Relapse of minimal change nephrotic syndrome after initiation of sulfamethoxazole-trimethoprim combination therapy: a case report.

Minimal change nephrotic syndrome (MCNS) is characterized by podocyte injury leading to severe proteinuria, mainly mediated by T-cell activation and cytokine imbalance. Relapses are often triggered by immunological stimuli such as infections, vaccinations, or drugs; however, relapse following administration of sulfamethoxazole-trimethoprim (ST) combination therapy has not been reported previously. We report an extremely rare case of MCNS relapse triggered by ST combination therapy.A 55-year-old woman with a history of breast cancer treated with tamoxifen developed nephrotic syndrome and was di

Two siblings with lipoid congenital adrenal hyperplasia: first reported case in Serbia and literature review.

BACKGROUND: Lipoid congenital adrenal hyperplasia (LCAH) is an uncommon but life-threatening autosomal recessive disorder. It is caused by a mutation in the steroidogenic acute regulatory protein (StAR), encoded by the STAR gene. METHODS: Clinical manifestations of two siblings with primary adrenal insufficiency (PAI) were described. A female infant was admitted to the hospital at six months of age for vomiting and failure to thrive. The diagnosis of PAI of unknown etiology was established. The girl's younger brother was diagnosed with PAI at the age of 1.5 years during a febrile illness. His

Cold agglutinin syndrome secondary to Epstein-Barr virus reactivation during corticosteroid therapy for minimal change nephrotic syndrome in a patient with human immunodeficiency virus infection.

Epstein-Barr virus (EBV), a member of the herpesvirus family, establishes lifelong latency following primary infection and may be reactivated under conditions of immunosuppression. EBV reactivation is classically associated with infectious mononucleosis and Burkitt lymphoma and rarely associated with cold agglutinin syndrome. Here, we describe a case of minimal change nephrotic syndrome complicated by cold agglutinin syndrome secondary to EBV reactivation in the setting of human immunodeficiency virus (HIV) infection and corticosteroid-induced immunosuppression. A 63-year-old man with well-con

Prevalence and correlates of active syphilis diagnosed using serological assays and the molecular research-use-only Aptima Treponema pallidum assay among pregnant women in Zambia, 2023.

INTRODUCTION: Syphilis is a leading cause of adverse pregnancy outcomes, excessively affecting sub-Saharan Africa. Early diagnosis and treatment are crucial to prevent mother-to-child transmission. The aims of the present study were to assess prevalence and epidemiological and clinical correlates of active syphilis diagnosed using serological [treponemal and non-treponemal (lipoidal)] assays and the research-use-only (RUO) Aptima Treponema pallidum assay [Hologic; transcription-mediated amplification (TMA) assay] among pregnant women attending four antenatal care facilities in Nchelenge, Zambi

Treponemal and Lipoidal Immunoglobulin M Tests in the Diagnosis and Management of Syphilis Among Nonpregnant Adults: A Systematic Review.

BACKGROUND: There are many commercially available tests that isolate immunoglobulin M (IgM) antibodies to either treponemal-specific or lipoidal (non-treponemal) antigens. These IgM tests for syphilis are distinct from standard serologies that are routinely used (e.g., Treponema pallidum particle agglutination), most of which detect a combination of IgM and immunoglobulin G antibodies. There are several clinical scenarios where IgM tests for syphilis may be of clinical utility: in diagnosing early primary syphilis, in differentiating early from late latent syphilis, in detecting reinfections,

Anti-PLA2R antibody positivity in adult minimal change disease with tuberculosis co-infection: A case report.

RATIONALE: Anti-phospholipase A2 receptor (PLA2R) antibody is a highly specific serological marker for primary membranous nephropathy, with a detection rate of <2% in minimal change disease (MCD). This case represents the first report of PLA2R-positive adult MCD complicated by active pulmonary tuberculosis (TB) and systemic osteoarthropathy, highlighting the potential for chronic infection to induce PLA2R antibodies through inflammatory activation or molecular mimicry. PATIENT CONCERNS: A 73-year-old male with 7-year steroid-dependent nephrotic syndrome developed pulmonary TB and systemic ost

Successful treatment of pneumonia due to Mycobacterium goodii in a severely underweight 45-year-old woman: case report and review of literature.

Mycobacterium goodii is a rapidly growing non-tuberculous mycobacterium (NTM), primarily associated with wound and prosthetic infections. Pulmonary infections are rare, typical of patients with predisposing conditions. We report a case of M.goodii pneumonia a severely underweight (BMI 13.8kg/m2) 45-year-old woman with restrictive eating behavior and self-induced vomiting. She presented with progressive dyspnea, fever, functional decline, neutrophilia and elevated C-reactive protein. Chest CT showed segmental pulmonary embolism and bilateral lower lobe consolidations with lipoid features. Despi

Refractory minimal change disease associated with Mycobacterium marinum infection.

This case report describes a rare instance of minimal change disease (MCD) associated with Mycobacterium marinum infection. MCD is typically idiopathic but can occasionally occur secondary to infections. The patient was a 61-year-old fisherman with a history of relapsing MCD who developed a subcutaneous abscess in the lower leg while undergoing rituximab and glucocorticoid therapy. Ziehl-Neelsen staining revealed acid-fast bacilli, and culture at 30 °C isolated M. marinum, a slow-growing nontuberculous mycobacterium (NTM) commonly found in aquatic environments. The infection was attributed to

A membranous nephropathy variant mimicking minimal change disease.

This study characterizes a novel disease pattern of membranous nephropathy (MN) that exhibits overlapping clinicopathological features with minimal change disease (MCD), termed 'MCD-like MN'. Patients with histologically confirmed MN showing sparse and segmental subepithelial electron-dense deposits (EDD) but clinically resembling MCD were enrolled, alongside age- and gender-matched classic MCD and MN controls. Clinicopathological parameters were compared across groups. MN-associated antigens and MCD-related podocyte antigens were analyzed in renal tissues and serum. Among 107 archival MCD cas

Intra-Familial Phenotypic Variability in Lipoid Proteinosis: A Case Series of Three Siblings.

Lipoid proteinosis (LP), or Urbach-Wiethe disease, is an ultra-rare autosomal-recessive disorder caused by loss-of-function variants in the ECM1 gene. It is characterized by the progressive deposition of hyaline-like material in the skin, mucosae, and central nervous system. This series describes three siblings (aged 16, 24, and 25) from Gujranwala, Pakistan, born to non-consanguineous parents. All patients presented with infant-onset hoarseness of voice and recurrent vesiculobullous eruptions that evolved into waxy papules and hyperkeratotic plaques. Physical examination revealed the pathogno

Paraneoplastic Minimal Change Disease Signaling Post-Transplant AML Relapse: Two Cases and a Literature Review.

Membranous nephropathy (MN) and minimal change disease (MCD) are the most common causes of nephrotic syndrome following hematopoietic stem cell transplantation (HSCT), a complication conventionally attributed to chronic graft-versus-host disease (GVHD). Paraneoplastic MCD is well described in lymphoid malignancies but is rarely reported in myeloid neoplasms. We report two cases of biopsy-confirmed MCD presenting as the initial manifestation of acute myeloid leukemia (AML) relapse following allogeneic HSCT. Both patients were White men in their sixties with relapsed/refractory AML who developed
