrullinated Protein Antibody-positive Arthralgia Associated with an L110P-E148Q Double Heterozygote: A Case-based Review.

A 42-year-old woman with positive anti-citrullinated protein antibody (ACPA) and rheumatoid factor (RF) levels exhibited arthralgia, but she did not fulfill the criteria for rheumatoid arthritis (RA). Subsequently, the patient exhibited episodic monoarthritis with a remarkable response to colchicine. An analysis of the MEFV gene identified heterozygous variants, L110P and E148Q, thus indicating a colchicine-responsive autoinflammatory condition. The patient was later diagnosed with RA and achieved remission with abatacept and colchicine. This case illustrates that ACPA-positive arthralgia evol

Artemisiaherba alba Outperforms Indomethacin with Multitarget Efficacy and Safety in CFA Arthritic Model.

Rheumatoid arthritis remains a major clinical challenge requiring safer and more effective alternatives to conventional non-steroidal anti-inflammatory drugs (NSAIDs). This pioneering study evaluated the anti-inflammatory, analgesic, antioxidant, and safety effects of Artemisia herba alba extract in complete Freund's adjuvant (CFA)-induced arthritis in rats. Animals received oral Artemisia herba alba (250 or 500 mg/kg), indomethacin (3 mg/kg), or saline for 15 days. CFA induced marked joint inflammation, mechanical allodynia, locomotor impairment, and oxidative stress. Treatment with Artemisia

Modulation of TNF-α, IL-1β and myeloperoxidase contributes to the anti-inflammatory and analgesic effects of Paullinia pinnata Linn (Sapindaceae) extracts in complete Freund's adjuvant-induced mono-arthritis in rats.

ETHNOPHARMACOLOGICAL RELEVANCE: Paullinia pinnata Linn (Sapindaceae) is used in Cameroonian ethnomedicine for the treatment of arthritis. Its leafy aqueous (AEPP) and methanol (MEPP) extracts have shown potent preventive effects in mono-arthritis but their curative effects and their mechanism of action remain unknown. AIM: This work evaluates the effects of AEPP and MEPP on Complete Freund's Adjuvant (CFA)-induced septic mono-arthritis (SM-A) and their mechanism of action. MATERIALS AND METHODS: SM-A was induced by intra-ankle injection of CFA (50 μl) in both male and female rats. On day 8 a

Ultrasound-Guided Synovial Biopsy in Chronic Knee Monoarthritis: A Case Series Highlighting Its Diagnostic Value.

Ultrasound-guided synovial biopsy represents a valuable diagnostic tool in the investigation of chronic knee monoarthritis, particularly in cases where conventional clinical, laboratory, and imaging findings remain inconclusive. We report a case series of three patients presenting with chronic knee monoarthritis evolving over several months who were managed in a tertiary rheumatology center. In all cases, ultrasound-guided synovial biopsy was performed after inconclusive routine investigations and enabled definitive etiological diagnosis. Histopathological examination revealed two cases of ost

Investigating synovial trace elements as diagnostic markers in acute septic arthritis: an exploratory study.

OBJECTIVES: Acute arthritis requires a precise and rapid diagnosis, particularly to rule out an infectious cause, which constitutes a rheumatologic emergency. Cytological, bacteriological, and microcrystal analyses do not always clearly differentiate causes. We hypothesized that variation in metal concentrations in synovial fluid could provide additional clues for etiological orientation. Our objective was to evaluate their diagnostic value in acute arthritis. METHODS: We conducted a retrospective, single-center study (Rennes University Hospital) using synovial fluids from the SYNOLACTATE-PLU

Arthroscopic one-stage management of diffuse shoulder pigmented villonodular synovitis with concomitant full-thickness rotator cuff tear and acromial bone erosion: a rare case report.

The coexistence of diffuse shoulder pigmented villonodular synovitis (PVNS), full-thickness rotator cuff tear, and PVNS-induced focal acromial bone erosion is extremely rare, with no prior reports documenting this complete pathological triad, which poses unique diagnostic and therapeutic challenges. We report a 52-year-old male with a three-month history of progressive right shoulder pain, swelling, and functional limitation. Radiographs showed cortical irregularity of the acromion, while MRI revealed extensive synovial proliferation with low signal on T1/T2 sequences, suggestive of PVNS, and

Atypical involvement of the proximal interphalangeal joint in oligoarticular juvenile idiopathic arthritis: a case report.

INTRODUCTION AND IMPORTANCE: Oligoarticular juvenile idiopathic arthritis (JIA) is the most common JIA subtype, typically involving large joints and presenting as mono- or oligoarthritis within 6 months of onset. Small joint involvement is rare and can complicate diagnosis. CASE PRESENTATION: We report an 8-year-old Palestinian girl presenting with isolated swelling of the proximal interphalangeal (PIP) joint of the right index finger. She experienced stiffness and restricted motion without systemic symptoms. Initial evaluations, including imaging and biopsy, suggested trauma. Persistent symp

Salmonella Septic Arthritis in Immunocompetent Young Children: A Rare Cause of Pediatric Monoarthritis.

Septic arthritis is an orthopedic emergency requiring prompt diagnosis and treatment to prevent irreversible joint damage. Staphylococcus aureus is the most common causative organism in pediatric septic arthritis. In contrast, Salmonella species are rare etiological agents, particularly in immunocompetent children without predisposing conditions. We report two cases of Salmonella-induced septic arthritis in healthy children. The first case involved an 11-month-old girl with acute right ankle septic arthritis. The second case involved a 15-month-old boy with subacute right hip septic arthritis

Reactive Arthritis Associated With Helicobacter Pylori Infection Presenting as Treatment-Refractory Monoarthritis: A Case Report.

Helicobacter pylori (H. pylori) is a prevalent gastric pathogen increasingly recognized as a potential trigger for extra-gastric inflammatory conditions. Despite this, its association with reactive arthritis (ReA) remains under-reported and is often overlooked in the differential diagnosis of seronegative arthritis. A 19-year-old female presented with a five-month history of persistent inflammatory monoarthritis of the left knee. The condition was refractory to non-steroidal anti-inflammatory drugs (NSAIDs), intra-articular corticosteroid injections, and conventional synthetic disease-modifyin

Differentiating septic arthritis from non-infectious inflammatory causes of acute monoarticular arthritis in children: A machine learning approach based on routine laboratory tests.

OBJECTIVE: Acute monoarthritis in children poses a diagnostic challenge, particularly in distinguishing septic arthritis from non-infectious inflammatory causes. Delayed or incorrect diagnosis may lead to serious complications or inappropriate treatment. This study aims to develop and validate machine learning (ML) models for distinguishing septic arthritis from non-infectious inflammatory arthritis in children presenting with acute monoarthritis, using routinely available laboratory markers including body temperature, conventional inflammatory markers (CRP and ESR), and complete blood count (

When Joint Aspiration Fails: Point-of-Care Ultrasound for Diagnosing Chronic Gout in Prolonged Monoarthritis.

BACKGROUND: A 34-year-old man presented to our primary care clinic with persistent pain and swelling of the right first metatarsophalangeal joint lasting 3 months. A previous clinic had suspected gout based on hyperuricemia, but urate-lowering therapy was repeatedly withheld because of ongoing inflammation, resulting in prolonged symptoms managed only with nonsteroidal anti-inflammatory drugs. CASE PRESENTATION: Ultrasound-guided joint aspiration yielded a dry tap owing to highly viscous chalky material. Point-of-care ultrasound demonstrated diffuse hyperechoic intra-articular deposits and a

Septic Arthritis and Its Mimics: Assessing the Incidence, Mortality Rate, Cause-of Death and Preventability in a Population-Based Cohort Study.

BACKGROUND: Septic arthritis (SA) is considered the most harmful etiology in patients presenting with an acutely painful swollen knee. This study aimed to estimate the incidence, mortality rate, and cause of death among patients diagnosed with SA and compare it to non-infectious causes. METHODS: We conducted a population-based observational cohort study in Skåne, southern Sweden, with a population of 1.4 million people. We included adults in the years 2020 and 2021 with cultures obtained at the emergency department from a native knee, where SA was considered a differential diagnosis based on

Joint pain: a practical guide to the diagnosis of arthritis.

Joint pain is one of the most common reasons for consultation in clinical practice, and its correct interpretation presents a diagnostic challenge due to the wide variety of causes and clinical presentations. The assessment of joint pain requires a structured clinical approach that allows differentiation between joint and extra-articular causes and distinguishes between inflammatory and non-inflammatory processes. A detailed medical history and targeted physical examination form the cornerstone of the diagnostic process, facilitating the identification of clinical patterns based on the number
