BACKGROUND: Severe combined immunodeficiency (SCID) is a rare group of primary immunodeficiency disorders characterized by profound defects primarily in T cells and, in certain subtypes, in B and NK cells as well. These immune deficiencies predispose affected patients to life-threatening infections and complicate their clinical management. Since the BCG vaccine is routinely administered to neonates in most countries, undiagnosed SCID patients face a significant risk of developing severe localized or disseminated BCG-related complications. Therefore, it should be taken into account in suspiciou

POEMS is an uncommon paraneoplastic plasma cell disorder known with the expansion of polyneuropathy, organomegaly, endocrine abnormalities, M-band, and skin changes. We report a patient presenting with monoarthritis and weight loss for 5 months, who was treated empirically for tuberculous arthritis and later developed debilitating axonal polyneuropathy due to delayed diagnosis. POEMS/Plasma cell dyscrasias rarely involve joints, and this case highlights this unique presentation of POEMS syndrome. Bone biopsy from the affected joint revealed the diagnosis. Chronic large-joint monoarthritis with

INTRODUCTION: Serratia marcescens is an opportunistic Gram-negative bacterium infrequently implicated in septic arthritis, with ankle involvement reported only in isolated cases.
CASE REPORT: We present a 45-year-old male with systemic lupus erythematosus on long-term corticosteroids, prior ischemic stroke with residual left hemiparesis, and end-stage renal disease on hemodialysis through a permanent catheter, who developed septic arthritis of the left ankle following a recent episode of presumptive hospital-acquired pneumonia. Concurrent blood and joint aspirate cultures both confirmed S. ma

Cryptosporidiosis is a zoonotic disease caused by a well-known parasitic protozoan called Cryptosporidium. Infection in livestock causes important economic losses among farm animals and its control has a global concern. In this study, internal white and external red layers were separated from pomegranate peels (Punica granatum) then; they were grinded to reach Nano form. Anticryptosporidial effect of their water extracts was investigated in experimentally infected mice. Also, their antioxidant activity, biochemical and histopathological changes were studied. Briefly, hot aqueous extracts of po

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

OBJECTIVES: Recurrent monoarthritis (RM) is a major challenge of many rheumatic diseases. Ablation is a well-known technique in the treatment of benign or malign lesions of different aetiologies. We aimed to investigate the success and safety of microwave ablation (MWA) as an adjunctive therapy in a cohort of medical treatment-resistant RM.
METHODS: Patients with RM associated with different inflammatory diseases were included. MWA was performed after measuring the size of synovial hypertrophy with 15 or 20 W power and different durations until microbubbles were shown, indicating necrosis. Bo

OBJECTIVES: Immune-mediated adverse events (irAEs) from immune checkpoint inhibitors (ICIs) often require high-dose glucocorticoids (GCs), which can promote cancer progression and counteract ICI benefits. This study evaluated the articular and oncologic clinical outcomes of ICI-induced arthritis treated with methotrexate (MTX) as a GC-sparing agent.
METHODS: Adult patients with ICI-induced arthritis in 2023 were included. Arthritis was assessed using the disease activity score on 28 joints by C-reactive protein (DAS28-CRP), with follow-ups every 3 months. All patients received subcutaneous MT

Fungal arthritis is a rare infection with a global distribution that affects neonates, the elderly, and immunocompromised individuals, resulting in severe outcomes. It presents a challenge for healthcare professionals due to its rarity, lack of characteristic features, and difficulty isolating and identifying responsible organisms. Studies on their pathophysiology and treatment have been limited, and evidence-based treatment options are lacking. The pathogens infect through direct inoculation, extension from adjacent infective foci, or distant dissemination via the bloodstream. Typically, it m

Pancreatitis panniculitis and polyarthritis syndrome (PPP) syndrome is a rare complication of existing pancreatic disorders which include a triad of pancreatitis, oligo or monoarthritis and lobular panniculitis. We describe a case of a 3-year-old female child who presented with chronic pancreatitis and later developed knee pain during hospitalization. Other causes of arthritis were ruled out and pancreatitis-related arthritis, PPP syndrome was suspected. PPP syndrome treatment primarily involves managing the underlying pancreatic cause. In our case, managing the chronic pancreatitis with NSAID

Background: Although tuberculosis (TB) can affect any organ, ruptured Baker's cyst due to TB is an uncommon phenomenon. Case Summary: A young lady presented with unilateral atraumatic knee and calf pain and swelling. Pain characteristics shared both mechanical and inflammatory natures. Constitutional features for TB were evident. Based on history, clinical examination, and initial investigation, a ruptured Baker's cyst with septic knee was suspected. Ultrasound-guided aspiration followed by a synovial fluid study revealed inflammatory fluid with high adenosine deaminase level but without any b

Acute calcific periarthritis (ACP) is a self-limiting but often underrecognized condition that can closely mimics more serious joint pathologies such as septic arthritis. We present the case of a 43-year-old woman who developed acute pain and swelling of the right second toe after a 4-mile run. Initial evaluation in the emergency department (ED) included point-of-care ultrasound and C-arm imaging, which revealed periarticular calcification at the distal interphalangeal joint. However, the diagnosis of ACP was not made at that time. The patient was treated with nonsteroidal anti-inflammatory me

Diagnosing and managing acute knee monoarthritis in children presents a complex challenge, and when an etiology is not clear, it might be the result of an occult presence of a radiolucent foreign body. We report on four instances of acute monoarticular knee arthritis in pediatric patients. Despite employing standard imaging techniques, patient history, and laboratory tests, no foreign bodies (FBs) were initially identified. However, arthroscopy conducted in all cases, unveiled FBs that might have otherwise gone unnoticed and led to proper treatment of the problem. Despite their rarity, a uniqu

OBJECTIVES: Granulomatosis with polyangiitis (GPA) formerly Wegener's granulomatosis is a type of anti-neutrophil cytoplasm antibody (ANCA)-associated vasculitis (AAV) typically associated with necrotizing, granulomatous inflammation. Musculoskeletal manifestations have been described in 40-65% of patients. Arthritis usually occurs as large, single and asymmetric joint involvement, but rarely symmetrical small joint involvement may also occur. We present 56 cases whose initial presentation was mostly symmetrical and polyarthritis, which caused a significant diagnostic delay.
METHODS: Fifty-si

Tuberculosis of the foot is an uncommon form of extrapulmonary tuberculosis, and tuberculosis of the first metatarsophalangeal (MTP) joint is extremely rare. We report the case of a healthy 31-year-old woman from the Philippines with a three-year history of swelling and pain in her left first MTP joint. Foot imaging revealed severe bone destruction and granulation tissue formation at the first MTP joint. Chest imaging showed no pulmonary involvement. Mycobacterium tuberculosis was identified from the granulation tissue at the first MTP joint, leading to a diagnosis of isolated tuberculosis of

Gout is an inflammatory arthropathy that develops due to the accumulation of monosodium urate crystals in the joints in adults. In approximately half of the cases, it presents as monoarthritis with an acute attack involving the first metatarsophalangeal joint. The first attack is rarely polyarticular. Herein, we present a male patient who presented with swelling and acute pain in the 5th toe but 18F-fluorodeoxyglucose positron emission tomography/computed tomography showed symmetric polyarticular involvement which was mimicking arthritis.

Juvenile Idiopathic Arthritis (JIA) is a major contributor to chronic diseases, affecting around 1-2 in 1000 children under the age of 16. With modern treatments, the morbidity has been reduced; however, there is increasing evidence that many, if not most, children with JIA will have a chronic disease with ongoing activity into adulthood. Many studies discuss the possibility of an early window of opportunity in which patients have the best chance of responding to therapy, thereby underscoring the importance of timely and appropriate imaging. Children typically present at 4-5 years of age with

Transarterial periarticular embolization (TAPE) is a novel interventional procedure for local pain therapy in osteoarthritis. The reduction of local hypervascularization is achieved by supraselective embolization of periarticular arteries using embolic agents. The procedure described herein results in a reduction in both pain and the use of analgesics. The drug was originally developed for the treatment of knee osteoarthritis and initial small studies and case reports indicated that it was effective for other joints, including the fingers and shoulders. In the domain of rheumatology TAPE opens
