Clinical Performance of the Elecsys HCV Duo Immunoassay Compared with a Standard-of-Care Algorithm Using Alinity Anti-HCV Screening and HCV RNA Confirmation.

Background/Objectives: The Elecsys HCV Duo immunoassay simultaneously detects HCV core antigen (HCV Ag) and anti-HCV antibodies in a single sample. We evaluated its clinical performance against a standard-of-care reference algorithm of anti-HCV antibody and HCV RNA testing. Methods: We retrospectively analyzed 1332 residual serum or plasma samples tested with reference HCV RNA assays and the Alinity i Anti-HCV assay. Samples were classified as reference-positive, indicating current or past HCV infection (n = 166; 88 RNA+/Ab+ with active infection and 78 RNA-/Ab+ with resolved or treated infect

SARS-CoV-2 spike protein enhances HBV replication via autophagy and aggravates inflammation in CHB patients with liver dysfunction.

This study investigated the effects of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spike protein (S protein) on hepatitis B virus (HBV) replication and immune function in patients with chronic hepatitis B (CHB), aiming to elucidate the underlying mechanisms of S protein-HBV interaction. Through in vitro cell experiments and clinical sample analyses, S protein expression was induced in HepG2.2.15 cells, leading to enhanced HBV replication; this effect was reduced by autophagy inhibitors, confirming the critical mediating role of autophagy. Furthermore, peripheral blood mono

Golgi-derived COPI vesicles fuel the biogenesis of coronavirus replication organelles.

Positive-strand RNA viruses remodel host endomembranes to form replication organelles (ROs). However, the membrane sources for coronavirus double-membrane vesicle (DMV) biogenesis remain unclear. Here, we used proteomic, genetic, pharmacological, and imaging approaches to identify the central membrane supply route for DMV formation. APEX2-based proximity proteomics first identified candidate pathways, followed by functional validation using siRNA knockdown, CRISPR/Cas9 editing, and Golgicide A (GCA) inhibition. DMV formation and viral RNA synthesis were assessed by electron microscopy, immunof

Protective mechanisms against murine coronavirus infection in individual mice and its transmission between mice.

Murine coronavirus, commonly known as mouse hepatitis virus (MHV), is a representative pathogenic virus in mice. MHV infection in mice is used as a model for viral infections and is utilized to analyze the mechanisms of protection and transmission. By establishing MHV-specific T cell clones and conducting adoptive transfer experiments in infected mice, it was demonstrated that T cells play a crucial role in the protection against MHV infection and that cytotoxic T lymphocytes induce apoptosis in infected cells and suppress viral production. Furthermore, studies using IFN-γ-deficient C57BL/6 mi

The new landscape of liver transplantation: Evolving candidates, new technologies, and personalized care.

Liver transplantation (LT) is entering a new era shaped by changes in disease epidemiology, candidate complexity, donor utilization, and post-transplant management. The decline of viral hepatitis-related cirrhosis, together with the rise of metabolic dysfunction-associated steatotic liver disease, alcohol-related liver disease, and hepatocellular carcinoma, has transformed the LT waitlist into an older, frailer, and more cardiometabolically complex population. At the same time, donation after circulatory death and machine perfusion are expanding the donor pool and redefining the boundaries of

Hepatitis-Associated Aplastic Anemia in Children and Adolescents: A Comprehensive Review.

Hepatitis-associated aplastic anemia (HAAA) is a rare but life-threatening subtype of acquired aplastic anemia (AA) that predominantly affects children and adolescents, characterized by bone marrow failure developing weeks to months following acute hepatitis. Although HAAA accounts for a small proportion of AA cases, it presents unique diagnostic and therapeutic challenges and remains poorly understood. Most cases are seronegative, with no identifiable viral pathogen, supporting an immune-mediated pathophysiology. Emerging evidence implicates dysregulated cytotoxic T-cell activation, reduced r

Hepatic Progenitor Cells and the Ductular Reaction: Pathophysiological Role in the Progression of Liver Disease.

Ductular reaction (DR), a defining histological hallmark of diverse acute and chronic liver injuries, is fundamentally driven by the activation of hepatic progenitor cells (HPCs) and their complex, spatiotemporal bidirectional crosstalk within the hepatic niche. As bipotent precursor cells, HPCs initiate compensatory regeneration when mature hepatocyte proliferation is compromised. However, DR is not a uniform process but a multiphase dynamic reaction: transient early activation facilitates parenchymal restoration, whereas persistent, dysregulated HPC expansion fuels progressive fibrogenesis.

Hepatitis A Virus (HAV) Recurrence and De Novo Hepatitis A Acquired at Time of Liver Transplantation: Comparison of Graft HAV Infection and Viral Kinetics.

Hepatitis A virus (HAV) infection is an uncommon indication for liver transplantation, and donor-derived or transfusion-associated HAV infection is rare. Data describing HAV recurrence or early acquisition following liver transplantation, particularly with longitudinal quantitative viral kinetics, are limited. We describe two liver transplant recipients with graft HAV infection occurring in different immunological contexts. One patient underwent emergency liver transplantation for HAV-related acute liver failure and subsequently developed recurrent HAV infection of the graft. The second patien

Numerous linked cases of acute genotype F hepatitis B virus infections in people living with HIV or on pre-exposure prophylaxis.

OBJECTIVES: At least 2.7 million people living with HIV are infected with HBV worldwide. Some antiretroviral treatments and tenofovir disoproxil fumarate-based Pre-Exposure Prophylaxis (PrEP) protect against HBV infection. However, emerging antiretroviral (ARV) strategies without any activity against HBV may promote spread of this virus in the future. In this context, we studied recent cases of acute HBV infection in people at risk of sexually transmitted infections. Our objective was to better understand HBV circulation and the dynamics of its transmission. METHODS: This retrospective study

Seroprevalence and risk factors of hepatitis E virus infection in a rural Argentine community: a cross-sectional study.

BACKGROUND: Hepatitis E virus (HEV) is a leading cause of acute viral hepatitis worldwide. Rural populations and pig production systems in Argentina may be at increased risk of HEV exposure; however, epidemiological data from these settings remain scarce. METHODS: A cross-sectional study was conducted among residents of a rural community. Serum samples from 90 residents were tested for anti-HEV immunoglobulin G. Participants completed structured questionnaires and residential addresses were georeferenced to evaluate exposures and spatial distribution. RESULTS: Rural residents reported greate

Disparate tetraparvovirus species linked to hepatitis.

The role of human tetraparvoviruses (PARV4) in cases of acute illness is poorly characterized, and the contribution of animal-associated tetraparvoviruses to human disease is unknown. In this study, we applied metagenomic and viral-enrichment next-generation sequencing to plasma from patients with diagnostically unresolved acute illness in Georgia (non-A-C hepatitis) and Bangladesh (acute febrile illness), and two tetraparvovirus genomes were recovered. Specimen HEP004 from a Georgian patient with acute hepatitis contained a virus clustering within Tetraparvovirus ungulate4, a species associat

The Association between Vitiligo and Infections: A Systematic Review.

Vitiligo is an acquired depigmentation disease with an unknown etiology. Several theories on its etiology have been proposed, including autoimmune mechanisms, oxidative stress, genetic factors, environmental factors, and lack of melanocyte growth factor. Recent studies have demonstrated a possible correlation between microbial infections and the development or consequences of vitiligo. This study aims to investigate the potential association between microbial infections and vitiligo, focusing on identifying any explainable correlations between these two. A systematic search was performed on al

Serum IgM crosslinks hepatitis B subviral particles into supramolecular immune complexes.

Hepatitis B virus (HBV)-infected cells release hepatitis B surface antigen (HBsAg)-rich subviral particles (SVPs) in large excess over infectious virions. SVPs bind antibodies against HBsAg (anti-HBs), including neutralizing antibodies, but the physical products of antibody capture in human blood are uncharacterized. Here we used cryo-electron tomography (cryo-ET) to image HBsAg-enriched serum from a patient with severe acute Hepatitis B and observed three-dimensional assemblies of intact IgM-SVP immune complexes. These associations persisted at a lower particle density and were validated by a

A divergent hepatitis A virus lineage in wild western lowland gorillas and central chimpanzees supports localized circulation and expands HAV diversity.

Hepatitis A virus (HAV) is a ubiquitous human pathogen that may cause acute liver disease. Despite the availability of effective vaccines, high prevalence rates are still reported in Southeast Asia and Africa, where sanitation conditions are poor. Although seven genotypes are known, including three in humans, three in non-human primates (NHP), and one recently identified in alpacas, the diversity, ecology, and evolutionary history of HAV remain poorly understood. In a pilot study of virome molecular characterization of wild African apes by using oligonucleotide probes enrichment, HAV sequences
